Abstract

Despite the initial hope inspired by the 2015 Paris Agreement, the world is now dangerously close to breaching its target of limiting global multiyear mean heating to 1·5°C. Annual mean surface temperature reached a record high of 1·45°C above the pre-industrial baseline in 2023, and new temperature highs were recorded throughout 2024. The resulting climatic extremes are increasingly claiming lives and livelihoods worldwide.

Abstract

BackgroundImpaired left ventricular diastolic filling is common in chronic thromboembolic pulmonary hypertension (CTEPH), and recent studies support left ventricular underfilling as a cause. To investigate this further, we assessed left atrial volume index (LAVI) in patients with CTEPH before and after pulmonary thromboendarterectomy (PTE).MethodsForty-eight consecutive CTEPH patients had pre- & post-PTE echocardiograms and right heart catheterizations. Parameters included mean pulmonary artery pressure (mPAP), pulmonary vascular resistance (PVR), cardiac index, LAVI, & mitral E/A ratio. Echocardiograms were performed 6 ± 3 days pre-PTE and 10 ± 4 days post-PTE. Regression analyses compared pre- and post-PTE LAVI with other parameters.ResultsPre-op LAVI (mean 19.0 ± 7 mL/m2) correlated significantly with pre-op PVR (R = -0.45, p = 0.001), mPAP (R = -0.28, p = 0.05) and cardiac index (R = 0.38, p = 0.006). Post-PTE, LAVI increased by 18% to 22.4 ± 7 mL/m2 (p = 0.003). This change correlated with change in PVR (765 to 311 dyne-s/cm5, p = 0.01), cardiac index (2.6 to 3.2 L/min/m2, p = 0.02), and E/A (.95 to 1.44, p = 0.002).ConclusionIn CTEPH, smaller LAVI is associated with lower cardiac output, higher mPAP, and higher PVR. LAVI increases by ~20% after PTE, and this change correlates with changes in PVR and mitral E/A. The rapid increase in LAVI supports the concept that left ventricular diastolic impairment and low E/A pre-PTE are due to left heart underfilling rather than inherent left ventricular diastolic dysfunction.

4. Development and optimisation of an immunological and cellular toolkit for pollutant biosensing

Abstract

Anti-atrazine and anti-mecoprop single chain antibody fragments (scAbs) were cloned from monoclonal cell lines and expressed in Escherichia coli. The anti-atrazine and anti-mecoprop scAbs showed the same specificity for atrazine and mecoprop respectively as the parental mAb, but showed a 2-3 fold increase in sensitivity for free atrazine. Antibody fragments against mecoprop and diuron were stabilised by the introduction of inter domain disulphide bonds and were capable of remaining more than 90% functional in 10% methanol and other harsh denaturing conditions. BIAcore analysis and competition ELISAs confirmed the stabilised antibodies (stAbs) bound to free pesticide with the same affinity and specificity as the scAb fragments. Lux marked bacteria were used to predict the toxicity of 7 pesticides over a range of concentrations in aqueous solutions and finally in aqueous and non aqueous fractions extracted from soil. The toxicity of pesticide mixtures was further investigated and synergistic interactions identified. Lux marked E. coli HB101 cells and 4 anti-pesticide stAbs were used as a tool-kit to measure firstly toxicity and secondly to quantify the pesticides present in both the aqueous and non aqueous fractions of soil. The E. coli HB101 biosensor was able to detect pesticides from soil at 10-100 times lower concentrations than would have had an effect in water, showing improved sensitivity. The pesticide concentration solvent extracted soil was analysed using either stab fragment in an indirect competition ELISA or by HPLC and in each case the results were almost identical. This toolkit combines the specificity of stAb antibodies, as a rapid and robust alternative to chemical analysis, with toxicity assessment of lux biosensors as a safety net to identity presence of unknown toxins and toxin mixtures in the field.

