Abstract

Background and purpose: There are different criteria for the diagnosis of different variants of chronic inflammatory demyelinating polyradiculoneuropathy (CIDP). The 2021 European Academy of Neurology/Peripheral Nerve Society (EAN/PNS) guidelines provide specific clinical criteria for each CIDP variant even if their therapeutical impact has not been investigated. Methods: We applied the clinical criteria for CIDP variants of the 2021 EAN/PNS guidelines to 369 patients included in the Italian CIDP database who fulfilled the 2021 EAN/PNS electrodiagnostic criteria for CIDP. Results: According to the 2021 EAN/PNS clinical criteria, 245 patients achieved a clinical diagnosis of typical CIDP or CIDP variant (66%). We identified 106 patients with typical CIDP (29%), 62 distal CIDP (17%), 28 multifocal or focal CIDP (7%), four sensory CIDP (1%), 27 sensory-predominant CIDP (7%), 10 motor CIDP (3%), and eight motor-predominant CIDP (2%). Patients with multifocal, distal, and sensory CIDP had milder impairment and symptoms. Patients with multifocal CIDP had less frequently reduced conduction velocity and prolonged F-wave latency and had lower levels of cerebrospinal fluid protein. Patients with distal CIDP more frequently had reduced distal compound muscle action potentials. Patients with motor CIDP did not improve after steroid therapy, whereas those with motor-predominant CIDP did. None of the patients with sensory CIDP responded to steroids, whereas most of those with sensory-predominant CIDP did. Conclusions: The 2021 EAN/PNS criteria for CIDP allow a better characterization of CIDP variants, permitting their distinction from typical CIDP and more appropriate treatment for patients.

Abstract

Background To assess the ability of the 2021 European Academy of Neurology/Peripheral Nerve Society (EAN/PNS) clinical criteria for chronic inflammatory demyelinating polyradiculoneuropathy (CIDP) to include within their classification the whole spectrum of clinical heterogeneity of the disease and to define the clinical characteristics of the unclassifiable clinical forms. Methods The 2021 EAN/PNS clinical criteria for CIDP were applied to 329 patients fulfilling the electrodiagnostic (and in some cases also the supportive) criteria for the diagnosis of CIDP. Clinical characteristics were reviewed for each patient not strictly fulfilling the clinical criteria ('unclassifiable'). Results At study inclusion, 124 (37.5%) patients had an unclassifiable clinical presentation, including 110 (89%) with a typical CIDP-like clinical phenotype in whom some segments of the four limbs were unaffected by weakness ('incomplete typical CIDP'), 10 (8%) with a mild distal, symmetric, sensory or sensorimotor polyneuropathy confined to the lower limbs with cranial nerve involvement ('cranial nerve predominant CIDP') and 4 (1%) with a symmetric sensorimotor polyneuropathy limited to the proximal and distal areas of the lower limbs ('paraparetic CIDP'). Eighty-one (65%) patients maintained an unclassifiable presentation during the entire disease follow-up while 13 patients progressed to typical CIDP. Patients with the unclassifiable clinical forms compared with patients with typical CIDP had a milder form of CIDP, while there was no difference in the distribution patterns of demyelination. Conclusions A proportion of patients with CIDP do not strictly fulfil the 2021 EAN/PNS clinical criteria for diagnosis. These unclassifiable clinical phenotypes may pose diagnostic challenges and thus deserve more attention in clinical practice and research.

Contains fulltext : 153534.pdf (Publisher’s version ) (Open Access) INTRODUCTION: Adults with end-stage kidney disease (ESKD) treated with haemodialysis experience mortality of between 15% and 20% each year. Effective interventions that improve health outcomes for long-term dialysis patients remain unproven. Novel and testable determinants of health in dialysis are needed. Nutrition and dietary patterns are potential factors influencing health in other health settings that warrant exploration in multinational studies in men and women treated with dialysis. We report the protocol of the "DIETary intake, death and hospitalisation in adults with end-stage kidney disease treated with HaemoDialysis (DIET-HD) study," a multinational prospective cohort study. DIET-HD will describe associations of nutrition and dietary patterns with major health outcomes for adults treated with dialysis in several countries. METHODS AND ANALYSIS: DIET-HD will recruit approximately 10,000 adults who have ESKD treated by clinics administered by a single dialysis provider in Argentina, France, Germany, Hungary, Italy, Poland, Portugal, Romania, Spain, Sweden and Turkey. Recruitment will take place between March 2014 and June 2015. The study has currently recruited 8000 participants who have completed baseline data. Nutritional intake and dietary patterns will be measured using the Global Allergy and Asthma European Network (GA(2)LEN) food frequency questionnaire. The primary dietary exposures will be n-3 and n-6 polyunsaturated fatty acid consumption. The primary outcome will be cardiovascular mortality and secondary outcomes will be all-cause mortality, infection-related mortality and hospitalisation. ETHICS AND DISSEMINATION: The study is approved by the relevant Ethics Committees in participating countries. All participants will provide written informed consent and be free to withdraw their data at any time. The findings of the study will be disseminated through peer-reviewed journals, conference presentations and to participants via regular newsletters. We expect that the DIET-HD study will inform large pragmatic trials of nutrition or dietary interventions in the setting of advanced kidney disease.

