Abstract

Abstract Background Selective serotonin reuptake inhibitors (SSRIs) are commonly prescribed to manage anxiety in adults with an autism diagnosis. However, their effectiveness and adverse effect profile in the autistic population are not well known. This trial aims to determine the effectiveness and cost-effectiveness of the SSRI sertraline in reducing symptoms of anxiety and improving quality of life in adults with a diagnosis of autism compared with placebo and to quantify any adverse effects. Methods STRATA is a two-parallel group, multi-centre, pragmatic, double-blind, randomised placebo-controlled trial with allocation at the level of the individual. It will be delivered through recruiting sites with autism services in 4 regional centres in the United Kingdom (UK) and 1 in Australia. Adults with an autism diagnosis and a Generalised Anxiety Disorder Assessment (GAD-7) score ≥ 10 at screening will be randomised 1:1 to either 25 mg sertraline or placebo, with subsequent flexible dose titration up to 200 mg. The primary outcome is GAD-7 scores at 16 weeks post-randomisation. Secondary outcomes include adverse effects, proportionate change in GAD-7 scores including 50% reduction, social anxiety, obsessive-compulsive symptoms, panic attacks, repetitive behaviours, meltdowns, depressive symptoms, composite depression and anxiety, functioning and disability and quality of life. Carer burden will be assessed in a linked carer sub-study. Outcome data will be collected using online/paper methods via video call, face-to-face or telephone according to participant preference at 16, 24 and 52 weeks post-randomisation, with brief safety checks and data collection at 1–2, 4, 8, 12 and 36 weeks. An economic evaluation to study the cost-effectiveness of sertraline vs placebo and a QuinteT Recruitment Intervention (QRI) to optimise recruitment and informed consent are embedded within the trial. Qualitative interviews at various times during the study will explore experiences of participating and taking the trial medication. Discussion Results from this study should help autistic adults and their clinicians make evidence-based decisions on the use of sertraline for managing anxiety in this population. Abstract

Since February 2020, strategies aimed at containing and managing the Covid-19 syndemic have been developed by the governments of European countries. Among these measures, the possibility of an early treatment of the disease has been considered of fundamental importance, both for curing the disease and governing the syndemic. Despite their potential, early therapies received a somehow unexpected treatment in Italy and the debate around them gave rise to a very evident conflict between proponents and opponents of those treatments, to the point that some of the former organised a properly political movement in order to promote the integration of early home therapies in the official health protocols. Not surprisingly, the issue of early therapies has been considered an exemplary case of politicisation of science. However, the assimilation of the early therapy controversy to the frame of politicisation of science cannot fully explain why these protocols were discarded by political and health authorities. Rather, the consideration of health protocols as socio-technical objects shifts the attention on the vast range of cultural, political and economic factors that contributed to the general resistance towards those treatments. Therefore, we aim to analyse the media coverage of the phenomenon, and investigate the protocols of home treatment of Covid-19, paying attention to the interaction of the factors that contributed to the exclusion of home therapies into national guidelines.

Abstract

In elevated atmospheric CO2 conditions, a prominent greenhouse gas contributing to climate change, there is a notable reduction in zinc (Zn) concentration within lowland rice crops. Zn deficiency diminishes rice productivity and lowers Zn content in grains, impacting human health, particularly stunting. This study aims to assess the effects of applying isolated Zn-solubilizing bacteria (ZnSB) strains and ZnSO4 on Zn uptake and yield in two rice varieties: Inpari IR Nutri Zinc (high Zn content) in the first trial and Ciherang (medium Zn content) in the second trial, conducted in an Inceptisol soil. The treatments were organized in a Randomized Block Design with three replications. The first factor involved ZnSO4 fertilizer (Z0 = without ZnSO4; and Z1 = with ZnSO4). The second factor was ZnSB strains (B0 = without ZnSB; B1 = Klebsiella pneumoniae Zn2; B2 = Enterobacter cloacae CmA12; B3 = E. bugandensis CmD2; B4 = S. marcescens CmT13; and B5 = B. thuringiensis PuT1). The results indicated that applying ZnSB strains without ZnSO4 significantly increased grain yields for both Ciherang and Inpari IR Nutri Zinc varieties (7.67% and 4.12%, respectively) and enhanced Zn uptake (28.05% and 13.58%, respectively). Moreover, combined ZnSB and ZnSO4 application also significantly increased yields for both varieties (12.14% and 25.03%, respectively) and Zn uptake (33.29% and 13.99%, respectively), demonstrating the potential of ZnSB for enhancing Zn uptake and rice yield. These findings suggest promising strategies for addressing Zn deficiency in rice cultivation under elevated CO2 conditions.

