Background Obesity is a known risk factor for developing endometrial cancer. However, the association of obesity with endometrial cancer (EC) outcomes has not been clearly established. This study examined how outcomes in women with early stage EC vary with body composition measured via computed tomography (CT). Methods In this retrospective study, patients diagnosed with EC international Federation of Gynecology and Obstetrics stages I-III and available CT scans were included. Automatica software was used to assess the areas of visceral adipose tissue, subcutaneous adipose tissue (SAT), and intermuscular adipose tissue (IMAT) and skeletal muscle area. Results Of 293 patient charts assessed, 199 met eligibility criteria. Median body mass index (BMI) was 32.8 kg/m2 (interquartile range [IQ] = 26.8-38.9); 61.8% had histologic subtype endometrioid carcinoma. Adjusted for age, international Federation of Gynecology and Obstetrics stage, and histologic subtype, a BMI of at least 30 vs less than 30 kg/m2 was associated with lower endometrial cancer–specific survival (ECSS) (hazard ratio [HR] = 2.32, 95% confidence interval [CI] = 1.27 to 4.25) and overall survival (OS) (HR = 2.7, 95% CI = 1.35 to 5.39). Higher IMAT 75th vs 25th percentile and SAT of at least 225.6 vs less than 225.6 cm2 were associated with lower ECSS (HR = 1.53, 95% CI = 1.1 to 2.13, and HR = 2.57, 95% CI = 1.13 to 5.88) and OS (HR = 1.50, 95% CI = 1.11 to 2.02, and HR = 2.46, 95% CI = 1.2 to 5.01), respectively. The association of visceral adipose tissue (75th vs 25th percentile) with ECSS and OS was not statistically significant (HR = 1.42, 95% CI = 0.91 to 2.22, and HR = 1.24, 95% CI = 0.81 to 1.89). Conclusion Higher BMI, IMAT, and SAT were associated with higher mortality from EC and lower OS. A better understanding of the mechanisms underlying these relationships could inform strategies to improve patient outcomes.

Purpose:The relevance of the MET/hepatocyte growth factor pathway in endometrial cancer tumor biology supports the clinical evaluation of cabozantinib in this disease.Patients and Methods:PHL86/NCI#9322 (NCT01935934) is a single arm study that evaluated cabozantinib (60 mg once daily) in women with endometrial cancer with progression after chemotherapy. Coprimary endpoints were response rate and 12-week progression-free-survival (PFS). Patients with uncommon histology endometrial cancer (eg, carcinosarcoma and clear cell) were enrolled in a parallel exploratory cohort.Results:A total of 102 patients were accrued. Among 36 endometrioid histology patients, response rate was 14%, 12-week PFS rate was 67%, and median PFS was 4.8 months. In serous cohort of 34 patients, response rate was 12%, 12-week PFS was 56%, and median PFS was 4.0 months. In a separate cohort of 32 patients with uncommon histology endometrial cancer (including carcinosarcoma), response rate was 6% and 12-week PFS was 47%. Six patients were on treatment for >12 months, including two for >30 months. Common cabozantinib-related toxicities (>30% patients) included hypertension, fatigue, diarrhea, nausea, and hand–foot syndrome. Gastrointestinal fistula/perforation occurred in four of 70 (6%) patients with serous/endometrioid cancer and five of 32 (16%) patients in exploratory cohort. We observed increased frequency of responses with somatic CTNNB1 mutation [four partial responses (PRs) in 10 patients, median PFS 7.6 months] and concurrent KRAS and PTEN/PIK3CA mutations (three PRs in 12 patients, median PFS 5.9 months).Conclusions:Cabozantinib has activity in serous and endometrioid histology endometrial cancer. These results support further evaluation in genomically characterized patient cohorts.

