Abstract

Summary: Background: The protection of fourth dose mRNA vaccination against SARS-CoV-2 is relevant to current global policy decisions regarding ongoing booster roll-out. We aimed to estimate the effect of fourth dose vaccination, prior infection, and duration of PCR positivity in a highly-vaccinated and largely prior-COVID-19 infected cohort of UK healthcare workers. Methods: Participants underwent fortnightly PCR and regular antibody testing for SARS-CoV-2 and completed symptoms questionnaires. A multi-state model was used to estimate vaccine effectiveness (VE) against infection from a fourth dose compared to a waned third dose, with protection from prior infection and duration of PCR positivity jointly estimated. Findings: 1298 infections were detected among 9560 individuals under active follow-up between September 2022 and March 2023. Compared to a waned third dose, fourth dose VE was 13.1% (95% CI 0.9 to 23.8) overall; 24.0% (95% CI 8.5 to 36.8) in the first 2 months post-vaccination, reducing to 10.3% (95% CI −11.4 to 27.8) and 1.7% (95% CI −17.0 to 17.4) at 2–4 and 4–6 months, respectively. Relative to an infection >2 years ago and controlling for vaccination, 63.6% (95% CI 46.9 to 75.0) and 29.1% (95% CI 3.8 to 43.1) greater protection against infection was estimated for an infection within the past 0–6, and 6–12 months, respectively. A fourth dose was associated with greater protection against asymptomatic infection than symptomatic infection, whilst prior infection independently provided more protection against symptomatic infection, particularly if the infection had occurred within the previous 6 months. Duration of PCR positivity was significantly lower for asymptomatic compared to symptomatic infection. Interpretation: Despite rapid waning of protection, vaccine boosters remain an important tool in responding to the dynamic COVID-19 landscape; boosting population immunity in advance of periods of anticipated pressure, such as surging infection rates or emerging variants of concern. Abstract

Objective. To determine the effectiveness of exercise and/or nutrition interventions and to address body weight changes during the menopause transition. Methods. A systematic review of the literature was conducted using electronic databases, grey literature, and hand searching. Two independent researchers screened for studies using experimental designs to evaluate the impact of exercise and/or nutrition interventions on body weight and/or central weight gain performed during the menopausal transition. Studies were quality appraised using Cochrane risk of bias. Included studies were analyzed descriptively. Results. Of 3,564 unique citations screened, 3 studies were eligible (2 randomized controlled trials, and 1 pre/post study). Study quality ranged from low to high risk of bias. One randomized controlled trial with lower risk of bias concluded that participation in an exercise program combined with dietary interventions might mitigate body adiposity increases, which is normally observed during the menopause transition. The other two studies with higher risk of bias suggested that exercise might attenuate weight loss or weight gain and change abdominal adiposity patterns. Conclusions. High quality studies evaluating the effectiveness of interventions targeting body weight changes in women during their menopause transition are needed. Evidence from one higher quality study indicates an effective multifaceted intervention for women to minimize changes in body adiposity.

6. Temozolomide hypersensitivity – A story of success

Abstract

Aims Background: Temozolomide is an oral chemotherapy drug widely used in the treatment of glial tumours. This is generally well tolerated however, immediate and delayed hypersensitivity reactions have been described, necessitating treatment interruption that could significantly impact survival. Method Case description: We report a case of a 39-year-old young woman with WHO grade 2 IDH1mutant diffuse astrocytoma (MGMT promoter methylated) of the left temporal lobe. She had tumour resection in 2012 followed by radical radiotherapy. Afterwards, she had close surveillance with regular brain imaging. The tumour started to progress in early 2020. We agreed to commence on temozolomide to control tumour growth. She developed an itchy rash with easy bruising towards the end of the first cycle. It responded to a course of prednisolone. We proceeded with the second cycle and she developed a worsening rash on the third day. However she did not have signs or symptoms of anaphylaxis. This episode was again managed by a course of steroids and oral antibiotics. Since Temozolomide was the optimal treatment to control the disease progression at this stage, we agreed to persevere with temozolomide using a rapid desensitisation protocol. Results On the first day of third cycle, she was brought to the day chemotherapy unit and given prednisolone 30mg, fexofenadine 180mg and ondansetron 8mg, 30 minutes before the treatment. Our protocol was as follows; 5mg, 10mg, 20mg, 30mg, 50mg and 90mg of temozolomide at an interval of 30 minutes between the doses. She was advised to follow the same regime for the next four days of the third cycle. On days 6 -28, she has been advised to take fexofenadine 180mg once daily. Although there was a minor rash appeared on days 6/7, they subsided gradually. She tolerated the treatment without sinister symptoms or signs. We proceeded with the next 4 chemotherapy cycles adhering to the same protocol. The intensity of the rash gradually improved, and she became almost completely asymptomatic by the 7th cycle. It was also encouraging to see the radiological response of the tumour with the treatment. Conclusion Although the published literature is minimal, rapid desensitisation is shown to be a safe and effective method to counteract temozolomide hypersensitivity. In an era where there is still a paucity of systemic treatment options for primary brain tumours, adopting rapid desensitisation to induce tolerability to temozolomide, a drug which has shown to improve survival, would be a valuable addition in managing our patients. Further, our experience suggests that this protocol is safe, effective, and does not necessitate inpatient admission.

