Abstract

In alpha-1-antitrypsin deficiency (A1ATD) there is excess neutrophil elastase activity, resulting in proteolytic destruction of the lung parenchyma. I hypothesised that the peptide fragments of proteins present in the lung might be detectable in plasma by mass spectrometry and that they might be useful biomarkers of disease activity and treatment efficacy. Calcium ionophore, neutrophil elastase and proteinase 3 were added to plasma from patients with A1ATD to create an in vitro model of the destructive processes. MALDI-based peptide profiling of plasma from patients pre and post treatment with intravenous A1AT was undertaken and MS/MS performed to identify differences. Plasma was also depleted of abundant plasma proteins, labelled with isobaric tags and analysed by shotgun proteomics. The readily detectible components of the plasma proteome remained unchanged with intravenous A1AT. Addition of ionophore, elastase and proteinase 3 to patient blood generated predominantly fragments of fibrinogen. In patients treated with intravenous A1AT, fragments of A1AT increased significantly with treatment: - 2 of these were fragments of a short C-terminal segment of the A1AT protein and were also present in healthy subjects. The shotgun experiments did not identify any robust biomarkers and illustrate the challenging nature of plasma proteomics.

13. Characterisation of two deubiquitinating enzymes in the DNA damage response and replication

Abstract

Ub has an essential role within the DNA double strand break (DSB) response which is well documented. However the role of ubiquitin (Ub) in the regulation of replication is an emerging area of research. This thesis investigates how two deubiquitinating enzymes (DUBs), POH1 and USP50, regulate DSB repair and replication respectively. A screen of 103 siRNAs against putative DUBs in the human genome, measuring the amount of conjugated Ub after release from HU-induced damage, identified the proteasome associated DUB, POH1 as being important in regulating Ub-conjuagtes after damage. Further work found that POH1 restricts the K63-linked Ub at DSBs and consequently 53BP1 foci formation. This appears to regulate repair of breaks by Non-homologous end-joining (NHEJ). The DUB screen also identified USP50 as having significantly reduced levels of conjugated Ub after damage. USP50 is an inactive DUB with Ub-binding activities, which has a role in preventing formation of Mus81-dependent DSBs during replication, with depletion sensitising cells to replication-stress. Therefore this works demonstrates a role for USP50 in genomic stability during replication. In this thesis I demonstrate the role of these two DUBs in DSB repair and replication respectively, providing potential therapeutic targets.

Abstract

This thesis explores the example of the archaeology of Fort Vancouver not as an end in itself, but as a pointer to a more general call for greater sensitivity in searching for and interpreting evidence. In archaeological interpretation men are most visible. The history of excavation at Fort Vancouver could be adduced as a perfect example. Chapters on feminist history and Fort Vancouver history are presented as essential preliminary background, in two parts. Part 1 describes the general background relating to historical archaeological practice, the growing visibility of women in historical investigation, the history of the fort, its occupants, and its excavations. Part 2 moves to the new story my research allows to be told. This new story is: 1) Mapping evidence establishes a layout of buildings, but with no clear material evidence of the presence of women. 2) Documentary evidence establishes a substantial presence of women with great clarity. 3) Excavations have tended to confirm the first pattern of evidence but to neglect the second pattern of evidence. 4) Finally, one building in particular provides an example of a structure used both by married with family and single occupants, and should have been excavated with that history in mind. It becomes an important test case – either as evidence of what can be proved, or as a cautionary tale of what should have been better explored, or as both. The story told is one of mixed success. Some of the evidence (extant maps and documentary evidence of families) demonstrates that women can be made more visible. However, some of the evidence (especially that of the physical remains and artifacts) is now largely lost or was neglected or overlooked, making it more difficult to present a clear picture.

29. Barriers and facilitators experienced in delivering alcohol screening and brief interventions in community pharmacy: a qualitative evidence synthesis.

Abstract

Background: Following increases in deaths due to alcohol during the COVID-19 pandemic, there have been renewed calls to increase resources in alcohol screening and brief intervention (SBI). Research has shown that community pharmacy could be a promising setting for SBI. This review aimed to investigate the barriers and facilitators to SBI delivery in community pharmacy to inform its further development. Methods: A systematic search of four databases (MEDLINE, EMBASE, CINAHL, and PsycINFO) was conducted in October 2021 to identify relevant published qualitative or mixed-method studies. Relevant qualitative data were extracted from the included studies and a framework synthesis was performed using the Capability–Opportunity–Motivation–Behaviour (COM-B) model. Results: Two thousand two hundred and ten articles were screened and nine studies were included in the review (seven in the United Kingdom and two in Australia). Identified barriers and facilitators to delivering SBI corresponded to all components of the COM-B model. Facilitators included non-confrontational communication skills, aligning SBI with existing pharmacy services and pharmacist role legitimacy. Barriers included multiple demands on staff time, a lack of staff experience with screening tools, and staff concerns of causing offence. Using the Behaviour Change Wheel (BCW), we propose five elements of a pharmacy SBI to address identified barriers. Conclusions: Research into SBI in community pharmacy is limited in comparison to other healthcare settings and this review provides an understanding of the barriers and facilitators to the delivery of SBI in community pharmacy from a behavioural perspective. AIM: To assess whether immunogenicity to a patient's first anti-TNF agent would be associated with immunogenicity to the second, irrespective of drug sequence METHODS: We conducted a UK-wide, multicentre, retrospective cohort study to report rates of immunogenicity and treatment failure of second anti-TNF therapies in 1058 patients with IBD who underwent therapeutic drug monitoring for both infliximab and adalimumab. The primary outcome was immunogenicity to the second anti-TNF agent, defined at any timepoint as an anti-TNF antibody concentration ≥9 AU/ml for infliximab and ≥6 AU/ml for adalimumab. RESULTS: In patients treated with infliximab and then adalimumab, those who developed antibodies to infliximab were more likely to develop antibodies to adalimumab, than patients who did not develop antibodies to infliximab (OR 1.99, 95%CI 1.27-3.20, p = 0.002).