Contains fulltext : 283536.pdf (Publisher’s version ) (Closed access) The Gynecologic Cancer InterGroup (GCIG) sixth Ovarian Cancer Conference on Clinical Research was held virtually in October, 2021, following published consensus guidelines. The goal of the consensus meeting was to achieve harmonisation on the design elements of upcoming trials in ovarian cancer, to select important questions for future study, and to identify unmet needs. All 33 GCIG member groups participated in the development, refinement, and adoption of 20 statements within four topic groups on clinical research in ovarian cancer including first line treatment, recurrent disease, disease subgroups, and future trials. Unanimous consensus was obtained for 14 of 20 statements, with greater than 90% concordance in the remaining six statements. The high acceptance rate following active deliberation among the GCIG groups confirmed that a consensus process could be applied in a virtual setting. Together with detailed categorisation of unmet needs, these consensus statements will promote the harmonisation of international clinical research in ovarian cancer.

Excessive steepness of grasslands, hills, or uneven terrain presents difficulties for farming with large conventional equipment. Therefore, a fleet of Autonomous Ground Vehicles (AGV) is proposed to perform primary agricultural operations on high sloped hills or terrain. However, it is imperative to understand how an individual AGV functions on sloping terrain under varying load and speed. Hence, this study aims to investigate the traction, mobility, and energy consumption characteristics of AGV on a sloped soil bin environment. A drawbar pull performance of the prototype AGV was evaluated on a level terrain and variable slope of 10° and 18°, both uphill and downhill, at varying drawbar pull (P) and AGV speed. The AGV’s performance metrics include power efficiency (PE), travel reduction (TR), and power number (PN) which relates to AGV’s traction, mobility, and energy usage, respectively. The AGV generated drawbar pull equivalent to its weight only on downhill run for reduced PE. On a level terrain (0°), the peak PE was 0.20 and was found to be 108.3% and 328.6% higher on 10° and 18° downhill run than uphill with 55.5% and 133% increase in drawbar pull, respectively. Both applied drawbar pull and uphill operation caused the AGV’s TR. The TR, corresponding to a peak PE, increased from 10% to 30%, respectively, on 0° and both 10° and 18° uphill. The optimum values of power number ranged from 2 to 4. The AGV delivers the optimum PE and generates enough drawbar pull with an optimum TR to perform a range of agricultural operations on a slope up to 18°. This study explored the suitability and established the boundary conditions of small size ground vehicles for high-sloped farming. Besides this, the study also aims to generate an AGV’s slope traction database to optimize its control variables, design optimization, and develop a mobility model for sloped terrain. This letter presents an experimental study on the use of ultrawideband (UWB) probing radar as a new type of sensor to efficiently and nondestructively measure plant structure for breeding applications, focusing here on wheat canopy height. Wheat canopy height is an important morphological and developmental phenotype that indicates plant growth and it is related to biomass. Currently, taking manual measurements of canopy height is labor-intensive and has become a bottleneck for genomic selection and breeding programs. Other recently proposed noncontact methods only sense the top of the canopy, thereby having to make assumptions about the elevation of the base of the canopy. We propose the use of UWB radar as a phenotyping sensor for measuring a variety of plant characteristics. A direct measurement of wheat canopy height is investigated with the capability of sensing both the top and the bottom of the canopy simultaneously. To test this idea, we developed a 2-18-GHz frequency-modulated-continuous-wave (FMCW) radar prototype and a mobile phenotyping platform. The sensor was tested by collecting near-nadir coherent radar measurements of wheat canopies of breeding plots at different growth stages. The performance to detect both the top and bottom of the canopy was examined at different frequency bands.

There is evidence that achieving a dose intensity &gt, 80% in adjuvant colon cancer treatment improves survival. In total, 192 consecutive patients with resected stage III and high-risk stage II colon cancer that received adjuvant chemotherapy were retrospectively analyzed. Patients who received at least 6 weeks of adjuvant therapy were included. The primary objective was to assess the influence of dose index (DI) and relative dose intensity (RDI) on DFS and OS at 3 and 5 years in patients receiving fluorouracil-based doublet therapy with oxaliplatin (FOLFOX)(5-FU and oxaliplatin assessed separately), or capecitabine monotherapy. In the capecitabine group, DFS rates for 3 and 5 years were 66.7% and 57.6%, respectively, while OS rates were 80.3% and 66.7%, respectively. Those who received FOLFOX had DFS rates of 76.9% and 71.2% at 3 and 5 years, respectively. OS rates were 86.4% and 76.7% at 3 and 5 years, respectively. Median RDI was 73.8% for capecitabine and 76.3% and 85.6% for the oxaliplatin and 5-FU components respectively. Based on a multivariate analysis in patients receiving FOLFOX, those with an oxaliplatin DI &gt, 80% had improvements in DFS and OS compared to those with an oxaliplatin DI of ≤ 80%. Otherwise, there was no significant difference in DFS or OS when comparing patients who achieved an RDI or a DI of above versus below 80% in the patients receiving adjuvant chemotherapy for resected colon cancer.

