Abstract

Background: Polycystic ovary syndrome (PCOS) is a heterogeneous metabolic disorder characterized by hyperinsulinemia, abnormal lipid levels, altered hormonal milieu such as elevated androgen levels, and chronic anovulation. PCOS is one of the important reasons for infertility. PCOS is the most common cause of irregular menstruation with diverse clinical manifestations affecting women of reproductive age. Aims and Objectives: The present study aimed to examine systemic inflammatory markers that can be detected in complete blood count, such as neutrophil–lymphocyte ratio (NLR) and platelet–lymphocyte ratio (PLR), in patients with PCOS and to compare them with healthy women. Materials and Methods: This case–control study was conducted in the Department of Obstetrics and Gynecology, Akash Institute of Medical Sciences and Research Centre, Devanahalli, Bengaluru, Karnataka. After following the inclusion and exclusion criteria, a total of 160 subjects were included. Among them, 80 were PCOS patients as cases and 80 were healthy women as controls. A detailed clinical and gynecological history was obtained from all subjects. Women presenting gynecology OPD in the group of 15–45 years with irregular menstrual cycles were included in the study. The diagnosis of PCOS was done according to the Rotterdam criteria, diagnosis of PCOS requires the presence of at least two of the following three symptoms: (1) oligomenorrhea/polymenorrhea; (2) biochemical hyperandrogenism (Ferriman–Gallwey scores of ≥8) or the presence of clinical hyperandrogenism; and (3) detection of polycystic ovaries in an ultrasonographic examination (≥12 follicles, 2–10 mm in size). Under aseptic conditions, venous blood samples were collected from all the subjects to assess the hormones such as follicle-stimulating hormone (FSH), luteinizing hormone (LH), inflammatory markers, white blood cells, neutrophils (N), lymphocytes (L), and platelets. NLR and PLR were calculated. Lipid profiles were obtained in the fasting state. Results: In the present study, BMI 24.5±4.1 kg/m2, total cholesterol 160.1±24.2 mg/dL, triglyceride 99.1±28.1 mg/dL, LDLC 99.1±25.1 mg/dL, LH 8.4±3.8 mIU/mL, LH/FSH ratio 1.2±0.91, lymphocytes 16.30±4.69, NLR 5.73±1.91, and PLR 17.01±7.13 were significantly increased in PCOS cases and HDLC 39.7±6.2 mg/dL and FSH 6.1±1.1 mIU/mL were decreased in PCOS cases compared to healthy controls. Conclusion: The study concludes that increased inflammatory markers such as NLR and PLR in PCOS cases compared to the age-matched individuals in the control group, suggesting the presence of chronic low-grade inflammation in PCOS cases of early reproductive age.

