1. Edge-based Modeling for Disease Transmission on Random Graphs: An Application to Mitigate a Syphilis Outbreak

Abstract: Edge-based network models, especially those based on bond percolation methods, can be used to model disease transmission on complex networks and accommodate social heterogeneity while keeping tractability. Here we present an application of an edge-based network model to the spread of syphilis in the Kingston, Frontenac and Lennox & Addington (KFL&A) region of Southeastern Ontario, Canada. We compared the results of using a network-based susceptible-infectious-recovered (SIR) model to those generated from using a traditional mass action SIR model. We found that the network model yields very different predictions, including a much lower estimate of the final epidemic size. We also used the network model to estimate the potential impact of introducing a rapid syphilis point of care test (POCT) and treatment intervention strategy that has recently been implemented by the public health unit to mitigate syphilis transmission.

2. Slowing and stopping the speed of sound

Abstract: Temporal interference between direct and surface-reflected paths induces large variation in the group speed of an acoustic signal between a source and receiver. This speed goes to zero when the source and receiver approach one another and are within $c \tilde{\delta t}/2$ of the surface, where $c$ is the in-situ speed of sound and $\tilde{\delta t}$ is the smallest temporal separation between the paths at which interference initiates. At greater depths, the group speed can drop by many orders of magnitude. The effect diminishes far from a receiver as the size of the delay shrinks relative the overall time of propagation. The phenomenon is of great importance for methods designed to locate sounds via time differences of arrival (TDOA) as the group speed between a sound and each receiver may differ by orders of magnitude, a phenomenon that invalidates the geometrical interpretation of location by hyperboloids. Isodiachronic geometries are required to derive valid locations. 3. The role of coal plant retrofitting strategies in developing India's net-zero power system: a data-driven sub-national analysis

Abstract: India set two Nationally Determined Contribution targets to achieve the net zero carbon emission goal by 2070, which requires deep decarbonization of India's power generation sector. Yet, coal power generation contributes to more than 60\% of its total power generation, and policies still permit further coal fleet expansion and lifetime extensions. In this paper, we investigate the role of retrofitting India's coal plants for carbon capture and storage and biomass co-firing in developing the net-zero power system. We model the power generation and transmission network expansions across 30 Indian states in four representative grid evolution scenarios under progressively tighter carbon emission caps, taking into account sub-national coal price variation and thermal efficiency of individual coal plants. We find that coal plant retrofitting could happen by 2035 when an annual carbon cap for the power generation sector is less than 1,000 million tons CO$_2$. This reduces the unabated coal plant capacity, electricity generation, and carbon abatement costs. Exploiting renewable energy potentials solely, such as wind resources, could reduce carbon abatement costs significantly but will result in low coal plant utilization and uneven renewable generation deployment between Southern and Central regions concerning energy justice.

4. A Dataset of the Operating Station Heat Rate for 806 Indian Coal Plant Units using Machine Learning

Abstract: India aims to achieve net-zero emissions by 2070 and has set an ambitious target of 500 GW of renewable power generation capacity by 2030. Coal plants currently contribute to more than 60\% of India's electricity generation in 2022. Upgrading and decarbonizing high-emission coal plants became a pressing energy issue. A key technical parameter for coal plants is the operating station heat rate (SHR), which represents the thermal efficiency of a coal plant. Yet, the operating SHR of Indian coal plants varies and is not comprehensively documented. This study extends from several existing databases and creates an SHR dataset for 806 Indian coal plant units using machine learning (ML), presenting the most comprehensive coverage to date. Additionally, it incorporates environmental factors such as water stress risk and coal prices as prediction features to improve accuracy. This dataset, easily downloadable from our visualization platform, could inform energy and environmental policies for India's coal power generation as the country transitions towards its renewable energy targets.

5. A unifying modelling approach for hierarchical distributed lag models

Abstract: We present a statistical modelling framework for implementing Distributed Lag Models (DLMs), encompassing several extensions of the approach to capture the temporally distributed effect from covariates via regression. We place DLMs in the context of penalised Generalized Additive Models (GAMs) and illustrate that implementation via the R package \texttt{mgcv}, which allows for flexible and interpretable inference in addition to thorough model assessment. We show how the interpretation of penalised splines as random quantities enables approximate Bayesian inference and hierarchical structures in the same practical setting. We focus on epidemiological studies and demonstrate the approach with application to mortality data from Cyprus and Greece. For the Cyprus case study, we investigate for the first time, the joint lagged effects from both temperature and humidity on mortality risk with the unexpected result that humidity severely increases risk during cold rather than hot conditions. 11. Repurposing Coal Power Plants into Thermal Energy Storage for Supporting Zero-carbon Data Centers

Abstract: Coal power plants will need to be phased out and face stranded asset risks under the net-zero energy system transition. Repurposing coal power plants could recoup profits and reduce carbon emissions using the existing infrastructure and grid connections. This paper investigates a retrofitting strategy that turns coal power plants into thermal energy storage (TES) and zero-carbon data centers (DCs). The proposed capacity expansion model considers the co-locations of DCs, local renewablewith the system-generation, andlevel coal retir energy storage ement and retrofitting. We optimize the DC system configurations under the hourly-matching carbon policy and flexible operations. Results show that under hourly-matching carbon constraints, the retrofitted TES could complement the operations of lithium-ion batteries (LIBs) to reduce system costs. This could render DCs with optimal co-located renewable generations and energy storage more cost-effective than unconstrained DCs.

