To ensure that the science, technology, engineering, and mathematics (STEM) workforce of the future is inclusive of diverse peoples and ideas, there is a need to ensure that STEM outreach activities that encourage students into STEM education and careers are inclusive of and provide equity for people from diverse racial/ethnic, socioeconomic, and academic backgrounds. Since 2004, the National Indigenous Science Education Program (NISEP), based at Macquarie University, Sydney, has been using best ethical and inclusion strategies to engage with Australian Aboriginal and Torres Strait Islander school students and their communities. This publication discusses the inclusion and equity strategies utilized by NISEP, including community consultation, student leadership and autonomy, volunteer mentors, and the respectful and best practice use of Indigenous knowledge. These factors have all contributed to NISEP's success in increasing Aboriginal and Torres Strait Islander students' aspiration toward and engagement with secondary and tertiary education.

Alphitonia Reissek ex Endl. is a relatively small genus of the family Rhamnaceae. Plants of this genus are found predominantly in the tropical regions of Southeast Asia, Australia and the Pacific, with some species being widely distributed and others endemic to a region. Almost half of the species of the Alphitonia genus have been reported for their customary (traditional and contemporary) medicinal uses. This includes for the treatment of skin conditions, headache, stomachache, inflammation, and body pain such as joint pain and childbirth.The aim of this review is to provide the first comprehensive account on the customary uses including ethnomedicinal uses, and phytochemistry and biological activities of the Alphitonia genus, and to identify gaps in current knowledge and scope for future research of plants of this genus.Information relevant to the genus Alphitonia was collected by searching the scientific databases (SciFinder, Google Scholar, ACS publications, PubMed, Wiley Online Library and International Plant name Index). Species names were validated using the World Flora Online database (www.worldfloraonline.org).Eight plants from the Alphitonia genus have been reported to be used as customary medicines, i.e. A. excelsa, A. ferruginea, A. franguloides, A. incana, A. neocaledonica, A. petriei, A. philippinensis and A. zizyphoides. A. excelsa, A. petriei, A. philippinensis and A. zizyphoides, have been shown to have biological activities that align with their customary uses, including antimicrobial, antioxidant and anti-inflammatory activities. Only five Alphitonia species reported for their medicinal customary uses have been explored for their phytochemistry, i.e. A. excelsa, A. neocaledonica, A. petriei, A. philippinensis and A. zizyphoides. Compounds identified from these plants include those that are well known for their medicinal importance. A. macrocarpa, A. whitei and A. xerocarpus have also been examined for their phytochemistry and have been found to have the same or similar bioactive compounds to those found in customarily used Alphitonia species. No biological activities or phytochemistry studies have been reported for the known customarily used medicinal plants A. ferruginea, A. franguloides and A. incana.This review highlights the customary uses, biological activities and phytochemistry of plants of the Alphitonia Reissek ex Endl. genus and highlights the significance of the knowledge systems of Indigenous peoples. Of the plants that have been researched for their biological activities and phytochemistry, there is good correlation with these properties and their customary medicinal uses. However, over half of the plants of the Alphitonia genus, including those that are already reported in the public domain for their customary medicinal uses, have had none or limited biological activities or phytochemistry studies conducted. While only eight species of the Alphitonia genus have been reported as customary medicines, other Alphitonia species also possess medicinally important compounds, and it is possible that they are customary medicines but their uses have not been shared publicly by the Indigenous knowledge custodians. There is clearly much scope for further investigation of this genus with regards to their ethnomedicinal uses and therapeutic potential.

Traditional medicinal knowledge possessed by Indigenous people is a significant resource, as exemplified by traditional medicines being the primary healthcare for ∼80% of people in developing countries and their study being a successful drug discovery avenue. The Indigenous Bioresources Research Group of Macquarie University Sydney Australia, work in collaborative partnership with Indigenous people to document and preserve first hand traditional medicinal knowledge; identify flora of significant medicinal potential and evaluate their effectiveness using biological assays; identify the major bioactive components responsible for the medicinal properties of the flora; and provide capacity strengthening opportunities for the Indigenous partners and their communities. The Indigenous Bioresources Research Group has well established partnerships with Aboriginal Elders of northern New South Wales, Australia, and Indian traditional custodians. This paper provides an overview of the importance of traditional medicinal knowledge and a brief review on traditional 'bush medicines' of New South Wales. It additionally presents the methodologies employed and some cultural, scientific and educational goals and achievements of the Indigenous Bioresources Research Group through showcasing of three major partnerships of the group, i.e., with Yaegl Aboriginal people of New South Wales Australia, a Siddha Medical Practitioner of Chennai, Southern India, and Chungtia villagers of Nagaland, North East India. These case studies particularly highlight the immense reward that truly collaborative partnerships between academics and Indigenous communities can bring, including the two-way exchange of knowledge and skills and various capacity strengthening outcomes.