Abstract

Summary: Pseudomonas corrugataCFBP 5454 produces two kinds of cyclic lipopeptides (CLPs), cormycin A and corpeptins, both of which possess surfactant, antimicrobial and phytotoxic activities. In this study, we identified genes coding for a putative non-ribosomal peptide synthetase and an ABC-type transport system involved in corpeptin production. These genes belong to the same transcriptional unit, designated crpCDE. The genetic organization of this locus is highly similar to other PseudomonasCLP biosynthetic clusters. Matrix-assisted laser desorption ionization-time of flight-mass spectrometry (MALDI-TOF-MS) analysis revealed that transporter and synthetase genomic knock-out mutants were unable to produce corpeptins, but continued to produce cormycin A. This suggests that CrpCDE is the only system involved in corpeptin production in P.corrugataCFBP 5454. In addition, phylogenetic analysis revealed that the CrpE ABC transporter clustered with the transporters of CLPs with a long peptide chain. Strains depleted in corpeptin production were significantly less virulent than the wild-type strain when inoculated in tomato plants and induced only chlorosis when infiltrated into Nicotiana benthamiana leaves. Thus, corpeptins are important effectors of P.corrugata interaction with plants. Expression analysis revealed that crpC transcription occurs at high cell density. Two LuxR transcriptional regulators, PcoR and RfiA, have a pivotal role in crpC expression and thus in corpeptin production.

Abstract

Xanthomonas campestris pv. campestris (Xcc) is the causal agent of black rot in Brassicaceae. It is widespread in Italy and severe outbreaks occur under conditions that favour disease development. In this study a multilocus sequence typing approach (MLST) based on the partial sequence of seven loci was applied to a selection of strains representative of the main areas of cultivation and hosts. The aim was to investigate whether the long tradition of brassica crops in Italy has influenced the evolution of different Xcc populations. All loci were polymorphic; 14 allelic profiles were identified of which 13 were unique to Italian strains. Based on the seven loci, the most common genotype within the Italian Xcc strains (AP1) was also the most representative genotype found in worldwide Xcc strains. This genotype was included in a new clonal complex in addition to three other clonal complexes already identified in Xcc populations. The phylogenetic reconstruction using a concatenated dataset of four conserved protein‐coding genes, dnaK, fuyA, gyrB and rpoD, showed that the Italian strains belonged to two genetic groups. Physiological races were also investigated for the first time in Italy. The race structure of Xcc was determined by inoculating eight differential Brassica lines belonging to five species and showed that, in Italy, race 4 is the most widespread, followed by races 1 and 6. No correlation was found between allelic profiles, host of isolation, geographical origin and races, although a prevalent race was identified within the same clonal complex. [ABSTRACT FROM AUTHOR]

Abstract

In the developed world, there comes a point in a person’s life when it is socially accepted that they should no longer be required to earn an income through personal exertion — generally quantitatively determined by their age. The level of support an older person receives is often correlated with the economic stability of the jurisdiction in which they reside. This support can range from basic services through to modest level pensions and healthcare. All support is funded by government revenue (i.e. taxes). Such revenue is predominately derived through taxpayers and, most notably, the working population. A large issue affecting countries globally is that of aging populations. Statistically, older persons are considered to be those over the age of 60. Aging populations are a direct result of increased mortality rates followed by reductions in fertility rates. The financial impact of aging populations is that a large network of people finds themselves being supported by a much smaller network. This places a greater burden on the younger population and risks a lower standard of living for older people.To combat the economic and social risks associated with an aging population, many countries over the past decade have implemented significant pension reforms which have included increasing age requirements for pension benefits, changing the way in which entitlements are calculated and introducing compulsory savings. The World Bank’s leading involvement in pension reform, globally, has identified that the main objectives of a pension system continues to be poverty alleviation and consumption smoothing — beneath the umbrella of social protection. This paper reviews each comparator country’s retirement income system using the World Banks Pension Conceptual Framework. It then considers each country’s system in terms such as adequacy, affordability, sustainability, equitability, predictability and robustness.