Aim: Cisplatin causes acute kidney injury (AKI) in approximately one-third of patients. Serum creatinine and urinary output are poor markers of cisplatin-induced (AKI). Metabolomics was utilized to identify predictive or early diagnostic biomarkers of cisplatin-induced AKI. Methods: Thirty-one adult head and neck cancer patients receiving cisplatin (dose ≥ 70 mg m2 -1) were recruited for metabolomics analysis. Urine and serum samples were collected prior to cisplatin (pre), 24-48 hours after cisplatin (24-48h), and 5-14 days (post) after cisplatin. Based on serum creatinine concentrations measured at the post timepoint, 11/31 patients were classified with clinical AKI. Untargeted metabolomics was performed using liquid chromatography-mass spectrometry. Results: Metabolic discrimination was observed between “AKI” patients and “no AKI” patients at all timepoints. Urinary glycine, hippuric acid sulfate, 3-hydroxydecanedioc acid, and suberate were significantly different between AKI patients and no AKI patients prior to cisplatin infusion. Urinary glycine and hippuric acid sulfate were lower (-2.22-fold and -8.85-fold), whereas 3-hydroxydecanedioc acid and suberate were higher (3.62-fold and 1.91-fold) in AKI patients relative to no AKI patients. Several urine and serum metabolites were found to be altered 24-48 hours following cisplatin infusion, particularly metabolites involved with mitochondrial energetics. Conclusion: We propose glycine, hippuric acid sulfate, 3-hydroxydecanedioc acid, and suberate as predictive biomarkers of predisposition to cisplatin-induced AKI. Metabolites indicative of mitochondrial dysfunction may serve as early markers of subclinical AKI.

Consensus guidelines exist for genotype‐guided fluoropyrimidine dosing based on variation in the gene dihydropyrimidine dehydrogenase (DPYD). However, these guidelines have not been widely implemented in North America and most studies of pretreatment DPYD screening have been conducted in Europe. Given regional differences in treatment practices and rates of adverse events (AEs), we investigated the impact of pretreatment DPYD genotyping on AEs in a Canadian context. Patients referred for DPYD genotyping prior to fluoropyrimidine treatment were enrolled from December 2013 through November 2019 and followed until completion of fluoropyrimidine treatment. Patients were genotyped for DPYD c.1905+1G>A, c.2846A>T, c.1679T>G, and c.1236G>A. Genotype‐guided dosing recommendations were informed by Clinical Pharmacogenetics Implementation Consortium guidelines. The primary outcome was the proportion of patients who experienced a severe fluoropyrimidine‐related AE (grade ≥3, Common Terminology Criteria for Adverse Events version 5.0). Secondary outcomes included early severe AEs, severe AEs by toxicity category, discontinuation of fluoropyrimidine treatment due to AEs, and fluoropyrimidine‐related death. Among 1394 patients, mean (SD) age was 64 (12) years, 764 (54.8%) were men, and 47 (3.4%) were DPYD variant carriers treated with dose reduction. Eleven variant carriers (23%) and 418 (31.0%) noncarriers experienced a severe fluoropyrimidine‐related AE (p = 0.265). Six carriers (15%) and 284 noncarriers (21.1%) experienced early severe fluoropyrimidine‐related AEs (p = 0.167). DPYD variant carriers treated with genotype‐guided dosing did not experience an increased risk for severe AEs. Our data support a role for DPYD genotyping in the use of fluoropyrimidines in North America.

