4. Comparison of Soil Arthropod Diversity and Community Structure in Various Types of Land Cover in Malang Region, East Java, Indonesia

Land cover heterogeneity can affect the structure of biodiversity in the supplied niche, so it is necessary to know the taxa community. This study aimed to understand the effect of land cover variation on the diversity and community structure of soil arthropods. The types of habitats used include urban areas, agroforestry, gardens, and natural forests which are determined in the Malang Region, East Java, Indonesia. Hand sorting and hay bait traps were applied in this study to obtain a variety of arthropod soils and the Berlese-Tullgren funnel was used to extract them. As a result, there are 25 families from 15 orders collected based on their ecological roles. The abundance of Philoscidae in sites S1 and S2 (urban green space), Talitridae in site S6 (agroforestry), and Isotomidae in sites S3, S4, and S5 (highland mixed forest) was highest and dominant. Site S7 has the highest diversity (H' = 2.56; 1-D = 0.90; J' = 0.76) even though its family richness is lower. The site S3 counter-site had relatively high taxa richness (TR = 13), but low diversity (H' = 1.02; 1-D = 0.39; J' = 0.16). Based on clustering analysis and NMDS ordination, 3 classifications of habitat types were obtained, namely I (S6 & S7), II (S3, S4 & S5), and III (S1 & S2). Mixed forest habitats contain a more complex diversity of soil arthropods, which can serve as a model for improving the fertility of disturbed ecosystems.

Background and Aim: The flavonoids from mistletoe are thought to have antimicrobial action. This encouraging finding supports the benefits of medicinal plants as a substitute for synthetic antimicrobials, thus promoting healthy lifestyles. In contrast, it is known that the use of topical drug formulations made from flavonoids of mistletoe (Dendrophthoe pentandra (L.) Miq. Loranthaceae) with Indonesian name, Benalu duku (BD) is required in skin cell irritation. This study aimed to assess the toxic effects of the flavonoid substances of BD, as an initial screening. Materials and Methods: A myeloma cell line was cultured in Roswell Park Memorial Institute medium, and the Baby Hamster Kidney clone 12 (BHK21) cell line was cultured in Dulbecco's Modified Eagle's Medium from stock (±9 × 107 cells/mL), and 1.2 mL of culture were distributed into each well of a microtiter plate. Subsequently, 0.2 mL of serially diluted flavonoid compounds (0.5–3 μg/mL) were added to 12 wells for each concentration, as trial groups (including control groups), followed by a 2-day incubation. Observations were performed based on the cytopathic effect (CPE) using an inverted microscope at a magnification of 100×. Results: Cytopathic effect was detected on the microtiter plate wells for the groups of myeloma and BHK21 cells at a flavonoid concentration of 0.5 μg/mL–3 μg/mL. 7. Characterization of Lignin Peroxidase from the Suspected Novel Strain Phanerochaete chrysosporium ITB Isolate

This study was aimed to characterize lignin peroxidase (LiP) obtained from Phanerochaete chrysosporium ITB isolate. The characterizations included molecular weight, the pH and optimum working temperature of the crude extract of the enzyme, the temperature stability, the effect of metal ions and inhibitors, their precipitation with ethanol, and the storage stability. The LiP of P. chrysosporium ITB isolates was 34 kDa. The crude extract of LiP displays high activity at pH between 3 until 5 and 26–32 °C, has good thermal stability at 26–32 °C for 20 h. The activity is affected by Pb2+, K+, Co2+, Fe2+, Cd2+, Mg2+, and Cu2+ EDTA, Na+, Cr3+, Hg2+, NaN3, Ni2+, and Ca2+ ions, is not affected by Mn2+ and Zn2+ ions, precipitated with the optimum ethanol at 64% ethanol saturation which results in an increase in specific activity of 2.3 times. The crude extract storage at 0 °C is more stable than the precipitate resulting from ethanol precipitation and resuspension from ethanol precipitation. These results strengthen that LiP from P. chrysosporium is another LiP isoenzyme that can be used for bioremediation processes. Unfortunately, the concentration using the ethanol precipitation method has not been effective, so further studies using other methods should be required.

8. Biological control of citrus canker pathogen Xanthomonas citri subsp. citri using Rangpur lime endophytic bacteria

Abstract Background Rangpur lime (RL) is a common rootstock cultivar used worldwide. However, it is known to have a high susceptibility to citrus canker (CC). To meet the increasing demand for healthy citrus seedlings, this research aimed to study the potency of RL endophytic bacteria as a biocontrol agent of the CC pathogen Xanthomonas citri subsp. citri. The isolates were collected from healthy RL leaves and subjected to in vitro and in planta antagonistic tests against XCC, alongside its cell-free supernatant (CFS). Potential isolates were identified according to their 16S rDNA sequence similarities. Results As many as 21 isolates were obtained from the leaves of healthy RL trees. Two (B1 and C8) isolates demonstrated promising inhibitory activity against XCC. Based on the in vitro assays, the cell suspensions (CS) of these isolates could effectively inhibit the growth of XCC, with an optimum clear zone diameter of 8.41 and 7.51 mm, respectively. Consistent with CS, their CFSs also displayed similar antagonistic potencies against XCC with the highest clear zone recorded being 7.23 mm and 6.22 mm, respectively. Further investigations revealed that the minimum inhibitory concentration of B1 and C8 CFSs was 25 µg/ml. It was also found that both CFSs were sensitive to high heat and very low pH, but stable in the presence of proteinase-K. In the in vivo assay, both CS and CFS treatments of B1 and C8 could effectively protect the Rangpur lime from CC. Treated plants had significantly lower disease incidences and developed less severe symptoms than the control plants. Based on their 16S rDNA sequence, B1 and C8 were identified as Staphylococcus pasteuri and Staphylococcus warneri, respectively. Conclusion It was concluded that Staphylococcus pasteuri and Staphylococcus warneri could be promising biocontrol agents of XCC. These bacterial isolates can be used as an alternative prevention measure to CC as opposed to commonly used chemicals.