Abstract

RNA interference (RNAi) is a powerful strategy for gene function analysis, and it is also widely studied in view of a promising use in pest control. The red palm weevil, Rhynchophorus ferrugineus (Oliver) (Coleoptera: Curculionidae), is one of the most devastating pests of palm in the world. Conventional pest management practices are not adequate to control this insect, thus the development of efficient approaches with minimal environmental impact are needed. In this work, the potential of RNAi in R. ferrugineus has been investigated through the silencing of three different genes (α-amylase, V-ATPase, Ecdysone receptor). For each gene we tested two different doses (1,500 and 5,500 ng) and two delivery techniques (injection and ingestion), evaluating both gene knockdown and mortality on insects. Results show that RNAi mediated gene silencing in R. ferrugineus varies from gene to gene, and that the response is dose-dependent, with stronger effects when dsRNA was administered by injection. In parallel, the same study was carried out with the model organism Tribolium castaneum Herbst (Coleoptera: Tenebrionidae), with results showing a different pattern of response, although the two insects belong to the same order.

Abstract

An ABC (ATP-binding cassette) transport system consisting of two genes and a truncated ORF encoding part of a putative non ribosomal peptide synthetase (NRPS) were identified in a cosmid insert of a genomic library of Pseudomonas corrugata, downstream of the PcoI/PcoR quorum sensing system and the transcriptional regulator rfiA. The genetic organization of this locus shows high similarity with other Pseudomonas cyclic lipopeptide (CLP) biosynthetic clusters. MALDI-TOF MS analysis of cultural filtrates showed that the wild type strain produced two kind of CLPs, cormycin A and corpeptins, whereas derivative mutants of the ABC transport system and NRPS produced only cormycin, thus suggesting that these genes are part of the corpeptin biosynthesis cluster. Moreover, RT-PCR with two set of primers overlapping the three genes demonstrated that they work as an operon. Quantitative real-time RT-PCR showed the presence of NRPS gene transcript in the transporter mutant revealing that corpeptins are still produced via NRPS but not released outside the cell, whereas in the NRPS mutant strain both transporter and synthetase transcripts were undetectable. Time-course analysis of the WT strain revealed that corpeptin NRPS is best expressed at high cell density (108 CFU ml-1), and that the expression could be anticipated by addition of exogenous AHL signals. No expression was detected in the pcoR and rfiA transcriptional regulator mutants. CLPs were undetectable both in the culture filtrates of these mutants and in the WT before the achievement a quorum, suggesting a similar regulation of the cormycin production. Abstract

Huanglongbing, the most destructive citrus disease, is induced by three Gram-negative, phloem-limited bacteria, 'Candidatus Liberibacter asiaticus', 'Ca. L. africanus' and 'Ca. L. americanus', which are spread by propagative material and the Asiatic and African citrus psyllids, and are listed as A1 quarantine pests by EPPO and in most of the NPPOs worldwide. Because of the potential risk of introduction of the pathogen in the Mediterranean area and in Sicily, where more than 90,000 ha of citrus are grown, the standardization of laboratory procedures to recognize symptoms, perform sampling and detection has been foreseen within the framework of a project on citrus harmful parasites supported by the Regione Sicilia. Thanks to an agreement with the National Center for Citrus Improvement, Hunan University of Agriculture, Changsha, the plant material for such study was sampled during a survey in Guangdong province (China). Young shoots of different trees showing HLB-like symptoms were partially processed in the farms. The leaf midribs of the young fully expanded leaves and the bark stripped from the shoots were comminuted and impressed both on FTA Plant Cards and on Whatman 3MM filter paper or were transferred to the Chinese laboratory and the DNAs extracted. Membranes and DNAs were divided among the three laboratories to detect the pathogen by PCR and/or real-time PCR. Ca. L. asiaticus was detected and identified in all samples showing leaf mottle and in some of those showing nutritional problems. Results of the three laboratories confirm that the standardization of procedures is a prerequisite for a proper detection of this harmful organism. [ABSTRACT FROM AUTHOR]