On behalf of the Canadian Association of Medical Oncologists, we are pleased to present the Abstracts of the 2021 Annual Meeting. The National CAMO Residents Research Day was held virtually on 1 April 2021 and the CAMO Virtual Annual Scientific Meeting (ASM) & Annual General Meeting (AGM) took place on 22 April 2021. Twenty (20) abstracts were selected for presentation as oral presentations and rapid-fire presentations. Awards for the top three (3) abstracts were presented during the ASM and AGM. All of them were marked as “Award Recipient”. We congratulate all the presenters on their research work and contribution.

The annual Eastern Canadian Gastrointestinal Cancer Consensus Conference 2019 was held in Morell, Prince Edward Island, 19–21 September 2019. Experts in medical oncology, radiation oncology, and surgical oncology who are involved in the management of patients with gastrointestinal malignancies participated in presentations and discussion sessions for the purpose of developing the recommendations presented here. This consensus statement addresses multiple topics in the management of anal, colorectal, biliary tract, and gastric cancers, including: radiotherapy and systemic therapy for localized and advanced anal cancer; watch and wait strategy for the management of rectal cancer; role of testing for dihydropyrimidine dehydrogenase (DPD) deficiency prior to commencement of fluoropyrimidine therapy; radiotherapy and systemic therapy in the adjuvant and unresectable settings for biliary tract cancer; and radiotherapy and systemic therapy in the perioperative setting for early-stage gastric cancer.

Purpose The purpose of this paper is to propose a simplified representation of the fire load in computational fluid dynamics (CFD) to represent the effect of large-scale travelling fire and to highlight the relevance of such an approach whilst coupling the CFD results with finite element method (FEM) to evaluate related steel temperatures, comparing the numerical outcomes with experimental measurements. Design/methodology/approach This paper presents the setup of the CFD simulations (FDS software), its corresponding assumptions and the calibration via two natural fire tests whilst focusing on gas temperatures and on steel temperatures measured on a central column. For the latter, two methods are presented: one based on EN 1993-1-2 and another linking CFD and FEM (SAFIR® software). Findings This paper suggests that such an approach can allow for an acceptable representation of the travelling fire both in terms of fire spread and steel temperatures. The inevitable limitations inherent to the simplifications made during the CFD simulations are also discussed. Regarding steel temperatures, the two methods lead to quite similar results, but with the ones obtained via CFD–FEM coupling are closer to those measured. Originality/value This work has revealed that the proposed simplified representation of the fire load appears to be appropriate to evaluate the temperature of steel structural elements within reasonable limits on computational time, making it potentially desirable for practical applications. This paper also presents the first comparisons of FDS–SAFIR® coupling with experimental results, highlighting promising outcomes.