Background: The sodium may aggravate synovial inflammation and cartilage thinning. This incidence can cause joint pain and reduce functional activity. Not many people know the effect of sodium on the incidence of OA. Objective: This study aims to determine the relationship between sodium in the body and knee joint pain which results in functional activity. Methods: The quantitative descriptive study used accidental sampling. The study was conducted at three outpatient polyclinic orthopedics of hospitals and was approved by the Health Ethics Committee. All data were collected during the interview. The Semi-Quantitative Food Frequency Questionnaire (SQ-FFQ) and the Nutrisurvey Indonesia 2007 application were used as a tool to collect daily sodium intake (mg). Knee joint pain score was measured using the Visual Analog Scale (VAS) while functional body activity was measured using the Western Ontario McMaster Osteoarthritis Index (WOMAC). The Pearson and Spearman test (p0.05) were used as a correlation test. Results: 80 subjects were according to the inclusion criteria. Characteristics of the subjects were pre-elderly (32, 40%), women (74, 92.5%), BMI≥ 30 kg/m2 (54, 67.5%) and occupation (43, 53.75%). Average sodium intake = 2090.78 ± 1084.33 mg, VAS score = 6.28 ± 1.95 and WOMAC score = 32.65 ± 14.88. The correlation sodium, VAS, and WOMAC were not significant (p=0.196, p=0.372). Conclusions: Increased sodium intake is not associated with knee joint pain and functional body activity.

Clinical use of cefadroxil, particularly in Bangka Tengah Hospital, is proven beneficial to overcome mild to moderate infections which especially occur in soft tissues such as skin, upper respiratory tract, pharyngitis, tonsillitis and urinary tract. For this reason, it is necessary to procure cefadroxil to be available enough for the treatment of cases of these diseases. The cefadroxil used by the Central Bangka Hospital was obtained from several pharmaceutical industries with different prices and distributions, due to the possibility that the active raw materials and ingredients had different origins, so there was concern that the microbiological quality would be different. Drug procurement is carried out using the e-catalog or non-e catalog method. This study aimed to examine the microbiological quality of six preparations (A, B, C, D, E, and F) in terms of their inhibitory activities against Gram-positive and Gram-negative bacteria. The bioassay was carried out by diffusion agar method using Escherichia coli ATCC 29522 and Staphylococcus aureus ATCC 29523 as the bacterial test, and nutrient agar as the test medium. The inhibitory activities were compared to cefadroxil standard for measuring the ratio potency. The results showed that all samples fulfilled USP 41 requirements with potential ratio of 90% to 120% and minimum inhibitory concentration of ≤ 8 ppm and ≤ 2 ppm against Escherichia coli and Staphylococcus aureus respectively. The potency ratios to cefadroxil standard were 95.9%, 99.1%, 100.0%, 96.7%, 96.2% and 98.2% against Staphylococcus aureus while the potency ratios of 95.6%, 99.3%, 103.8%, 97.1%, 95.7% and 100.4% were achieved against Escherichia coli for A, B, C, D, E, and F samples, respectively.

Penelitian ini bertujuan untuk mengetahui kandungan hara makro yaitu N-total, P2O5 dan K2O pada ekstrak keong mas yang difermentasi dengan menggunakan EM4 (Effective Microorganism 4). Penelitian ini dilakukan dengan cara menambahkan EM4 aktif yang terdiri dari 6 level konsentrasi (0%; 5%; 10%; 15%; 20%; 25%) dan air gula ke dalam reaktor yang berisi limbah keong mas. Proses fermentasi dilakukakan selama 14 hari. Selanjutnya dilakukan pengambilan sampel pada hari ke 14 untuk dianalisis kandungan hara makro yang terdiri dari P2O5 dan K2O dengan metode SNI 7763:2018 dan N-total dengan metode Kjeldhal (IKA-B.005). Hasil penelitian menunjukkan bahwa kadar N-Total dan Kalium berbeda sangat nyata, dimana terjadi penurunan kadar N total dari ekstrak keong mas pada beberapa konsentrasi EM4 aktif selama proses fermentasi, kandungan nitrogen paling tinggi terdeteksi pada fermentasi tanpa EM4 (0%), yaitu sebesar 1,766 % (v/v). Kadar Nitrogen paling kecil pada fermentasi menggunakan 25% EM4 aktif yaitu 1,474%. 15. Effect of Atorvastatin in Lipid Profile Changes and Inflammation Marker TNF-alpha on Diabetes Patient with Dyslipidemia

Diabetes is one of the risk factors for cardiovascular disease (CVD). Diabetics patients have 2 to 4 times increased risk of cardiovascular disease compared with non-diabetics. TNF-alpha is a proinflammatory cytokine that can be used to determine the risk of atherosclerosis complications triggered by inflammation in diabetes. Statins are a class of HMG CoA reductase inhibitors that inhibit cholesterol biosynthesis and have pleioropic effects that inhibit the release of inflammatory cytokines like TNF-alpha and stabilize atherosclerotic plaques. This study aims to determine the effect of atorvastatin 20 mg/day for 30 days in reducing the lipid profile and TNF-alpha inflammatory markers in patients with diabetes dyslipidemia. Diabetes patient with dyslipidemia who included the inclusion criteria in this observational prospective cohorts studies treated with atorvastatin for 30 days (n = 19). The efficacy of statin therapy was measured by lipid profiles (LDL, TG, HDL, and total cholesterol) and TNF-alpha. The results of the study showed that atorvastatin decreased 40.55% of LDL levels, 15.34% of TG levels, and 30.70% oftotal cholesterol levels which statistically significant (P 0.05). TNF-alpha levels increased by 11.30% which statistically non-significant (P >0.05). The use of atorvastatin 20 mg for 30 days gave reduction in LDL, TG, and total cholesterol and increased in HDL. Atorvastatin does not have a reducing effect on TNF-alpha. There was no correlation between lipid profile changes with TNF-alpha changes.

