Introducción: La celiaquía es una enfermedad sistémica, asociada a numerosas manifestaciones hematológicas, a menudo presentes desde antes del diagnóstico de la enfermedad celiaca, como la anemia ferropénica o la leucopenia. Objetivos: Analizar las anomalías del hemograma al diagnóstico de la enfermedad celiaca en los tres últimos años en el Hospital Universitario Central de Asturias (HUCA) de Oviedo. Metodología: Se revisaron los hemogramas previos al diagnóstico de 225 pacientes diagnosticados de enfermedad celiaca en los años 2009-2011 en el HUCA, con un estadio I o superior de Marsh en el estudio anatomopatológico duodenal. Resultados: Se han encontrado alteraciones en el hemograma en el 75,6% de los pacientes estudiados al diagnóstico. Dentro de las anomalías vinculadas a las serie roja, la mas frecuente ha sido la anemia, con cifras de hemoglobina inferiores a 12 g/dl en un 75% de los casos, así como un incremento del ADE (área de distribución eritrocitaria) en un 51,8% del total de la muestra a estudio. Otros hallazgos relativos a la serie blanca, han sido la Basopenia en un 40% de los pacientes, Leucopenia en un 35% y Eosinopenia en un 5,7% de los casos. Conclusiones: El hemograma constituye una herramienta sencilla y útil para la sospecha diagnóstica de enfermedad celiaca, cuyo uso sistemático podría disminuir el retraso diagnóstico en la enfermedad celiaca. Objetivos: Analizar las diferencias entre las anomalías halladas en los hemogramas de niños y adultos al diagnóstico de la enfermedad celiaca. Metodología: Se analizaron 225 pacientes diagnosticados de enfermedad celiaca en los años 2009-2011 en el Hospital Universitario Central de Asturias (HUCA), que se encuentren en un estadio de Marsh I, o superior, en el estudio anatomopatológico duodenal. Resultados: El hallazgo más importante en los niños, fue la Basopenia en un 35,3%, seguido de un incremento del ADE en un 18% de los casos, Linfopenia en un 18% de los casos y descenso de la hemoglobina en el 12% de los niños. Los parámetros afectados y sus porcentajes no difieren de forma significativa con respecto a los hallados en el caso de los adultos. Conclusiones: El hemograma constituye una herramienta clave, sencilla y útil, para el diagnóstico de la enfermedad celiaca, tanto en las formas de comienzo en la edad infantil como en el adulto. Background: Postoperative intracranial neurosurgical infections (PINI) complicate < 5% neurosurgeries. Scarce attention was dedicated to the extension and characteristics of its antimicrobial management considering their high morbidity, not negligible mortality, delayed hospital stay and increased healthcare costs., Methods: We analyzed retrospectively (2014-2023) 162 PINI from eight Spanish third-level academic hospitals., Results: Elective clean craniotomies after tumor or vascular causes were the leading procedures. Epidural abscess (24.7%), scalp infections (19.8%), postsurgical meningitis (16.7%) and cranioplasty infections (16.7%) were the most frequent PINI. Gram negative bacteria (38.6%) and Staphylococcus spp (28.6%) were the predominant isolates. Overall 85.2% patients underwent pus drainage, mostly by craniotomy (40.3%). Interestingly 34% were already receiving antibiotics for extracranial infections before developing PINI while 16.8% did not receive pre-operative antibiotic prophylaxis. In total 77.2% patients started a combined intravenous (IV) antimicrobial therapy, of which 85.2% switched after 5 days to a second-line IV antibiotic regimen, in 41.3% cases combined, after pus culture results, for a median of 21 days. Overall 61.1% patients continued on oral antimicrobials after hospital discharge, 30.3% as a combined regimen, for a median of 42 days. Complete cure was obtained in 81.5% cases, while 11.1% relapsed, 7.4% failed to cure and 6.8% died after PINI complications. In the multivariate analysis oral antimicrobial therapy after hospital discharge (p = 0.001) was significantly associated with PINI cure with no effect on survival., Conclusions: We conclude that an extended 6 weeks sequential IV and oral antimicrobial therapy in addition to