Ethnopharmacological relevance The bark of Erythrina stricta Roxb. (Fabaceae) has been used in Indian indigenous systems as a remedy for rheumatism, stomach-ache, asthma, dysentery, contact dermatitis, eczema and skin infections. However, there have been limited phytochemical or biological studies on the bark of E. stricta and there are no studies that align with its traditional medicinal uses. Aim of the study The aim of this study was to assess the antimicrobial and antioxidant activity of the stem bark of E. stricta to support its topical use in the treatment of contact dermatitis, eczema and skin infections and to isolate and identify any bioactive compounds. Materials and methods MTT microdilution and disc diffusion assays were used to determine the antimicrobial activities of n-hexane, dichloromethane, ethyl acetate, methanol and water extracts of the bark of E. stricta. Column and preparative thin layer chromatography were used for the purification of the dichloromethane extract. The structures of the compounds isolated were elucidated by extensive 1D and 2D NMR spectroscopic techniques and comparison with published data. The antioxidant activities of the extracts were determined by DPPH free radical scavenging and FRAP assays and the antioxidant activity of the pure compounds by dot-blot and DPPH staining methods. Results The dichloromethane, ethyl acetate, and n-hexane extracts showed the most significant activity with MIC values of 7.8 µg/mL, 125 µg/mL, and 125 µg/mL against a sensitive strain of Staphylococcus aureus. The dichloromethane and ethyl acetate extracts also showed significant activity against Candida albicans with MIC values of 125 µg/mL and 1 mg/mL respectively. GC-MS analysis of the n-hexane extract showed the presence of the antibacterial and antifungal compounds β-caryophyllene, caryophyllene oxide, α-selinene, β-selinene, selin-11-en-4-α-ol, α-copaene and δ-cadenine. Phytochemical studies of the dichloromethane extract led to the isolation of the novel compound erynone (1), together with six known compounds; wighteone (2), alpinum isoflavone (3), luteone (4), obovatin (5), erythrinassinate B (6) and isovanillin (7). Luteone (4) exhibited the most significant antibacterial activity with minimum inhibitory quantity (MIQ) values of 1.88 µg, 1.88 µg and 3.75 µg, respectively, against sensitive (MSSA) and resistant strains (MRSA and MDRSA) of S. aureus using a TLC bioautography assay. Erynone (1) exhibited the greatest DPPH free radical scavenging activity. Conclusions Seven compounds, including a new chromanone, were isolated from the antimicrobial dichloromethane extract of the stem bark of E. stricta. Six of the seven compounds showed antibacterial and/or antioxidant activities. These findings provide support for the customary (traditional and contemporary) use of E. stricta bark for the treatment of skin and wound infections.

Randomized clinical trial.To investigate the effect of including manual therapy (MT) in a pulmonary rehabilitation program for patients with chronic obstructive pulmonary disease (COPD).The primary source of exercise limitation in people with COPD is dyspnea. The dyspnea is partly caused by changes in chest wall mechanics, with an increase in chest wall rigidity (CWR) contributing to a decrease in lung function. As MT is known to increase joint mobility, administering MT to people with COPD carries with it the potential to influence CWR and lung function.Thirty-three participants with COPD, aged between 55 and 70 years (mean = 65·5±4 years), were randomly assigned to three groups: pulmonary rehabilitation (PR) only, soft tissue therapy (ST) and PR, and ST, spinal manipulative therapy (SM), and PR. Outcome measures including forced expiratory volume in the 1st second (FEV1), forced vital capacity (FVC), 6-minute walking test (6MWT), St. George's respiratory questionnaire (SGRQ), and the hospital anxiety and depression (HAD) scale were recorded at 0, 8, 16, and 24 weeks.There was a significant difference in FVC between the three groups at 24 weeks (P = 0·04). For the ST+SM+PR group versus PR only the increase was 0·40 l (CI: 0·02, 0·79; P = 0·03). No major or moderate adverse events (AE) were reported following the administration of 131 ST and 272 SM interventions.The increase in FVC is a unique finding. Although the underlying mechanisms responsible for this outcome are not yet understood, the most likely explanation is the synergistic effect resulting from the combination of interventions. These results support the call for a larger clinical trial in the use of MT for COPD.