16. Changes on Serum Troponin T Level before and after Taking Standard Therapy Medication in Heart Failure Patients

Patients with advanced heart failure (NYHA FC III and IV heart failure) had positive cardiac troponin levels in previous cohort studies. In heart failure, cardiac troponin T (cTnT) is a biomarker that is sensitive to myocardial damage, especially myocardial necrosis. However, there is still little information regarding changes in cTnT levels during standard therapy. This prospective observational study is aimed at evaluating changes in cTnT levels before and after theadministration of standard therapy and evaluating symptom improvement before and after the administration of standard therapy in patients with severe heart failure. Measurement of cTnT levels and symptom improvement parameters before treatment was carried out on the first day of the inpatient and measurement after therapy was carried out on the last day of the inpatient. Sampling was done by consecutive sampling and found 30 patients in the inpatient room of the SMF Cardiovascular Disease, Dr. Soetomo Hospital, Surabaya during the months of May-July 2017. The results of the study obtained the average cTnT levels before therapy 33.48 + 31.88 pg/ml and the average cTnT levels after therapy 46.32 + 52.68 pg/ml. Based on the statistical difference test with the Wilcoxon sign-ranked test, there was no significant change in cTnT levels (p = 0.318). 17. Monitoring Serum Creatinine, Blood Urea Nitrogen in Patients Brain Injury with Mannitol Therapy

Increased intracranial pressure is a further effect of brain injury due to structural damage and osmotic and water imbalances (Edema). Mannitol works in the proximal tubules and mannitol in the absorption of tubular cells by the mechanism of pinocytosis. The fluid transfer will draw fluid into the intracellular, so that the cell will be switched and broke. This phenomenon is referred to as the phenomenon of "Nephrosis Osmotic", in which mannitol administration may occur as a result of accumulation of drugs in the kidneys due to prolonged exposure to mannitol in the kidney and given dosage. The effects of osmotic diuresis occurring plus the dose and duration of mannitol administration are reported to cause renal function disorders (Scr and BUN). The aim of this study was to analyze changes in serum creatinine and BUN in patients with brain injury from before receiving mannitol therapy and during mannitol treatment. From the results of the study the number of patients who met the inclusion criteria, 32 patients. Serum creatinine, the initial average of 0.85 ± 0.17 mg / dl and the last day of the mean SCr 0.74 ± 0.30 mg / dl. While the mean BUN (Blood Urea Nitrogen) was 11.27 ± 2.75 mg / dl and the mean last day was 17.08 mg / dl ± 8.59 mg / dl. From Serum Creatinine and BUN data it can be concluded that there is no significant change.

18. The Impacts of Traditional Fermentation Method on the Chemical Characteristics of Arabica Coffee Beans from Bondowoso District, East Java

Bondowoso district is the predominant supplier of coffee beans, and also known as Republik Kopi. However, there was still insufficient data about the chemical characteristics of coffee from Bondowoso. This research has main aims to characterize the chemical characteristic of Coffea Arabica L. from Bondowoso, and determine the impact of traditional fermentation on them. Coffee beans were naturally-fermented through soaking in water for less than 12 hours. Unfermented coffee beans were used as a control. Both unfermented and fermented coffee beans were subjected to chemical analysis. Results show that total nitrogen and lipid contents of both fermented and unfermented beans decreased after fermentation into 0.19%±0.03 and 10.03%±0.14, respectively. LCMS analysis of coffee beans revealed that the majority of amino acid contents in fermented beans were higher than that found in unfermented beans, while caffeine and derivatives to be lower in fermented coffee beans than in unfermented beans. One exception for this was 7-methylxanthine, which was found only in fermented beans. Other metabolites, including procyanidines A and B, were found to decrease through fermentation. Interestingly, 3-flavanol was found only in fermented coffee beans. This research results would benefit on improving the quality of coffee through fermentation step.

Porang (Amorphophallus muelleri Blume) has high economic value in Asia. Porang tuber is high in glucomannan and thus becomes one of the exportable food commodities. Porang plants are susceptible to wilt disease caused by Fusarium sp. Trichoderma sp. has the ability to inhibit the growth of pathogenic fungi such as Fusarium sp. This research aimed to study the potency of Trichoderma sp. in controlling the growth of Fusarium sp. Trichoderma sp. was isolated from the rhizosphere soil in A. muelleri plantation, while Fusarium sp. was isolated from A. muelleri tuber infected by Fusarium. The fungi isolates were identified phylogenetically based on the similarity of Internal Transcribed Spacers (ITS) sequence. Both fungi were antagonistically assessed based on the dual culture method. The antagonistic assay showed that the two isolates of Trichoderma had the potency to inhibit the growth of the two isolates of Fusarium. Trichoderma sp. 2 has higher antagonistic potency than Trichoderma sp. 1. Based on ITS sequence similarity, Trichoderma sp. 1 and Trichoderma sp. 2 were identified as Trichoderma longipale and Trichoderma spirale respectively, while both pathogenic Fusarium were identified as Fusarium oxysporum. Trichoderma spirale therefore could be developed as a biopesticide agent in controlling Fusarium oxysporum.

Twenty-four intestinal amoebiasis patients have been treated with a new amoebicid tiberal (Roche) or metronidazole as a control in a dosage of 15- 30 mg/kg body weight per day for 5 consecutive days. The results show that both drugs achieve a 100% cure rate. No side effects or signs of drug toxicity as evaluated from the results of the safety tests were observed. No signs of clinical or parasitological relapses were seen (after discharge. In comparison with the previous trial with a lower dosage (7~ - 15 mg.) and longer course (7 days) the results were even better, e.g. clinical improvement and parasitological disappearance were achieved in a shorter time.

Pendahuluan: Fondaparinux 2,5 mg sebagai terapi antikoagulan pada pasien Sindroma Koroner Akut (SKA) di RSUD Dr. Soetomo Surabaya diberikan selama 5 hari, sedangkan pustaka menyebutkan bahwa fondaparinux diberikan selama 3 - 7 hari. Kadar steady-state dapat dicapai setelah 3 - 4 hari dan efek antikoagulan dari fondaparinux dapat bertahan selama 2 - 4 hari setelah terapi dihentikan pada pasien dengan fungsi ginjal normal. Diperlukan penelitian untuk mengetahui pengaruh lama pemberian fondaparinux 2,5 mg subcutan sekali sehari pada hari ke-3, 4, dan 5 terhadap Activated Partial Thromboplastin Time (APTT). Tujuan: Penelitian ini bertujuan untuk mengetahui apakah ada perbedaan pemberian fondaparinux terhadap nilai APTT pada penderita SKA. Metode: Sampel penelitian ini adalah seluruh pasien yang didiagnosis SKA yang diberikan terapi fondaparinux di RSUD Dr. Soetomo Surabaya. Jenis penelitian ini adalah observasional dengan rancang bangun prospektif kohort. Teknik pengambilan sampel dengan purposive sampling. Instrumen yang digunakan adalah rekam medis pasien dan hasil rekam pengukuran nilai APTT. Data demografi pasien dan nilai APTT dianalisis secara deskriptif dan One Way Anova digunakan untuk mengetahui perbedaan antara masing-masing kelompok terhadap lama pemberian fondaparinux terhadap APTT. Hasil: Penggunaan fondaparinux meningkatkan nilai rata-rata APTT dan mencapai target (25 - 40 detik) pada 9 orang pasien selama 3 hari (39,39 ± 8,02 detik), 4 hari (39,98 ± 12,15 detik), dan 5 hari (40,06 ± 6,21 detik). Tidak ada perbedaan bermakna antara APTT dengan lama pemberian fondaparinux 2,5 mg subcutan sekali sehari selama 3, 4, dan 5 hari (p > 0,05). Kesimpulan: Lama pemberian fondaparinux pada hari ke-3, 4, dan 5 tidak berbeda bermakna dalam meningkatkan nilai APTT.

