16. Persistence of Nontraditional Undergraduate Online Students: Towards a Contemporary Conceptual Framework

- Abstract
Since the mid-1970s, researchers have provided theoretical frameworks that continue to be used today to explain persistence and attrition of nontraditional undergraduate students in higher education. Previous frameworks emphasized specific skills nontraditional undergraduate students need upon admission into an online program to support their persistence. Recent research has confirmed the importance of these skills to student persistence in an online program. While these frameworks remain relevant, none reflect advancements and developments that have shaped today's online learning environment. The characteristics of nontraditional undergraduate online students and programs have evolved over the years. Hence, this article introduces a modified Contemporary Persistence Model, predicated on existing frameworks, and informed by the results of recent research into nontraditional undergraduate online students. The projected growth in online learning, coupled with the increased interest among nontraditional undergraduate students to enroll in online programs, necessitates a persistence model that is aligned with the contemporary needs of students.

21. Identifying Significant Personal and Program Factors That Predict Online EdD Students' Program Integration

- Abstract
Based on a synthesis of persistence theory and the empirical literature, an online doctoral program integration model was developed using data from 232 online EdD students. A predictive, correlation design and regression analysis were used to examine if personal factors (gender, race, age, marital status, and presence of children in the home) and program factors (stage in doctoral journey, synchronous interactions, cohorts, and orientations) could predict program integration. The entire model was significant. The variables of gender, race, participation in a cohort, and engagement in synchronous communication individually contributed to the variance in program integration.

29. A Pilot Study on Concurrent Learning/Teaching Model (CLTM) for Online and In-Class Informatics Students

- Abstract
The Informatics program at Mercer University is offered at four regional academic centers located throughout the state of Georgia. We serve non-traditional students who have primary responsibilities such as caring for family, working, and participating in their communities. We aim to offer availability and access to all required courses, access to full-time faculty instruction, and a realistic schedule to graduation. We face two challenges. One is how to operate multiple courses with limited faculty members while maintaining the minimum operation cost. We are offering online education as one possible solution and this has its own challenges. Subsequently, our second challenge is how to deliver the same quality in-class learning environment in an online format for a course in the technology domain. This past year, Informatics faculty conducted a pilot study on concurrent learning environments for online and in-class Informatics students. Surprisingly, 73% of students favor the concurrent online learning environment. In this paper, we describe the planning, designing, implementation and assessment of the first pilot of our concurrent learning/teaching model. [For the full proceedings, see ED575713.]

30. A High-Impact Practice for Online Students: The Use of a First-Semester Seminar Course to Promote Self-Regulation, Self-Direction, Online Learning Self-Efficacy

- Abstract
Student enrollment continues to increase in online programs, but there is concern surrounding the reportedly high rates of attrition in online classes compared to face-to-face classes. Undergraduate students are poorly prepared and lack the human agency necessary for success in the online learning environment. To address the lack of persistence of undergraduate online students, universities must create and implement interventions that prepare students for the online learning environment and help them develop as autonomous learners. This study examined whether differences in self-regulation, self-direction, and online learning self-efficacy exist between students participating in an experimental high-impact First-Semester Seminar (FSS) class and a traditional FSS class while controlling for pre-existing factors. A quantitative, quasi-experimental, pretest-posttest research design was used for this study with nonequivalent control groups, and multivariate analysis of covariance (MANCOVA) and follow up analyses of covariances (ANCOVA) were used to analyze the data. MANCOVA results revealed a statistically significant difference between groups. Follow-up ANCOVAs revealed differences between the posttest scores of the traditional FSS class and the high-impact FSS class on the measurements for self-directed learning and self-regulated learning.

- Abstract
This quantitative non-experimental correlational study examined the associations among the predictor variables of first year, first semester nontraditional online learners' self-regulation, self-direction, and self-efficacy with the criterion variable of semester-to semester persistence. A nonprobability convenience sampling method was used to select nontraditional learners enrolled in online undergraduate degree level courses during the Fall semester of the 2018-2019 academic year at a private higher education institution in the Southeast region of the United States. A logistical regression analysis demonstrated that nontraditional online learner semester-to-semester persistence can be explained by the combination of self-efficacy, self-regulation, and self-directedness. The entire model containing all the predictor and criterion variables significantly predicted whether or not first-semester, first-year nontraditional, online learners would persist beyond their second semester at the current institution. The adequacy of the model was further supported by the results of the Hosmer and Lemeshow test, which confirmed that the model is a good fit for predicting online, nontraditional learner persistence.

