Abstract

Purpose: To explore the issue of counterintuitive data via analysis of a representative case and further discussion of those situations in which the data appear to be inconsistent with current knowledge. Case: 844 postoperative CABG patients, who were extubated within 24 hours of surgery were identified in a critical care database (MIMIC-III). Nurse elicited pain scores were documented throughout their hospital stay on a scale of 0 to 10. Levels were tracked as mean, median, and maximum values, and categorized as no (0/10), mild (1-3), moderate (4-6) and severe pain (7-10). Regression analysis was employed to analyze the relationship between pain scores and outcomes of interest (mortality and hospital LOS). After covariate adjustment, increased levels of pain were found to be associated with lower mortality rates and reduced hospital LOS. Conclusion: These counterintuitive results for post-CABG pain related outcomes have not been previously reported. While not representing strong enough evidence to alter clinical practice, confirmed and reliable results such as these should serve as a research trigger and prompt further studies into unexpected associations between pain and patient outcomes. With the advent of frequent secondary analysis of electronic health records, such counterintuitive data results are likely to become more frequent. 9. Withholding or withdrawing invasive interventions may not accelerate time to death among dying ICU patients

Abstract

Background Critically ill patients may die despite invasive intervention. In this study, we examine trends in the application of two such treatments over a decade, namely, endotracheal ventilation and vasopressors and inotropes administration, as well as the impact of these trends on survival durations in patients who die within a month of ICU admission. Methods We considered observational data available from the MIMIC-III open-access ICU database and collected within a study period between year 2002 up to 2011. If a patient had multiple admissions to the ICU during the 30 days before death, only the first stay was analyzed, leading to a final set of 6,436 unique ICU admissions during the study period. We tested two hypotheses: (i) administration of invasive intervention during the ICU stay immediately preceding end-of-life would decrease over the study time period and (ii) time-to-death from ICU admission would also decrease, due to the decrease in invasive intervention administration. To investigate the latter hypothesis, we performed a subgroups analysis by considering patients with lowest and highest severity. To do so, we stratified the patients based on their SAPS I scores, and we considered patients within the first and the third tertiles of the score. We then assessed differences in trends within these groups between years 2002–05 vs. 2008–11. Results Comparing the period 2002–2005 vs. 2008–2011, we found a reduction in endotracheal ventilation among patients who died within 30 days of ICU admission (120.8 vs. 68.5 hours for the lowest severity patients, p<0.001; 47.7 vs. 46.0 hours for the highest severity patients, p = 0.004). This is explained in part by an increase in the use of non-invasive ventilation. Comparing the period 2002–2005 vs. 2008–2011, we found a reduction in the use of vasopressors and inotropes among patients with the lowest severity who died within 30 days of ICU admission (41.8 vs. 36.2 hours, p<0.001) but not among those with the

Abstract

Objective: To identify shared polygenic risk and causal associations in amyotrophic lateral sclerosis (ALS). Methods: Linkage disequilibrium score regression and Mendelian randomization were applied in a large-scale, data-driven manner to explore genetic correlations and causal relationships between >700 phenotypic traits and ALS. Exposures consisted of publicly available genome-wide association studies (GWASes) summary statistics from MR Base and LD-hub. The outcome data came from the recently published ALS GWAS involving 20,806 cases and 59,804 controls. Multivariate analyses, genetic risk profiling, and Bayesian colocalization analyses were also performed. Results: We have shown, by linkage disequilibrium score regression, that ALS shares polygenic risk genetic factors with a number of traits and conditions, including positive correlations with smoking status and moderate levels of physical activity, and negative correlations with higher cognitive performance, higher educational attainment, and light levels of physical activity. Using Mendelian randomization, we found evidence that hyperlipidemia is a causal risk factor for ALS and localized putative functional signals within loci of interest. Interpretation: Here, we have developed a public resource (https://lng-nia.shinyapps.io/mrshiny) which we hope will become a valuable tool for the ALS community, and that will be expanded and updated as new data become available. Shared polygenic risk exists between ALS and educational attainment, physical activity, smoking, and tenseness/restlessness. We also found evidence that elevated low-desnity lipoprotein cholesterol is a causal risk factor for ALS. Future randomized controlled trials should be considered as a proof of causality. Abstract