Abstract

Glutathione transferases (GSTs) mainly catalyze the nucleophilic addition of glutathione to a large variety of hydrophobic molecules participating to the vacuole compartmentalization of many toxic compounds. In this work, the putative tolerance of transgenic tobacco plants over-expressing CsGSTU genes towards the chloroacetanilide herbicide alachlor was investigated. Our results show that the treatment with 0.0075 mg cm-3 of alachlor strongly affects the growth of both wild type and transformed tobacco seedlings with the sole exception of the transgenic lines overexpressing CsGSTU2 isoform that are barely influenced by herbicide treatment. In order to correlate the in planta studies with enzyme properties, recombinant CsGSTs were in vitro expressed and tested for GST activity using alachlor as substrate. The recombinant GSTU2 enzyme was twice more active than GSTU1 in conjugating alachlor to GSH thus indicating that CsGSTU2 might play a crucial role in the plant defense against the herbicide. Moreover, as a consequence of the infiltration with a bacterial suspension of the P. syringae pv. tabaci, transgenic tobacco plants but not wild type plants bestowed the capability to limit toxic metabolite diffusion through plant tissues as indicated by the absence of chlorotic halos formation. Consequently, the transgenic tobacco plants described in the present study might be utilized for phytoremediation of residual xenobiotics in the environment and might represent a model for engineering plants that resist to pathogen attack.

Abstract

Accurate and sensitive detection of Citrus Huanglongbing associated ‘Ca. Liberibacter’species, not currently reported in the European and Mediterranean area, is an imperative need to define certification programs, to prevent introduction of the bacteria and/or their vectors in the unaffected areas, and to delineate efficient management strategies in those areas where the disease has spread. In this study, we compared different citrus sample preparation procedures for PCR based detection methods of ‘Candidatus Liberibacter asiaticus’, in order to find out the best a way to transport and preserve samples of Shatangju mandarin and fingered citron obtained during a survey in citrus orchards in Guangdong in 2012. Results show that silica gel dehydrated midrib and bark citrus samples are amenable for DNA extraction and further processing by PCR and real-time PCR. FTA plant collecting card were useful for easily collect DNA samples in the field and processing by real-time PCR although at a lower concentration than extracted DNAs.

Abstract

The occurrence of interaction between insects and fungi is interesting from an ecological point of view, particularly when these interactions involve insect pests and plant pathogens within an agroecosystem. In this study, we aimed to perform an accurate analysis on the fungal microbiota associated to Bactrocera oleae (Rossi) through a metabarcoding approach based on 454 pyrosequencing. From this analysis, we retrieved 43,549 reads that clustered into 128 operational taxonomic units (OTUs), of which 29 resulted in the “core” associate fungi of B. oleae. This fungal community was mainly represented by sooty mould fungi, such as Cladosporium spp., Alternaria spp. and Aureobasidium spp., by plant pathogens like Colletotrichum spp. and Pseudocercospora spp., along with several other less abundant taxa whose ecology is unclear in most of the cases. Our findings lead to new insights into the microbial ecology of this specific ecological niche, enabling the understanding of a complex network of interactions within the olive agroecosystem.

Abstract

Clavibacter michiganensis subsp. michiganensis (Cmm) strains, collected in greenhouses from 17 farms during tomato bacterial canker outbreaks occurring between 2005 and 2008 in Sicily, were analysed by a multiphasic approach. Population studies were conducted to investigate the possible sources of inocula. Cmm strains were characterized by PCR assays targeting virulence genes, fingerprinting techniques, metabolic profiles and virulence. These strains were comparatively analysed with Cmm strains isolated in other parts of Italy over a period of 15 years. Chromosomal genes encoding virulence determinants tomA, ppaA, chpC, and the plasmid-encoded genes pat-1 and celA were detected by PCR in all tested strains, except for four Sicilian Cmm strains where the pat-1 gene was not amplified. Using BOX-PCR, Cmm strains were differentiated into 13 haplotypes and clonal populations were identified. Cmm strains isolated from different farms in 2008 showed the same BOX-PCR haplotype. A distinct BOX-PCR haplotype was obtained from atypical Cmm strains lacking pat-1 and isolated in 2006/7 from three farms. Cmm strains with two different haplotypes were detected in one farm, whereas the other farms contained strains with only a single haplotype. A new fAFLP protocol based on the amplification of ApaI/MseI fragments was developed and was able to differentiate C. michiganensis subspecies. Different populations were delineated for the multiple outbreaks occurring in Sicily, whereas similar populations were recorded in other Italian regions over a period of 12 years. The results are consistent with previous studies that demonstrate that Cmm outbreaks are associated with propagation material.