neurosurgical correction increases PINI cure rate with no effect on survival., Clinical Trial Number: Not applicable., Competing Interests: Declarations. The present work evaluates the predictive value of low-density-granulocytes (LDG) for the development of cardiovascular disease (CVD) and/or bone deterioration (BD) in a six-year prospective study in Systemic Lupus Erythematosus (SLE). Considering the high SLE-LDG capacity to form Neutrophil Extracellular Traps (NETs), circulating levels of total cell-free DNA (cirDNA) and relative amounts of mitochondrial and nuclear DNA (mtDNA and nDNA, respectively) were tested as LDG-associated biomarkers to identify SLE patients at risk of CVD and BD. To this end, the frequency of total blood LDGs, as well as the CD16negCD14neg (nLDG) and CD16posCD14low (pLDG) subsets, was quantified by flow cytometry in 33 controls and 144 SLE patients. Total cirDNA and relative amounts of mitochondrial (mtDNA) and nuclear (nDNA) cell-free DNA were measured by fluorometry or qPCR in plasma from a subgroup of 117 patients and 23 controls at enrolment. Our findings showed increased blood levels of SLE-nLDGs at enrolment associated with prospective CVD development (pCVD) and the presence of BD, thus revealing LDG expansion as a predictor of both comorbidities in SLE progression. The amounts of the different types of circulating DNA analysed were increased in patients, especially those presenting traditional CV-risk factors or subclinical atheromatosis. IgG4-related disease is characterized by a lymphoplasmacytic infiltrate rich in IgG4-positive plasma cells, storiform fibrosis and obliterative phlebitis. It can present as parotid gland enlargement, tubulointerstitial nephritis, retroperitoneal fibrosis or pancreatitis, although nearly any organ can be affected. We report the case of a 37-year-old woman who presented with severe dysphonia and recurrent painful aphthous ulcers, with histopathological findings at the level of the larynx that revealed a lymphoplasmacytic infiltrate and IgG4 positivity. In addition, extensive studies were performed to rule out other diseases. Objective: The aim was to evaluate the most relevant cell populations involved in vascular homeostasis as potential biomarkers of SLE-related cardiovascular disease (CVD)., Methods: Low-density granulocytes (LDGs), monocyte subsets, endothelial progenitor cells, angiogenic T (Tang) cells, CD4+CD28null and Th1/Th17 lymphocytes and serum cytokine levels were quantified in 109 SLE patients and 33 controls in relationship to the presence of subclinical carotid atheromatosis or cardiovascular disease. A second cohort including 31 recent-onset SLE patients was also included., Results: Raised monocyte and LDG counts, particularly those LDGs negative for CD16/CD14 expression (nLDGs), in addition to the ratios of monocytes and nLDGs to high-density lipoprotein-cholesterol (HDLc) molecules (MHR and nLHR, respectively), were present in SLE patients with traditional risk factors or subclinical atheromatosis but not in those who were CV-free, thus revealing their value in the identification of patients at risk of CVD, even at the onset of disease. Accordingly, nLDGs were correlated positively with carotid intima-media thickness (cIMT) and with inflammatory markers (CRP and IL-6). A bias towards more differentiated monocyte subsets, related to increased IFN-α and IL-17 serum levels, was also observed in patients. Intermediate monocytes were especially expanded, but independently of their involvement in CVD. Finally, CD4+CD28null, Th17 and Th1 lymphocytes were increased, with CD4+CD28null and Th17 cells being associated with cIMT, whereas endothelial progenitor and Tang cell levels were reduced in all SLE patients., Conclusion: The present study highlights the potential use of MHR and nLHR as valuable biomarkers of CVD risk in SLE patients, even at diagnosis. The increased amounts of nLDGs, monocytes, Th17 and senescent-CD28null subsets, coupled with reduced pro-angiogenic endothelial progenitor cells and Tang cells, could underlie the development of atheromatosis in SLE., (© The Author(s) 2020.