Abstract

Preimplantation culture of mouse embryos has been suggested to result in reduced anxiety-like behavior in adulthood. Here, we investigated the effects of in vitro fertilization (IVF), embryo culture, and different diets on anxiety-like behavior using the elevated plus maze (EPM). We hypothesized that exposure to suboptimal conditions during the preimplantation stage would interact with the suboptimal diet to alter behavior. The expression of genes related to anxiety was then assessed by quantitative real-time polymerase chain reaction in various brain regions. When fed a normal diet during gestation and a moderately high-fat Western diet (WD) postnatally, naturally conceived (NC) and IVF mice showed similar anxiety-like behavior on the EPM. However, when fed a low-protein diet prenatally and a high-fat diet postnatally (LP/HF), NC mice showed a modest increase in anxiety-like behavior, whereas IVF mice showed the opposite: a strongly reduced anxiety-like behavior on the EPM. The robust reduction in anxiety-like behavior in IVF males fed the LP/HF diets was, intriguingly, associated with reduced expression of MAO-A, CRFR2, and GABA markers in the hypothalamus and cortex. These findings are discussed in relation to the developmental origin of health and disease hypothesis and the 2-hit model, which suggests that 2 events, occurring at different times in development, can act synergistically with long-term consequences observed during adulthood.

We employ realistic constraints on astrophysical and instrumental selection effects to model the Gamma-Ray Burst (GRB) redshift distribution using {\it Swift} triggered redshift samples acquired from optical afterglows (OA) and the TOUGH survey. Models for the Malmquist bias, redshift desert, and the fraction of afterglows missing because of host galaxy dust extinction, are used to show how the "true" GRB redshift distribution is distorted to its presently observed biased distribution. We also investigate another selection effect arising from a correlation between $E_{{\rm iso}}$ and $L_{{\rm opt}}$. The analysis, which accounts for the missing fraction of redshifts in the two data subsets, shows that a combination of selection effects (both instrumental and astrophysical) can describe the observed GRB redshift distribution. Furthermore, the observed distribution is compatible with a GRB rate evolution that tracks the global SFR, although the rate at high-$z$ cannot be constrained with confidence. Taking selection effects into account, it is not necessary to invoke high-energy GRB luminosity evolution with redshift to explain the observed GRB rate at high-$z$., Comment: Version 2. Abstract

Perinatal asphyxia, a naturally and commonly occurring risk factor in birthing, represents one of the major causes of neonatal encephalopathy with long term consequences for infants. Here, degraded spectral and temporal responses to sounds were recorded from neurons in the primary auditory cortex (A1) of adult rats exposed to asphyxia at birth. Response onset latencies and durations were increased. Response amplitudes were reduced. Tuning curves were broader. Degraded successive-stimulus masking inhibitory mechanisms were associated with a reduced capability of neurons to follow higher-rate repetitive stimuli. The architecture of peripheral inner ear sensory epithelium was preserved, suggesting that recorded abnormalities can be of central origin. Some implications of these findings for the genesis of language perception deficits or for impaired language expression recorded in developmental disorders, such as autism spectrum disorders, contributed to by perinatal asphyxia, are discussed.

In this paper, I shall illustrate the personality of Rita Levi-Montalcini and highlight how she first emerged within a prestigious school, while under the direction of Giuseppe Levi, a most inspiring and stimulating figure. Between 1919 and 1938, he created a rich environment capable of influencing the minds of innumerable students; among them, three future Nobel Prize winners, Renato Dulbecco, Salvador Luria, and Rita Levi-Montalcini, who were companions in the same classroom. In 1938, due to the racial laws for the defence of the race, Luria and Levi-Montalcini were banned from entering the university premises. In a small working space in her bedroom—“a minuscule laboratory not unlike a convent cell”—Levi-Montalcini, supported by Levi, made a most outstanding discovery that opened a new chapter in neurobiology. Further collaborations with Viktor Hamburger and Stanley Cohen led Levi-Montalcini to the discovery of the nerve growth factor (NGF): a remarkable accomplishment which turned out to represent a milestone in the development of modern cell biology.