23. Isolation and Identification of Hydrocarbon-Degrading Bacteria that Tolerant to Saponin of Sapindus Rarak Plant

A commercial saponin as biosurfactant can reduce the surface tension of water and increase of hydrocarbon degradation. However, this saponin can be toxic to some hydrocarbonoclastic bac-teria. This study aimed to obtain bacterial isolates that were tolerant and incapable to degrade saponin, and to identify them based on 16S rDNA sequence. Bacteria were isolated from petroleum contaminated soil in Wonocolo Village, Bojonegoro Regency, East Java, Indonesia. The soil samples were acclimated using Bushnell-Haas (BH) broth with 0.5% crude oil at room temperature for 3 weeks. The culture was spread onto BH agar incubated at 30°C for 7 days. The first screened, isolates were grown in nutrient broth with addition of sap-onin 0%, 8%, and 12% (v/v) then incubated at 30°C for three days. The bacterial cell density was measured using a spectrophotometer. Second screened, the isolates were grown on BH broth with addition of 0.5% saponin as a sole carbon source, and their cell densities were measured. The selected isolates were identified based on 16S rDNA sequences. Among 34 bacterial isolates, nine isolates were tol-erant to 12% saponin. Three bacterial isolates IHT1.3, IHT1.5, and IHT3.24 tolerant to high concentration of saponin and did not use this substance as growth nutrition. The IHT1.3, IHT1.5, and IHT3.24 isolates were identified as Ochrobactrum pseudogrignonense (99% similarity), Pseudomonas mendocina (99%), and Ochrobactrum pi-tuitosum; (97%), respectively. Those three selected isolates are good candidates as hydrocarbon-degrading bacteria to bioremediation of soil contaminated crude oil. However, the combined activity of bacteria and saponin to degrade hydrocarbon needs further study.

Background: Meningitis defined as an inflammation of the meninges, a membrane that surrounds the brain. The inflammation is a result from bacterial infection. Central nervous system infection is a medical emergency because of the progressivity and potentially life-threatening. Antibiotics usage become crucial to reduce morbidity and mortality. Objective: This study was aimed to assess the use of antibiotics, prescribed for patients with bacterial meningitis in Neurology Department of Dr. Soetomo General Hospital Surabaya from January 2010 until June 2015. Methods: Retrospective study using medical record of inpatients obtained from January 1st 2010 until June 30th 2015. The inclusion criteria were inpatients with bacterial meningitis (diagnosed by physician using CT scan data, lumbal puncture and clinical signs and symptoms) and prescribed with antibiotics. Patients with antibiotics therapy less than 3 days and infection other than bacterials were excluded. Results: From 85 patients, male to female ratio was 1.66. Meningitis appear to be mostly occured in productive age, 39 (45.88%) in 15 - 29 years old and 40 (47.06%) in 30 - 60 years old. Number of antibiotics prescribed were 130 prescriptions, as single and combination therapy. The most prescribed antibiotic was cephalosporins (97 prescriptions), dominated by 2 g twice daily ceftriaxone with 86 (66.15%) from overall prescriptions. Followed by metronidazole and fluoroquinolons,11 (8.46%) and 8 (6.15%) respectively. In addition, there were 26 prescriptions of combination therapy, mostly ceftriaxone and metronidazole. Conclusion: Bacterial meningitis are mostly treated using ceftriaxone which is an appropriate drug of choice.

Uji daya hasil pada beberapa galur jagung mays L) dilakukan di salah satu perusahaan perbenihan yang bergerak bidang produksi benih jagung hibrida perusahaan tersebut adalah PT. Syngenta Seed Indonesia dimana kegiatan ini merupakan akhir dari penelitian. Uji Daya Hasil ini hasilnya akan menjadi acuan perusahaan untuk galur yang pantas untuk dilepas menjadi varietas baru. Rancangan percobaan yang digunakan dalam penelitian ini yaitu Rancangan Acak Kelompok Non-faktorial (RAK) yang diulang sebanyak 6 kali pada setiap galurnya dan apabila pada parameter mempunyi notasi sangat berbeda nyata maka akan dilanjut ke uji selanjutnya yaitu uji DMRT (Duncan Multipe Range Test). Kegiatan uji daya hasil dilakukan pada galur yang terpilih yaitu Syn 212 , Syn 22, Syn 306 dan Syn 7328 yang diamati adalah tinggi tanaman, tinggi tongkol, panjang tongkol, berat segar panen tanpa kelobot, diameter tongkol, berat 1000 butir, berat kering pipil per plot, berat basah pipil pr plot, dan Produktivitas. Hasil penelitian menunjukan bahwa pada galur Syn 22 mempunyai keunggulan dari yang lain karena mempunyai nilai yang besar dibandingkan galur lain. Pada parameter galur nilainya Syn 22 dibanding Syn 7328 panjang tongkol sebesar 17.15 : 14.96 cm, berat segar panen tanpa kelobot 62.94 : 49.01 kg/plot, berat kering pipil 30.8 : 25.1 kg/plot, berat segar pipil 37: 32.3 kg/plot, Produktivitas 19.51: 15.19 ton/ha dan berat 1000 butir 247:247 gram. Sehingga dapat ditarik kesimpulan bahwa galur pra komersial yang dapat dilepas terdapat pada galur V2 yang mempunyai keunggulan dari galur yang lain.