Contains fulltext : 241226.pdf (Publisher’s version ) (Closed access) BACKGROUND: Olaparib, a poly (ADP-ribose) polymerase (PARP) inhibitor, has previously been shown to extend progression-free survival versus placebo when given to patients with relapsed high-grade serous or endometrioid ovarian cancer who were platinum sensitive and who had a BRCA1 or BRCA2 (BRCA1/2) mutation, as part of the SOLO2/ENGOT-Ov21 trial. The aim of this final analysis is to investigate the effect of olaparib on overall survival. METHODS: This double-blind, randomised, placebo-controlled, phase 3 trial was done across 123 medical centres in 16 countries. Eligible patients were aged 18 years or older, had an Eastern Cooperative Oncology Group performance status at baseline of 0-1, had histologically confirmed, relapsed, high-grade serous or high-grade endometrioid ovarian cancer, including primary peritoneal or fallopian tube cancer, and had received two or more previous platinum regimens. Patients were randomly assigned (2:1) to receive olaparib tablets (300 mg in two 150 mg tablets twice daily) or matching placebo tablets using an interactive web or voice-response system. Stratification was by response to previous chemotherapy and length of platinum-free interval. Treatment assignment was masked to patients, treatment providers, and data assessors. The primary endpoint of progression-free survival has been reported previously. Overall survival was a key secondary endpoint and was analysed in all patients as randomly allocated. Safety was assessed in all patients who received at least one treatment dose. This trial is registered with ClinicalTrials.gov, NCT01874353, and is no longer recruiting patients. FINDINGS: Between Sept 3, 2013 and Nov 21, 2014, 295 patients were enrolled. Patients were randomly assigned to receive either olaparib (n=196 [66%]) or placebo (n=99 [34%]). One patient, randomised in error, did not receive olaparib. Median follow-up was 65·7 months (IQR 63·6-69·3) with olaparib and 64·5 months (63·4-68·7) with placebo. Median overall survival was 51·7 months (95% CI 41·5-59·1) with olaparib and 38·8 months (31·4-48·6) with placebo (hazard ratio 0·74 [95% CI 0·54-1·00]; p=0·054), unadjusted for the 38% of patients in the placebo group who received subsequent PARP inhibitor therapy. The most common grade 3 or worse treatment-emergent adverse event was anaemia (which occurred in 41 [21%] of 195 patients in the olaparib group and two [2%] of 99 patients in the placebo group). Serious treatment-emergent adverse events were reported in 50 (26%) of 195 patients receiving olaparib and eight (8%) of 99 patients receiving placebo. Treatment-emergent adverse events with a fatal outcome occurred in eight (4%) of the 195 patients receiving olaparib, six of which were judged to be treatment-related (attributed to myelodysplastic syndrome [n=3] and acute myeloid leukaemia [n=3]). INTERPRETATION: Olaparib provided a median overall survival benefit of 12·9 months compared with placebo in patients with platinum-sensitive, relapsed ovarian cancer and a BRCA1/2 mutation. Although statistical significance was not reached, these findings are arguably clinically meaningful and support the use of maintenance olaparib in these patients. FUNDING: AstraZeneca and Merck.

In the frame of the European RFCS-TRAFIR project, natural fire tests in large compartment were conducted by Ulster University, involving steel structure and aiming at understanding the conditions in which a travelling fire develops, how it behaves and impacts the surrounding structure. During the experimental programme, the path and geometry of the travelling fire was studied and temperatures, heat fluxes and spread rates were measured. The experimental data is presented in terms of gas temperatures recorded in the test compartment at different positions and levels. The influence of the travelling fire on the surround structure is presented in terms of the temperatures recorded in the selected steel columns and beams. The temperatures in the test compartment were dependent on the positioning of the travelling fire band as well as the height from the floor level. The non-uniform temperatures in the compartment lead to transient heating of the nearby structural steel elements, resulting in a reduction of their resistance which may influence the global structural stability. The results obtained will help to understand the behaviour of travelling fires and their influence on the structural members. This knowledge will help to reduce the travelling fire associated risks in future.

Characterising the risk of the fire spread in informal settlements relies on the ability to understand compartment fires with boundary conditions that are significantly different to normal residential compartments. Informal settlement dwellings frequently have thermally thin and leaky boundaries. Due to the unique design of these compartments, detailed experimental studies were conducted to understand their fire dynamics. This paper presents the ability of FDS to model these under-ventilated steel sheeted fire tests. Four compartment fire tests were modelled with different wall boundary conditions, namely sealed walls (no leakage), non-sealed walls (leaky), leaky walls with cardboard lining, and highly insulated walls; with wood cribs as fuel and ISO-9705 room dimensions. FDS managed to capture the main fire dynamics and trends both qualitatively and quantitatively. However, using a cell size of 6 cm, the ability of FDS to accurately model the combustion at locations with high turbulent flows (using the infinitely fast chemistry mixing controlled combustion model), and the effect of leakage, was relatively poor and both factors should be further studied with finer LES filter width. Using the validated FDS models, new flashover criteria for thermally thin compartments were defined as a combination of critical hot gas layer and wall temperatures. Additionally, a parametric study was conducted to propose an empirical correlation to estimate the onset Heat Release Rate required for flashover, as current knowledge fails to account properly for large scale compartments with thermally thin boundaries. The empirical correlation is demonstrated to have an accuracy of ≈ ± 10% compared with the FDS models.