Abstract

[Importance] About 20% to 30% of people with schizophrenia have psychotic symptoms that do not respond adequately to first-line antipsychotic treatment. This clinical presentation, chronic and highly disabling, is known as treatment-resistant schizophrenia (TRS). The causes of treatment resistance and their relationships with causes underlying schizophrenia are largely unknown. Adequately powered genetic studies of TRS are scarce because of the difficulty in collecting data from well-characterized TRS cohorts., [Objective] To examine the genetic architecture of TRS through the reassessment of genetic data from schizophrenia studies and its validation in carefully ascertained clinical samples., [Design, Setting, and Participants] Two case-control genome-wide association studies (GWASs) of schizophrenia were performed in which the case samples were defined as individuals with TRS (n = 10 501) and individuals with non-TRS (n = 20 325). The differences in effect sizes for allelic associations were then determined between both studies, the reasoning being such differences reflect treatment resistance instead of schizophrenia. Genotype data were retrieved from the CLOZUK and Psychiatric Genomics Consortium (PGC) schizophrenia studies. The output was validated using polygenic risk score (PRS) profiling of 2 independent schizophrenia cohorts with TRS and non-TRS: a prevalence sample with 817 individuals (Cardiff Cognition in Schizophrenia [CardiffCOGS]) and an incidence sample with 563 individuals (Genetics Workstream of the Schizophrenia Treatment Resistance and Therapeutic Advances [STRATA-G])., [Main Outcomes and Measures] GWAS of treatment resistance in schizophrenia. The results of the GWAS were compared with complex polygenic traits through a genetic correlation approach and were used for PRS analysis on the independent validation cohorts using the same TRS definition., [Results] The study included a total of 85 490 participants (48 635 [56.9%] male) in its GWAS stage and 1380 participants (859 [62.2%] male) in its PRS validation stage. Treatment resistance in schizophrenia emerged as a polygenic trait with detectable heritability (1% to 4%), and several traits related to intelligence and cognition were found to be genetically correlated with it (genetic correlation, 0.41-0.69). PRS analysis in the CardiffCOGS prevalence sample showed a positive association between TRS and a history of taking clozapine (r2 = 2.03%; P = .001), which was replicated in the STRATA-G incidence sample (r2 = 1.09%; P = .04)., [Conclusions and Relevance] In this GWAS, common genetic variants were differentially associated with TRS, and these associations may have been obscured through the amalgamation of large GWAS samples in previous studies of broadly defined schizophrenia. Findings of this study suggest the validity of meta-analytic approaches for studies on patient outcomes, including treatment resistance.

We sought to evaluate the prognostic role of cardiac magnetic resonance (CMR) in patients with definite, borderline and possible diagnosis of arrhythmogenic right ventricular cardiomyopathy (ARVC) as defined by the International Task Force (TF) in 2010. CMR was performed in 175 patients: 52 with definite, 50 with borderline and 73 possible ARVC. Abnormal-CMR was defined as the presence of ≥1 CMR abnormalities (including abnormalities of right ventricular and left ventricular wall motion, fat infiltration, late gadolinium enhancement, dilation and dysfunction of either ventricles). During the follow-up time 35 patients had hard cardiac events (sudden cardiac death, appropriate implantable cardioverter defibrillator shock and resuscitated cardiac arrest), and 34 of them occurred in patients with abnormal-CMR (negative predictive value = 96.9%). At the multivariate Cox-regression analysis LV involvement at CMR (fat infiltration and/or late gadolinium enhancement), and episode of nonsustained ventricular tachycardia (NSVT) were independent predictors of cardiac events in both the whole population (LV involvement: HR 3.69, 95% CI 1.57-8.65, p = 0.0002; NSVT: HR 5.8, 95% CI 2.82-11.9, p < 0.0001), and in the group of patients with definite ARVC (LV involvement: HR 3.03, 95% CI 1.15 to 8.02, p = 0.02; NSVT: HR 12.1, 95% CI 4.02-36.5, p < 0.0001). In conclusion, CMR evidence of LV involvement is a strong independent predictor of cardiac events in patients with definite, borderline or possible ARVC diagnosis. Abnormal CMR has very high negative predictive value for hard cardiac events

We determined total Purkinje cell (PC) numbers in cerebella of wild-type (+/+) and heterozygous (rl/+) reeler mice of either sex during early postnatal development; in parallel, we quantified levels of neuroactive steroids in the cerebellum with mass spectrometry. We also quantified reelin mRNA and protein expression with RT-PCR and Western blotting. PC numbers are selectively reduced at postnatal day 15 (P15) in rl/+ males in comparison to +/+ males, +/+ females, and rl/+ females. Administration of 17β-estradiol (17β-E) into the cisterna magna at P5 increases PC numbers in rl/+ males, but not in the other groups; conversely, estrogen antagonists 4-OH-tamoxifen or ICI 182,780 reduce PC numbers in +/+ and rl/+ females, but have no effect in males. Testosterone (T) levels at P5 are much higher in males than in females, reflecting the perinatal testosterone surge in males. In addition, rl/+ male cerebella at P5 show a peculiar hormonal profile in comparison with the other groups, consisting of increased levels of T and 17β-E, and decreased levels of dihydrotestosterone. RT-PCR analysis indicated that heterozygosity leads to a 50% reduction of reelin mRNA in the cerebellum in both sexes, as expected, and that 17β-E upregulates reelin mRNA, particularly in rl/+ males; reelin mRNA upregulation is associated with an increase of all major reelin isoforms. These effects may represent a novel model of how reelin deficiency interacts with variable perinatal levels of neuroactive steroids, leading to gender-dependent differences in genetic vulnerability.