Ethnopharmacological relevance Traditional medicinal plant knowledge is an integral and very important part of Indigenous cultures worldwide. For many communities there is a great urgency in recording this knowledge in written form. This is the first ethnobotanical report of medicinal plant knowledge of the Nagaland Ao tribe of Chungtia village and is an important step in the preservation of this culturally and medicinally significant knowledge. Aim of the study The aim of the presented work was to perform an ethnobotanical study on plants of medicinal and other significance to the Chungtia villagers of Nagaland, North East India. Materials and methods Ethnobotanical data were collected from traditional practitioners and Elders of Chungtia village by means of open group discussions and semi-structured interviews of groups and individuals using questionnaires. The interviews were also recorded in an audio format in the local Mongsen language. The gathered ethnobotanical knowledge was compared with reported ethnobotanical usages worldwide and reported biological properties and phytochemical studies relevant to the Chungtia villagers׳ applications. Results A total of 135 plant species of 69 families and 123 genera were recorded for medicinal and household maintenance applications. Those applications were grouped into 13 categories based on Chungtia villagers׳ classification system. The families most represented were Asteraceae, Euphorbiaceae and Solanaceae. The most reported uses were for gastrointestinal problems, followed by dermatological problems. The most commonly used plant parts were leaves, followed by fruits and stems and they were most commonly administered as a paste, decoction, infusion, juice or poultice, or taken orally with no preparation. There was strong agreement among the informants as to the usages of the plants (informant consensus factor 0.80–0.91). The use value of 6 for Cassia floribunda, Dolichos lablab, Hedyotis scandens, Phyllanthus urinaria and Rhus javanica indicated these are the most important species. Forty four of the 135 plants had a fidelity level of 100%. Conclusion This study has helped to document and preserve in written format important traditional plant knowledge of 135 plants of the Chungtia villagers, assisting them in the continued preservation of their cultural values.

Aims: To test the feasibility of identifying Staphylococcus aureus with a fluorescence in situ hybridization (FISH) assay that uses a single hot-plate and urea-NaCl reagents. Methods and Results: Slides spotted with S. aureus and treated with methanol and lysozyme were incubated with urea-NaCl reagents on a hot-plate with a precise temperature control and identified with specific DNA probes. Conclusions: Staphylococcus aureus was detected and differentiated from Staphylococcus epidermidis in 1 h with a novel FISH method that used a single hot-plate and in the absence of dimethyl formamide. Significance and Impact of Study: A rapid hot-plate FISH assay with urea-NaCl and without toxic dimethyl formamide might be useful if FISH is run infrequently or where resources are limited.

Background: Staphylococcus aureus is a clinically important pathogen. A small number of whole-cell fluorescence in situ hybridization (FISH) probes have been reported to detect S. aureus. New online computational tools for in silico design and testing make it possible to assess candidate FISH probes for S. aureus. Materials and Methods: Six online tools, NCBI-Nucleotide, Ribosomal Database Project, NCBI-Blast, Reverse-Complement, Probecheck, and mathFISH, were employed in a workflow to evaluate FISH probes for S. aureus. A previously reported probe, Staaur-16S69, was compared to a new probe, KT18-16S68, predicted by mathFISH to have the same performance. Results: A number of new probes for S. aureus were predicted to perform as well or better in silico as those previously reported. When tested in a FISH assay, Staaur and a new probe, KT18, were found to have the same performance. Conclusion: Existing and new FISH probes for S. aureus were found to be accurately identified and characterized with online computational tools. In silico evaluation of probes has the potential to reduce the time spent evaluating probes in the laboratory.