26. Scoping Review: Effects of Probiotics against The Immune System in Burn patients

Background: One of the issues in burn patients is decreasing the body immune system and making it difficult to treat. Probiotics, which are commonly used to treat GI tract imbalances, are also known to be able to modulate the immune system. Objectives: This scoping review aims to explore literature about the effects of probiotics on the immune system in burn patients and to identify gaps in the existing literature. Methods: A systematic search was conducted in six electronic databases (PubMed, ScienceDirect, Scopus, Cochrane, EBSCO/CINAHL, DOAJ and other databases) to identify relevant peer-reviewed studies, with time limits from June 2005 until November 2020, using search terms with database-appropriate keywords. Articles were screened and assessed for eligibility. Results: We identified 901 articles. Of these, 10 articles met the inclusion criteria. In this Scoping Review, the proportion of probiotic combination types mostly used multi-strain probiotic combinations. The frequency and types of probiotic strains most widely used was Lactobacillus spp (58%). The highest concentration of oral probiotics route used was in the total probiotic cell content of 109 CFU (42%) and the duration of probiotic administration was 14 days (50%). Meanwhile, improvement of the immune system in burns has been shown by the laboratory outcome parameters (increased the secretion of IgA, decreased of CRP serum, IL-6, leukocytes, and neutrophils), and also the clinical outcome parameters (improvement of GI imbalance, decreased the mortality, decreased the risk of SIRS/sepsis, and shortened Length of Hospital Stay). Conclusions: To perform the modulation of the immune system in burns, the optimal dose, strain, and duration of probiotic administration has not been established or still varies widely. Therefore, more clinical studies are needed using placebos or controls to get better validity regarding the evidence of effectiveness and safety at various degrees of burns.

Osteoarthritis is a chronic, degenerative disease with unknown etiology. Sodium can induce the secretion of IL-17 which causes cartilage destruction, decreases the secretion of chondrocytes and proteoglycans and induces the secretion of inflammatory mediators. No one knows that sodium is a risk factor for osteoarthritis. The aim of this study was to determine the effect of sodium intake and biopsychosocial factors on knee joint pain in Osteoarthritis patients. This study was an observational study conducted at the Orthopedic Outpatient Poly with a quantitative descriptive method with cross sectional sampling. Data were taken through interviews using the Semi Quantitative Food Frequency Questionnaire and Visual Analog Score. The data obtained were analyzed using Partial Least Square (p≤0.05). This research has obtained permission No. B/404/V/2021 and ethical approval No. 44/LE/2021. There were 52 subject in the inclusion criteria. The analysis of biopsychosocial factors (age, gender, body mass index, education, occupation) that affect knee joint pain was body mass index (p < 0.044). Sodium levels also affect gender (p< 0.046). The results of the correlation test between sodium levels and knee joint pain were 0.024. The higher the level of sodium intake caused an increase in knee joint pain in osteoarthritis patients. The study shows that there is a correlation between sodium intake, knee joint pain, gender and body mass index. It is necessary to reduce sodium intake in the diet of osteoarthritis patients

Rice bran is the waste product of rice milling which is abundant in Indonesia, it can be used as a raw material for the manufacture of bioethanol by fermentation. Before being fermented, rice bran must be hydrolyzed into glucose by biomass degrading. This study was aimed to isolate indigenous cellulolytic bacteria from rice bran as producer of cellulolytic enzymes and resulted in 22 bacterial isolates that demonstrated cellulolytic activity being identified. Among them, BE 8 and BE 14 isolates showed the highest endoglucanase activity at 2.16 and 1.31 U/mL respectively. Identification of the 16S rDNA showed that BE 8 belongs to Bacillus subtilis and BE 14 in Bacillus cereus.

Penelitian ini bertujuan mendeskripsikan dan mengekspalanasi kinerja Bawaslu Kabupaten Buton Selatan dalam pelaksanaan pemilu tahun 2019. Metode yang digunakan yaitu deskriptif kualitatif. Peneliti berperan sebagai instrumen utama. Sumber data yaitu data primer dan data sekunder, informan dipilih dengan menggunakan teknik purposif sampling. Data dikumpulkan dengan teknik wawancara, observasi dan dokumentasi. Teknis data dianalisis dengan menyiapkan data dan mengorganisir data, mereduksi data dan menyajikan data. Pemeriksaan keabsahan data dilakukan dengan uji kredibilitas, transfermabilitas, dependabilitas dan konformabilitas. Hasil penelitian menunjukan bahwa pertama kinerja Bawaslu dalam penyelenggaraan pemilu tahun 2019 telah dilaksanakan secara baik dengan tetap mengacu pada tugas pokok dan fungsinya, namun terdapat pengawas pemilu pada tingkat desa yang belum memahami tugas pokok dan fungsinya dengan baik. Kedua Bawaslu dalam penyelenggaraan pemilu masih memiliki keterbatasan inovasi. Inovasi Bawaslu dalam pelaksanaan pemilu lebih pada inovasi layanan publik dan itupun masih terbatas. Trik dan strategi untuk mensukseskan pemilu masih sebatas pengawasan dan tindakan yang selama ini pernah dilakukan oleh Bwaslu. Ketiga Bawaslu Buton Selatan telah bekerja sesuai dengan jadwal dan ketentuan waktu pengawasan, pencegahan dan penindakan pelanggaran pemilu, namun waktu yang digunakan terbatas sehingga keputusan yang diambil cenderung dipaksakan. Keputusan diambil terkadang memaksakan keadaan dan kurang memperhatikan kondisi yang berdampak buruk pada keputusan. Keempat Keakuratan kinerja Bawaslu Buton Selatan dalam melakukan pengawasan, pencegahan dan penindakan sudah akurat walaupun masih terdapat kekurangan. Kelima Bawaslu menjalin kerjasama yang baik dengan pihak Kepolisian Kabupaten Buton, Kejaksaaan, Gakkumdu, organisasi kemasyarakat lainnya.

