Abstract

Introduction: Selection of analgesics should be based on the World Health Organization (WHO) analgesic ladder, beginning with non-opioid analgesics in combination with adjuvants for mild pain, weak opioids with adjuvants are indicated for moderate pain, while potent opioids, non-opioids and adjuvants are recommended for severe pain. Methods: The facts presented in this paper are expanded by searching for recent literature data in the following index-data-bases: SCI index, PubMed, Google Scholar, Scopus, and by using adequate key words. The idea supporting this paper was to make practice easier for clinicians who are engaged in supportive oncology and to help in adequate and up-to-date malignant pain management in oncology patients in everyday practice. Topic: Initial opioid dose should be low, and long-acting opioid dose should be gradually increased and titrated considering daily requirements of short-acting opioid formulation due to pain breakthrough. It is mandatory for patients on long-acting opioid treatment to be provided with fast-acting medication for breakthrough pain treatment. The following long-acting strong opioids formulations are available in Serbia - oxycodone, oxycodone/ naloxone fixed combination, hydromorphone, tapentadol, fentanyl. A patient and his/her family should be warned about possible side effects of opioids, primarily morphine. There is great fear of prescribing opioid analgesics due to their possible side-effect of tory depression. Opiophobia is defined as exaggerated concern about the consequences of medical use of opioids. The risk of opioids is estimated to overweight the benefits of the treatment, resulting in the fact that they are not used where indicated. Avoiding opioid prescriptions is one of the biggest barriers for successful treatment of cancer pain. Conclusion: Oncology patients with moderate and severe pain have a constitutional right not to tolerate pain and they have a right to strong opioid treatment. Implementation of analgesic elevator is recommended. There should not be any fear of addiction or other side-effects that can be controlled, and the benefits greatly outweigh the risks of possible side-effects in these patients. Cancer pain management should be an equally important element of overall cancer treatment. Only by multimodal and multidisciplinary treatment approach satisfactory analgesic effects may be achieved while minimizing adverse side-effects.

5. Neurophysiological confirmation of phrenic nerve affection in a patient with dyspnea and herpes zoster

Abstract

Introduction. Herpetic lesion of the phrenic nerve is quite uncommon. These lesions are usually unilateral, and for most clinicians, the clinical manifestation of herpes zoster in the cervical or thoracic region and diaphragmatic paralysis on the same side is sufficient for making a diagnosis of segmental herpes zoster phrenic nerve palsy. We report a patient with a classic clinical picture, in which we confirmed phrenic nerve affection on nerve conduction study. Case report. A 58-year-old female patient came for an examination due to shortness of breath. The patient had a herpetic rash on her right shoulder two and a half months earlier. The elevation of the right hemidiaphragm was seen on chest X-ray imaging. Asymmetry was evident in the nerve conduction study of the phrenic nerve: prolonged latency and reduced amplitude of her right phrenic nerve. The patient was treated with acyclovir, pregabalin, and B complex vitamins. After six months, the motor deficit was reduced completely. Conclusion. A nerve conduction study of the phrenic nerve is useful in making the definitive diagnosis. Good outcome, as in this patient, is rare in patients with this diagnosis and may be linked to timely treatment with acyclovir.

Abstract

The new pandemic, caused by the corona viral disease 2019 (Covid-19), has brought with it a series of restrictive measures that have drastically altered people's way of life and which restrict freedom of movement, behavior and social engagement. At the time of pandemia, almost all aspects of each individual lifestyle have been impaired, especially in members of vulnerable groups, particularly women in postpartum women. Acute psychological reactions to a pandemia may include insomnia, fear of infection, anxiety, frustration, panic attacks, loneliness, but also depressive episodes. Non-psychotic postpartum mood and anxiety disorders can occur as an inability to adapt to stressful life situations. A comprehensive understanding of the impact of the pandemia on the mental health of women in the postpartum period is needed, in order to devise adequate approaches to recovery from such a major disaster as the Covid-19 pandemic.

