- Abstract
ABSTRACTIntroductionPsoriasis affecting the genital, palmoplantar, and scalp regions is recognized as difficult-to-treat, and data on the efficacy of biologics in these areas remains limited.Research design and methodsThis single-center study evaluated the effectiveness of anti-IL-17 and anti-IL-23 agents on scalp, genital, and palmoplantar psoriasis. We retrospectively analyzed data from all patients with psoriasis being treated with IL inhibitors at our clinic. Effectiveness was evaluated at 16, 28, and 52 weeks, according to the achievement of relative and mean PSSI, PGA-G, and ppPASI.ResultsIn all, 308 patients showed involvement of the scalp, 136 in the genital area, and 94 in the palmoplantar regions. On scalp psoriasis, anti-IL-17 agents demonstrated superiority in disease control compared to anti-IL-23 agents. PSSI100 at week 16 was reached by 59% of patients on an anti-IL17 vs 39.8% on an anti-IL-23 (p < 0.003). At genital sites, no significant differences between anti-IL-17 and anti-IL-23 agents were observed, and all classes achieved PGA-G 0/1. No significant differences between anti-IL-17 and anti-IL-23 agents were observed in palmoplantar areas.ConclusionsThe present data support the utility of both anti-IL-17 and anti-IL-23 agents for the treatment of difficult-to-treat areas in patients with psoriasis. Anti-IL-17 agents achieved better control of scalp psoriasis.

Background There are no Italian data regarding the strategies for preventing neonatal group B streptococcal (GBS) infection. We conducted a national survey in order to explore obstetrical, neonatal and microbiological practices for the GBS prevention. Methods Three distinct questionnaires were sent to obstetricians, neonatologists and microbiologists. Questionnaires included data on prenatal GBS screening, maternal risk factors, intrapartum antibiotic prophylaxis, microbiological information concerning specimen processing and GBS antimicrobial susceptibility. Results All respondent obstetrical units used the culture-based screening approach to identify women who should receive intrapartum antibiotic prophylaxis, and more than half of the microbiological laboratories (58%) reported using specimen processing consistent with CDC guidelines. Most neonatal units (89 out of 107, 82%) reported using protocols for preventing GBS early-onset sepsis consistent with CDC guidelines. Conclusions The screening-based strategy is largely prevalent in Italy, and most protocols for preventing GBS early-onset sepsis are consistent with CDC guidelines. However, we found discrepancies in practices among centers that may reflect the lack of Italian guidelines issued by public health organizations.

OBJECTIVE: To study the prevalence of congenital syphilis and its risk factors in Italy. STUDY DESIGN: Prospective study from 1 July 2006 to 30 June 2007. Data on mother-child pairs were collected for every syphilis seropositive mother. RESULTS: Maternal syphilis seroprevalence at delivery was 0.17%. 207 infants were born to 203 syphilis seropositive mothers. In 25 newborns it was possible to diagnose congenital syphilis (20/100,000 live births). Maternal risk factors included age

- Abstract
The Sahelian belt of West Africa is a region characterized by wide climate variations, which can in turn affect the survival of local populations especially in rangeland, as happened during the dramatic food crisis in the 70-80s caused by severe drought. This work has been carried out in the framework of the EU FP7 Geoland2 project as a contribution to the ECOWAS component (Economic Community Of West African States) of the AMESD (African Monitoring of the Environment for Sustainable Development) programme with the purpose of establishing the reliability of Dry Matter Productivity (DMP) developed by Flemish Institute for Technological Research (VITO), a spatial estimation of dry matter (DM) obtained from remotely sensed data. DMP can be of great help in monitoring savanna pasturelands in a region characterized by food insecurity and a significant variability of biomass production, linked to climate variations, which can in turn affect the survival of local populations. The evaluation of DMP was carried out thanks to the Centre de Suivi Ecologique (CSE) and Action Contre la Fame (ACF), the partners who provided the field biomass measurements. The paper shows the correlation of DMP with field measurements of herbaceous biomass, and discusses the differences among the different sites where ground data were collected. The analysis of other environmental variables (land cover, rainfall), which can be influential on rangeland biomass production, is presented in order to better explain the variance of field measurements among the different years.

20. Advantages and drawbacks of NOAA-AVHRR and SPOT-VGT for burnt area mapping in a tropical savanna ecosystem

- Abstract
In this paper we compare and contrast the performances of two low spatial resolution Earth observing systems, National Oceanographic and Atmospheric Administration ‐ advanced very high resolution radiometer (NOAA‐AVHRR) and Système pour l'Observation de la Terre ‐ VEGETATION (SPOT‐VGT), for burnt area mapping in a tropical savanna ecosystem of northern Australia. A multi-temporal composite image was derived from each of the two data sets acquired at the beginning of the 1999 fire season, and a set of rules for extracting burnt areas was compiled using a classification and regression trees (CART) methodology. In the case of the VGT data, the rules were used to segment the entire composite image; for the AVHRR data, a seed-growing approach was tested, with the aim of reducing overestimation of burnt areas. Since burnt areas are often confused with other land cover types, active fires were first identified (the seeds), and then the mapping of burnt areas was restricted to the neighbourhood of the pixels labelled as active fires. For the accuracy assessment, a reference map was derived by visual interpretation of two Landsat thematic mapper (TM) frames. First, the proportions of burnt areas were calculated for 14 × 14 km cells on a regular grid overlaid on AVHRR, VGT, and TM maps and the low-resolution areas were regressed against the TM estimates. Second, the spatial agreement between burnt area maps derived using high and low spatial resolution data was assessed using confusion matrices. We conclude that NOAA‐AVHRR and SPOT‐VGT show complementary characteristics. The better geometry of the VGT sensor, compared to the AVHRR, allows a higher accuracy in burnt area estimation, but the lack of a thermal channel on VGT is certainly a drawback for both the compositing and the burnt area mapping processes. Both systems tend to overestimate burnt areas when these areas are very fragmented but clearly underestimate when the burn scars are too small to be detected. Lastly, the seed-growing method is promising and could form the basis for a combined use of NOAA-AVHRR and SPOT-VGT systems.

In this work we propose an approach for mapping flooded areas from Sentinel-2 MSI (Multispectral Instrument) data based on soft fuzzy integration of evidence scores derived from both band combinations (i.e. Spectral Indices - SIs) and components of the Hue, Saturation and Value (HSV) colour transformation. Evidence scores are integrated with Ordered Weighted Averaging (OWA) operators, which model user's decision attitude varying smoothly between optimistic and pessimistic approach. Output is a map of global evidence degree showing the plausibility of being flooded for each pixel of the input Sentinel-2 (S2) image. Algorithm set up and validation were carried out with data over three sites in Italy where water surfaces are extracted from stable water bodies (lakes and rivers), natural hazard flooding, and irrigated paddy rice fields. Validation showed more than satisfactory accuracy for the OR-like OWA operators (F-score > 0.90) with performance slightly decreased (F-score < 0.75) over heterogeneous conditions (e.g. rice fields). The algorithm was applied with no changes and/or tuning to independent sites from the Copernicus Emergency Management Service (EMS) activations to simulate operational conditions. Over these sites, the proposed approach achieved greater, more consistent and robust mapping accuracy compared to traditional approaches based on the segmentation of single input features.