Personalised medicine is replacing the one-drug-fits-all approach with many prognostic models incorporating biomarkers available for risk stratifying patients. Evidence has been emerging that the effects of biomarkers change over time and therefore violate the assumption of proportional hazards when performing Cox regression. Analysis using the Cox model when the assumptions are invalid can result in misleading conclusions. This thesis reviews existing approaches for the analysis of non-proportional effects with respect to survival data. A number of well-developed approaches were identified but to date their uptake in practice has been limited. There is a need for more widespread use of flexible modelling to move away from standard analysis using a Cox model when the assumption of proportional hazards is violated. Two novel approaches were applied to investigate the impact of follow-up duration on two residual risk models, IHC4 and Mammostrat, for predicting risk in early breast cancers using two studies with different lengths of follow up; the Edinburgh Breast Conservation Series (BCS) and the Tamoxifen versus Exemestane Adjuvant Multinational (TEAM) trial. Similar results were observed between the two approaches that were considered, the multivariable fractional polynomial time (MFPT) approach and Royston-Parmer flexible parametric models, with their respective advantages and disadvantages being discussed. The analyses identified a strong time-varying effect of IHC4 score with the prognostic effect of IHC4 score on time-to distant recurrence decreasing with increasing follow-up time. Mammostrat score identified a group of patients with an increased risk of distant recurrence over full follow-up in the TEAM and Edinburgh BCS cohorts. The results suggest a combined IHC4 and Mammostrat risk score could provide information on the risk of recurrence and warrants further study.

Purpose Reliable biomarkers for predicting subsequent sepsis among patients with suspected acute infection are lacking. In patients presenting to emergency departments (EDs) with suspected acute infection, we aimed to evaluate the reliability and discriminant ability of 47 leukocyte biomarkers as predictors of sepsis (Sequential Organ Failure Assessment score [greater than or equal to] 2 at 24 h and/or 72 h following ED presentation). Methods In a multi-centre cohort study in four EDs and intensive care units (ICUs), we standardised flow-cytometric leukocyte biomarker measurement and compared patients with suspected acute infection (cohort-1) with two comparator cohorts: ICU patients with established sepsis (cohort-2), and ED patients without infection or systemic inflammation but requiring hospitalization (cohort-3). Results Between January 2014 and February 2016, we recruited 272, 59 and 75 patients to cohorts 1, 2, and 3, respectively. Of 47 leukocyte biomarkers, 14 were non-reliable, and 17 did not discriminate between the three cohorts. Discriminant analyses for predicting sepsis within cohort-1 were undertaken for eight neutrophil (cluster of differentiation antigens (CD) CD15; CD24; CD35; CD64; CD312; CD11b; CD274; CD279), seven monocyte (CD35; CD64; CD312; CD11b; HLA-DR; CD274; CD279) and a CD8 T-lymphocyte biomarker (CD279). Individually, only higher neutrophil CD279 [OR 1.78 (95% CI 1.23-2.57); P = 0.002], higher monocyte CD279 [1.32 (1.03-1.70); P = 0.03], and lower monocyte HLA-DR [0.73 (0.55-0.97); P = 0.03] expression were associated with subsequent sepsis. With logistic regression the optimum biomarker combination was increased neutrophil CD24 and neutrophil CD279, and reduced monocyte HLA-DR expression, but no combination had clinically relevant predictive validity. Conclusions From a large panel of leukocyte biomarkers, immunosuppression biomarkers were associated with subsequent sepsis in ED patients with suspected acute infection. 47. Cell-surface signatures of immune dysfunction risk-stratify critically ill patients: INFECT study

Purpose Cellular immune dysfunctions, which are common in intensive care patients, predict a number of significant complications. In order to effectively target treatments, clinically applicable measures need to be developed to detect dysfunction. The objective was to confirm the ability of cellular markers associated with immune dysfunction to stratify risk of secondary infection in critically ill patients. Methods Multi-centre, prospective observational cohort study of critically ill patients in four UK intensive care units. Serial blood samples were taken, and three cell surface markers associated with immune cell dysfunction [neutrophil CD88, monocyte human leucocyte antigen-DR (HLA-DR) and percentage of regulatory T cells (T.sub.regs)] were assayed on-site using standardized flow cytometric measures. Patients were followed up for the development of secondary infections. Results A total of 148 patients were recruited, with data available from 138. Reduced neutrophil CD88, reduced monocyte HLA-DR and elevated proportions of T.sub.regs were all associated with subsequent development of infection with odds ratios (95% CI) of 2.18 (1.00-4.74), 3.44 (1.58-7.47) and 2.41 (1.14-5.11), respectively. Burden of immune dysfunction predicted a progressive increase in risk of infection, from 14% for patients with no dysfunction to 59% for patients with dysfunction of all three markers. The tests failed to risk stratify patients shortly after ICU admission but were effective between days 3 and 9. Conclusions This study confirms our previous findings that three cell surface markers can predict risk of subsequent secondary infection, demonstrates the feasibility of standardized multisite flow cytometry and presents a tool which can be used to target future immunomodulatory therapies.