Female patients with breast cancer frequently develop arthralgia when treated with aromatase inhibitors (AI). Although the mechanism of AI-induced arthralgia is unknown, potential biomarkers have been identified. The purpose of this study was to investigate the clinical and genetic predictors of AI-induced arthralgia in a prospective cohort of patients with estrogen receptor-positive breast cancer. One hundred and ninety-six patients were enrolled at initiation of AI therapy with either letrozole or anastrozole. Patients completed two validated self-report questionnaires assessing pain, stiffness, and physical function at baseline, and repeated the questionnaires at two and at six months after the initiation of treatment with an AI. Germline DNA of all patients was genotyped for seven single-nucleotide polymorphisms (SNPs) previously identified by genetic screens and genome-wide association studies as associated with AI-induced arthralgia. More than 50% of the study group experienced arthralgia symptoms. Genetic analysis revealed that four SNPs, in CYP19A1 (rs4775936) and ESR1 (rs9322336, rs2234693, rs9340799), were associated with the development of arthralgia (adjusted P = 0.016, 0.018, 0.017, 0.047). High body mass index (BMI) was also associated with the development of arthralgia symptoms (adjusted P = 0.001). Patients prescribed letrozole were significantly more likely to develop arthralgia than patients on anastrozole (P = 0.018), and also more likely to discontinue AI therapy due to arthralgia. The CYP19A1 (rs4775936) SNP was significantly associated with discontinuation of therapy due to intolerable arthralgia. Our results suggested that BMI and AI drug (letrozole versus anastrozole) were clinical predictors of arthralgia, while genetic variants rs4775936, rs9322336, rs2234693, and rs9340799 were genetic predictors of AI-induced arthralgia. Significantly, rs4775936 was also a predictor of discontinuation of therapy.

Title: Association between genetic mutations and risk of venous thromboembolism in patients with solid tumor malignancies: A systematic review and meta-analysis. Introduction: Venous thromboembolisms (VTE) is a frequent complication in cancer patients with an overall incidence of approximately 20% and is associated with significant morbidity, mortality, and burden on the health care system [1]. VTE risk varies according to tumor type and it is possible that certain genetic mutations may promote hypercoagulability resulting in a prothrombotic state. We have studied the association between genetic mutations and risk of VTE in patients with melanoma, small cell lung cancer (SCLC), non-small cell lung cancer (NSCLC), colon, gastric and ovarian cancers. Aim: To evaluate the association of genetic mutations in melanoma, lung (SCLC, NSCLC), colon, gastric and ovarian cancer with the risk of venous thromboembolism. Search strategy: We conducted a systematic literature search in MEDLINE and EMBASE electronic databases from inception to May 2020 using a combination of the following subject headings and text words: gene mutation, genetic polymorphism, oncogenes, tumor-related genes, proto-oncogenes, oncoproteins, venous thromboembolism, venous thrombosis, pulmonary embolism, lung neoplasms (SCLC, NSCLC), gastrointestinal neoplasms, ovarian neoplasms, colorectal neoplasms, and melanoma. Selection criteria: We included studies presenting data on genetic mutations with > 5% prevalence in our population of interest [adult patients with melanoma, lung (SCLC, NSCLC), colon, gastric and ovarian cancer]. Studies with thrombosis events given, but not listed according to the mutation status of patients were excluded. Retrospective and prospective cohort studies, case-control studies and randomized clinical trials (RCTs) were included. Data collection and analysis: General descriptors for the patients in each study were collected, as well as number of patients with wildtype or mutated genes of interest that developed thrombosis. Fixed and random effects models were used to generate pooled adjusted ratios with 95% confidence intervals using data for thrombosis events in patients with mutated and wildtype driver genes of interest. The quality of the studies: Study qualities were assessed using Jadad score for randomized controlled trials and the Newcastle-Ottawa Scale for case-control and cohort studies. Main results: Of 616 eligible articles, we included 31 articles based on our inclusion criteria and 10 were included in the meta-analysis. In patients with lung cancer with EGFR, KRAS and ALK mutations the relative risk (RR) of VTE was 1.006 (40.084-11.150, P=0.965), 1.034 (0.514-2.081, P=0.925) and 1.571 (1.314-1.878, P Conclusion: In patients with lung cancer, those bearing ALK rearrangements carried a significantly higher relative risk of developing thrombosis than those with wild-type ALK.