Introduction: The role of oxidative stres in patients with Meniere disease (MD), is still unresearched. The aim: The aim was to compare serum levels of total and direct bilirubin, albumins, uric acid and creatinine as antioxidative status parameters in patients with MD with healthy controls, and patients with multiple sclerosis (MS). Introduction: Some psychosociodemographic and characteristics of pregnancy and childbirth have been associated with the onset of postpartum depression in the literature. Aim: To examine certain psychosociodemographic and characteristics of pregnancy and childbirth as potential risk factors for the onset of postpartum depression in the early postpartum period. Material and Methods: The study involved 120 subjects, which included the use of the Edinburgh Postpartum Depression Scale and a sociodemographic questionnaire constructed for the purposes of this study, in the early postpartum period. 20. STUDY ON THE TWO-DIRECTIONAL RELATIONSHIP BETWEEN MYASTHENIA GRAVIS AND PREGNANCY

Abstract

Due to the high prevalence of myasthenia gravis (MG) in women of reproductive age, pregnancy in patients with MG is not uncommon. This requires special clinical and therapeutic caution. There is a two-way relationship between MG and pregnancy: Pregnancy can affect the course of the disease, but MG can affect childbirth and the occurrence of postnatal complications. The purpose of our study was to evaluate the clinical course, delivery, and neonatal outcome of pregnant women with the diagnosis of myasthenia gravis. The clinical course of the disease during pregnancy, labor, and postpartum period was reviewed, as well as the neonatal period in the 23 infants born to 15 MG mothers. Spontaneous abortion was observed in two pregnant women (8%) in the second month of pregnancy. One newborn was diagnosed with transitory neonatal MG. There were no stillbirths or physical anomalies. Clinical worsening was recorded during 10 pregnancies (40%), in 8 pregnant women. The clinical course of MG in pregnancy is variable and unpredictable, but pregnancy does not affect the long-term course of MG. Spontaneous abortion is a potential complication in pregnant women with MG. Cesarean section is a more frequent intervention in pregnant women with MG compared to the general population of women. Thymectomy in mothers with MG before pregnancy could potentially have a positive benefit for the newborn. Neonatal transient myasthenia was uncommon in our patient population. No congenital abnormalities were discovered in the 23 babies delivered at our institution.

Abstract

Regular assessments of species’ status are an essential component of conservation planning and adaptive management. They allow the progress of past or ongoing conservation actions to be evaluated and can be used to redirect and prioritize future conservation actions. Most countries perform periodic assessments for their own national adaptive management procedures or national red lists. Furthermore, the countries of the European Union have to report on the status of all species listed on the directives of the Habitats Directive every 6 years as part of their obligations under Article 17. However, these national level assessments are often made using non-standardized procedures and do not always adequately reflect the biological units (i.e., the populations) which are needed for ecologically meaningful assessments. Since the early 2000’s the Large Carnivore Initiative for Europe (a Specialist Group of the IUCN’s Species Survival Commission) has been coordinating periodic surveys of the status of large carnivores across Europe (e.g., von Arx et al. 2004; Salvatori & Linnell 2005, Kaczensky et al. 2013). These have covered the Eurasian lynx (Lynx lynx), the wolf (Canis lupus), the brown bear (Ursus arctos) and the wolverine (Gulo gulo). The golden jackal (Canis aureus) has been added to the LCIE prerogatives in 2014. The species is rapidly expanding in Europe (Trouwborst et al. 2015; Männil & Ranc 2022), a large-scale phenomenon that resembles that of the other large carnivores. Golden jackals are thriving in human-dominated landscapes (Ćirović et al. 2016; Lanszki et al. 2018; Fenton et al. 2021), where they are often functioning as the top predators, despite having smaller body size that is typical for large carnivores. The expansion of the species triggers many questions among scientists, stakeholders, and policy makers (Trouwborst et al. 2015; Hatlauf et al. 2021), that are closely connected to those raised by the other large carnivores (e.g., potential conflicts with livestock or hunting). In this context, monitoring the species’ expansion, delineating populations, assessing the species' legal and protection status, and addressing the concerns raised by this rapidly expanding carnivore requires a high level of coordination among regional experts. These surveys involve the contributions of the best available experts and sources of information. While the underlying data quality and field methodology varies widely across Europe, these coordinated assessments do their best to integrate the diverse data in a comparable manner and make the differences transparent. They also endeavor to conduct the assessments on the most important scales. This includes the continental scale (all countries except for Russia, Belarus, Moldova and the parts of Ukraine outside the Carpathian Mountain range), the scale of the EU 28 (where the Habitats Directive operates) and of the biological populations which reflect the scale at which ecological processes occur (Linnell et al. 2008). In this way, the independent LCIE assessments provide a valuable complement to the ongoing national processes. Our last assessments covered the period 2006-2011 (Kaczensky et al. 2013; Chapron et al. 2014) but, at the time, did not include golden jackals. The current assessment is based on the period 2012-2016 and broadly follows the same methodology. Explicit distinctions are made between classification based on empirical data and expert opinion. The population definitions used in this report follow those proposed in (Ranc et al. 2018); areas whose presence category was defined by expert opinion were not assigned to a specific population, though.

