4. Prolonged exercise causes an increase in endothelin-1 production in the heart in rats

Abstract

A study was conducted to test whether the production of endothelin-1 in hearts is changed by prolonged exercise in rats. The left ventricle and lungs were homogenized with a Polytron homogenizer for 60 seconds. Reverse transcription polymerase chain reaction was then carried out to determine the expression of prepro-endothelin-1 mRNA. Results indicated that the production of endothelin-1 was significantly increased in the heart by exercise without alterations in endothelin-converting enzyme and receptor expression.

Abstract

Uterine smooth muscle preparations from pregnant humans were analyzed to determine the alterations in endothelin-1 (ET-1)-induced contraction, ET receptors and receptor subtypes due to pregnancy. Analysis of human uterine membrane preparations from gravid humans indicated the presence of increased ET-1-induced uterine contractions that was attenuated by ETA receptor antagonist. ET-1-induced contractions were also mediated by increased ETA receptors and reduced ETB receptors in pregnant human uterus.

13. Acute effects of intravenous nicardipine on hemodynamics and cardiac function in patients with a healed myocardial infarction and no evidence of congestive heart failure

Abstract

Acute effects of intravenous nicardipine (10 [micro]g/kg) on systemic hemodynamics and cardiac function were evaluated in 17 patients with a healed myocardial infarction and no evidence of congestive heart failure. Mean New York Heart Association functional class was 1.6 [+ or -] 0.5 (mean [+ or -] standard deviation). Aortic systolic pressure (p [is not greater than] 0.001) and left ventricular end-diastolic pressure decreased (10 [+ or -] 3 to 8 [+ or -] 3 mm Hg, p [is not greater than] 0.01), and systemic vascular resistance decreased significantly (p [is not greater than] 0.001), whereas pulmonary and right atrial pressure and pulmonary arteriolar resistance did not change. Cardiac and stroke indexes showed biphasic changes. Although positive and negative maximal rate of left ventricular pressures decreased significantly (p [is not greater than] 0.05 and p [is not greater than] 0.01, respectively), they did not change significantly when aortic systolic pressure was corrected. There was a significant inverse correlation between the negative rate of left ventricular pressure/aortic systolic pressure before nicardipine infusion and its maximal percent increase after infusion (r = -0.56, p [is not greater than] 0.05), indicating a beneficial effect on diastolic relaxation in patients with impaired diastolic function. Our data show that a low dose (10, [micro]g/kg) of intravenous nicardipine experts a favorable effect on impaired diastolic function, but depresses left ventricular pump function with much less effect on right heart circulation. (Am J Cardiol 1991;68;301-305), Nicardipine is one of a group of calcium channel blockers, drugs that prevent entry of calcium into some tissues. The effects of nicardipine are not completely clear. Effects from laboratory studies differ from those found in patients, and those in patients with congestive heart failure may differ from those in other patients. To clarify the physiological effects of nicardipine, 17 patients who had a previous myocardial infarction (heart attack) but had no apparent congestive heart failure were studied. Patients' function was affected mildly or moderately by their heart status. Blood pressure decreased significantly one to 15 minutes after infusion of nicardipine, and pressure in the left side of the heart (where the pumping action for most of the body occurs) decreased significantly within five to 30 minutes after treatment. Other measurements further indicated that nicardipine depresses pump function in the left ventricle (the major pumping chamber). Few effects were seen in the right side of the heart, which pumps blood into the lungs. Abstract

Coronary artery disease (CAD) results when the blood vessels supplying the heart muscle (coronary arteries) become narrowed (stenotic) or occluded, causing a reduction of blood flow to that organ (myocardial ischemia). Mental stress can aggravate myocardial ischemia and cardiac dysrhythmia (irregular heartbeat). Part of this response may be mediated by the peripheral nervous system, but changes in endocrine function may also play a role. A simple method for inducing mild mental stress is to have subjects perform mental arithmetic. To determine the effect of mental stress on neuroendocrine function in healthy subjects, 12 men and 14 women volunteers were subjected to blood sampling before and during the performance of seven moderately difficult subtraction problems; the blood pressure and electrical activity of the heart were concurrently monitored. Both heart rate and blood pressure increased during stress. In addition, the rate-pressure product (derived by multiplying heart rate by blood pressure; an index of myocardial oxygen consumption) was also significantly increased. Levels of norepinephrine and epinephrine, two hormones associated with stress responses, were increased, as were thromboxane B-2 levels. Glucocorticoid and prostaglandin levels were not altered. Norepinephrine levels were correlated with both heart rate and blood pressure increases, while epinephrine levels were correlated only with heart rate increases. These data indicate that there are several different physiological mechanisms that may contribute to the worsening of cardiac dysfunction during stress.
