BACKGROUND: Pathogenic Leptospira species are globally important zoonotic pathogens capable of infecting a wide range of host species. In marine mammals, reports of Leptospira have predominantly been in pinnipeds, with isolated reports of infections in cetaceans. CASE PRESENTATION: On 28 June 2021, a 150.5 cm long female, short-beaked common dolphin (Delphinus delphis delphis) stranded alive on the coast of southern California and subsequently died. Gross necropsy revealed multifocal cortical pallor within the reniculi of the kidney, and lymphoplasmacytic tubulointerstitial nephritis was observed histologically. Immunohistochemistry confirmed Leptospira infection, and PCR followed by lfb1 gene amplicon sequencing suggested that the infecting organism was L.kirschneri. Leptospira DNA capture and enrichment allowed for whole-genome sequencing to be conducted. Phylogenetic analyses confirmed the causative agent was a previously undescribed, divergent lineage of L.kirschneri. CONCLUSIONS: We report the first detection of pathogenic Leptospira in a short-beaked common dolphin, and the first detection in any cetacean in the northeastern Pacific Ocean. Renal lesions were consistent with leptospirosis in other host species, including marine mammals, and were the most significant lesions detected overall, suggesting leptospirosis as the likely cause of death. We identified the cause of the infection as L.kirschneri, a species detected only once before in a marine mammal - a northern elephant seal (Mirounga angustirostris) of the northeastern Pacific. These findings raise questions about the mechanism of transmission, given the obligate marine lifestyle of cetaceans (in contrast to pinnipeds, which spend time on land) and the commonly accepted view that Leptospira are quickly killed by salt water. They also raise important questions regarding the source of infection, and whether it arose from transmission among marine mammals or from terrestrial-to-marine spillover. Moving forward, surveillance and sampling must be expanded to better understand the extent to which Leptospira infections occur in the marine ecosystem and possible epidemiological linkages between and among marine and terrestrial host species. Generating Leptospira genomes from different host species will yield crucial information about possible transmission links, and our study highlights the power of new techniques such as DNA enrichment to illuminate the complex ecology of this important zoonotic pathogen.

Leptospirosis, the most widespread zoonotic disease in the world, is broadly understudied in multi-host wildlife systems. Knowledge gaps regarding Leptospira circulation in wildlife, particularly in densely populated areas, contribute to frequent misdiagnoses in humans and domestic animals. We assessed Leptospira prevalence levels and risk factors in five target wildlife species across the greater Los Angeles region: striped skunks (Mephitis mephitis), raccoons (Procyon lotor), coyotes (Canis latrans), Virginia opossums (Didelphis virginiana), and fox squirrels (Sciurus niger). We sampled more than 960 individual animals, including over 700 from target species in the greater Los Angeles region, and an additional 266 sampled opportunistically from other California regions and species. In the five target species seroprevalences ranged from 5 to 60%, and infection prevalences ranged from 0.8 to 15.2% in all except fox squirrels (0%). Leptospira phylogenomics and patterns of serologic reactivity suggest that mainland terrestrial wildlife, particularly mesocarnivores, could be the source of repeated observed introductions of Leptospira into local marine and island ecosystems. Overall, we found evidence of widespread Leptospira exposure in wildlife across Los Angeles and surrounding regions. This indicates exposure risk for humans and domestic animals and highlights that this pathogen can circulate endemically in many wildlife species even in densely populated urban areas.

Feed conversion ratio (FCR) is widely used as a measure of feed use efficiency in commercial aquaculture production and research. Surprisingly, little work has focused on the factors that affect FCR in plankton‐based, commercial ponds despite the widespread use of FCR as a production metric. Proposals to use FCR as a numeric standard for environmental regulation (mandatory or voluntary) raise questions as to whether FCR meets the reliability and consistency requirements of a good metric, and whether it would lead to desired behavioral changes. This article summarizes FCR data from university research verification programs conducted on US catfish farms and databases developed from commercial farm records. The relevant research literature is synthesized with respect to factors that affect FCR. US catfish is used as a case study because pond‐level data from verification trials (Extension/research‐led on‐farm trials of best available science) and commercial farm records are available. While specific to US catfish production, the analysis provides insights into factors affecting FCR that likely apply to other pond‐based aquaculture species. Commercial verification of pond data showed (1) significant variation in FCR (coefficients of variation from 1% to 47%) among ponds with similar stocking, feeding, and aeration rates; (2) significant effects of survival on FCR; and (3) no significant effects on FCR of yield, feed type, management, or production system. Improvements in FCR would occur with improved survival that requires improved management of federally protected fish‐eating bird populations and development of additional, effective vaccines for diseases. FCRs were found to not exhibit the degree of reliability and consistency required for use as an effective metric or numeric standard for regulatory purposes. BackgroundTreatments for health care-associated infections (HAIs) caused by antibiotic-resistant bacteria and Clostridium difficile are limited, and some patients have developed untreatable infections. Evidence-supported interventions are available, but coordinated approaches to interrupt the spread of HAIs could have a greater impact on reversing the increasing incidence of these infections than independent facility-based program efforts.MethodsData from CDC's National Healthcare Safety Network and Emerging Infections Program were analyzed to project the number of health care-associated infections from antibiotic-resistant bacteria or C. difficile both with and without a large scale national intervention that would include interrupting transmission and improved antibiotic stewardship. As an example, the impact of reducing transmission of one antibiotic-resistant infection (carbapenem-resistant Enterobacteriaceae [CRE]) on cumulative prevalence and number of HAI transmission events within interconnected groups of health care facilities was modeled using two distinct approaches, a large scale and a smaller scale health care network.ResultsImmediate nationwide infection control and antibiotic stewardship interventions, over 5 years, could avert an estimated 619,000 HAIs resulting from CRE, multidrug-resistant Pseudomonas aeruginosa, invasive methicillin-resistant Staphylococcus aureus (MRSA), or C. difficile. Compared with independent efforts, a coordinated response to prevent CRE spread across a group of inter-connected health care facilities resulted in a cumulative 74% reduction in acquisitions over 5 years in a 10-facility network model, and 55% reduction over 15 years in a 102-facility network model.ConclusionsWith effective action now, more than half a million antibiotic-resistant health care-associated infections could be prevented over 5 years. Models representing both large and small groups of interconnected health care facilities illustrate that a coordinated approach to interrupting transmission is more effective than historical independent facilitybased efforts.Implications for public healthPublic health-led coordinated prevention approaches have the potential to more completely address the emergence and dissemination of these antibiotic-resistant organisms and C. difficile than independent facility-based efforts.

In this article the author shares the story of the Whittier Drum Project and how it succeeded. The Whittier Drum Project has brought the community together through the talents of youth and their dedication to drumming, and has used drumming to link professionals to their own communities. The author adapted the model to meet the therapeutic needs of troubled teens, taking it a step further by adding a clinical component. In collaboration with the community court of Denver (which takes a restorative justice model approach to youth offenders in their neighborhood) and the Bread for the Journey Foundation, the Multi-Family Therapy Group with Drumming model was developed. The program provides eight weeks of therapy for four or five youth offenders and their caregivers for the purpose of decreasing at-risk behaviors in the community. Each one-hour weekly group consists of approximately thirty minutes of hand-drumming and thirty minutes of talk therapy. The drumming is used to build ties in a nonthreatening way between group members and between youth and their families. It also helps to address cultural and trust barriers. The verbal component of the group provides a space for participants to examine issues that led to the offending behaviors and to explore alternative ways to cope with such issues and stressors.
