We demonstrate the rotating-crystal method in femtosecond x-ray diffraction. Structural dynamics of a photoexcited bismuth crystal is mapped in a pump-probe scheme by measuring intensity changes of many Bragg reflections simultaneously.

Abstract

Transient electron density maps are derived from x-ray diffraction patterns of photoexcited [Fe(bpy)3]2+(PF6−)2 powder. Upon photoexcitation, the 5T2 quintet state reveals a charge transfer from the PF6− ions and from the Fe atoms to neighboring bpy units. The charge transfer from the Fe points to a partial and weak charge-transfer character of this state.

Abstract

Transient electron density maps of potassium dihydrogen phosphate(KH2PO4, KDP) are derived from femtosecond x-ray powder diffraction patterns. Upon photoexcitation, the low-frequency TO soft mode is elongated impulsively and modulates the electronic charge distribution on the length scale of interatomic distances, much larger than the vibrational amplitude of the nuclear motion. The results demonstrate a charge transfer from the volumes around the P-atoms to those containing the O—H···O units and a quadrupolar distortion of the K+ charge distribution. This behavior reflects the interplay of nuclear motions and electric polarizations in the ionic crystal lattice.

Abstract

Background: The benefit of pharmacogenetic testing before starting drug therapy has been well documented for several single gene–drug combinations. However, the clinical utility of a pre-emptive genotyping strategy using a pharmacogenetic panel has not been rigorously assessed. Methods: We conducted an open-label, multicentre, controlled, cluster-randomised, crossover implementation study of a 12-gene pharmacogenetic panel in 18 hospitals, nine community health centres, and 28 community pharmacies in seven European countries (Austria, Greece, Italy, the Netherlands, Slovenia, Spain, and the UK). Patients aged 18 years or older receiving a first prescription for a drug clinically recommended in the guidelines of the Dutch Pharmacogenetics Working Group (ie, the index drug) as part of routine care were eligible for inclusion. Exclusion criteria included previous genetic testing for a gene relevant to the index drug, a planned duration of treatment of less than 7 consecutive days, and severe renal or liver insufficiency. All patients gave written informed consent before taking part in the study. Participants were genotyped for 50 germline variants in 12 genes, and those with an actionable variant (ie, a drug–gene interaction test result for which the Dutch Pharmacogenetics Working Group [DPWG] recommended a change to standard-of-care drug treatment) were treated according to DPWG recommendations. Patients in the control group received standard treatment. To prepare clinicians for pre-emptive pharmacogenetic testing, local teams were educated during a site-initiation visit and online educational material was made available. The primary outcome was the occurrence of clinically relevant adverse drug reactions within the 12-week follow-up period. Analyses were irrespective of patient adherence to the DPWG guidelines. Die Pharmakogenetik ist ein wichtiger Baustein in der Individualisierung von Therapien. Eine pharmakogenetische Diagnostik findet jedoch bislang noch wenig Anwendung in der medizinischen Praxis. Eine konsequente Berücksichtigung individueller Patientenfaktoren in der Arzneimitteltherapie kann helfen, diese sicherer und effektiver zu gestalten. Es wird ein kurzer Überblick über Strukturen und Auswirkungen von genetischen Varianten auf Arzneimittelwirkungen gegeben. Einige häufig verordnete Medikamente werden exemplarisch herausgegriffen. Des Weiteren wird die Durchführbarkeit von pharmakogenetischer Diagnostik und Dosisanpassungen beschrieben. Die European Medicines Agency (EMA) als europäische Zulassungsbehörde hat bereits über 70, die U.S. Food and Drug Administration (FDA) sogar über 150 Arzneimittelinformationen um Angaben zu Biomarkern erweitert. Dies ist ein entscheidender Schritt in Richtung gezielte Therapie. Internationale Leitlinien für die Dosis- und Therapieanpassung basieren auf einer systematischen Datenauswertung der klinischen Evidenz beispielsweise durch das Clinical Pharmacogenetics Implementation Consortium des Pharmacogenomics Research Network. Wesentlich für den Erfolg einer Arzneimitteltherapie ist auch die Berücksichtigung individueller Risikofaktoren der Patienten. Eine pharmakogenetische Diagnostik muss von konkreten Dosierungs- und Therapieempfehlungen begleitet werden, um einen Beitrag zur Individualisierung und zur Verbesserung der Sicherheit und Effizienz der Arzneimitteltherapie leisten zu können. Pharmacogenetics are an important component in the individualization of treatment; however, pharmacogenetic diagnostics have so far not been used to any great extent in clinical practice. A consistent consideration of individual patient factors, such as pharmacogenetics may help to improve drug therapy and increase individual safety and efficacy aspects. A brief summary of structures and effects of genetic variations on drug efficacy is presented. Some frequently prescribed pharmaceuticals are specified. Furthermore, the feasibility of pharmacogenetic diagnostics and dose recommendations in the clinical practice are described. The European Medicines Agency (EMA) as the European approval authority has already extended the drug labels of more than 70 pharmaceuticals by information on pharmacogenetic biomarkers and the U.S. Food and Drug Administration (FDA) more than 150. This is a crucial step towards targeted medicine. Guidelines on dose and therapy adjustments are provided by the Clinical Pharmacogenetics Implementation Consortium of the Pharmacogenomics Research Network. It is fundamental to consider individual patient factors for successful drug therapy. Dose and therapy recommendations based on pharmacogenetic diagnostics are highly important for individualization as well as improvement of safety and efficiency of drug therapy.

