OBJECTIVES: It is unclear how newer methods of respiratory support for infants born extremely preterm (EP; 22-27 weeks gestation) have affected in-hospital sequelae. We aimed to determine changes in respiratory support, survival and morbidity in EP infants since the early 1990s. DESIGN: Prospective longitudinal cohort study. SETTING: The State of Victoria, Australia. PARTICIPANTS: All EP births offered intensive care in four discrete eras (1991-1992 (24 months): n=332, 1997 (12 months): n=190, 2005 (12 months): n=229, and April 2016-March 2017 (12 months): n=250). OUTCOME MEASURES: Consumption of respiratory support, survival and morbidity to discharge home. Cost-effectiveness ratios describing the average additional days of respiratory support associated per additional survivor were calculated. RESULT(S): Median duration of any respiratory support increased from 22 days (1991-1992) to 66 days (2016-2017). The increase occurred in non-invasive respiratory support (2 days (1991-1992) to 51 days (2016-2017)), with high-flow nasal cannulae, unavailable in earlier cohorts, comprising almost one-half of the duration in 2016-2017. Survival to discharge home increased (68% (1991-1992) to 87% (2016-2017)). Cystic periventricular leukomalacia decreased (6.3% (1991-1992) to 1.2% (2016-2017)), whereas retinopathy of prematurity requiring treatment increased (4.0% (1991-1992) to 10.0% (2016-2017)). The average additional costs associated with one additional infant surviving in 2016-2017 were 200 (95% CI 150 to 297) days, 326 (183 to 1127) days and 130 (70 to 267) days compared with 1991-1992, 1997 and 2005, respectively. CONCLUSION(S): Consumption of resources for respiratory support has escalated with improved survival over time. Cystic periventricular leukomalacia reduced in incidence but retinopathy of prematurity requiring treatment increased. Background: Little evidence is available on the use of telehealth for antenatal care. In response to the COVID-19 pandemic, we developed and implemented a new antenatal care schedule integrating telehealth across all models of pregnancy care. To inform this clinical initiative, we aimed to assess the effectiveness and safety of telehealth in antenatal care. Method(s): We analysed routinely collected health data on all women giving birth at Monash Health, a large health service in Victoria (Australia), using an interrupted time-series design. We assessed the impact of telehealth integration into antenatal care from March 23, 2020, across low-risk and high-risk care models. Allowing a 1-month implementation period from March 23, 2020, we compared the first 3 months of telehealth integrated care delivered between April 20 and July 26, 2020, with conventional care delivered between Jan 1, 2018, and March 22, 2020. The primary outcomes were detection and outcomes of fetal growth restriction, pre-eclampsia, and gestational diabetes. Secondary outcomes were stillbirth, neonatal intensive care unit admission, and preterm birth (birth before 37 weeks' gestation). Finding(s): Between Jan 1, 2018, and March 22, 2020, 20 031 women gave birth at Monash Health during the conventional care period and 2292 women gave birth during the telehealth integrated care period. Of 20 154 antenatal consultations provided in the integrated care period, 10 731 (53%) were delivered via telehealth. Background: Phenobarbitone (PB) is the first-line anti-convulsant for neonatal seizures. The use of peritoneal dialysis (PD) to enhance drug elimination in cases of neonatal PB overdose has not been reported. Objective(s): To report a case of neonatal severe PB toxicity and review the elimination of PB by PD. Method(s): Assessment of PD drug clearance. Result(s): A neonate with prolonged seizures was administered PB. Encephalopathy and myocardial failure developed, which were initially suspected to be secondary to hypoxia. At 42 h of age, the serum PB concentration was in the toxic range at 131 mg/L. Despite supportive care, the infant's condition deteriorated with escalating inotropes and the need for CPR. Enhanced PB elimination via multiple-dose activated charcoal and exchange transfusion were considered too risky. Hourly PD cycles via Tenckhoff catheter were commenced, based on reports suggesting that PD enhances PB clearance. The clinical state of the infant then improved. PD administration was continued for 60 h, recovering 20% of the estimated total PB body load. The infant survived and there were no PD complications. Conclusion(s): PD increased PB clearance in this neonate, correlating with clinical recovery. Background: Effective interprofessional collaboration (IPC) improves quality and safety of patient care and clinician job satisfaction. However, the reality of IPC is complex, influenced by interpersonal, organisational and cultural factors. The purpose of this study was to explore the nature of IPC between midwives and junior neonatal doctors in a post-natal ward setting. Method(s): An ethnographic methodology was used, comprising observations of ward-based work on the post-natal ward of a tertiary Australian hospital and individual interviews with staff between June and August 2014. Activity theory informed data collection, analysis and synthesis. Result(s): Sixteen hours of observation and eight interviews (n = 4 midwives, n = 4 neonatal registrars) were completed. Solo work predominated. Collaboration was brief, opportunistic and task-focussed. The midwife in charge of the ward was a conduit for communication, but other midwives were often 'out of the loop'. The primary objective of the activity system was 'sign off', facilitating discharge of mothers and babies and allowing beds to be reallocated. Contradictions within the system ncluded competing objectives, differing models of care, lack of clarity in division of labour, questioning of process rules and duplicate treatment guidelines. Enabling and constraining factors were conceptualised as relational, processual and organisational. Conclusion(s): The sporadic and improvised nature of collaboration between midwives and neonatal registrars is in keeping with published IPC literature and with the concept of 'knotworking'. There is a need to prepare junior clinicians for capability and adaptability, and for socialisation into interprofessionalism.

Background: WHO guidelines recommend exclusive breastfeeding from birth until at least 6months of age. Barriers to establishment of breastfeeding in newborn infants with complex congenital surgical anomalies include maternal-infant separation, complex nutritional needs, the neonatal intensive care unit (NICU) environment and staff attitudes. The primary aim of this study was to determine breastfeeding outcomes at discharge in newborn infants with complex congenital surgical anomalies who were admitted to an Australian NICU. Method(s): Retrospective audit of medical records of all infants admitted with a diagnosis of complex congenital surgical anomalies (tracheoesophageal fistula, esophageal atresia, congenital diaphragmatic hernia, gastroschisis, omphalocele) between 1/1/ 2012 and 30/6/2014. Result(s): 42 infants met inclusion criteria during the audit period, of whom 38 survived to discharge. Gastroschisis accounted for 52% of cases. Median gestational age and birth weight was 37.1weeks, and 2785 g, respectively. All infants received expressed breastmilk as their first feed, at a median age of 7 days. At discharge, all infants received at least partial breastmilk feeds, and 82% of infants were exclusively breastfed. At the age of 6months, feeding data was only available for 60% of infants; of these, exclusive breastfeeding rates had fallen to 39%. Conclusion(s): The breastfeeding outcomes at discharge in this cohort of infants with complex surgical anomalies compares favourably with published breastfeeding rates at hospital discharge for preterm and unwell newborn infants, which are in the range of 49-91%. The decline in breastfeeding rates at 6months is also consistent with the literature but needs further prospective investigation.