12. Risk of SARS-CoV-2 infection before and after the Omicron wave in a cohort of healthcare workers in Ontario, Canada

Abstract: Abstract Background Longitudinal healthcare worker (HCW) cohorts throughout the COVID-19 pandemic provide a unique opportunity to study the relative contributions of various exposures to infection risk over time. This study aimed to examine how demographic, health, occupational, household and community factors influenced the SARS-CoV-2 infection risk in a cohort of HCWs in Southeastern Ontario, Canada, during the early pandemic and the Omicron waves. We compared the contribution of these factors to infection risk and explored the implications for future epidemic preparedness and the protection of HCWs. Methods We conducted a longitudinal analysis using data from a cohort of HCWs recruited from one acute care hospital and four long-term care homes. The analysis was divided into two periods: the initial phase of the pandemic (period #1) and the first three Omicron waves (period #2). We employed Poisson regression for period #1 and Cox regression for period #2 to examine associations of demographic factors (age, sex, ethnicity, migration status, income insufficiency), health factors (chronic conditions, smoking history, SARS-CoV-2 vaccination status), household factors (exposure to COVID-19), occupational factors (work role, exposure to COVID-19 patients, personal protective equipment access, aerosol-generating procedures) and community exposures (use of masks, distance, hand-washing) with SARS-CoV-2 infection. Results At period #1, 17/208 (8.2%) HCWs reported having had SARS-CoV-2 infection. At period #2, 65/167 (38.3%) reported at least one SARS-CoV-2 infection. In period #1, factors associated with increased risk of infection included working in a long-term care home, exposure to more COVID-19-positive patients, working as a nurse or therapist, and inadequate use of personal protective equipment. In period #2, the hazard of infection was higher among HCWs who had COVID-19-infected children at home, whereas the use of protective measures in the community (maintaining social distance, mask-wearing) and receiving a vaccine booster were associated with reduced risk. Providing care to COVID-19 patients was not associated with the risk of acquiring SARS-CoV-2 infection at period #2. Conclusions During the Omicron wave, community and household exposures, but not occupational exposure to COVID-19 cases, were the primary factors contributing to infection risk in HCWs. This contrasts with the early waves of the pandemic where occupational exposures played a significant role. These findings may be explained by the effectiveness of institutional interventions in reducing the risk of SARS-CoV-2 transmission in healthcare settings, alongside the failure of community-level interventions to mitigate risk during the Omicron period.

13. Student correlates with interprofessional attitudes within undergraduate medical education

Abstract: Abstract Background Interprofessional education (IPE) is defined as students from two or more professions learning together to collaborate and advance health outcomes and is a required component of osteopathic medical education. Factors that influence osteopathic medical students’ (OMS) interprofessional attitudes are currently unknown. This study sought to examine differences in interprofessional attitudes after completing an IPE didactic curriculum and identify specific student or curricular factors correlated with higher interprofessional attitudes. Methods This study utilized a two-phase longitudinal approach. During phase one, a pre/post survey was disseminated to OMS before and after exposure to a standardized IPE didactic curriculum. The survey collected demographic information, previous interprofessional exposures, and baseline interprofessional attitudes using the Interprofessional Attitudes Scale (IPAS). A Wilcoxon signed rank test was used to assess differences in pre/post IPAS scores. For phase two of the study, the survey was additionally disseminated to second, third-, and fourth-year OMS at the end of each academic year to identify student factors associated with higher interprofessional attitudes, including specialty interest and type of core rotation site for third year clinical clerkships and analyzed using a Kruskal Wallis test. Results Over two years, 628 OMS responded to the survey, with 585 providing sufficient information to calculate IPAS scores. Of these 585, responses were from 267 first, 133 second, 84 third, and 65 fourth year students (36 missing). For phase one, 74 of the 267 survey responders completed both the pre and post survey. There was no significant change in IPAS scores following participation in the IPE curriculum (mean change ± standard deviation − 2.22 ± 8.89; p = 0.13). Abstract: Abstract Self-replicating RNA (srRNA) technology, in comparison to mRNA vaccines, has shown dose-sparing by approximately 10-fold and more durable immune responses. However, no improvements are observed in the adverse events profile. Here, we develop an srRNA vaccine platform with optimized non-coding regions and demonstrate immunogenicity and safety in preclinical and clinical development. Optimized srRNA vaccines generate protective immunity (according to the WHO defined thresholds) at doses up to 1,000,000-fold lower than mRNA in female mouse models of influenza and rabies. Clinically, safety and immunogenicity of RBI-4000, an srRNA vector encoding the rabies glycoprotein, was evaluated in a Phase I study (NCT06048770). RBI-4000 was able to elicit de novo protective immunity in the majority of healthy participants when administered at a dose of 0.1, 1, or 10 microgram (71%, 94%, 100%, respectively) in a prime-boost schedule. Similarly, we observe immunity above the WHO benchmark of protection following a single administration in most participants at both 1 and 10 microgram doses. There are no serious adverse events reported across all cohorts. These data establish the high therapeutic index of optimized srRNA vectors, demonstrating feasibility of both low dose and single dose approaches for vaccine applications.