2. Aetiology and Outcomes of Thrombocytopenia in Pregnancy: A Cross-Sectional Study in a University Hospital, India

Abstract

Background: Thrombocytopenia (TCP) is the second most common haematological finding in pregnancy next to anaemia. It carries a risk for both the mother and the fetus, associated with substantial maternal or neonatal morbidity and mortality. However, a specific therapy, if instituted promptly, improves the outcome for affected patients and their offspring. In patients in India, TCP during pregnancy is an underexplored condition. Objectives: To assess the aetiology of TCP in pregnancy and to assess the maternal outcomes of TCP in pregnancy. Methodology: The authors included a total of 133 patients in their third trimester (>32 weeks), with a platelet count less than 149,000 /mm3, admitted to the authors’ institution from 1st January 2021 to 31st December 2021. Patient-related data such as menstrual and obstetric history, presenting complaints, obstetric examination, and basic investigations were collected in a pre-designed, pre-tested proforma. All cases were followed until delivery to record any maternal complications, or any other morbidities. The data were analysed using SPSS (International Business Machines Corporation, Armonk, New York, USA) software. χ2 test was used to compare the proportions between the groups. p less than 0.05 was considered significant. Results: Overall, 64.7 percent of patients were in the 18–25 years age group and 49.6 percent of patients were primigravida. Furthermore, 60.9 percent of patients were diagnosed to have mild TCP, 32.3 percent had moderate TCP, and only 6.8 percent patients had severe TCP. The majority (75.2%) of cases were of gestational TCP. In total, 15.8% of cases had pregnancy-induced hypertension (PIH); 3.0 percent had dengue; 2.3 percent were COVID-19 positive; 1.5 percent were diagnosed with haemolysis, elevated liver enzymes, and low platelets syndrome; 1.5 percent had immune TCP; and only one patient had leptospirosis. Four percent of cases had gestational TCP, 9.5% had PIH, one patient (25.0%) had dengue, and both cases of immune TCP had severe TCP. Twenty-eight percent of gestational TCP cases; 47.6 percent of PIH cases, both cases of haemolysis, elevated liver enzymes, and low platelets syndrome; 50.0 percent of dengue cases; and one COVID-19 positive case (33.0%) had moderate TCP. Finally, 6.25% of patients who underwent lower segment caesarean section had severe TCP, 6.00% of patients who underwent vaginal delivery had severe TCP, and out of two patients who had a spontaneous abortion, one (50.00%) had severe TCP at the time of admission. The association was significant (p less than 0.05). Conclusion: TCP is a crucial condition among pregnant patients. Mild TCP is a common type. Correct aetiological diagnosis, and promptly administered adequate and specific therapy are, therefore, essential to significantly improve the outcomes of pregnant patients and their offspring.

Abstract

We describe the epidemiological characteristics and associated risk factors of those presenting at a large testing centre for SARS-CoV-2 infection. This is a retrospective record review of individuals who underwent SARS-CoV-2 testing by reverse transcription–polymerase chain reaction (RT-PCR) at a high-throughput national-level government facility located in the north of India. Samples collected from 6 April to 31 December 2020 are included in this work and represent four highly populous regions. Additionally, there was a prospective follow-up of 1729 cases through telephone interviews from 25 May 2020 to 20 June 2020. Descriptive analysis has been performed for profiling clinic-epidemiological aspects of suspect cases. Multivariable logistic regression analysis was undertaken to determine risk factors that are associated with SARS-CoV-2 test positivity and symptom status. A total of 125 600 participants' details have been included in this report. The mean (s.d.) age of the participants was 33.1 (±15.3) years and 66% were male. Among these tested, 9515 (7.6%) were positive for COVID-19. A large proportion of positive cases were asymptomatic. In symptomatic positive cases, the commonest symptoms were cough and fever. Increasing age (groups 20–59 and ≥60 years compared to age group less than 5 years), male sex, history of international travel, symptoms for SARS-CoV-2, and participants from Delhi and Madhya Pradesh were positively associated with SARS-CoV-2 test positivity. Having co-morbidity, risk behaviours and intra-familial positivity were associated with a positive odds ratio for exhibiting SARS-CoV-2 symptoms. Intensified testing and isolation of cases, identification of both asymptomatic and symptomatic individuals and additional care of those with co-morbidities and risk behaviours will all be collectively important for disease containment in India. Reasons for differentials in testing between men and women remain an important area for in-depth study. The increased deployment of vaccines is likely to impact the trajectory of COVID-19 in the coming time, and therefore our data will serve as a comparative resource as India experiences the second wave of infection in light of newer variants that are likely to accelerate disease spread.

Abstract

The Chevrel phase (CP) (Mo _6 S _8 ), which is used as an electrode material in Mg rechargeable batteries, has a capacity limit owing to ion insertion and trapping. To address this problem, we modify the wire structure of the CP. Mo _6 S _3 I _6 nanowires, in which iodiene is substituted for Mo _6 S _9 nanowires as infinite CP structures, can be synthesized in various ways. When synthesizing stoichiometrically, an unwanted secondary phase may appear. We solved these problems by reducing the synthesis time. Electrochemical analysis was performed using these nanowires as an active material in Mg batteries.