Kopi adalah salah satu komoditas perkebunan yang memiliki nilai ekonomis yang tinggi. Dari berbagai jenis kopi yang ada, terdapat beberapa golongan kopi yang paling sering dibudidayakan di Indoneisa, yaitu : kopi arabika, robusta, dan liberika. Jember adalah salah satu daerah yang terkenal sebagai penghasil kopi, salah satunya adalah di kecamatan Arjasa. Proses pengolahan yang dilakukan berupa, pengolahan basah dan kering. Selama proses pengolahan kopi, dihasilkan limbah berupa kulit kopi yang menimbulkan bau busuk jika didiamkan. Salah satu modifikasi yang dapat digunakan untuk memanfaatkan limbah adalah mengolahnya menjadi produk yang lebih bernilai ekonomis, yaitu tepung roti. Tepung roti yang dihasilkan akan digunakan sebagai bahan substitusi dalam pembuatan mie kering, cookies dan brownis. Terdapat berbagai kendala dalam proses pemasaran dan juga pengenalan produk ini kepada masyarakat luas sehingga tidak banyak yang mengetahui tentang produk tepung roti dari bahan kulit kopi ini. Untuk itu diperlukan berbagai usaha demi mengenalkan produk ini kepada masyarakat luas karena melihat manfaatnya yang kurang dimanfaatkan secara maksimal. Salah satunya adalah branding melalui kemasan dan labelling. Tujuan jangka panjang yang diharapkan dari kegiatan ini adalah menghasilkan produk yang memiliki daya jual yang tinggi dengan kemasan yang informatif.

31. The Satisfaction of Covid-19 patients Self-home Care with Telepharmacy Services

Background: The impact of Covid-19 pandemic makes the hospital innovate using technology services, one of which was telepharmacy. Telepharmacy has the potential to improve Covid-19 outpatient through the use of telecommunications with self-home care patient. In July 2021, 120 patients received telepharmacy services with teleconsultation with pulmonologist, pediatrist, internist, otorhinolaryngologist, neurologist, psychiatrist and general practioners. In August 2021, telepharmacy patients were reduced to 39 patients, and in September 2021, telepharmacy patients were reduced to 11 patients. Objectives: Evaluating outpatients with telepharmacy services by measuring satisfaction patients. Methods: This program integrated with interprofessional doctor, pharmacy, nurse, driver and customer care unit. The satisfaction of patient depends on the multiprofessional services. This research to evaluate the satisfaction Covid-19 outpatients with received telepharmacy services. Desain study were descriptive, non-experimental and study cross-sectional. Data collected 3 months dan the samples were 170 patients with patient surveys to evaluate patient satisfaction. Results: 170 patients received telepharmacy ranged in age from 5-70 years, consisting of 93 women (54.7%) and 77 men (45.3%) with 76,5% satisfaction with telepharmacy services. Patients reported telepharmacy services from pulmonologist by 61.7%, internist 8.2%, otorhinolaryngologist 1.2%, pediatrist 23.5%, neurologist 0.5%, psychiatrist 0.5% and general practitioner from medical check-up by 4.1%. Conclusions: The satisfaction of self-home care telepharmacy patients overall reached 76.5%, and the patient felt dissatisfied were 23.5%. The decrease of patients in August and September 2021 because the telepharmacy services has not been maximized services and needs improvement. The hospital can improve the quality of telepharmacy services to increase patient satisfaction and visit telepharmacy patients.

The golden snail is one of the pests that damage rice plants, this pest has the ability to reproduce quickly, causing the golden snail to become one of the main pests in lowland rice. But the golden snail can be used as a basic material for liquid organic fertilizer (LOF) because it contains protein, Azotobacter, Azospirillum, phosphate solubilizing microbes, Pseudomonas, Staphylococcus, enzymes and the hormone auxin. The effectiveness of the golden snail LOF has been tested on several agricultural crops, one of which is rice. In the Podo Tentrem Farmers Group, these pests are simply thrown away and have not been utilized properly. Whereas the potential of this golden snail can be very high if it is processed into organic fertilizer. Therefore, with this community service activity, it is hoped that it can improve competence, skills and increase the production output of the Podo Tentrem farmer group. This can be measured by the enthusiasm of the participants who were attended by 100% of the invitees, besides that the participants also knew the stages of work and succeeded in making Keong Mas LOF well. In addition, other assessment indicators are using quantitative data on pre-test and post-test scores. Before the counseling was carried out, the pretest value was 50 and after the knowledge training, it increased with an average post-test score of 76. These results can indicate that in theory and practice, on average, participants understand and can make golden snail LOF.

Objectives: As it has been known that sodium may aggravate joint inflammation throuh IL-17A pathway and it can cause joint pain, the purpose of this study was to determine the effect of the Self Education Programs for Osteoarthritis (SEPO) intervention on sodium intake, knee join pain and IL-17A level in patients with knee osteoarthritis. Methods: This multicenter study was conducted in orthopedic and internal medicine outpatient wards in three hospitals. This research has received ethical approval. Patients were recruited based on accidental sampling, who met the inclusion criteria. SEPO intervention was carried out for 30 days, followed by provision of leaflets, posters, and handbook about osteoarthritis, daily WhatsApp education chat and/or video. Patients’ demographic, knee joint pain, and sodium intake data was collected during interview, using the Semi Quantitative Food Frequency Questionnaire (SQ_FFQ), and WOMAC (Western Ontario McMaster Osteoarthritis Index) whereas serum IL-17A was determined by using ELISA. The effect of SEPO was analyzed statistically using Mann Whitney tests (p< 0.05). Results: Total subjects recruited were 80, namely 30 patients in control group and 50 patients in treatment group. Majority of patients were female (74, 92.5%), age 56-65 yo (32, 40%), obese (54, 67.5%) and senior high school (23, 46%). Statistical results showed that following SEPO intervention daily sodium intake was lower (1289.45±457.98 mg) compared to the control group (2143.94±744.75 mg), p= 0.04. WOMAC score of control group (28±12.41) higher than treatment group (23.88±13.61), p=0.02. Serum IL-17A levels were also lower in the treatment group (3.974±1.06 pg/mL) than the control group (5.542±1.99 pg/ml), p=0.01. Discussion: Education as a non-pharmacological therapy is needed to reduce disease progression. Education is provided according to the patient's needs. Educational models can be offline (face to face) or online (digital). SEPO intervention in patients with knee osteoarthritis can reduce sodium intake, knee joint pain, and may impact on reduction of serum IL-17A.

In Indonesia, there is a change in diet due to refined carbohydrates, a type of food that is favoured and consumed by children. If we do not brush our teeth right away, the rest of the food is turned into acid by bacteria and causes dental caries. This study aims to determine the most appropriate time for brushing teeth to neutralize the acidity of Saliva after consuming a snack. Research design Quasi-Experimental One Group Pre and post-test design by involving the entire population of 60 people. Data analysis using the Wilcoxon test to prove the effect of before and after brushing teeth after a snack and Friedman’s test to see the effect on the treatment time group. The results of the study are the degree of acidity (pH) of Saliva pre and post brushing teeth 5 minutes after eating an average of 5.85 decreased by 5.60, 15 minutes after eating an average of 5.78 increased by 6.02, and 30 minutes after eating an average of 5.68 increased by 6.10.