8. An Evaluation of a Community Health Intervention Programme Aimed at Improving Health and Wellbeing

Abstract

Objective: The objective of this evaluation was to examine the extent to which participants in the Tailor Made Leisure Package programme experienced any improvement in their health and wellbeing. Design: A quantitative survey. Setting: The Healthy Living Centre initiative is an example of a community-based intervention which was formalized as part of the British government's health policy. The purpose of this initiative is to reduce inequalities in health and promote good health and wellbeing. The programme was offered by a Healthy Living Centre located in Scotland. Method: The benefits of the initiative were investigated through the use of a postal questionnaire which was used to examine the perceived benefits of the Tailor Made Leisure Package programme. This was distributed to a total of 177 people who had applied to take part in the programme during the period February 2004 to February 2005. Results: Of the 177 people surveyed 90 returned completed questionnaires giving a response rate of 51 per cent. Of these, 70 people started the programme. Those who started the programme took part in a variety of activities including the gym, GP Referral programme and complementary therapies. The reported benefits were that individuals felt fitter and healthier, they were taking part in additional leisure activities and that they were socializing more frequently. Those participants who had complementary therapy reported a relief of symptoms associated with their particular health condition. All of these benefits are important in the process of improving health and wellbeing. The 20 individuals who did not start their programme reported that personal circumstances, health reasons, transport problems, financial difficulties and the activities/programme being inappropriate were the main reasons for not starting the programme. Conclusion: The results of the evaluation suggest that an initiative such as this is worthwhile in promoting positive health and wellbeing. (Contains 4 tables.)

Abstract

The COVID-19 pandemic triggered a migration of workforces to work from home. A key issue for academics was the implications for the ability to carve out ‘thinking time’ to engage in what we term sustained knowledge work, the type of work essential for producing research. We administered an employee survey to academics from seven Australian and seven Canadian Universities, receiving over 3000 responses. We report on both quantitative and qualitative findings from the survey, with a particular emphasis on the latter. The two countries displayed broadly similar patterns in responses, but these patterns were gendered in specific ways. We distinguished between episodic and sustained knowledge work and found the shift of the location for sustained knowledge work from the workplace to the home affected academics unevenly, with disproportionate negative impacts on women. There are implications for all knowledge workers: while gendered, domestic norms continue to exist, the sustained knowledge work that is critical to career advancement can become especially problematic for women knowledge workers.

Abstract

To determine the extent of exposure to Zika virus (ZIKV) and chikungunya virus (CHIKV) in Jamaica, we collected serum from 584 pregnant women during 2017-2019. We found that 15.6% had antibodies against ZIKV and 83.6% against CHIKV. These results indicate potential recirculation of ZIKV but not CHIKV in the near future.

Abstract

Objective: Gastroesophageal reflux disease (GERD) is one of the most common conditions seen in general medical practice, which is associated with an impaired quality of life. This study determined the prevalence of GERD symptoms in a Jamaican population Methods: Eligible individuals between the ages of 18 and 75 years were invited to complete a questionnaire, which included demographic data as well as the frequency and severity of typical GERD symptoms. Results: The questionnaire was completed by 422 subjects with 51% males and 49% females. The prevalence of any GERD symptoms within the previous year was 71.1%. Females were more frequently affected, 75.7%, compared to 67.3% of males. Weekly symptoms were present in 18.6%. Symptoms of moderate or greater severity were reported in 11.7%. The most common and bothersome symptom experienced was heartburn. Nocturnal symptoms that awaken the affected subjects occurred in 17.8%. Gastroesophageal reflux disease was strongly associated with food, supine posture and heavy meals (p < 0.0001). There was no significant association with GERD and age or body mass index. In persons with GERD symptoms, 24.2% (p = 0.000) saw a doctor for their symptoms and 38.6% took medication for their symptoms (p < 0.0001), which included prescription medications in 42.7%, over the counter medications in 36.3% and combination of both in 15.3%. Conclusion: Symptoms of GERD are common and significant problem in the Jamaican population studied. Heartburn was the most bothersome symptom reported. Abstract

In recent years a new type of teaching-focused academic position has emerged in the university system, the ‘Scholarly Teaching Fellow’ (STF). These continuing positions are designed to replace casual teachers, and to enable a more ‘sustained’ engagement with scholarship as required under Commonwealth higher education standards. There has been agrowing reliance on casual academics to deliver university courses, and the rise of the ‘gig’ academic has undermined scholarship as well as job security. In 2012 the sector’s lead trade union, the National Tertiary Education Union (NTEU), proposed a novel approach to extend teaching capacity and provide job security for a portion of the estimated 50,000 casual university teaching staff. By creating a career path for the casual academics who currently perform the bulk of face-to-face teaching in Australian universities, it was envisaged that not only would casual academics benefit from enhanced job security, but that the employment ofmore continuing staff would improve teaching and learning and enhance the student experience. Between 2012 and 2015, industrial agreements negotiated between the NTEU and the majority of the sector’s universities led to a commitment to create 850 positions for a new type of academic role: the Scholarly Teaching Fellow (STF). Around 800 of these positions had been created by August 2018. This research, funded as a ‘Strategic Project’ by the former Office of Learning and Teaching, examined the introduction of STFs into the Australian university system between 2013and 2016. The project explored the impact of this new category of employment in Australian universities on the organisation and future prospects of academic work.

