Abstract

Abstract Background The seasonality, clinical and radiographic features and outcome of aseptic meningitis have been described for regional outbreaks but data from a wider geographic area is necessary to delineate the epidemiology of this condition. Methods A retrospective chart review was completed of children presenting with aseptic meningitis to eight Canadian pediatric hospitals over a two-year period. Results There were 233 cases of proven enteroviral (EV) meningitis, 495 cases of clinical aseptic meningitis and 74 cases of possible aseptic meningitis with most cases occurring July to October. Headache, vomiting, meningismus and photophobia were more common in children ≥ 5 years of age, while rash, diarrhea and cough were more common in children < 5 years of age. Pleocytosis was absent in 22.3% of children < 30 days of age with proven EV meningitis. Enterovirus was isolated in cerebrospinal fluid (CSF) from 154 of 389 patients (39.6%) who had viral culture performed, and a nucleic acid amplification test for enterovirus was positive in CSF from 81 of 149 patients (54.3%). Imaging of the head by computerized tomography or magnetic resonance imaging was completed in 96 cases (19.7%) and 24 had abnormal findings that were possibly related to meningitis while none had changes that were definitely related to meningitis. There was minimal morbidity and there were no deaths. Conclusion The clinical presentation of aseptic meningitis varies with the age of the child. Absence of CSF pleocytosis is common in infants < 30 days of age. Enterovirus is the predominant isolate, but no etiologic agent is identified in the majority of cases of aseptic meningitis in Canadian children.

6. Marine heatwaves and cold spells in the Northeast Atlantic: what should the UK be prepared for?

Abstract

Up to now, the UK has avoided major marine heatwaves (MHWs) that cause severe damage to marine ecosystems and the blue economy. However, an unprecedented in its intensity, though short-lived, MHWoccurred in UK waters in June 2023. This event sounded an alarm bell, highlighting gaps in our understanding of MHW characteristics and their potential future impacts in the UK. Here, we use a combination of remote sensing data and model output to characterise MHWs and Marine Cold Spells (MCSs) around the UK and the wider North Atlantic, and to assess the potential for concurrent biogeochemical extreme events. Results indicate that across the wider North Atlantic, the UK is not a hot spot for MHWs or MCSs but, regionally, the southern North Sea experiences the most activity. This is also the location of extreme chlorophyll-a concentrations, here termed blue waves (low chlorophyll-a) and green waves (high chlorophyll-a). However, there is not a very pronounced relationship between temperature and chlorophyll-a extremes, which may be impacted by the exact location, drivers and season of occurrence. In contrast, the southern North Sea and English Channel may experience a MHW and low near-bottom oxygen compound events year-round, which, due to the combination of thermal stress and reduced oxygen availability, may negatively impact benthic marine ecosystems. While MHWs in UK waters do not appear to be as long-lasting or intense as other well-documented events around the world, they are projected to increase. Thus, the UK has a unique opportunity to learn from other nations and so develop robust and comprehensive policies to increase preparedness and response capability for future extreme events. 15. Monopile-induced turbulence and sediment redistribution form visible wakes in offshore wind farms

Abstract

Offshore wind farms are becoming an increasingly common feature in the marine environment as a renewable energy source. There is a growing body of evidence on the effects of wind farms on the seabed and its organisms. However, an important and understudied aspect of site development is the interaction of turbine foundations on the surrounding marine environment. Structures exert significant disturbance on tides, waves and currents; these are visible as optically-distinct, elongate wakes at the sea surface with elevated suspended particulate matter. Despite this, there is uncertainty on the mechanisms that lead to the visible manifestation of wakes at turbine foundations, primarily due to a lack of direct measurements. Here, in situ measurements along with a 15-year time series of satellite images of the Thanet offshore wind farm, located within the Thames Estuary, were used to investigate the formation of visible monopile wakes, and the effects these have on the surrounding water column. We show the optically distinct wakes are near-constant at Thanet; visible in >90% of all satellite images, yet no regional change in sea surface turbidity could be attributed to wind farm construction or operation. Monopile wake in situ water samples and acoustic Doppler current profiler (ADCP) backscatter measurements demonstrated colour change related to elevated sea surface sediment concentration. However, averaged water column measurements of suspended sediment within wakes, and upstream of monopiles, remained consistent. These measurements demonstrate that sediment was redistributed towards surface waters, rather than additional sediment becoming suspended in the wake. ADCP velocity measurements supported a mechanism of sediment lofting towards the surface, with enhanced vertically upwards flow recorded in wakes.