Abstract

We tested differences in attitudes towards bears, wolves and lynx among the rural public in Albania and Macedonia through information collected from a questionnaire survey (n = 759). Wolves were the species with the least positive attitudes among the rural public and had the lowest support for conservation compared with bears and lynx. In addition, conflict perception of wolves was higher than for bears and lynx. We argue that, based on species specific differences in public attitudes, conservation initiatives and management plans for large carnivores should deal with wolves separately from bears and lynx, as lower public support for wolves might jeopardise the conservation of the two other large carnivores. Bears and lynx can be potentially treated together in conservation initiatives based on the similar levels of public support for conservation, however, from a conflict-management point of view, all three species need to be addressed separately.

Abstract

Guillain-Barre syndrome is an immunologically mediated polyradiculoneuropathy characterized by a monophasic course, with a clinical peak within 4 weeks of disease onset. There have been several reports ofGuillain-Barré syndrome, related to COVID-19, days or weeks after the onset of respiratory symptoms. In contrast to that, we describe a case of acute sensorimotor demyelinating polyradiculoneuropathy, followed by COVID-19 infection. Our patient was successfully treated with intravenous immunoglobulins while COVID-19 was treated according to the latest clinical management protocol. In our case, neuropathy symptoms showed aparainfectious profile rather than a post-infectious one, which is uncommon inGuillain-Barré syndrome. [ABSTRACT FROM AUTHOR]

Abstract

Spatial and foraging ecology of the Eurasian lynx (Lynx lynx) has been well recognized, however due to the distinct taxonomic position and geographic isolation of its Balkan population, it is important to learn and compare its ecology to other populations of this felid. Therefore, the paper offers the first ever investigation into the spatial and foraging ecology of this predator. To that aim, we used modern GPS/GSM telemetry methods, allowing proper research of animal spatial requirements and diet preferences. Individuals were captured using walk-through, double-door box-traps and foot-snares placed on fresh lynx kills. Average home range size of males is 373 km2 (95% MCP) and 400 km2 (0.7 Kernel), while the female’s home range is 119 km2 (95% MCP) and 108 km2 (0.7 Kernel). GPS clusters showed prey remains of 153 kills from five different species: roe deer, chamois, brown hare, red fox and marten. Data collected for the Balkan lynx suggest lower kill rates, probably associated with lower ungulate densities in the study area compared to most of Central Europe, also indicated by the relatively long search time. Although Eurasian lynx can adapt to lower ungulate prey densities by increasing hunting effort, changing spatial organization or switching to smaller prey, this, in turn, can have adverse demographic effects on the critically endangered Balkan population. Using GPS telemetry, we provided first insight into the space use of this small population, and show that the spatial and foraging ecology of the Balkan lynx appear similar to other European populations of this species, especially those from Central Europe with similar home range size and principal prey preference.