Thiamine, also called vitamin B1, is a water soluble vitamin that is involved in the formation of ATP in cells. The active metabolite of thiamine is a co-enzyme thiamine pyrophosphate (TPP) that plays an active role in carbohydrate metabolism and the formation of amino acid binding conjugates. Directly, thiamine may increase energy production in heart muscle cells through such mechanism, whereas in conditions of heart failure, a decrease in the contractility of heart muscle may be found. Therefore, thiamine supplementation is needed in stage II heart failure patients due to long-term use of furosemide that triggers the thiamine deficiency condition. Thiamine supplementation here is enabled to increase heart cells contractility which may ultimately increase the ejection fraction value of the heart muscle as a parameter of heart work efficiency measurement. The objective of this study was to evaluate the effect of thiamine administration as a therapeutic supplement in stage II of heart failure patients on the ejection fraction rate changes in pre and post thiamine supplement therapy for 28 days. The study was conducted using Cross Sectional Prospective Observational Analysis method between April and August 2016 in male patients with NYHA II heart failure who received furosemide therapy meeting the inclusion criteria at Heart Clinic Jemursari Islamic Hospital, Surabaya. Patients who fulfilled the inclusion and exclusion criteria in this study were 32 patients, 16 control patients and 16 treatment group patients between the ages of 35-75 years. The objective of this study was to analyze the dynamics community structure of Cyanobacteria and trophic status in ponds of intensive shrimp culture (Litopenaeus vannamei) in Situbondo based on Trophic Diatom Index (TDI). The ex post facto research was conducted in situ in the hamlet of Pond Mutiara Mas III Klatakan Situbondo East Java Indonesia. Observation of Cyanobacteria and Diatoms community structure were done every week during four cycles of shrimp farming ponds. Cycle of shrimp farming ponds is the times for enlargement process of the shrimps from seed to mature which ranges from 90-120 days. The dependent variables were the density of Cyanobacteria and Diatoms community, as well as chemical parameters nitrite and orthophosphate. Trophic status was determined from TDI be counted from data of diatom density. All of the data were then classified using cluster and biplot analysis program PAST Ver. 3.11 to discribe the profile of ecosystem quality. The research results showed that there were four taxa of Cyanobacteria during farming cycle which the highest density was found from the genus of Oscillatoria. Based on the value of TDI, we found that the trophic status of water in shrimps pond during the production process was eutrophic until hyper-eutrophic. The water quality was decreases along with the length of incubation time.

b-thalassemia major is a genetic disease and a decline in production due to imperfect of hemoglobin. Clinical manifestations are anemia, treated with regular blood transfusions. Blood transfusions lead to an increase in iron in the body that can lead to organ complications. Iron chelation drug delivery is expected to reduce complications due to an increase in the amount of iron in the body by measuring serum ferritin. The study aims to analyze the use of deferasirox and deferipron in patients with b-thalassemia major children at Dr. Kariadi Hospital of January 1, 2012 until December 31, 2013 as well as the need for blood transfusions. Patients who met the inclusion criteria, ie patients with b-thalassemia major children, retrospectively conducted observations of medical records include basic data and laboratory data. After descriptive analysis was performed to determine the use of two types of iron chelating drugs. In this study, 9 patients included in the inclusion criteria. A total of 5 people using deferasirox and 4 using deferipron at baseline. There are 3 people who turned deferipron be deferasirox. Serum ferritin values at study entry was> 1200 mcg/L, and at the end of the study serum ferritin is> 1200 mcg/L. The mean dose of deferasirox study was 19 ± 4.3 mg, and the dose deferipron mean was 80.8 ± 7.7 mg. Mean hemoglobin levels before transfusion was 6.60 ± 0.89 mg/dL. The average number of blood transfusions given was 336.52 ± 73.85 ml. Type of blood transfusion is used Washed erythrocyte. Splenomegaly occurred in 2 patients. There is no change in renal function, and hepatic meaning. The final conclusion until the reduction target serum ferritin

In the treatment of heart failure is recommended the use of a combination of ACEI (captopril) with diuretics (furosemide), ACEI has the side effect of hyperkalemia 10-38% in hospitalized patients and 10% in outpatients, is increased in patients with renal failure and diabetes mellitus, the use diuretics occur hypokalemia 5-20% in outpatients. Hyperkalemia and hypokalemia both should be avoided in heart failure patients, because it can cause cardiac conduction disorders such as cardiac arrhythmias This study aims to monitor levels of potassium in the use of combination therapy with furosemide captopril in heart failure patients. The study was conducted using the method Prospective Observational Cross Sectional Analysis, conducted in January-May 2015 in heart failure patients male and female NYHA II-III receiving combination therapy with furosemide Captopril met the inclusion criteria were treated in the department CVCU Dr. Wahidin Sudirohusodo Makassar. Potassium levels were measured pre and post administration of the combination therapy. A total of 37 patients met the inclusion and exclusion criteria in this study, 31 male patients (83.8%) and 6 female patients (16.2%) aged between 35-80 years, during this study after administration of the combination therapy, 5 patients (13.5%) had hypokalemia ( 5 mmol/L) and 30 patients (81.1%) of normal potassium levels (3,5 to 5.0 mmol/L). The test results paired t test p = 0.432 (p> α;α = 0.05) showed no significant differences between potassium levels pre and post potassium levels. In conclusion, the use of a combination of captopril and furosemide therapy, the risk of hyperkalemia and hypokalemia not significant but need to be monitored levels of potassium and ECG patient.