15. User Experience Design in the Information Systems Curriculum: Lessons Learned and Best Practices

Abstract: User Experience Design (UXD) is an often-neglected area of the information systems (IS) curriculum. UXD classes specifically designed for IS students are still uncommon in IS programs and this study aims to add to the body of knowledge to prepare a more well-rounded future generation of IS professionals. With this goal in mind, this study describes the redesign of an introductory UXD course following Kolb's learning cycle and constructivist instructional models. This paper describes the implementation of the supporting pedagogy and the opportunity for students to better master core UXD concepts. UXD, a multi-disciplinary area is built on skills learned in systems analysis and design class while students learn to apply relevant concepts through a hands-on, instructor-led, individual, in-class sample project. The skills are then applied by engaging students in active learning in a team setting to deliver value to a local organization by solving related, real-life challenges. Students work on community-engaged team projects to enhance their appreciation of the impact and relevance of their semester-long project deliverable. Constructivism guides the instructional models of the framework, in which problem-based learning is used to help students build and apply relevant skills. The instructional models and implications for instructional design are discussed along with a proposed pedagogical approach, course setting and structure, tools and techniques engaged, student feedback analyzed, and lessons learned.

16. Challenge or Support? Exploring Academic Burnout in Residential Learning Communities

Abstract: Residential learning communities (RLCs) are on-campus programs aimed at seamlessly blending the in- and out-of-class experiences for college students. Scholars have demonstrated how RLC initiatives are valuable because they build and reinforce an academic environment, and help facilitate related outcomes (e.g., academic transition, GPA, critical thinking). With such an emphasis on aligning academic and residential experiences for students, this article explored if RLC participants were susceptible to higher rates of academic burnout. Using survey data to assess RLC and non-RLC students, we established that participation was not associated with meaningful differences in academic burnout. Conversely, by measuring thriving we also determined that the ways students experience success in RLCs is in fact different than their non-RLC counterparts. Student affairs practitioners can better understand how their students experience burnout when they also understand how their students succeed, as demonstrated in the study's findings. Recommendations to alleviate burnout while promoting success are discussed in light of RLC participation.

Abstract: The Microbicide Trials Network 042 study (MTN-042/DELIVER) is a two-arm, randomized, open-label Phase 3b trial that is evaluating the safety, adherence, and acceptability of the monthly ring and daily oral PrEP among HIV-uninfected pregnant people in four African countries. This analysis focuses on acceptability data captured qualitatively from a subset (n = 48) of the 150 people in the first cohort of the trial who were enrolled in late-stage pregnancy at 36 to 38 weeks gestational age and followed until after delivery. Single IDIs were conducted by trained interviewers at each clinic site using a semi-structured guide. Data excerpts of key codes pertaining to acceptability, pregnancy, and maternal health were summarized, reviewed and interpreted by multinational analyst teams. Although the product use period was relatively short, the data suggested several acceptability findings that may directly translate to longer durations of product use in pregnancy. The first was the overarching maternal sentiment that being able to protect both oneself and their baby was highly valued. The second was the importance of counseling support from providers not only because participants used methods that might generate side effects, but because pregnancy itself is a period with its own set of side effects. The third was that, similar to non-pregnant participants in other trials, here study products were generally liked and described as easy to use. Concerns about ring and oral PrEP use could be addressed with provider counseling and support and should form an essential component rollout among pregnant people.

Abstract: T(8;21)(q22;q22), which generates the AML1-ETO fusion oncoprotein, is a common chromosomal abnormality in acute myeloid leukemia (AML) patients. Despite having favorable prognosis, 40% of patients will relapse, highlighting the need for innovative models and application of the newest technologies to study t(8;21) leukemogenesis. Currently, available AML1-ETO mouse models have limited utility for studying the pre-leukemic stage because AML1-ETO produces mild hematopoietic phenotypes and no leukemic transformation. Conversely, overexpression of a truncated variant, AML1-ETO9a (AE9a), promotes fully penetrant leukemia and is too potent for studying pre-leukemic changes. To overcome these limitations, we devised a germline-transmitted Rosa26 locus AE9a knock-in mouse model that moderately overexpressed AE9a and developed leukemia with long latency and low penetrance. We observed pre-leukemic alterations in AE9a mice, including skewing of progenitors towards granulocyte/monocyte lineages and replating of stem and progenitor cells. Next, we performed single-cell RNA sequencing to identify specific cell populations that contribute to these pre-leukemic phenotypes. We discovered a subset of common myeloid progenitors that have heightened granulocyte/monocyte bias in AE9a mice. We also observed dysregulation of key hematopoietic transcription factor target gene networks, blocking cellular differentiation. Finally, we identified Sox4 activation as a potential contributor to stem cell self-renewal during the pre-leukemic stage.