Objective The objective of this case series is to report the results of spinal manipulative therapy (SMT) for people with chronic obstructive pulmonary disease (COPD) who were older than 65 years. Methods The study design was a prospective case series. Six patients of a long-term care center who were older than 65 years and having COPD underwent a course of 12 SMT sessions over a 4-week period. Each SMT session consisted of manually applied spinal manipulation and instrument-assisted spinal manipulation delivered by a doctor of chiropractic. Lung function measurements were recorded at baseline and at 2 and 4 weeks. The occurrence and type of any adverse events (AEs) related to SMT were recorded at each SMT session. Results One male and 5 female patients took part in the study. The average age was 79.1 years (range, 68-89 years). There was a clinically significant increase in forced expiratory volume in the first second after SMT in 4 of the 6 patients at 2 weeks. This was sustained in only 1 patient at 4 weeks. No clinically significant changes were observed for forced vital capacity at 2 or 4 weeks. One hundred forty-four manually applied spinal manipulations and 72 instrument-assisted spinal manipulations were administered during the intervention period. No major or moderate AEs were reported. Only minor AEs were reported after 29% of the intervention sessions, with 1 AE being reported for each patient. All AEs resolved within 48 hours. Conclusions This case series offers preliminary evidence that SMT may have the potential to benefit lung function in patients with COPD who are older than 65 years.

Objectives The purpose of this study was to systematically review studies that quantify the high-velocity, low-amplitude (HVLA) spinal thrust, to qualitatively compare the apparatus used and the force-time profiles generated, and to critically appraise studies involving the quantification of thrust as an augmented feedback tool in psychomotor learning. Methods A search of the literature was conducted to identify the sources that reported quantification of the HVLA spinal thrust. MEDLINE-OVID (1966-present), MANTIS-OVID (1950-present), and CINAHL-EBSCO host (1981-present) were searched. Eligibility criteria included that thrust subjects were human, animal, or manikin and that the thrust type was a hand-delivered HVLA spinal thrust. Data recorded were single force, force-time, or displacement-time histories. Publications were in English language and after 1980. The relatively small number of studies, combined with the diversity of method and data interpretation, did not enable meta-analysis. Results Twenty-seven studies met eligibility criteria: 17 studies measured thrust as a primary outcome (13 human, 2 cadaver, and 2 porcine). Ten studies demonstrated changes in psychomotor learning related to quantified thrust data on human, manikin, or other device. Conclusions Quantifiable parameters of the HVLA spinal thrust exist and have been described. There remain a number of variables in recording that prevent a standardized kinematic description of HVLA spinal manipulative therapy. Despite differences in data between studies, a relationship between preload, peak force, and thrust duration was evident. Psychomotor learning outcomes were enhanced by the application of thrust data as an augmented feedback tool.

Urinary tract infections (UTIs) are known to alter the normal urine composition which, in principle, can lead to changes in urine autofluorescence. This paper describes the study of human urine (normal and UTI) by using UV fluorescence excitation/emission matrices and synchronous spectra and proposes a method of diagnosing UTI without any sample preparation. The method is based on excitation in the shorter UV region (250-350 nm) which shows good discrimination between the normal urine and UTI samples. The synchronous scans with an offset of Deltalambda=90 nm were also able to differentiate between normal urines and UTI samples. These differences were observed even though the two known major urine fluorophores, tryptophan and indoxyl sulfate were present in the normal urine and UTI samples in similar concentration as established by HPLC analysis. Although the identity of substances responsible for the altered autofluorescence in UTI is not established, our study shows that autofluorescence has the potential to differentiate between normal human urine samples and those with UTI.

