2. Artificial Neural Network Analysis of Volatile Organic Compounds for the detection of lung cancer

Abstract

Lung cancer is a widespread disease and it is well understood that systematic, non-invasive and early detection of this progressive and life-threatening disorder is of vital importance for patient outcomes. In this work we present a convergence of familiar and less familiar artificial neural network techniques to help address this task. Our preliminary results demonstrate that improved, automated, early diagnosis of lung cancer based on the classification of volatile organic compounds detected in the exhaled gases of patients seems possible. Under strictly controlled conditions, using Selected Ion Flow Tube Mass Spectrometry (SIFT-MS), the naturally occurring concentrations of a range of volatile organic compounds in the exhaled gases of 20 lung cancer patients and 20 healthy individuals provided the dataset that has been analysed. We investigated the performance of several artificial neural network architectures, each with complementary pattern recognition properties, from the domains of supervised, unsupervised and recurrent neural networks. The neural networks were trained on a subset of the data, with their performance evaluated using unseen test data and classification accuracies ranging from 56% to 74% were obtained. In addition, there is promise that the topological ordering properties of the unsupervised networks’ clusters will be able to provide further diagnostic insights, for example into patients who may have been heavy smokers but so far have not presented with any lung cancer. With the collection of data from a larger number of subjects across a long time period there is promise that an automated assistive tool in the diagnosis of lung cancer via breath analysis could soon be possible.

Abstract

Cytokine gene transfer into mouse tumor cells has been shown to stimulate a strong immune response resulting in the rejection of the transduced tumor when injected in vivo. Therefore, retroviral vectors containing the human interleukin (IL)-2 or IL-4 gene have been constructed to transduce human melanoma cells to explore whether their immunogenicity can be increased both in vitro and in vivo. Our preliminary results indicate that retroviral vectors can efficiently transduce the IL-2 and or IL-4 gene into melanoma clones, inducing production of either cytokine in the range of 0.5-2 ng/ml/10(5) cells in 48-72 h. No modifications of the growth rate, morphology and antigenicity of the transduced tumor cells were found.

17. Human stem cell metabolomics : headspace volatile gas analysis as an indicator of self-renewal and differentiation status

Abstract

Self-renewal and an ability to differentiate are key hallmarks of stem cells. Pluripotent stem cells (PSC) have the capacity to differentiate into all cell types found within the three germ layers; ectoderm, endoderm, mesoderm. Multipotent stem cells including mesenchymal stem cells (MSC) customarily differentiate into cell types representative of one germ layer only. Metabolomics focusses on characterising the low molecular weight organic compounds which are by-products of protein-protein interactions and metabolic enzymatic processes; volatile organic compounds (VOCs) emitted from or consumed by cells can be correlated with metabolic status. Selected Ion Flow Tube Mass Spectrometry (SIFT-MS) has been used to identify and discriminate between metabolites (VOCs) from both undifferentiated and differentiated stem cells in specific cell culture oxygen conditions (air (21% O2) and physiological oxygen (2% O2)). The suitability of SIFT-MS for detecting and identifying VOCs from hPSCs was evaluated with SIFT-MS spectral data analysed via OPLS-DA. hPSCs cultured in 2% O2 displayed a distinct metabolic profile to those cultured in 21% O2. Metabolite markers differing between culture conditions for undifferentiated hPSCs included ethanol and acetaldehyde. Inhibition of ethanol/acetaldehyde conversion enzymes revealed mechanistic control of ethanol and acetaldehyde levels linked to environmental oxygen. hPSC differentiation-linked differences in metabolic profiles included immediate reductions in acetaldehyde and DMS/ethanethiol levels upon onset of differentiation. For hMSCs, OPLS-DA score plots indicated that hMSCs cultured in a controlled, hermetic, workstation maintained at 2% O2 were distinct to those cultured in either 2% O2 or 21% O2. The VOC profile of hMSCs varied with oxygen condition and degree of differentiation during osteogenic differentiation. In summary, this thesis demonstrates that SIFT-MS can detect and discriminate VOC profiles in two distinct stem cell populations. These profiles are sensitive to oxygen and differentiation status and therefore provide a potential valuable tool for non-invasive explorations of stem cell biology.

20. Improving the management of lung cancer using mass spectrometry and spectroscopy techniques

Abstract

Lung cancer is a worldwide health problem associated with poor prognosis. The survival at 5 years remains between 5% and 15% in spite of the development of new drugs. One of the main reasons for this is the disease being diagnosed in late stages when curative treatments might not be available. Therefore, some of the most important factors within improving prognosis are both refining diagnostic techniques for early detection, and better assessing tumour response to treatment. Here lies a need for novel diagnostic tools for lung cancer. Spectroscopic and spectrometric analysis of the molecular underpinnings of the disease may provide biochemical signatures for use in diagnostics. Selected Ion Flow Tube – Mass Spectrometry (SIFT-MS) and Fourier Transform Infrared (FTIR) Spectroscopy may provide the gold standard of diagnostic assessment that is needed. Given both techniques previous contributions and technological advancements, their clinical requirements are being increasingly met. This is leading towards the opportunity for the study of lung cancer to benefit from the rapid, non-destructive and sensitive qualities they have to offer. In this thesis, both techniques have been used with the aim of improving the diagnosis and management of lung cancer.

Abstract

In most industrialized countries, different epidemiologic studies show that chronic renal failure is dramatically increasing. Such major public health problem is a consequence of acquired systemic diseases such as type II diabetes, which is now the first cause for end stage renal failure. Furthermore, lithogenic diseases may also induce intratubular crystallization, which may finally result in end-stage renal failure (ESRF). Up to now, such rare diseases are often misdiagnosed. In this study, based on twenty four biopsies, we show that SR µFTIR (Synchrotron Radiation-µFourier transform infrared) spectroscopy constitutes a significant opportunity to characterize such pathological µcalcifications giving not only their chemical composition but also their spatial distribution in the tissues. This experimental approach offers new opportunities to the clinicians to describe at the cell level the physico-chemical processes leading to the formation of the pathological calcifications which lead to ESRF.

Abstract

Identifying early cellular events in response to a chemotherapy drug treatment, in particular at low doses that will destroy the highest possible number of cancer cells, is an important issue in patient management. In this study, we employed Fourier transform infrared spectroscopy as a potential tool to access such information. We used as model the non-small cell lung cancer cell line, Calu-1. They were exposed to cytostatic doses (0.1 to 100 nM for 24, 48 and 72 h) of gemcitabine, an anti-tumour drug, currently used in treatment of lung cancer patients. In these conditions, inhibition of cell proliferation ranges from weak (< or = 5%), to moderate (approximately 23%), to high (82-95%) without affecting cell viability. Following drug treatment as a function of doses and incubation times, the spectra of cell populations and of individual cells were acquired using a bench-top IR source and a synchrotron infrared microscope. It is demonstrated that spectral cell response to gemcitabine is detectable at sublethal doses and that effects observed on cell populations are similar to those from single cells. Using cluster analysis, spectra could be classified in two main groups: a first group that contains spectra of cells exhibiting a weak or moderate proliferation rate and a second group with spectra from cells presenting a high growth inhibition. These results are promising since they show that effects of subtoxic doses can also be monitored at the single-cell level with the clinical implications that this may have in terms of patient benefit and response to chemotherapy.