Abstract

EMB Future Science Brief No. 11 ‘Marine habitat mapping’ presents science and policy needs and recommendations to advance next-generation marine habitat mapping. It highlights current methods and future trends in the acquisition of data from the seabed and water column via remote sensing and direct, in situ techniques. It discusses combining data to produce maps using modelling approaches and presents recommendations for adopting fit-for-purpose habitat classification schemes. It also provides an overview of what has been mapped and where within the European sea-basins, highlights the need to increase the quality and resolution of marine habitat maps. It identifies critical gaps in habitat types and geographic extent, including the coastal areas, the deep sea, Natura 2000 sites and other Marine Protected Areas across all regional seas. Finally, it describes the need to improve the assessment and communication of uncertainty and confidence in maps, and to make maps more easily accessible to a variety of stakeholders to increase their value for end-users and to the public for Ocean literacy.

Abstract

During the 2020/2021 academic year, I conducted a mixed quantitative and qualitative analysis of the drivers of student engagement in the School of Politics and International Relations at Queen Mary University of London. My main finding was that students vary widely in their ability to manage the competing demands on their time. Those who are able to avoid scheduling conflicts between taught classes and off-campus commitments engage at higher rates and achieve better marks. Those who struggle – whether due to under-developed time management skills, a lack of confidence in asking for assistance, or both – miss out on vital learning opportunities, and their attainment suffers. While there is little a department can do about the broader socio-economic forces that require students to take on significant responsibilities beyond the classroom, my findings suggest there are things we can do to improve engagement among those who struggle. I conclude by recommending actions to help students resolve clashing commitments, to build their confidence in and ability to seek support from staff, and to develop positive peer networks. Abstract