Individual selection of the optimal drug and dose is required to attain the highest possible efficacy and acceptable tolerability for every patient.Methods: This review, which includes more than 500 articles selected by 30 experts, combines relevant knowledge on studies investigating the pharmacokinetics, pharmacodynamics and pharmacogenetics of 33 antidepressant drugs and of 4 drugs approved for augmentation in cases of insufficient response to antidepressant monotherapy. Such studies typically measure drug concentrations in blood (i.e. therapeutic drug monitoring) and genotype relevant genetic polymorphisms of enzymes, transporters or receptors involved in drug metabolism or mechanism of action. Imaging studies, primarily positron emission tomography that relates drug concentrations in blood and radioligand binding, are considered to quantify target structure occupancy by the antidepressant drugs in vivo. Results: Evidence is given that in vivo imaging, therapeutic drug monitoring and genotyping and/or phenotyping of drug metabolising enzymes should be an integral part in the development of any new antidepressant drug.Conclusions: To guide antidepressant drug therapy in everyday practice, there are multiple indications such as uncertain adherence, polypharmacy, nonresponse and/or adverse reactions under therapeutically recommended doses, where therapeutic drug monitoring and cytochrome P450 genotyping and/or phenotyping should be applied as valid tools of precision medicine.

Abstract

The aim of personalized medicine is to detach from a “one-size fits all approach” and improve patient health by individualization to achieve the best outcomes in disease prevention, diagnosis and treatment. Technological advances in sequencing, improved knowledge of omics, integration with bioinformatics and new in vitro testing formats, have enabled personalized medicine to become a reality. Individual variation in response to environmental factors can affect susceptibility to disease and response to treatments. Space travel exposes humans to environmental stressors that lead to physiological adaptations, from altered cell behavior to abnormal tissue responses, including immune system impairment. In the context of human space flight research, human health studies have shown a significant inter-individual variability in response to space analogue conditions. A substantial degree of variability has been noticed in response to medications (from both an efficacy and toxicity perspective) as well as in susceptibility to damage from radiation exposure and in physiological changes such as loss of bone mineral density and muscle mass in response to deconditioning. At present, personalized medicine for astronauts is limited. With the advent of longer duration missions beyond low Earth orbit, it is imperative that space agencies adopt a personalized strategy for each astronaut, starting from pre-emptive personalized pre-clinical approaches through to individualized countermeasures to minimize harmful physiological changes and find targeted treatment for disease. Advances in space medicine can also be translated to terrestrial applications, and vice versa. This review places the astronaut at the center of personalized medicine, will appraise existing evidence and future preclinical tools as well as clinical, ethical and legal considerations for future space travel.

Abstract

Background: There is a need for a comprehensive evaluation of the associations between varieties of weather conditions on the time spent out-of-home (TOH) and on walking duration (WD) among older adults. We aim to investigate the extent to which various weather parameters (temperature, solar radiation, sunshine duration, humidity, windspeed, and rain) determine daily WD the TOH in older adults. Methods: The ActiFE (Activity and Function in Older People in Ulm) study is a prospective study of participants aged 65 years or older who wore an accelerometer and kept a movement diary in up to three temporally separated waves from 2009 to 2018 for a duration up to seven days per wave (up to three weeks in summary). We used weather data from a weather station near the participants' homes. Age-adjusted and gender-stratified generalized mixed models were used to predict WD and TOH (with 95% confidence interval (CI)) within and between weather categories. Generalized additive models were computed for the single predictions at the weather quartile boundaries. Cubic splines (with 95% pointwise confidence bands (CB)) visualized the continuous course of the weather values. Results: Higher temperatures, solar radiation and more hours of sunshine, led to an increase in WD and TOH, while higher precipitation, humidities and windspeeds led to a decrease. Women had in general higher WD and TOH times than men. Conclusions: Our data suggest that weather parameters have a considerable influence on PA and TOH. Abstract