Although an expansion in $1/N$ corresponds to interactions in the field theory, others, such as in a small breaking of the statistical independence of network parameters, can also lead to interacting theories. These other expansions can be advantageous over the $1/N$-expansion, for example by improved behavior with respect to the universal approximation theorem. Given the connected correlators of a field theory, one can systematically reconstruct the action order-by-order in the expansion parameter, using a new Feynman diagram prescription whose vertices are the connected correlators. This method is motivated by the Edgeworth expansion and allows one to derive actions for neural network field theories. Conversely, the correspondence allows one to engineer architectures realizing a given field theory by representing action deformations as deformations of neural network parameter densities. Abstract: Abstract Background The primary aim of this scoping review was to synthesise key domains and sub-domains described in existing clinical decision support systems (CDSS) implementation frameworks into a novel taxonomy and demonstrate most-studied and least-studied areas. Secondary objectives were to evaluate the frequency and manner of use of each framework, and catalogue frameworks by implementation stage. Methods A scoping review of Pubmed, Scopus, Web of Science, PsychInfo and Embase was conducted on 12/01/2022, limited to English language, including 2000–2021. Each framework was categorised as addressing one or multiple stages of implementation: design and development, evaluation, acceptance and integration, and adoption and maintenance. Key parts of each framework were grouped into domains and sub-domains. Results Of 3550 titles identified, 58 papers were included. The most-studied implementation stage was acceptance and integration, while the least-studied was design and development. The three main framework uses were: for evaluating adoption, for understanding attitudes toward implementation, and for framework validation. The most frequently used framework was the Consolidated Framework for Implementation Research. Conclusions Many frameworks have been published to overcome barriers to CDSS implementation and offer guidance towards successful adoption. However, for co-developers, choosing relevant frameworks may be a challenge. A taxonomy of domains addressed by CDSS implementation frameworks is provided, as well as a description of their use, and a catalogue of frameworks listed by the implementation stages they address. Future work should ensure best practices for CDSS design are adequately described, and existing frameworks are well-validated. An emphasis on collaboration between clinician and non-clinician affected parties may help advance the field.

Abstract: Abstract Background Adaptation of evidence-based interventions (EBIs) often occurs when implemented in new local contexts and settings. It is unclear, however, during which phase of implementation adaptations are most frequently made and how these changes may impact the fidelity, effectiveness, and sustainability of the EBI. Pediatric Early Warning Systems (PEWS) are EBIs for early identification of deterioration in hospitalized children with cancer. This study evaluates adaptations of PEWS made among resource-variable pediatric oncology hospitals in Latin America implementing and sustaining PEWS. Methods We conducted a cross-sectional survey among pediatric oncology centers participating in Proyecto Escala de Valoración de Alerta Temprana (EVAT), a collaborative to implement PEWS. Adaptations to PEWS were assessed via 3 multiple choice and 1 free text question administered as part of a larger study of PEWS sustainability. Descriptive statistics quantitatively described what, when, and why adaptations were made. Qualitative analysis of free text responses applied the Framework for Reporting Adaptations and Modifications Expanded (FRAME) to describe respondent perspectives on PEWS adaptations. Results We analyzed 2,094 responses from 58 pediatric oncology centers across 19 countries in Latin America. Participants were predominantly female (82.5%), consisting of nurses (57.4%) and physicians (38.2%) who were PEWS implementation leaders (22.1%) or clinical staff (69.1%). Respondents described multiple PEWS adaptations across all implementation phases, with most occurring during the planning and piloting of EBIs. Adaptations included changes to PEWS content (algorithm, scoring tool, terminology, and use frequency) and context (personnel delivering or population). Respondents felt adaptations streamlined monitoring, enhanced effectiveness, improved workflow, increased comprehension, and addressed local resource limitations. Qualitative analysis indicated that most adaptations were categorized as fidelity consistent and planned; fidelity inconsistent adaptations were unplanned responses to unanticipated challenges. Conclusion Adaptations made to PEWS across implementation phases demonstrate how EBIs are adapted to fit dynamic, real-world clinical settings. This research advances implementation science by highlighting EBI adaptation as a potential strategy to promote widespread implementation and sustainability in hospitals of all resource levels.

24. Student Engagement in the HyFlex and Online Classrooms: Lessons from the COVID-19 Pandemic

Abstract: Student engagement is determined by the degree to which students perceive that their psychological needs for autonomy, competence, and relatedness are met as prescribed. The COVID-19 pandemic, with its sudden and dramatic shift to mandatory HyFlex (alternating online and face-to-face student attendance) or online learning only, meant less or no face-to-face interactions with peers and faculty, which had an impact on student motivation and engagement in the classroom, as reflected in the National Survey of Student Engagement. This new educational landscape will likely remain in effect to a certain extent, and, thus, there is a need for a deeper understanding of its impact on students' basic psychological needs and ultimately their engagement in the classroom. Building on self-determination theory and self-system processes, we studied 329 student responses to a survey conducted at a private Northeastern university in the United States and analyzed. We found that the impact of need for autonomy on student engagement is mediated by need for competence, and need for relatedness is directly impacting student engagement. Theoretical and practical implications are discussed in detail.