Introduction: Complexities of robotic distal gastrectomy (RDG) give reason to assess physician's surgical skill. Varying levels in surgical skill affect patient outcomes. We aim to investigate how a novel artificial intelligence (AI) model can be used to evaluate surgical skill in RDG by recognizing surgical instruments. Methods: Fifty‐five consecutive robotic surgical videos of RDG for gastric cancer were analyzed. We used Deeplab, a multi‐stage temporal convolutional network, and it trained on 1234 manually annotated images. The model was then tested on 149 annotated images for accuracy. Deep learning metrics such as Intersection over Union (IoU) and accuracy were assessed, and the comparison between experienced and non‐experienced surgeons based on usage of instruments during infrapyloric lymph node dissection was performed. Results: We annotated 540 Cadiere forceps, 898 Fenestrated bipolars, 359 Suction tubes, 307 Maryland bipolars, 688 Harmonic scalpels, 400 Staplers, and 59 Large clips. The average IoU and accuracy were 0.82 ± 0.12 and 87.2 ± 11.9% respectively. Moreover, the percentage of each instrument's usage to overall infrapyloric lymphadenectomy duration predicted by AI were compared. The use of Stapler and Large clip were significantly shorter in the experienced group compared to the non‐experienced group. Conclusions: This study is the first to report that surgical skill can be successfully and accurately determined by an AI model for RDG. Our AI gives us a way to recognize and automatically generate instance segmentation of the surgical instruments present in this procedure. CONTEXTE: Le role des enfants dans la propagation et la transmission communautaire du coronavirus du syndrome respiratoire aigu severe 2 (SRAS-CoV-2) est encore mal compris. Nous visons a quantifier l'infectivite du SRAS-CoV-2 d'echantillons nasopharynges provenant d'enfants comparativement a ceux provenant d'adultes. METHODES: Nous avons obtenu des ecouvillons nasopharynges de cas adultes et pediatriques de la maladie a coronavirus 2019 (COVID-19) ainsi que de leurs contacts qui ont obtenu un resultat positif a la presence du SRAS-CoV-2 lors d'un test de depistage au Manitoba entre les mois de mars et decembre 2020. Nous avons compare la croissance virale en culture cellulaire, les valeurs de cycle seuil de test d'amplification en charne par polymerase couple a une transcription inverse (RT-PCR) de l'enveloppe (E) du gene du SRAS-CoV-2 et de la dose infectieuse pour 50% de la culture tissulaire ([DICT.sub.50]/mL) entre les adultes et les enfants. RESULTATS: Parmi les 305 echantillons positifs a la presence du SRAS-CoV-2 valides par RT-PCR, 97 echantillons provenaient d'enfants de 10 ans et moins, 78 echantillons d'enfants de 11-17 ans et 130 echantillons d'adultes ([greater than or equal to] 18 ans). On a observe une croissance virale en culture dans 31% des echantillons, dont 18 (19%) echantillons d'enfants de 10 ans et moins, 18 (23%) d'enfants de 11-17 ans et 57 (44%) d'adultes (enfants c. adultes, rapport de cotes 0,45; intervalle de confiance [IC] a 95% 0,28-0,72). Le cycle seuil etait de 25,1 (IC a 95% 17,7-31,3) chez les enfants de 10 ans et moins, 22,2 (IC a 95% 18,3-29,0) chez les enfants de 11-17 ans et 18,7 (IC a 95% 17,9-30,4) chez les adultes (p < 0,001). La [DICT.sub.50]/mL mediane etait considerablement plus faible chez les enfants de 11-17 ans (316, ecart interquartile [EI] 178-2125) que chez les adultes (5620, EI 1171-17 800, p < 0,001). BACKGROUND: The role of children in the transmission and community spread of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is unclear. We aimed to quantify the infectivity of SARS-CoV-2 in nasopharyngeal samples from children compared with adults. METHODS: We obtained nasopharyngeal swabs from adult and pediatric cases of coronavirus disease 2019 (COVID-19) and from their contacts who tested positive for SARS-CoV-2 in Manitoba between March and December 2020. We compared viral growth in cell culture, cycle threshold values from the reverse transcription polymerase chain reaction (RT-PCR) of the SARS-CoV-2 envelope (E) gene and the 50% tissue culture infective dose ([TCID.sub.50]/mL) between adults and children. RESULTS: Among 305 samples positive for SARS-CoV-2 by RT-PCR, 97 samples were from children aged 10 years or younger, 78 were from children aged 11-17 years and 130 were from adults ([greater than or equal to] 18 yr). Viral growth in culture was present in 31% of samples, including 18 (19%) samples from children 10 years or younger, 18 (23%) from children aged 11-17 years and 57 (44%) from adults (children v. adults, odds ratio 0.45, 95% confidence interval [CI] 0.28-0.72). The cycle threshold was 25.1 (95% CI 17.7-31.3) in children 10 years or younger, 22.2 (95% CI 18.3-29.0) in children aged 11-17 years and 18.7 (95% CI 17.9-30.4) in adults (p < 0.001). The median [TCID.sub.50]/mL was significantly lower in children aged 11-17 years (316, interquartile range [IQR] 178-2125) than adults (5620, IQR 1171 to 17 800, p < 0.001). Cycle threshold was an accurate predictor of positive culture in both children and adults (area under the receiver-operator curve, 0.87, 95% CI 0.81-0.93 v. 0.89, 95% CI 0.83-0.96, p = 0.6). 34. More spinn'd against than spinning? : public opinion, political communication, and Britain's involvement in the 2003 invasion of Iraq