Background The aim of this study was to assess the effect of day of the week and wearing a device (reactivity) on objectively measured physical activity (PA) in older people. Methods Walking duration as a measure for PA was recorded from 1333 German community-dwelling older people (≥65 years, 43.8% women) over 5 days using accelerometers (activPAL). Least-square means of PA with 95%-confidence intervals (95%-CI) from multi-level analysis were calculated for each day of the week and each measurement day (days after sensor attachment). Results Walking duration on Sundays was significantly lower compared to working days (Sunday vs. Monday-Friday: − 12.8 min (95%-CI: − 14.7; − 10.9)). No statistically significant difference compared to working days was present for Saturdays. The linear slope for measurement day and walking duration was marginal and not statistically significant. Conclusions Studies using PA sensors in older people should assess Sundays and working days to adequately determine the activity level of the participants.

Abstract

Im Rahmen des EMPAR-Projektes – Einfluss metabolischer Profile auf die Arzneimitteltherapiesicherheit in der Routineversorgung – wird in den Forschungsschwerpunkten „Pharmakogenomik und individualisierte Medizin“ sowie „Pharmakoepidemiologie“ des BfArM untersucht, inwieweit Menschen aufgrund ihrer genetischen Veranlagungen auf ein Arzneimittel unterschiedlich reagieren und welchen Einfluss die genetische Variabilität auf die Arzneimittelwirksamkeit, Arzneimittelsicherheit und auf die Inanspruchnahme von Versorgungsleistungen besitzt. Sowohl die Pharmakokinetik als auch die Pharmakodynamik von Arzneimitteln unterliegen zum Teil erheblichen genetischen Einflüssen. Beim Abbau von Arzneimitteln im Körper spielen Enzyme eine wichtige Rolle, wobei über ein Drittel aller in der Versorgungspraxis verwendeten Arzneistoffe (oder ihrer Metaboliten) über polymorphe Cytochrom-P450-Enzyme (z.B. CYP2D6, CYP2C9, CYP2C19) verstoffwechselt werden.1 Sind polymorphe Enzyme am Metabolismus eines Arzneimittels beteiligt, kann das schwer vorherzusagende Auswirkungen auf die Wirkstoffspiegel im Körper haben und zu Unterschieden in der Wirksamkeit und Sicherheit der Arzneimitteltherapie führen. Denn aufgrund der genetischen Variante kann die Enzymaktivität erhöht oder vermindert sein. Sogenannte Schnell-Metabolisierer bauen die Wirkstoffe in Arzneimitteln deutlich schneller ab als die meisten Menschen, was das Risiko erhöht, dass die therapeutische Wirkung ausbleibt. Im Gegensatz zu Schnell-Metabolisierern bauen sogenannte Langsam-Metabolisierer Wirkstoffe weniger schnell ab als die meisten Menschen. Dies kann aufgrund erhöhter Wirkspiegel bei normaler Dosis zu stärkeren Nebenwirkungen führen. Bei Prodrugs, Arzneimitteln, bei denen der Metabolit die aktive Substanz ist, verhält sich der Zusammenhang zwischen Wirkstoffspiegel und Metabolisierungsstatus genau umgekehrt. Schnell-Metabolisierer unterliegen dem Risiko eines erhöhten Wirkspiegels und Langsam-Metabolisierer dem eines erniedrigten. Die Pharmakogenetik spielt somit eine wichtige Rolle im Hinblick auf die Arzneimitteltherapiesicherheit im Rahmen einer individuell auf den Patienten abgestimmten Therapie und personalisierten Medizin.

Abstract

Aim: The need for pharmacogenomic education is becoming more and more urgent. Our aim was to evaluate the progress in pharmacogenomics education since then, and to put forward further recommendations. Methods: A survey was sent to 248 schools of medicine, pharmacy, nursing and health professions around the world. Results: The majority of the study programs (87%) include pharmacogenomics education, which is generally taught as part of the pharmacology curriculum. On average, educators and teachers have selected appropriate and highly relevant pharmacogenomics biomarkers to include in their teaching programs. Conclusions: Based on the results, we can conclude that the state of pharmacogenomics education at the surveyed universities has improved substantially since 2005.