Cisplatin is one of platinum cytostatic drug for the medication of solid cancers, one of which is head and neck cancer. Adverse event that resulted during drug treatment was acute or chronic nephrotoxicity. Cisplatin concentration in proximal tubular epithelial cells is about 5 times the serum concentration. Platinum exposure on renal tubular cells bonding covalent complex which stimulate production of inflammatory factors that lead to apoptosis and necrosis cell. Cisplatin nephrotoxicity can be prevented by aggressive hydration or alternate method of administration. The aim of this study was to analyze the effectiveness of NaCl-Mannitol hydration on renal function of head and neck cancer patients receiving cisplatin 100 mg/m2 chemotherapy combination with 5FU or paclitaxel. This was a cohort, prospective, and observational study to analyze renal function of head and neck cancer patients receiving cisplatin 100 mg/m2 chemotherapy combination with 5FU or paclitaxel. Inclusion criteria were BUN 7-18 mg/dl and serum creatinine < 2 mg/dl of any cycle. All patients received infuse NaCl-Mannitol hydration with term that provided in Surgeon Departement of Dr. Soetomo General Hospital. Data obtained were BUN, SCr, and eClCr Cockroft-Gault, each was measured pre- and post-hydration. In cisplatin and 5FU chemotherapy combination value BUN pre-hydration (11,99 + 4,62) mg/dl, value BUN post-hydration (12,14 + 4,74) mg/dl and value serum creatinine pre-hydration (0,97 + 0,34) mg/dl, value serum creatinine post-hydration (1,02 + 0,37) mg/dl. Meanwhile to the combination of cisplatin and paclitaxel chemotherapy, value BUN pre-hydration (10,19 + 2,58) mg/dl, value of BUN post-hydration (10,43 + 2,31) mg/dl and value of serum creatinine post- hydration (0,98 + 0,26) mg/dl. In conclusion, NaCl-Mannitol hydration administration is adequate which is shown by BUN and serum creatinine in pre- and post-hydration data within normal limits.

Heart failure (HF) is the final common stage of many diseases of the heart. NT-proBNP levels are increased in HF and correlate well with ventricular wall stress and severity of HF. Combination therapy with Bisoprolol and ACE-inhibitor decreases NT-proBNP level in patient with HF. The use of Bisoprolol as a combination with ACE-inhibitor is still dominate in outpatient setting at Dr. Soetomo teaching hospital. The objective of this study is to analyze NT-proBNP level changes as an indicator in cardiac function after combination therapy with Bisoprolol and ACE-inhibitor in patient with HF.Methods: This study was prospective, observational and conducted in outpatient setting. Consecutive patients who meet the inclusion criteria of the study were included. Blood samples were taken at pre and 2 months post combination therapy with Bisoprolol and ACE-inhibitor, then NT-proBNP level was measured with IMMULITE®. There were 14 patients enrolled in this study (7 males, 7 females). The result showed that NT-proBNP 2 months post combination therapy with Bisoprolol and ACE-inhibitor is significantly decreased than baseline with mean baseline of NT-proBNP level is 4191.43 ± 4367.277 pg/ml to 2786.79 ± 2485.199 pg/ml (p=0.025). From a total 14 patients, 9 patients had NT-proBNP decreases >20% (20.1% – 56.4%) and 3 patients had NT-proBNP decreases 40% (43.4% and 40.4%). In conclusion, there was a significant decreases in NT-proBNP level after 2 months combination therapy with Bisoprolol and ACE-inhibitor in patient with HF.

Caprine milk CSN1S2 protein contains bioactive peptides that have a specific function in regulating the metabolic homeostasis mechanism between different organs. However, the safe dosage of goat milk consumption has not been established previously. The aim of this study was to evaluate the safe dosage of caprine CSN1S2 protein from fresh local goat milk using acute and repeated doses (for 4 weeks) in oral toxicity studies on three month old normal Wistar rats. The CSN1S2 protein was isolated from fresh goat milk. This casein protein administered in a single oral gavage dose to 5 rat groups at a daily dose of 0, 500, 1000, 2000, 4000 mg/kg BW for acute and 28-day sub-chronic toxicity testing of rats of both sexes. The result showed no evident mortality or abnormal effects. Data analysis of body weight, water and food intake, blood biochemical parameters, clinical observation and organ weight revealed no significant differences between all groups for both sexes. The weights of some organs showed differences between groups, but these values were within the normal weight ranges. The conclusion is that caprine CSN1S2 from fresh milk is safe for food consumption because there was no indication of toxicity, no mortality, and no hematological and clinical chemistry changes.

Inflammation is the underlying cause of several comorbid diseases, including macrovascular complications that cause the highest mortality in diabetes patients. One of the pro-inflammatory cytokines used to assess inflammatory conditions in diabetes and its complications is IL-6. This study intends to analyze the effect of atorvastatin administration on lipid profile and inflammatory markers after 30 days and the correlation between lipid profile and IL-6. An observational prospective cohort study was conducted from November 2017 to January 2018 and approved by the ethical committee of General Hajj Hospital, Surabaya. Nineteen patients who met the inclusion criteria and signed the informed consent enrolled in this study. The measurement of lipid profile and IL-6 level were done twice, before and after 30 days of atorvastatin therapy. After atorvastatin administration, there was a 40.55% decrease in LDL level, a 15.34% decrease in TG level, a 30.70% decrease in total cholesterol level, which was statistically significant (p0.05). In conclusion, atorvastatin administration can improve lipid profile in diabetes patients with dyslipidemia. There was a decrease in IL-6 but not statistically different. From statistical analysis, there is no correlation found between lipid profile and IL-6

Background: Patients with cardiovascular diseases (CVD) are vulnerable to experiencing drug-drug interactions (DDIs). DDIs are a concern among patients receiving multiple drug regimens but they are also an avoidable cause of adverse drug reactions. The study of potential DDIs (pDDIs) would make it easier for the healthcare provider to deliver better patient care and mitigate pDDIs incidence. Objective: This study aimed to assess the frequency, severity level and risk factors associated with pDDIs among medications used to treat hypertensive ambulatory patients in Universitas Airlangga hospital. Methods: A retrospective observational study was carried out from electronic prescriptions received by hypertensive patients in March 2021. Data collection includes demographic data, the profile of antihypertensive drug use, and pDDIs. pDDIs were identified by severity using Lexicomp Drug Interaction Checker (Application). Univariate logistic regression analysis was used to find associated factors of major pDDIs. A p-value less than 0.05 (≤ 0.05) was considered statistically significant. Results: From 704 patients, 53.98% women and 46.02% men, 89.06% (n = 627) patients had minor to major pDDIs; 1354 pDDIs were identified, 89.4% (n = 1,210) were moderate and 9.8% (n = 133) were major class. Multiple antihypertensive drug regimens had significance associated with the major pDDIs occurrence. Conclusion: We found a high prevalence of pDDIs among hypertensive patients. The majority of pDDIs were of moderate severity. Multiple antihypertensive drug regimens were associated factors in the presence of major pDDIs.