25. Transcending Attitudes: Teacher Responses to Professional Development in Trauma-Informed Education

Abstract: The adoption of trauma-informed practices in schools is a significant and growing area of school reform efforts. It has been assumed that professional development aimed at influencing teacher attitudes toward trauma-informed care in schools is an important first step in adopting trauma-informed practices and improving student and school outcomes. However, there are gaps in the literature assessing the impact of trauma-informed professional development on teacher attitudes toward trauma-informed care, and there is little evidence linking changes in teacher attitudes to changes in practices. This exploratory study assessed teacher responses to a year-long pilot program providing intensive professional development and consultation on trauma-informed practices in one urban elementary school in the U.S. Southwest. Findings raise questions about the impact of professional development on teacher attitudes and subsequent changes to practice.

Abstract: This pilot study assessed the feasibility and combined effect of aerobic exercise (AE) and mindfulness meditation (MM), compared with MM alone and a control (CON) condition, on stress, anxiety, and depression in high-stress college-based young adults. Thirty-two participants (84.4% F, 20.5 ± 2.7 years, 23.9 ± 5.0 kg/m[superscript 2]) were randomized to a four-week, AE + MM (n = 16), MM (n = 10), or control intervention (n = 6). ANOVA revealed non-significant, but noteworthy group x time interactions (perceived stress: p = 0.09; anxiety/depression: p = 0.07). Both AE + MM and MM seem to be feasible strategies to reduce levels of stress, anxiety and depression in college-based young adults.

Abstract: BACKGROUND AND OBJECTIVES: The Pediatric Emergency Care Applied Research Network Fluid Therapies Under Investigation in Diabetic Ketoacidosis (DKA) (FLUID) Trial found that rapid fluid infusion does not increase the risk of cerebral injury. Concern persists, however, whether fluid rates should be adjusted for overweight or obese patients. We used the FLUID Trial database to evaluate associations between fluid infusion rate and outcomes in these patients. METHODS: We compared children and youth who were overweight, obese, or normal weight, in regard to protocol adherence, mental status changes, time to DKA resolution, and electrolyte abnormalities. We investigated associations between outcomes and the amount of fluid received in these groups. RESULTS: Obese children and youth were more likely to receive fluids at rates slower than dictated by protocol. Overweight and obese children and youth in the fast fluid arms, who received fluids per the study protocol based on their measured weight, had similar rates of mental status changes or clinically apparent cerebral injury as those with normal weights. Risk of hypophosphatemia was increased in those receiving larger initial bolus volumes and reduced in those receiving higher rehydration rates. No other metabolic outcomes were associated with rehydration. CONCLUSIONS: Protocol adherence data in the FLUID Trial suggest that physicians are uncomfortable using weight-based fluid calculations for overweight or obese children. However, higher rates of fluid infusion were not associated with increased risk of mental status changes or cerebral injury, suggesting that physicians should not limit fluid resuscitation in obese children and youth with DKA.