Aboriginal people of Australia possess a rich knowledge on the use of medicinal plants for the treatment of sores, wounds, and skin infections, ailments which impose a high global disease burden and require effective treatments. The antibacterial and antioxidant activities and phytochemical contents of extracts, obtained from eight medicinal plants used by Aboriginal people of New South Wales, Australia, for the treatment of skin related ailments, were assessed to add value to and provide an evidence-base for their traditional uses. Extracts ofAcacia implexa,Acacia falcata,Cassytha glabella,Eucalyptus haemastoma,Smilax glyciphylla,Sterculia quadrifida, andSyncarpia glomuliferawere evaluated. All extracts except that ofS. quadrifidashowed activity against sensitive and multidrug resistant strains ofStaphylococcus aureuswith minimum inhibitory concentration values ranging from 7.81 to 1000 μg/mL. The sap ofE. haemastomaand bark ofA. implexapossessed high total phenolic contents (TPC) and strong DPPH radical scavenging abilities. A positive correlation was observed between TPC and free radical scavenging ability. GC-MS analysis of then-hexane extract ofS. glomuliferaidentified known antimicrobial compounds. Together, these results support the traditional uses of the examined plants for the treatment of skin related ailments and infections by Aboriginal people of New South Wales, Australia.

Background This study is a collaboration between Macquarie University researchers and the Yaegl Aboriginal Community of northern NSW, Australia to investigate the antimicrobial potential of plants used in the topical treatment of wounds, sores and skin infections. Based on previously documented medicinal applications, aqueous and aqueous ethanolic extracts of Alocasia brisbanensis, Canavalia rosea, Corymbia intermedia, Hibbertia scandens, Ipomoea brasiliensis, Lophostemon suaveolens and Syncarpia glomulifera and the aqueous extracts of Smilax australis and Smilax glyciphylla were tested against common wound pathogens, including antibiotic resistant bacterial strains. Methods Plant material was prepared as aqueous extractions modelled on customary preparations and using 80% aqueous ethanol. Extracts were assayed against a selection of clinically relevant Gram positive (Streptococcus pyogenes and sensitive and resistant strains of Staphylococcus aureus) and Gram negative (Pseudomonas aeruginosa, Escherichia coli and Salmonella typhimurium) bacteria and a fungus (Candida albicans) using disc diffusion and MTT microdilution methods. Viability of treated microorganisms was determined by subculturing from microdilution assays. Results The extracts of Corymbia intermedia, Lophostemon suaveolens and Syncarpia glomulifera had promising levels of antimicrobial activity (MIC 31–1,000 µg/mL) against both antibiotic sensitive and resistant Staphylococcus aureus as well as the fungus Candida albicans (clinical isolate). Conclusion Aqueous and 80% aqueous ethanolic extracts of Lophostemon suaveolens, Corymbia intermedia and Syncarpia glomulifera exhibited promising levels of antimicrobial activity against a range of both antibiotic sensitive and resistant strains of microorganisms. This is the first report of antimicrobial activities for C. intermedia and L. suaveolens and the leaves of S. glomulifera. This study demonstrates the value of customary knowledge in the identification of new sources of antimicrobial treatments. Lophostemon suaveolens is a relatively unexplored endemic medicinal plant of Australia. Extracts of fresh leaves of L. suaveolens obtained from sequential extraction with n-hexane and dichloromethane exhibited antibacterial activity in the disc diffusion and MTT microdilution assays against Streptococcus pyogenes and methicillin sensitive and resistant strains of Staphylococcus aureus (minimum bactericidal concentration63 μg/mL). The dichloromethane extract and chromatographic fractions therein inhibited nitric oxide in RAW264.7 murine macrophages (IC50 3.7-11.6 μg/mL) and also PGE2 in 3T3 murine fibroblasts (IC50 2.8-19.7 μg/mL). The crude n-hexane, dichloromethane and water extracts of the leaves and chromatographic fractions from the dichloromethane extract also showed modest antioxidant activity in the ORAC assay. GC-MS analysis of the n-hexane fraction showed the presence of the antibacterial compounds aromadendrene, spathulenol, β-caryophyllene, α-humulene and α-pinene and the anti-inflammatory compounds β-caryophyllene and spathulenol. Fractionation of the dichloromethane extract led to the isolation of eucalyptin and the known anti-inflammatory compound betulinic acid.