6. Raising Expectations in Literacy for Students with Extensive Support Needs Using Multiliteracies

Multiliteracies provide a unique opportunity to students with extensive support needs (ESN) because they extend literacy practices to include print-based texts along with the visual, audio, gestural, and spatial modes, thereby engaging students in literacy practices and higher-level skills regardless of their fluency in language or learning modality. However, many teachers of students with ESN are not familiar with multiliteracies or how to implement these in their classrooms. This paper provides a framework for implementing multiliteracies in the classroom using the four knowledge processes specified in the pedagogy: experiencing, conceptualizing, analyzing, and applying.

Abstract

Phaeohyphomycosis is a rare mycotic infection caused by various heterogenous groups of phaeoid (dematiaceous) fungi involving the skin and subcutaneous tissue. Common clinical manifestations are subcutaneous abscesses or cystic swellings. Here, we report a case of subcutaneous phaeohyphomycosis presenting as multiple asymptomatic cystic swellings over the hands and feet without any predisposing factors. Histopathology showed granulomatous inflammation and special stain with Grocott's methanamine silver stain revealed broad pigmented hyphae. Culture showed black-colored colonies identified as Exophiala jeanselmi. The patient was treated with surgical excision of the lesions.

Abstract

BackgroundPediatric tuberculous meningitis (TBM) commonly causes death or disability. In adults, high-dose rifampicin may reduce mortality. The role of fluoroquinolones remains unclear. There have been no antimicrobial treatment trials for pediatric TBM.MethodsTBM-KIDS was a phase 2 open-label randomized trial among children with TBM in India and Malawi. Participants received isoniazid and pyrazinamide plus: (i) high-dose rifampicin (30 mg/kg) and ethambutol (R30HZE, arm 1); (ii) high-dose rifampicin and levofloxacin (R30HZL, arm 2); or (iii) standard-dose rifampicin and ethambutol (R15HZE, arm 3) for 8 weeks, followed by 10 months of standard treatment. Functional and neurocognitive outcomes were measured longitudinally using Modified Rankin Scale (MRS) and Mullen Scales of Early Learning (MSEL).ResultsOf 2487 children prescreened, 79 were screened and 37 enrolled. Median age was 72 months; 49%, 43%, and 8% had stage I, II, and III disease, respectively. Grade 3 or higher adverse events occurred in 58%, 55%, and 36% of children in arms 1, 2, and 3, with 1 death (arm 1) and 6 early treatment discontinuations (4 in arm 1, 1 each in arms 2 and 3). 14. Optimization of multimeric human papillomavirus L2 vaccines.

Abstract

We sought to define the protective epitopes within the amino terminus of human papillomavirus (HPV) type 16 minor capsid protein L2. Passive transfer of mice with rabbit antisera to HPV16 L2 peptides 17-36, 32-51 and 65-81 provided significant protection against vaginal HPV16 challenge, whereas antisera to 47-66, 108-120 or 373-392 did not. Vaccination with L1 virus-like particles induces a high titer, but generally type-restricted neutralizing antibody response. Conversely, vaccination with L2 11-88, especially multimers thereof, induces antibodies that neutralize a broad range of papillomavirus types, albeit at lower titers than for L1 VLP. With the intent of enhancing the immunogenicity and the breadth of protection by focusing the immune response to the key protective epitopes, we designed L2 fusion proteins consisting of residues ∼11-88 of eight divergent mucosal HPV types 6, 16, 18, 31, 39, 51, 56, 73 (11-88×8) or residues ∼13-47 of fifteen HPV types (13-47×15). The 11-88×8 was significantly more immunogenic than 13-47×15 in Balb/c mice regardless of the adjuvant used, suggesting the value of including the 65-81 protective epitope in the vaccine. Since the L2 47-66 peptide antiserum failed to elicit significant protection, we generated an 11-88×8 construct deleted for this region in each subunit (11-88×8Δ). Mice were vaccinated with 11-88×8 and 11-88×8Δ to determine if deletion of this non-protective epitope enhanced the neutralizing antibody response. However, 11-88×8Δ was significantly less immunogenic than 11-88×8, and even the addition of a known T helper epitope, PADRE, to the construct (11-88×8ΔPADRE) failed to recover the immunogenicity of 11-88×8 in C57BL/6 mice, suggesting that while L2 47-66 is not a critical protective or T helper epitope, it nevertheless contributes to the immunogenicity of the L2 11-88×8 multimer vaccine.