Abstract

When Tony Blair took Britain to war in Iraq in 2003, he overruled vociferous opposition from both the wider public and members of his own governing party. Public opinion was exercised by the issue on a vast scale. Over one million marched in London against the war. Opinion polls uniformly showed majority opposition to the use of force. Newspapers, the engine of media debate in this country, mostly attacked the government line, and encouraged their readers to protest or even, in one case, to rebel. The story of Iraq, however, is not simply one of an ideological or misguided premier dragging the entire nation to battle against its will. It is not simply one of ‘spin’, dossiers, Alastair Campbell, and Weapons of Mass Destruction. Much of the debate, and much of the hostility it generated, focused on areas that foreign policy analysts would consider peripheral; the domestic political consequences of war, the role of ‘spin doctors’ in the assessment of intelligence, and the question of whether the Prime Minister’s (successful) efforts to build a strong alliance with the world’s last superpower had transformed him into the President’s ‘poodle’. Interactions between ministers and the media were conditioned on both sides by an intimidating array of structural pressures. Diplomatic and journalistic calculations often clashed, trapping the government in the middle of an immensely complex ‘multi-level game’. News management influenced substantive foreign policy just as policy influenced news management, and the media arguably affected both, albeit often indirectly. The substance and the communication of the decision to go to war proved to be inseparable, both in the course of decision-making, and in their later retrospective assessment. Public Opinion, broadly defined, had a significant impact on British foreign policy at this time. Crucially, however, this impact operated through political communication mechanisms usually ignored by FPA.

35. Professional Standards and Performance Evaluation for Principals in China: A Policy Analysis of the Development of Principal Standards

Abstract

This article presents the results of an interpretive policy analysis of China's Ministry of Education Standards (2013) for the professional practice of principals. In addition to revealing the evolution of the evaluation of principals in China and the processes by which this policy is formulated, a comparative analysis was conducted to compare it with the Interstate School Leaders Licensure Standards 2008 (ISLLC 2008). The analysis was conducted on the specific standards and indicators across the two documents. The results indicate the presence of both significant similarities and differences in performance expectations for principals: differences are explained by the cultural and national contexts within which school leaders work in both countries. In February 2013 the Ministry of Education in China issued for the first time the national Professional Standards for Compulsory Education School Principals, which provide the specific expectations of quality school leadership. The unprecedented interest in international benchmarking of student academic performance has led to the belief that there are common elements in education policy and school leadership practices. It is hoped that this research sheds new light onto the current thinking on the expectations and evaluation of principal leadership.

49. Recovery and restoration potential of cold-water corals: experience from a deep-sea marine protected area

Abstract

Cold-water corals (CWCs) are important species that provide habitat for other taxa but are sensitive to mechanical damage from bottom trawling. CWC conservation has been implemented in the form of Marine Protected Areas (MPAs), but recovery from impact may be particularly slow in the deep-sea environment; consequently, the use of restoration techniques has been considered. To gain some insight into CWC recruitment and growth, in 2011 we deployed small seabed moorings in the Darwin Mounds MPA (~1000 m water depth). This site hosts hundreds of CWC mounds, that had previously (until 2003) been impacted by deep-water trawling. In 2019, we carried out in situ visual surveys of these moorings and the surrounding seabed environment, then recovered two of the moorings. The mooring buoys, glass floats with plastic covers, were extensively colonised by a diverse epifauna that included the CWCs Desmophyllum pertusum and Desmophyllum dianthus. The presence of coral recruits indicated that environmental conditions, and larval supply, remained favourable for the settlement and growth of CWCs within the MPA. Based on our observations, we consider four possible restoration methods, together with a ‘do-nothing’ option, for the Darwin Mounds CWCs that have shown little, if any, natural recovery despite 16-years of protection. We conclude that seabed emplacement of high-relief artificial substrata is likely to be the most efficient and cost-efficient means of promoting enhanced recovery of the CWCs.