A modified fluorescein diacetate (FDA) assay has been compared with standard NCCLS broth macrodilution and broth microdilution methods for the detection of antifungal activity. The FDA assay was performed in a medium containing bacteriological peptone, NaCl, yeast extract and glucose (0.2%, 0.1%, 0.1% and 1% w/v, respectively) and buffered with 10 mM BES buffer. The MICs of amphotericin B, fluconazole, miconazole and flucytosine (representing three major classes of antifungal agents) obtained by the three methods were compared. The results obtained with the FDA assays correlated well with the NCCLS macrodilution method for MICs of amphotericin B, miconazole and fluconazole, but not for flucytosine. However, the MIC values of flucytosine obtained with the FDA assay were well within the quality control range for the two reference strains recommended by the NCCLS. The FDA assay described is an attractive alternative to the NCCLS methods for screening for antifungal agents, with the added advantage of objectivity of fluorescence measurement.

Two serrulatane diterpenes, 3,8-dihydroxyserrulatic acid (1) and serrulatic acid (2), have been isolated from Eremophila sturtii through bioassay-guided fractionation. These compounds inhibit the inflammation pathway enzymes cyclooxygenase 1 and 2, and exhibit bactericidal activity against Staphylococcus aureus.

The Australian Aboriginal people have used plants as medicine and food for thousands of years, however, this traditional knowledge is documented only to a limited extent, and is in danger of being lost. The Indigenous Bioresources Research Group (IBRG) aims to help Australian Aboriginal communities to preserve their customary medicinal knowledge, and to provide information that can be used for their cultural or educational purposes, as well as for scientific advancement. This work is undertaken in close collaboration with Australian Aboriginal communities in New South Wales. The project is multidisciplinary, combining an ethnobotanical and an ethnopharmacological approach, which includes biological and chemical investigations, as well as developing best practices for protecting traditional knowledge. This paper describes the general strategy of the project as well as methods used in the ethnopharmacological study. Ethnobotanical databases are set up for each participating community. Plant material is collected, extracted, and active compounds are isolated using a bioassay-guided fractionation approach. All extracts and compounds are tested for biological activity in antimicrobial assays (disc diffusion, resazurin, fluorescein diacetate), neurological assays or anti-inflammatory assays, depending on their traditional use.

The fluorescein diacetate (FDA) antibacterial assay relies on the cleavage of fluorescein diacetate by metabolically active bacteria. The recent finding that microbiological media can lead to significant levels of cleavage has reduced the reliability of the assay. Using the nucleophilic scavengers N-ethylmaleimide and maleic anhydride, we have demonstrated that this abiotic cleavage is most likely due to nucleophiles such as cysteine and histidine commonly present in the media. To increase the reliability of the assay we have modified the original assay conditions to include use of dilute medium (peptone 0.2% w/v, yeast extract 0.1% w/v and NaCl 0.1% w/v) in a non-nucleophilic buffer and overnight incubation of the medium after addition of antibacterial agents. The optimised fluorescein diacetate assay has been used to determine the MIC of gentamicin, tetracycline and chloramphenicol for Escherichia coli, Staphyloccocus aureus and Pseudomonas aeruginosa and gave quantitative results that were reproducible and consistent with published data.

Background A variety of fluorophores are present in normal human urine. Alteration in the autofluorescence of urine could result from physiological or pathological changes. Method This study investigates the differences in the autofluorescence of 45 normal urine samples from 25 individuals with bacteriuria. Results Excitation at 290 nm showed good discrimination between these 2 groups. Principal Component Analysis (PCA) of the data revealed statistically significant differences between the fluorescence spectra for samples with bacteriuria as compared to the control group. Conclusion The findings indicate the potential of the fluorescence spectrum of urine to be developed as a simple and rapid diagnostic tool.

Objective The purpose of this preliminary study was to demonstrate the feasibility of a study that measures the short-term effects of a course of manual therapy (MT) and exercise (Ex) in people with moderate chronic obstructive pulmonary disease (COPD). Methods Fifteen participants (9 males; mean age, 56.1 years), with moderate COPD (mean % predicted forced expiratory volume in the first second [FEV 1 % predicted], 61.8%), were randomly allocated to 1 of 3 groups: soft tissue therapy only (ST); ST and spinal manipulation (SM); or ST, SM, and Ex. The intervention continued for 4 weeks. Outcome measures included FEV 1 , forced vital capacity (FVC), chronic respiratory questionnaire (CRQ-SAS) scores, distance walked in a 6-minute walking test, and monitoring for adverse events. Results There was an increase in FVC for the SM + ST + Ex group compared with ST only and ST + SM (1.01 and 1.00 L, respectively). Distance walked increased in the ST + SM and ST + SM + Ex groups compared with ST only (120.0 and 168.0 m, respectively). Dyspnea levels decreased in the ST + SM and ST + SM + Ex groups compared with ST only (0.64 and 0.44, respectively). There were no major or moderate adverse events reported following ST or SM interventions. Conclusions For this small group of patients, combining MT with Ex produced short improvements in FVC, distance walked, and dyspnea levels, with no major or moderate adverse events. This preliminary study showed that a larger study evaluating the clinical outcomes of MT for people with moderate COPD appears feasible.

