1. Validating Parallel-Forms Tests for Assessing Anesthesia Resident Knowledge

Abstract

We created a serious game to teach first year anesthesiology (CA-1) residents to perform general anesthesia for cesarean delivery. We aimed to investigate resident knowledge gains after playing the game and having received one of 2 modalities of debriefing. We report on the development and validation of scores from parallel test forms for criterion-referenced interpretations of resident knowledge. The test forms were intended for use as pre- and posttests for the experiment. Validation of instruments measuring the study's primary outcome was considered essential for adding rigor to the planned experiment, to be able to trust the study's results. Parallel, multiple-choice test forms development steps included: (1) assessment purpose and population specification; (2) content domain specification and writing/selection of items; (3) content validation by experts of paired items by topic and cognitive level; and (4) empirical validation of scores from the parallel test forms using Classical Test Theory (CTT) techniques. Field testing involved online administration of 52 shuffled items from both test forms to 24 CA-1's, 21 second-year anesthesiology (CA-2) residents, 2 fellows, 1 attending anesthesiologist, and 1 of unknown rank at 3 US institutions. Items from each form yielded near-normal score distributions, with similar medians, ranges, and standard deviations. Evaluations of CTT item difficulty (item p values) and discrimination (D) indices indicated that most items met assumptions of criterion-referenced test design, separating experienced from novice residents. Abstract

This study with 20 adults explored adding career biographies and career narrative writing to the career interest assessment report process. Participants reported that biographies and narratives helped them identify themes to consider how their career plans fit their personal lifestyle, meaning making, and values. This study offers 1 way to incorporate narratives and story to explore the self and self-in-context to activate meaning-making processes in career interest assessment reports. Until career interest assessment reports include career biographies and a process for writing career narratives, only career counselors can offer this type of service to clients.

Abstract

1. RLD is characterized by a limitation of lung expansion; the hallmark of the condition is decreased total lung capacity (TLC) below the fifth percentile of the predicted value [1]. 2. This results in decreased functional residual capacity (FRC), increased work of breathing, and often hypoxemia. 3. Reduced forced expiratory volume (FEV1) and forced vital capacity (FVC) (with normal or elevated ratio) are characteristic findings. 4. RLD may result from [1] pathology of the parenchyma itself (e.g., idiopathic pulmonary fibrosis, granulomatous disease, scleroderma, systemic lupus erythematosus), [2] neuromuscular disease (e.g., myasthenia gravis, amyotrophic lateral sclerosis), or [3] structural abnormality (e.g., kyphoscoliosis, morbid obesity). 5. The diffusion capacity of carbon monoxide (DLCO) differentiates intrinsic lung disease (decreased DLCO) from extrapulmonary causes (normal DLCO). 6. Patients with intrinsic parenchymal disease and pulmonary fibrosis may also have secondary pulmonary hypertension, which carries a significantly higher morbidity and mortality [2]. 7. TLC is used to classify RLD as mild (65–80% predicted value), moderate (50–65%), or severe (less than 50%).

5. Left Lateral Table Tilt for Elective Cesarean Delivery under Spinal Anesthesia Has No Effect on Neonatal Acid-Base Status: A Randomized Controlled Trial

Abstract

Current recommendations for women undergoing cesarean delivery include 15° left tilt for uterine displacement to prevent aortocaval compression, although this degree of tilt is practically never achieved. We hypothesized that under contemporary clinical practice, including a crystalloid coload and phenylephrine infusion targeted at maintaining baseline systolic blood pressure, there would be no effect of maternal position on neonatal acid base status in women undergoing elective cesarean delivery with spinal anesthesia.Healthy women undergoing elective cesarean delivery were randomized (nonblinded) to supine horizontal (supine, n = 50) or 15° left tilt of the surgical table (tilt, n = 50) after spinal anesthesia (hyperbaric bupivacaine 12 mg, fentanyl 15 μg, preservative-free morphine 150 μg). Lactated Ringer's 10 ml/kg and a phenylephrine infusion titrated to 100% baseline systolic blood pressure were initiated with intrathecal injection. The primary outcome was umbilical artery base excess.There were no differences in umbilical artery base excess or pH between groups. The mean umbilical artery base excess (± SD) was -0.5 mM (± 1.6) in the supine group (n = 50) versus -0.6 mM (± 1.5) in the tilt group (n = 47) (P = 0.64). During 15 min after spinal anesthesia, mean phenylephrine requirement was greater (P = 0.002), and mean cardiac output was lower (P = 0.014) in the supine group.Maternal supine position during elective cesarean delivery with spinal anesthesia in healthy term women does not impair neonatal acid-base status compared to 15° left tilt, when maternal systolic blood pressure is maintained with a coload and phenylephrine infusion. These findings may not be generalized to emergency situations or nonreassuring fetal status.

Abstract

A healthy 25-year-old G3P2002 patient at 38-week gestation was referred to the high-risk obstetric anesthesia service due to the presence of abnormal placentation. Her past surgical history included 2 previous cesarean deliveries. The ultrasound examination was suspicious for a complete anterior placenta previa with multiple placental lakes, which looked even more likely after magnetic resonance imaging (MRI).

Abstract

We investigated whether GnRH has the properties of a growth hormone-releasing factor in vivo in a mammalian species. Sexually mature female Sprague-Dawley rats were prepared for in vivo dynamic hormone testing. The rats were assigned to one of four groups: I) GnRH, II) apomorphine and GnRH, III) somatostatin and GnRH, or IV) saline control. Blood samples were drawn at 0, 30, 60, 90, and 120 minutes after treatment. Growth hormone (GH) and LH were measured by radioimmunoassay. Results were analyzed using repeated-measures analysis of variance, Student test, and comparisons of the area under the curve where appropriate. Gonadotropin-releasing hormone caused a marked rise in both GH and LH levels as a junction of time. Apomorphine caused a significant blunting of the LH response to GnRH but did not blunt the GnRH-induced rise in GH. Somatostatin blocked the GnRH-induced rise in GH but did not block the increase in LH. In saline-treated controls, neither the GH nor LH levels exhibited a statistically significant variance over the 2-hour test period. Gonadotropin-releasing hormone appears to have the properties of a growth hormone-releasing factor.