Abstract

Optimizing downlink coordinated multipoint (CoMP) performance through advanced scheduling algorithms enhances V2X communication technology, enabling efficient resource allocation, minimizing interference, and maximizing data rates for reliable and synchronized communication between vehicles and infrastructure. In this paper, several scheduling algorithms were compared, including support vector machine (SVM) linear, SVM Radial Basis Function (RBF), SVM Sigmoid, Deep Neural Networks (DNN), Convolutional Neural Networks (CNN), and Graph Convolutional Networks (GCN). The performance metrics used in this comparison included CoMP decision, throughput, and cell edge throughput. The results showed that the out-rated GCN algorithm had the best-triggering composition for 5G radio networks, outperforming the other algorithms in terms of CoMP decision accuracy and overall throughput. In particular, the GCN algorithm demonstrated significant improvements in cell edge throughput, which is critical for ensuring reliable communication in areas with weaker signal strength. The reported results proves that the integration of advanced scheduling algorithms in the downlink CoMP framework enhances the efficiency of V2X communication, enabling optimized resource allocation, interference mitigation, and maximized throughput, thereby improving system efficiency, reducing latency, and ensuring reliable and seamless information exchange for connected vehicles, smart cities, and industrial automation. Abstract

To meet the needs of society, the next generation of communication systems will have to learn to juggle multiple responsibilities effectively. This is leading to exponential growth in both circuit complexity and error rates. It is possible to solve these issues by using the intelligent reflecting surface (IRS)-Assisted space shift keying (SSK) modulation technique. IRS is a cutting-edge technology that shows promise for future networks due to its low mistake rates and low-energy consumption. Similarly, SSK modulation is favored due to its preference for simpler hardware. Therefore, integrating IRS with SSK is preferable for minimizing both error rates and circuit complexity. In this article, a new framework is developed to characterize the average bit error rate (ABER) with the help of the moment generating function (MGF) over Nakagami-m fading channels. The ABER is derived by factoring the direct path and IRS reflected path at the receiver and also comparing the performance analysis over the Rayleigh fading channel. Abstract

This paper aims to examine the subject of this research - the effect of online shopping applications on the overconsumption of young people with corresponding buying behaviours, psychological and social factors, the effect of such advertising as targeted advertising and individual recommendations. In this respect, the study adopts a descriptive research approach to gather data from 261 respondents aged 18-30 using a convenient online survey conducted in the region of Kerala. In this case, stratified random sampling ensured that all the participants are sampled in each age category and geographical region. Thus, ANOVA and Friedman tests were used to analyse the variables. From the results, it can be ascertained that the youth are influenced a lot by apps involved in online shopping and perceptions of convenience, trust and satisfaction across educational background are metamorphosed. Related to this, psychological and social factors such as peer pressure concern, stress, and social media advertisements also help in controlling consumption whereas consumers having different employment statuses also show some variation in that regard. Among the analysed variables, consumption related to targeted advertising and suggestion of concrete products have been identified as playing a major role in influencing overall consumption; Further, consumers' satisfaction with the ads has been described as most decisive. Some of these features include the fact that the study involves a particular region/area and particular group while others may disagree. Also, data collected using self-reporting may also have biases. Nevertheless, it offers insights to marketers and app developer to market specific strategies according to the needs and wants of young consumers focusing on the educational and employment setting influencing their online shopping. This study delves into the intricate financial dynamics within agricultural value chains, aiming to uncover specific needs, challenges, interdependencies, and collaborative potentials. As the agricultural sector plays a pivotal role in global economies, understanding these financial nuances is crucial for sustainable development. The objectives of the study are twofold: firstly, to analyse the financial needs and challenges across value chain actors; secondly, to examine interdependencies and cascading effects of financial challenges. This research provides a comprehensive understanding of financial interactions among diverse actors within agricultural value chains. Employing a mixed methods approach, structured surveys and in-depth interviews were conducted with a representative sample of value chain actors. Quantitative data were analysed using appropriate statistical techniques, while qualitative insights were extracted through thematic analysis. This approach ensures a holistic exploration of financial trends, challenges, and interconnectedness. Analysis of financial challenges unveiled the critical roles of access to capital, working capital adequacy, technology investment, timely credit access, and collaborative financial planning in determining actors' financial resilience. Common hurdles encompassed commodity price fluctuations, limited access to suitable financial products, investment difficulties, financial literacy gaps, and inadequate risk management strategies. The findings highlight the interconnected nature of financial challenges, emphasizing the potential ripple effects across the value chain. Collaboration emerges as a key strategy to mitigate these effects. Abstract