As a proof-of-concept, the feasibility of detecting Staphylococcus aureus faster than previous whole-cell fluorescent in situ hybridization (FISH) methods was tested.Isolates of Staphylococcus were treated with three rapid slide-based FISH protocols and DNA probes. Protocols were shortened by optimizing, combining or omitting steps.All FISH protocols detected S. aureus and not the phenotypically similar Staphylococcus epidermidis. The express FISH assay was completed in 24 minutes. The one-step FISH assay with NaCl and the one-step with phosphate buffered saline (PBS) assay took 19 minutes, but yielded a weaker signal.The exploratory study identified S. aureus two to three times faster than previous methods. Additional testing in a clinical laboratory scenario (for example with positive blood-culture bottles) is warranted.

Background: Aspects of the fluorescence in situ hybridization (FISH) method for the detection of clinically important bacteria, such as Staphylococcus aureus, Staphylococcus epidermidis, and Escherichia coli, were investigated for optimization. Methods: Various approaches to optimizing the FISH procedure were taken and different methods were compared. To save time, hybridization and washing buffers were prepared beforehand and stored at −20°C and mixed to their final formamide and NaCl concentrations just before use. The use of 50-ml tubes for hybridizationincubation reduced drying out, reagent wastage, and reaction times. Results: A two-step permeabilization FISH assay was developed that used phosphate-buffered saline as a buffer for lysostaphin. It could detect bacteria with DNA probes conjugated to fluorophores with a higher signal intensity and the less expensive biotinylated DNA probes with minimal cell lysis in 1 hr. Conclusions: The two-step assay might be used when the FISH signal is weak, bacterial numbers are low or if there is a need to use other reporter molecules. J. Clin. Lab. The premise that lung function can regulate chest wall mobility is an accepted concept. Descriptions of the primary and accessory respiratory structures do not usually include spinal components as a part of these classifications. The case for including these components as a part of the respiratory mechanism and their role in the development of dyspnea and chest wall rigidity in chronic obstructive pulmonary disease (COPD) is reviewed. Mechanical impairment of the chest wall is a contributing factor in the prognosis of COPD. Reducing this impairment improves prognosis. Because spinal manipulation and soft-tissue therapy increase joint mobility and decrease muscle hypertonicity, respectively, applying these interventions to the chest wall in COPD could reduce chest wall rigidity, thereby improving breathing mechanics. Improvements in breathing mechanics reduce the work of the respiratory muscles and delay the onset of dyspnea. Exercise capacity is reliant on the ability to overcome activity-limiting dyspnea, which usually occurs prior to maximum exercise capacity being reached. Delaying the onset of dyspnea permits more exercise to be performed before dyspnea develops. Spinal manipulation and soft-tissue therapy have the potential to deliver such a delay. Because exercise tolerance is considered to be a strong predictor of quality of life and survival in COPD, any increase in exercise capacity would therefore improve prognosis for the disease.

