Objective — the aim of the work is to analyze the liver function in patients with first diagnosed tuberculosis (FDTB) of the lungs before the beginning of antimycobacterial therapy (AMBT) using ultrasound (ultrasound) of the hepatobiliary system.Materials and methods. The clinical study involved 47 patients with FDTB of the lungs, who were examined according to national methods and standards before the start of AMBT. Patients are divided into 2 identical groups, depending on their concomitant pathology, viral hepatitis B and C. Patients of both groups did not differ in age, sex, type of tuberculosis process and comorbidity.Results and discussion. The majority of the examined patients with concomitant viral hepatitis showed signs of liver damage, which were manifested by hepatomegaly and splenomegaly, decreased or increased echogenicity of the parenchyma, heterogeneity of its structure and parameters of the portal blood flow, in contrast to patients with VDTB of the lungs without viral hepatitis.Conclusions. It was established that even before the beginning of AMBT, in patients with concomitant viral hepatitis, there was an increase in the level of some biochemical parameters and changes in the liver parenchyma were observed in ultrasound, which requires special attention when using antimycobacterial drugs (AMBD)., Цель работы — проанализировать состояние функции печени у больных с впервые диаг­нос­тированным туберкулезом (ВДТБ) легких до начала антимикобактериальной терапии (АМБТ) с помощью ультразвукового исследования (УЗИ) системы печени.Материалы и методы. В клиническом исследовании приняли участие 47 больных с ВДТБ легких, которых обследовали согласно национальным методам и стандартам до начала АМБТ. Пациенты разделены на две группы в зависимости от сопутствующей патологии, а именно: вирусных гепатитов В и С. Больные обеих групп не отличались по возрасту, полу, типу туберкулезного процесса и коморбидности.Результаты и обсуждение. У большинства пациентов с сопутствующими вирусными гепатитами выявлены признаки поражения печени, которые проявлялись гепатомегалией и спленомегалией, снижением или повышением эхогенности паренхимы, неоднородностью ее структуры и параметров портального кровотока, в отличие от пациентов с ВДТБ легких без вирусных гепатитов.Выводы. Еще до начала антимикобактериальной терапии у пациентов с сопутствующими вирусными гепатитами отмечались повышение некоторых биохимических показателей и изменения в паренхиме печени при УЗИ. Это требует особого внимания при применении антимикобактериальних препаратов., Мета роботи — проаналізувати стан функції печінки у хворих на вперше діагностованим туберкульозом (ВДТБ) легень до початку антимікобактеріальної терапії (АМБТ) за допомогою ультразвукового дослідження (УЗД) гепатобіліарної системи печінки.Матеріали та методи. У клінічному дослідженні взяли участь 47 дорослих хворих із ВДТБ легень, яких обстежували за національними методами і стандартами до початку АМБТ. Пацієнтів розподілено на дві групи залежно від супутньої патології, а саме: вірусних гепатитів В і/або С. Хворі обох груп не відрізнялися за віком, статтю, типом туберкульозного процесу та коморбідністю.Результати та обговорення. У більшості пацієнтів із супутніми вірусними гепатитами виявлено ознаки ураження печінки, які проявлялися гепатомегалією і спленомегалією, зниженням або підвищенням ехогенності паренхіми печінки, неоднорідністю її структури та параметрів портального кро­воплину, на відміну від пацієнтів із ВДТБ легень без вірусних гепатитів.Висновки. Ще до початку антимікобактеріальної терапії у пацієнтів із супутніми вірусними гепатитами були збільшені деякі біохімічні показники та спостерігалися зміни в паренхімі печінки при УЗД. Це потребує особливої уваги під час застосування антимікобактеріальних препаратів.

Background Adequate identification and testing of people at risk for HIV is fundamental for the HIV care continuum. A key strategy to improve timely testing is HIV indicator condition (IC) guided testing. Aim To evaluate the uptake of HIV testing recommendations in HIV IC-specific guidelines in European countries. Methods Between 2019 and 2021, European HIV experts reviewed guideline databases to identify all national guidelines of 62 HIV ICs. The proportion of HIV IC guidelines recommending HIV testing was reported, stratified by subgroup (HIV IC, country, eastern/western Europe, achievement of 90–90–90 goals and medical specialty). Results Of 30 invited European countries, 15 participated. A total of 791 HIV IC guidelines were identified: median 47 (IQR: 38–68) per country. Association with HIV was reported in 69% (545/791) of the guidelines, and 46% (366/791) recommended HIV testing, while 42% (101/242) of the AIDS-defining conditions recommended HIV testing. HIV testing recommendations were observed more frequently in guidelines in eastern (53%) than western (42%) European countries and in countries yet to achieve the 90–90–90 goals (52%) compared to those that had (38%). The medical specialties internal medicine, neurology/neurosurgery, ophthalmology, pulmonology and gynaecology/obstetrics had an HIV testing recommendation uptake below the 46% average. None of the 62 HIV ICs, countries or medical specialties had 100% accurate testing recommendation coverage in all their available HIV IC guidelines. Conclusion Fewer than half the HIV IC guidelines recommended HIV testing. This signals an insufficient adoption of this recommendation in non-HIV specialty guidelines across Europe.