Abstract: Organic carbon (OC) association with soil minerals stabilizes OC on timescales reflecting the strength of mineral-C interactions. We applied ramped thermal oxidation to subsoil B horizons with different mineral-C associations to separate OC according to increasing temperature of oxidation, i.e. thermal activation energy. Generally, OC released at lower temperatures was richer in bioavailable forms like polysaccharides, while OC released at higher temperatures was more aromatic. Organic carbon associated with pedogenic oxides was released at lower temperatures and had a narrow range of 14C content. By contrast, N-rich compounds were released at higher temperatures from samples with 2 : 1 clays and short-range ordered (SRO) amorphous minerals. Temperatures of release overlapped for SRO minerals and crystalline oxides, although the mean age of OC released was older for the SRO. In soils with more mixed mineralogy, the added presence of older OC released at temperatures greater than 450°C from clays resulted in a broader distribution of OC ages within the sample, especially for soils rich in 2 : 1 layer expandable clays such as smectite. While pedogenic setting affects mineral stability and absolute OC age, mineralogy controls the structure of OC age distribution within a sample, which may provide insight into model structures and OC dynamics under changing conditions. Abstract: IntroductionYoung adults with HIV (YWH) experience worse clinical outcomes than adults and have high rates of substance use (SU) and mental illness that impact their engagement in care and adherence to antiretroviral therapy (ART). The intervention for Virologic Suppression in Youth (iVY) aims to address treatment engagement/adherence, mental health (MH) and SU in a tailored manner using a differentiated care approach that is youth friendly. Findings will provide information about the impact of iVY on HIV virological suppression, MH and SU among YWH who are disproportionately impacted by HIV and at elevated risk for poor health outcomes.Methods and analysisThe iVY study will test the effect of a technology-based intervention with differing levels of resource requirements (ie, financial and personnel time) in a randomised clinical trial with an adaptive treatment strategy among 200 YWH (18-29 years old). The primary outcome is HIV virological suppression measured via dried blood spot. This piloted and protocolised intervention combines: (1) brief weekly sessions with a counsellor via a video-chat platform (video-counselling) to discuss MH, SU, HIV care engagement/adherence and other barriers to care; and (2) a mobile health app to address barriers such as ART forgetfulness, and social isolation. iVY has the potential to address important, distinct and changing barriers to HIV care engagement (eg, MH, SU) to increase virological suppression among YWH at elevated risk for poor health outcomes.Ethics and disseminationThis study and its protocols have been approved by the University of California, San Francisco Institutional Review Board. Abstract: This field guide is a first step by a collaborative of California child-serving education, health, and social service experts and leaders to further California's current efforts toward one effective, integrated, comprehensive, school-based child-serving system. The guide includes guidance for both local education agencies (LEAs) and state leaders, with details and implementation guidance on a wide variety of cross-sector initiatives to support the whole child. Using a whole-child approach, this guide outlines resources and strategies for LEAs and state leaders to use to support the implementation of funding that centers children and families. The guide includes six major initiatives that can serve as part of an integrated, school-based system of care: Adverse Childhood Experiences (ACES) Aware Initiative, Children and Youth Behavioral Health Initiative (CYBHI), Children and Youth System of Care, community schools initiatives, early childhood initiatives, and expanded learning. The guide also focuses specifically on how to fund various components of an integrated, school-based system of care, providing strategies for funding sustainability and further details about funding sources for comprehensive services. The guide concludes with a discussion of how state leaders facilitate the ability of LEAs to establish integrated, school-based systems of care by modeling cross-sector collaboration at the state level to reduce the current barriers that LEAs face. Abstract: Summary: Background: Low dose corticosteroids (e.g., 6 mg dexamethasone) have been shown to reduce mortality for hypoxic COVID-19 patients. We have previously reported that higher dose corticosteroids cause harm in patients with clinical hypoxia but not receiving ventilatory support (the combination of non-invasive mechanical ventilation, including high-flow nasal oxygen, continuous positive airway pressure and bilevel positive airway pressure ventilation, and invasive mechanical ventilation or extra-corporeal membrane oxygenation), but the balance of efficacy and safety in patients receiving ventilatory support is uncertain. Methods: This randomised, controlled, open-label platform trial (Randomised Evaluation of COVID-19 Therapy [RECOVERY]) assessed multiple possible treatments in patients hospitalised for COVID-19. Eligible and consenting adult patients receiving ventilatory support were randomly allocated (1:1) to either usual care with higher dose corticosteroids (dexamethasone 20 mg once daily for 5 days followed by 10 mg once daily for 5 days or until discharge if sooner) or usual standard of care alone (which includes dexamethasone 6 mg once daily for 10 days or until discharge if sooner). The primary outcome was 28-day mortality; secondary outcomes were duration of hospitalisation and (among participants not on invasive mechanical ventilation at baseline) the composite of invasive mechanical ventilation or death. Recruitment closed on 31 March 2024 when funding for the trial ended. The RECOVERY trial is registered with ISRCTN (50189673) and clinicaltrials.gov (NCT04381936). Findings: Between 25 May 2021 and 9 January 2024, 477 COVID-19 patients receiving ventilatory support were randomly allocated to receive usual care plus higher dose corticosteroids vs. usual care alone (of whom 99% received corticosteroids during the follow-up period). Of those randomised, 221 (46%) were in Asia, 245 (51%) in the UK and 11 (2%) in Africa. 143 (30%) had diabetes mellitus. Overall, 86 (35%) of 246 patients allocated to higher dose corticosteroids vs. 86 (37%) of 231 patients allocated to usual care died within 28 days (rate ratio [RR] 0.87; 95% CI 0.64–1.18; p = 0.37). There was no significant difference in the proportion of patients discharged from hospital alive within 28 days (128 [52%] in the higher dose corticosteroids group vs. 120 [52%] in the usual care group; RR 1.04, 0.81–1.33]; p = 0.78). Among those not on invasive mechanical ventilation at baseline, there was no clear reduction in the proportion meeting the composite endpoint of invasive mechanical ventilation or death (76 [37%] of 206 vs. 93 [45%] of 205; RR 0.79 [95% CI 0.63–1.00]; p = 0.05). Interpretation: In patients hospitalised for COVID-19 receiving ventilatory support, we found no evidence that higher dose corticosteroids reduced the risk of death compared to usual care, which included low dose corticosteroids. Abstract: This article highlights strategies for implementing successful rural public health campaigns by sharing insights from the Spit It Out – West Virginia Project. Active since 2007, the project expanded to Mingo County in 2023 to address high smokeless tobacco use through culturally relevant education and cessation workshops. Key components included leveraging local partnerships, tailoring campaigns to community norms, and integrating accessible support services like the West Virginia Tobacco Quitline. Outcomes demonstrated increased awareness, behavioral change, and community engagement. Our perspective offers practical lessons for designing effective, community-based interventions in rural settings to address pressing public health challenges.