Ethnopharmacological relevance Documentation of Australian bush medicines is of utmost importance to the preservation of this disappearing and invaluable knowledge. This collaboration between the Yaegl Aboriginal community in northern New South Wales (NSW), Australia and an academic institution, demonstrates an effective means of preserving and adding value to this information. Materials and methods Questionnaire-guided interviews were performed with community Elders under a framework of participatory action research. Medicinal plant knowledge was collated in a handbook to aid interviews and to be used as an ongoing resource by the community. Specimens for all non-cultivar plants that were discussed were collected and deposited in herbaria with unique voucher numbers. This medicinal knowledge was checked against the literature for reports of related use and studies of biological activity. Results Nineteen Elders were interviewed, leading to discussions on fifty four plant preparations used for medicinal purposes. These plant preparations involved thirty two plants coming from twenty one families, reflecting the botanical diversity of the area. The plants retained in the Yaegl pharmacopoeia correspond to their accessibility and ease of preparation, reflected in their ongoing utilisation. Several plant uses did not appear elsewhere in the literature. Conclusions This study is the first comprehensive documentation of the medicinal knowledge of the Yaegl Aboriginal community. It has been conducted using participatory action research methods and adds to the recorded customary knowledge of the region. The customary medicinal knowledge retained by the Yaegl Aboriginal community is related to the evolving needs of the community and accessibility of plants.

To detect with whole-cell fluorescence in situ hybridization (FISH), Staphylococcus aureus is typically permeabilized with lyozyme and lysostaphin. We tested whether it was feasible to detect S. aureus and differentiate it from Staphylococcus epidermidis with lysozyme-only permeabilization. We compared lysozyme permeabilizationto S. aureus permeabilized with lysozyme in combination with lysostaphin. It was determined that S. aureus treated with agarose, methanol, and lysozyme could be detected with FISH. The 1 hr protocol is a useful alternative to conventional FISH. J. Clin. Lab. Despite biological variability the spectral characteristics of undiluted human urine show relatively low autofluorescence at short UV (250-300nm) excitation. However with dilution the fluorescence intensity remarkably increases. This paper examines the mechanisms behind this effect, by using excitation-emission matrices. Corrections for the inner filter effect were made for improved understanding of the spectral patterns. We focused on three major fluorophores (tryptophan, indoxyl sulfate and 5-hydroxyindole-3-acetate) that are excited at these wavelengths, and whose content in urine is strongly linked with various health conditions. Their fluorescence was studied both individually and in combinations. We also examined the effect of ammonium on the fluorescence of these major fluorophores individually and in combinations. Through these studies we have identified the leading effects that reduce the UV fluorescence, namely higher concentration of indoxyl sulfate producing the inner filter effect and concentration quenching and quenching of fluorophores by ammonium. This result will assist in broader utilisation of UV fluorescence in medical diagnostics.

A number of predisposing factors are recognised as increasing the risk of developing chronic pulmonary obstructive disease (COPD). There is increasing recognition that COPD may be an inflammatory disease with systemic consequences. However, the trigger for the transition from 'at risk' (stage 0) to COPD state remains unclear. The current approach to intervention for the 'at risk' group is risk factor avoidance. We propose that if interventions shown to improve chronic respiratory symptoms in COPD sufferers could be applied to the 'at risk' group, then moderation or even reversal of the changes typical of this transition becomes a possibility. Exercise training has been shown to be beneficial at all stages of COPD. Mobility of the chest wall influences lung function. We hypothesise that the application to 'at risk' individuals (stage 0) of therapeutic interventions known to improve chronic respiratory symptoms and cardiovascular function in mild/moderate COPD (stages 1 and 2) could delay progression of the disease (i.e. manifestation of mild/moderate COPD). If the hypothesis were confirmed, the potential to delay or even prevent the onset of COPD would be feasible.

Summary Objectives The aim of this study was to examine the role of complementary and alternative medical (CAM) practitioners in integrative practices where general practitioners (GPs) and CAM practitioners were co-located. Design This study used grounded theory, a qualitative methodology from the interpretive paradigm. Setting A total of 23 integrative practitioners (10 general practitioners and 13 naturopaths) were interviewed. The informants were drawn from 16 integrative practices and one non-integrative general medical practice. Results In 11 out of 16 integrative practices the CAM practitioners yielded their primary contact role to the GPs. CAM practitioners were restricted to expanding the range of treatment options available to patients. However, the role of the CAM practitioners was influenced by the level of CAM training the GP(s) in the practice had undertaken. The more CAM training the GPs had undertaken, the more CAM practitioners were enlisted as diagnosticians as well as treatment providers. Conclusion CAM practitioners in integrative practices had an accessory role to the GPs in the practice, deferring diagnosis and assessment activities to the GPs. However, where GPs had significant training in CAM, the CAM practitioner's role included both treatment and diagnostic activities.