There are several obstacles that prevent smallholder farmers in poor nations from realizing their full output potential. In order to help underprivileged communities, have access to value chain financing, researchers are studying Agricultural Value Chain Finance (AVCF). A great deal of research on AVCF has been carried out using different types of agricultural farming. Among these, cultivation of paddy was still limited. However, this topic has yet to receive considerable attention based on paddy cultivation and marketing which was limited to the Palakkad District. Thus, a study was done to analyze the effectiveness of AVCF in Paddy cultivation in the Palakkad district. By using the methodology of random sampling technique, the sample has been collected from 350 respondents in Palakkad. The result showed that the AVCF approach has a leverage chain relationship with more creditworthy chain members to attract direct and indirect finance of individuals and smallholder farmers in value chains. Abstract

Rapid technological advancements have permeated every sector, including education. Teachers in government colleges are facing the necessity to adopt and adapt these changes in their teaching, research, and administrative roles. While these advancements can lead to improved teaching methods and expanded research possibilities, the process of change and the associated demands can also contribute to increased job stress. However, this relationship and its implications on teachers' roles and responsibilities in government colleges remains underresearched. This study aims to explore the impact of rapid technological advancements on the roles and responsibilities of government college teachers and to examine the relationship between these technological changes and job stress levels. A mixed-method approach was adopted, involving a self-administered questionnaire and qualitative interviews with teachers from government colleges in Kerala, India. The findings indicated that technological advancements have had a profound impact on teachers' daily teaching responsibilities and research capabilities. However, these changes also increased their workload which leads to higher stress levels. Many teachers also expressed anxiety about not being able to keep up with the pace of technological changes in their field. The study highlights the dual impact of technological advancements on government college teachers, with benefits being offset by heightened job stress. The findings underscore the need for comprehensive institutional support, including ongoing training, effective stress management programs, and prioritization of resource allocation for technological infrastructure. Abstract

This paper investigates the symbol error rate (SER) and coverage probability of single reconfigurable intelligent surface (S-RIS) and double reconfigurable intelligent surface (D-RIS) systems operating over Weibull fading channels. In the S-RIS system, the RIS is positioned between the base station and user link, while in the D-RIS system, RIS-1 and RIS-2 are placed near the base station and user respectively. By leveraging the central limit theorem, we derive the statistical parameters of the received signal-to-noise ratio (SNR) and subsequently closed-form expressions for SER and coverage probability of both systems using the moment-generating function. Also, we determine the optimal location of the RISs in D-RIS system from the simulations of SER. Comparative analysis reveals that the D-RIS system exhibits superior error performance and provides better coverage than the S-RIS system, despite having an equal number of reflecting elements.