37. Living Away From Specialized Pollinators: The Pollination System of Cieba Pentandra in the Yucatan Peninsula

Abstract: ABSTRACT Plant‐pollinator systems can exist along the gradient from extreme specialization to extreme generalization. Theoretical work predicts that high pollinator reliability may lead to specialized pollination systems, while pollinator unpredictability may promote generalized pollination systems. However, empirical tests of these predictions are constrained by the availability of accurate field observations, particularly in some groups of plants, such as trees pollinated by nocturnal animals. Plant species that face variable pollinator availability within their distributional range represent an ideal study system to address the effect of pollination predictability on the specialization level of pollination systems. This is the case of Ceiba pentandra, a tropical tree with chiropterophilic flowers. Most previous studies have described the pollination system of this tree species as mainly or exclusively bat‐pollinated. Here we studied the pollination system of C. pentandra in the Yucatan Peninsula, a region where no species of specialized nectarivorous bats occur. We assessed quantity (visitation rate) and quality pollination components (pollen deposition, fruit/seed set) for diurnal and nocturnal visitors in two different locations and years. We expected to find a generalized pollination system due to the absence of specialized pollinators. As predicted, we found five functional groups of nocturnal and diurnal pollinators. Diurnal and nocturnal pollinators contributed to a similar extent to quantity (visitation rate) and quality pollination components (fruit & seed set). However, the contribution of diurnal and nocturnal pollinators also varied either spatially or temporally. We conclude that the pollination system of C. pentandra in the Yucatan Peninsula is highly generalized and that temporal and/or spatial unpredictability in the effectiveness of its pollinators may explain this finding.

38. Ocean warming, icebergs, and productivity in the Gulf of Alaska during the Last Interglacial

Abstract: The Pacific Ocean is an important region for carbon storage, yet the past ocean–climate interactions are relatively underexplored in explaining glacial/interglacial climate variability during the late Pleistocene re-expansion of the Cordilleran Ice Sheet (CIS). We analysed marine sediment samples from IODP 341 Expedition Site U1418 in the Gulf of Alaska (GOA) using marine and terrestrial biomarkers to study the periodicity of glacial–interglacial cycles over the last 500 thousand years and reconstruct the North Pacific oceanography and marine productivity. Our sea surface temperature (SST) results show that marine isotope stages (MISs) 4 and 6 are the coldest and freshest glacials and MIS 5 and MIS 1 are the warmest and saltiest interglacials. The glacial–interglacials during the last 200 kyr are best observed due to increased sedimentation rates in the GOA during the expansion of the CIS, when coccolithophores increase relative to diatom productivity export. We observe lower %IRDs during cold MIS 6 than during warm MIS 5, and overall lower carbon and nitrogen accumulation rates (TOC and TN MAR) and stable isotope ratios (δ15N and δ13C). We conclude that during MIS 6, the stratification of the cold and fresh surface ocean helped the growth of the CIS, and coccolithophores did not fully exploit the nutrient availability in the ocean but contributed to deep carbon storage and climate cooling. During MIS 5, although SSTs were 6°C warmer, icebergs were more abundant. We infer that higher coccolithophore abundance with different alkenone configurations suggests a change in the dominant coccolithophore groups and changes in the available nutrients, probably as a result of higher influence of the more saline Alaska Current (AC) and lower influence of the Alaska Coastal Current (ACC) at Site U1418. This higher AC influence in the GOA contributed to sedimentary carbon storage and contributed to climate cooling despite the warm climate and higher iceberg discharge. As the last interglacial (MIS 5e) was a time period with similar than modern atmospheric CO2 concentrations, our results provide new understandings of the possible behaviours of CIS and GOA under the current changes in climate.

Abstract: Epithelioid hemangioendothelioma (EHE) is a rare vascular tumor. We present the case of a 64-year-old woman who presented with new-onset claudication and an atypical vascular mass involving the superficial femoral artery and femoral vein. En bloc resection and vascular reconstruction was performed which revealed a G1 EHE involving the walls of the superficial femoral artery and femoral vein with tumor cells positive for ERG-, CD31, and CAMTA-1. We discuss the preoperative workup for atypical vascular masses and the challenges associated with EHE.

Abstract: ABSTRACT Introduction The cognitive side‐effects of medication are common, but often overlooked in practice, and not routinely considered in interventional trials or post‐market surveillance. The cognitive footprint of a medication seeks to quantify the impact of its cognitive effects based on magnitude, duration, and interaction with other factors, evaluated across the exposed population. Methods Bayesian multivariable regression analysis of retrospective population‐based cross‐sectional cohorts. Results We replicate positive and negative cognitive effects of commonly used medications in UK Biobank, and extend observed associations to two additional cohorts, the EPIC Norfolk, and the Caerphilly Prospective Cohort. We quantify the resultant cumulative impact at the population level given known patterns of prescribing and compare it with exemplar common diseases. Conclusion The cognitive side‐effects of commonly used drugs may have significant impact at the population level. Consideration should be given to a routine structured assessment of cognition in interventional trials and post‐market surveillance.