To identify novel genes associated with ALS, we undertook two lines of investigation. We carried out a genome-wide association study comparing 20,806 ALS cases and 59,804 controls. Independently, we performed a rare variant burden analysis comparing 1,138 index familial ALS cases and 19,494 controls. Through both approaches, we identified kinesin family member 5A (KIF5A) as a novel gene associated with ALS. Interestingly, mutations predominantly in the N-terminal motor domain of KIF5A are causative for two neurodegenerative diseases: hereditary spastic paraplegia (SPG10) and Charcot-Marie-Tooth type 2 (CMT2). In contrast, ALS-associated mutations are primarily located at the C-terminal cargo-binding tail domain and patients harboring loss-of-function mutations displayed an extended survival relative to typical ALS cases. Taken together, these results broaden the phenotype spectrum resulting from mutations in KIF5A and strengthen the role of cytoskeletal defects in the pathogenesis of ALS. Using a large-scale genome-wide association study and exome sequencing, we identified KIF5A as a novel gene associated with ALS. Our data broaden the phenotype resulting from mutations in KIF5A and highlight the importance of cytoskeletal defects in the pathogenesis of ALS.

Abstract

We examine the hypothesis that isotopic techniques are applicable to hydrological predictions in difficult-to-simulate semi-arid basins, using the Murray-Darling Basin as an example. Isotopic data from three aquifers in the Murray-Darling characterize precipitation intensity for evaluation of GCMs. Applying these to 'good' (water conserving) and 'poor' (non-water-conserving) climate model simulations of the Murray-Darling gives rise to large differences in rainfall amount (30-62%). Selecting only 'good' models shows a greater than 150 mm annual groundwater recharge loss in El Niño cf. La Niña climates. 2002-2003 El Niño drought data are used to refine isotopic calculation of water lost in evaporation from rivers and irrigation, giving a cumulative loss of 64% of river water during 2002 (cf. 80% using a previous method). This substantiates recent identification of this El Niño drought as evaporatively most extreme and we conclude that stable water isotopes, used synergistically with hydro-climate models, have great potential in future water resource predictions. Abstract

Deuterium and oxygen-18 enrichment in river water during its transit across dryland region is found to occur systematically along evaporation lines with slopes of close to 4 in 2H-18O space, largely consistent with trends predicted by the Craig-Gordon model for an open-water dominated evaporating system. This, in combination with reach balance assessments and derived runoff ratios, strongly suggests that the enrichment signal and its variability in the Barwon-Darling river, Southeastern Australia is acquired during the process of evaporation from the river channel itself, as enhanced by the presence of abundant weirs, dams and other storages, rather than reflecting inherited enrichment signals from soil water evaporation in the watershed. Using a steady-state isotope mass balance analysis based on monthly 18O and 2H, we use the isotopic evolution of river water to re-construct a perspective of net exchange between the river and its contributing area along eight reaches of the river during a drought period from July 2002 to December 2003, including the duration of a minor flow event. The resulting scenario, which uses a combination of climatological averages and available real-time meteorological data, should be viewed as a preliminary test of the application rather than as a definitive inventory of reach water balance. As expected for a flood-driven dryland system, considerable temporal variability in exchange is predicted. While requiring additional real-time isotopic data for operational use, the method demonstrates potential as a non-invasive tool for detecting and quantifying water diversions, one that can be easily incorporated within existing water quality monitoring activities. Abstract

A small series of novel, imidazoles 4 have been prepared that exhibit very good binding affinities for the δ and μ opioid receptors (ORs), as well as demonstrate potent agonist functional activity at the δ OR. Representative imidazole 4a (Ki δ = 0.9 nM; Ki μ = 55 nM; Ki κ = 124 nM; EC50 δ =13−25 nM) was further profiled for OR related in vivo effects. Compound 4a reduced gastrointestinal (GI) propulsive motility in a dose-dependent and naloxone-reversible manner, based on the results of the mouse glass bead expulsion test (3, 5, and 10 mg/kg, ip) and the mouse fecal pellet output test (1 and 3 mg/kg, ip). Compound 4a showed no analgesic activity as measured by the mouse abdominal irritant test (MAIT) when dosed at 100 mg/kg, sc, but did show significant MAIT activity at doses of both 10 μg (40% inhibition) and 100 μg (100% inhibition) when dosed intracerebroventricularly (icv). Taken together, these in vivo results suggest that 4a acts peripherally when dosed systemically, and that these prototypical compounds may prove promising as medicinal leads for GI indications.