Abstract

Fisheries bycatch is a major threat to marine megafauna such as seabirds. Population monitoring has revealed low survival of juvenile seabirds over recent decades, potentially because naïve individuals are more susceptible to bycatch than adults. However, major gaps remain in our knowledge of behavior and interaction of juveniles with fisheries. Here, we tracked juvenile grey-headed albatrosses (Thalassarche chrysostoma) from South Georgia - the largest global population of this endangered species, and in rapid decline - to investigate their at-sea distribution and assess bycatch risk. Fledged juveniles dispersed to the northeast, overlapping with a bycatch hotspot for grey-headed albatrosses reported by the Japanese pelagic longline fleet in the southeast Atlantic Ocean. Given adult grey-headed albatrosses use regions less exposed to fishing activity (< 40°S), the majority of birds bycaught in this area are probably juveniles, and possibly immatures, from South Georgia, likely representing a key factor explaining the sustained population decline. Our study highlights the urgent need to uncover the ‘lost-years’ for marine megafauna to enable focused conservation efforts.

Abstract

Introduction: Previous research has shown that nurses are not meeting recommended moderate-to-vigorous intensity PA (MVPA) guidelines (≥150 minutes/week) for optimal cardiovascular disease risk reduction. Socio-ecological approaches have been used to explore the determinants of PA levels. We examined personal, social and environmental factors associated with the MVPA levels of Canadian nurses. Methods: Secondary analysis of data from a multi-site cross-sectional study was undertaken. Nurses were recruited from 14 hospitals in Ontario, Canada. An accelerometer (ActiGraph GT3X) was used to measure MVPA levels (minutes/day). Socio-ecological variables were derived from sociodemographic, anthropometric and cardiometabolic data, and questionnaires assessing determinants of PA (Table 1). Multivariate generalized estimating equations (GEE) were used to explore associations between socio-ecological variables and MVPA levels while accounting for hospital sites. Variables were selected for multivariate analyses if they were significant ( p Results: A total of 257 nurses (42±12 years) had complete accelerometer (≥10 hours of wear time for ≥4 days) and questionnaire data. Of these nurses, 54% were overweight/obese and 6% were smokers. Multivariate analyses showed positive associations between MVPA levels and high perceived capability to exercise despite common barriers (e.g. poor weather; β=0.13, p =0.02) and feeling connected to their exercise peers (β=1.34, p =0.01). Greater shiftwork associated daytime sleepiness and/or insomnia were inversely associated with MVPA levels (β=-4.87, p Conclusion: Future endeavors to accentuate nurses’ PA levels should address modifiable socio-ecological variables such as encouraging exercise despite common barriers, and with peers to whom they feel connected. Nurses should consider increasing time spent engaging in MVPA to minimize the negative effects of shiftwork.

Abstract

Many industrial activities impose a threat on biodiversity, and it is unclear to what extent environmental regulations can reduce the threat of such activities. Bycatch in industrial fisheries is one of the greatest sources of mortality for seabirds, but a threat for which effective mitigation exists. Here we quantify whether the introduction of a new regulation that required the use of bird-scaring lines reduced seabird mortality in two of the most hazardous fisheries in the South Atlantic. The Namibian hake demersal trawl and longline fisheries, estimated to be killing 20,000–30,000 birds/year, have been required to use bird-scaring lines since 2015. We used data from BirdLife International's Albatross Task Force and the Namibian Fisheries Observer Agency to quantify changes in seabird mortality in these fisheries before and after the introduction of these regulations. Our estimated bycatch rates in the longline fleet were 0.468 birds/1000 hooks (95% confidence interval 0.067–1.450) before regulations and 0.004 birds/1000 hooks (0.001–0.013) following their introduction, a 98.4% reduction. Our estimate suggests that 215 (1–751) seabirds were killed across this fleet in 2018 compared to 22,222 (3187–68,786) in 2009. In the trawl fleet, observers recorded seabird mortality resulting from interactions with trawl cables. The average rate of heavy interactions was 1.09 interactions/h (0.81–1.39) before the regulation came into effect, and 0.49 interactions/h (0.23–0.84) since then. Extrapolations based on the number of observed fatal interactions suggest 1452 (0–3865) birds were killed by this fleet in 2017 compared to 7030 (0–16,374) in 2009. The lower mortality reduction in the trawl fleet is likely due to incomplete implementation of regulations and highlights the importance of adequate enforcement for effective bycatch mitigation. Overall, we demonstrate that regulations that mandate that well-tested safeguards are used during industrial operations can have enormous benefits for the conservation of threatened species.

Abstract

•Based on results from previous European (Assessing Levels of PHysical Activity, ALPHA) and North American (Institute of Medicine report) projects, we have made an evidence-based proposal of core health-related fitness tests and protocols to be used worldwide.•We have gathered, through a Delphi survey, the opinions of 169 experts from major regions of the world and obtained a high response rate (78 %) and high agreement (≥85 %) for all tests and protocols.•This study therefore has achieved international consensus (initial goal, ≥80 %) on the proposed Youth Fitness International Test (YFIT) battery, which is valid, reliable, health-related, safe, feasible, and inclusive. The YFIT battery tests are: weight and height (to compute body mass index as a proxy of body size/composition), the 20-m shuttle run (cardiorespiratory fitness), handgrip strength, and standing long jump (muscular fitness).•This study contributes to standardizing fitness tests and protocols used for research, monitoring, and surveillance across the world, which will allow for future data pooling and the development of international and regional sex- and age-specific reference values, health-related cut-points, inter-country and inter-continental fitness comparisons, and ultimately provide global data that can inform surveillance and guide public health policies.