Summary: In the preclinical sciences, statistically significant predictive values have been reported between the performances in one discipline and the others, supporting the hypothesis that students who perform well in one discipline were likely to perform well in the other disciplines. We therefore decided to conduct a retrospective study to investigate the predictive effects of preclinical subjects on clinical subjects from 87 students of The University of the West Indies (UWI), Mona Campus who took the MBBS Stage II examination at various times between May 2000 and May 2002. The grade in Pathology was significantly predicted by scores in Anatomy and Pharmacology; Medicine by Physiology and Pharmacology scores; Surgery by Anatomy and Social and Preventive Medicine scores; while, the Obstetrics and Gynecology grade was predicted by the Anatomy score. The results support the hypothesis that the scores in some preclinical subjects can predict the performance in specific clinical subjects, which could be interpreted to suggest that poor performance in specific preclinical disciplines could be a warning sign of future poor performance in the related clinical disciplines.Keywords: Medical education, preclinical grades, clinical grades, predictors of performance

30. Acute Pyelonephritis in Pregnancy: A Retrospective Descriptive Hospital Based-Study

Introduction. Pyelonephritis is a common complication of pregnancy. It is also exacerbated by immunocompromised states and also the sickle cell gene. We reviewed this condition in Jamaican women. Method. We did a six year hospital database docket review. We found 102 confirmed cases. Results. Pyelonephritis was found in 0.7% of deliveries. The mean maternal age was years with 51% primiparity. Most (58.8%) occurred in the second trimester. The main symptoms were loin pain (96.2%) and abdominal pain (84.6%). It was more common on the right side in 67% of cases. On urinalysis, 81.4% had pyuria. The commonest organism was Escherichia coli, in 61% of cases. Patients given Antibiotics prior to admission had quicker resolution, . Haemoglobin S was found in 16% cases (general population 10%; . However diabetes was only found in 1.3% cases (1.5% expected). 61.3% had positive urine culture after treatment showed that 61.3% and 25% had recurrent pyelonephritis. Complications included 32% threatened preterm labour and 17% preterm delivery. About 6% of neonates had intrauterine growth restriction. There were no ICU admissions and no deaths. Conclusion. Early recognition and treatment of pyelonephritis result in good outcome. The condition is more prevalent in patients with the sickle cell gene and recurrence is high.

Abstract

Diversity management is recognised as a major challenge for organizations throughout the world. There is broad acceptance that when it comes to all aspects of workforce management major differences exist among individuals in terms of age, gender, national origin, physical capability, sexuality, religion and others. This chapter discusses the concept, meaning and application of managing that difference or ‘diversity’ through programs known as diversity management. It identifies and discusses the different contextual and theoretical approaches that frame diversity programs found in organizations today. A number of programs within different country contexts are examined. The discussion examines the challenges of diversity management its programs and its outcomes with a view to understanding the lessons learned and recommending future directions.

Abstract

Issues of equity and inequity have always been part of employment relations and are a fundamental part of the industrial landscape. For example, in most countries in the nineteenth century and a large part of the twentieth century women and members of ethnic groups (often a minority in the workforce) were barred from certain occupations, industries or work locations, and received less pay than the dominant male ethnic group for the same work. In recent decades attention has been focused on issues of equity between groups, predominantly women and different ethnic groups in the workforce. This has been embodied in industrial legislation, for example in equal pay for women and men, and frequently in specific equity legislation. In this way a whole new area of law and associated workplace practice has developed in many countries. Historically, employment relations and industrial relations research has not examined employment issues disaggregated by gender or ethnic group. Born out of concern with conflict and regulation at the workplace, studies tended to concentrate on white, male, unionized workers in manufacturing and heavy industry (Ackers, 2002, p. 4).