41. Determinants of Familiarity and Experience with HIV Pre-Exposure Prophylaxis in Primary Care Providers in Ontario, Canada

Abstract: Background: Despite increased access to HIV pre-exposure prophylaxis (PrEP) in Canada, familiarity and experience among primary care providers (PCPs)—including family doctors and those working with key populations—remains limited. To understand the barriers and facilitators of PrEP familiarity and experience, we conducted a situational analysis in PCPs in sub-urban and rural Ontario. Methods: We surveyed a non-probabilistic sample of PCPs using an online questionnaire, designed with the Consolidated Framework for Implementation Research (CFIR). Poisson regressions with robust variance were used to assess the relationship between CFIR domains, sociodemographic, and practice characteristics on both PrEP familiarity and experience. Results: A total of 54 PCPs participated (6% response rate), comprising 80% physicians and 20% nurses. Nearly 30% of the sample worked with key populations, including sexual health clinics and community care centers, 18% of respondents reported high familiarity with PrEP, and 44% reported PrEP experience (referred, started a conversation, or prescribed). PrEP familiarity and experience were associated with working in an organization serving key populations, working with gender minorities, and having colleagues providing PrEP. Providers with a positive perception of PrEP and its necessity for populations at risk were more likely to have PrEP-related experience. Higher familiarity and experience were reported by PCPs with specific clinical skills related to PrEP, and with the perception that PrEP was compatible with their practice as primary provider. Conclusions: Our findings suggest that organizational support, and additional training and education would facilitate PrEP provision by PCPs in suburban/rural Ontario.

Abstract: Introduction Urgent, tailored and equitable action is needed to address the alarming rise in syphilis rates in Canada. In the last decade, the rates of infectious syphilis have increased by 345% in Ontario, Canada. Underserved populations—people who use drugs, un(der)housed individuals and those living in rural and remote areas—face unique social and healthcare challenges that increase their vulnerability to syphilis infections and hinder their access to timely diagnosis and treatment. This study assesses the real-world implementation and effectiveness of using a recently approved syphilis point-of-care test in conjunction with public health outreach to break barriers and bring services to the population at the highest risk.Methods and analysis The Syphilis Rapid Point-of-Care Testing and Immediate Treatment Evaluation (SPRITE) study includes eight public health units in Ontario, Canada. Implementation and evaluation of this rapid ‘test and treat’ outreach model of care will be assessed using the Practical, Robust Implementation and Sustainability Model (PRISM)/Reach, Effectiveness, Adoption, Implementation, Maintenance (RE-AIM) Framework, following a community-based participatory approach. Network models will be used to estimate the population-level impact of implementing this model of care to curb transmission. Abstract: STUDY OBJECTIVE: Our primary objective was to characterize the degree of dehydration in children with diabetic ketoacidosis (DKA) and identify physical examination and biochemical factors associated with dehydration severity. Secondary objectives included describing relationships between dehydration severity and other clinical outcomes. METHODS: In this cohort study, we analyzed data from 753 children with 811 episodes of DKA in the Pediatric Emergency Care Applied Research Network Fluid Therapies Under Investigation Study, a randomized clinical trial of fluid resuscitation protocols for children with DKA. We used multivariable regression analyses to identify physical examination and biochemical factors associated with dehydration severity, and we described associations between dehydration severity and DKA outcomes. Abstract: For graphs $G$ and $H$, what relations can be determined between $t(G,W)$ and $t(H,W)$ for a general graph $W$? We study this problem through the framework of the density domination exponent, which is defined to be the smallest constant $c$ such that $t(G,W)\ge t(H,W)^c$ for every graph $W$. This broad generalization encompasses the Sidorenko conjecture, the Erd\H{o}s-Simonovits Theorem on paths, and a variety of other statements relating graph homomorphism densities. We introduce some general tools for estimating the density domination exponent, and extend previous results to new graph regimes.

Abstract: IntroductionYoung children with type 1 diabetes (T1D) may be at particularly high risk of cognitive decline following diabetic ketoacidosis (DKA). However, studies of cognitive functioning in T1D typically examine school-age children. The goal of this study was to examine whether a single experience of DKA is associated with lower cognitive functioning in young children. We found that recently diagnosed 3- to 5-year-olds who experienced one DKA episode, regardless of its severity, exhibited lower IQ scores than those with no DKA exposure.MethodsWe prospectively enrolled 46 3- to 5-year-old children, who presented with DKA at the onset of T1D, in a randomized multi-site clinical trial evaluating intravenous fluid protocols for DKA treatment. DKA was moderate/severe in 22 children and mild in 24 children. Neurocognitive function was assessed once 2-6 months after the DKA episode. A comparison group of 27 children with T1D, but no DKA exposure, was also assessed. Patient groups were matched for age and T1D duration at the time of neurocognitive testing.ResultsChildren who experienced DKA, regardless of its severity, exhibited significantly lower IQ scores than children who did not experience DKA, F(2, 70) = 6.26, p = .003, partial η2 = .15. This effect persisted after accounting for socioeconomic status and ethnicity.ConclusionsA single DKA episode is associated with lower IQ scores soon after exposure to DKA in young children.
