Surgical stabilization of rib fractures (SSRF) improves functional outcomes compared to controls, partly due to reduction in pain. We investigated the impact of early SSRF on pulmonary complications, mortality, and length of stay compared to non-operative analgesia with epidural analgesia (EA).Retrospective cohort study of the Trauma Quality Improvement Program (TQIP) 2017 dataset for adults with rib fractures, excluding those with traumatic brain injury or death within twenty-four hours. Early SSRF and EA occurred within 72 h, and we excluded those who received both or neither intervention. Our primary outcome was a composite of pulmonary complications including acute respiratory distress syndrome (ARDS) or ventilator-associated pneumonia (VAP). Additional outcomes included unplanned endotracheal intubation, in-hospital mortality, and hospital and intensive care unit (ICU) length of stay (LOS) for those surviving to discharge. Multiple logistic and linear regressions were controlled for variables including age, sex, flail chest (FC), injury severity, additional procedures, and medical comorbidities.We included 1,024 and 1,109 patients undergoing early SSRF and EA, respectively. SSRF patients were more severely injured with higher rates of FC (42.8 vs 13.3%, p0.001), Injury Severity Score (ISS)16 (56.9 vs 36.1%, p0.001), and Abbreviated Injury Scale (AIS) Thorax3 (33.3 vs 12.2%, p0.001). Overall, 49 (2.3%) of patients developed ARDS or VAP, 111 (5.2%) required unplanned intubation, and 58 (2.7%) expired prior to discharge. On multivariable analysis, SSRF was not associated with the primary composite outcome (OR: 1.65, 95%CI: 0.85-3.21). Early SSRF significantly predicted decreased risk of unplanned intubation (OR:0.59, 95%CI: 0.38-0.92) compared with early EA alone, however, was not a significant predictor of in-hospital mortality (OR: 1.27, 95%CI: 0.68-2.39). SSRF was associated with significantly longer hospital (Exp(β): 1.06, 95%CI: 1.00-1.12, p = 0.047) and ICU LOS (Exp(β): 1.17, 95%CI: 1.08-1.27, p0.001).Aside from unplanned intubation, we observed no statistically significant difference in the adjusted odds of in-hospital pulmonary morbidity or mortality for patients undergoing early SSRF compared with early EA. Chest wall injury patients may benefit from referral to trauma centers where both interventions are available and appropriate surgical candidates may receive timely intervention.

Anastomotic leak is the most dreaded complication following colonic resection. While patient frailty is increasingly being recognized as a risk factor for surgical morbidity and mortality, the current colorectal body of literature has not assessed the relationship between frailty and anastomotic leak.Evaluate the relationship between patient frailty and anastomotic leak as well as patient frailty and failure to rescue in patients who experienced an anastomotic leak.Retrospective cohort study.American College of Surgeons National Surgical Quality Improvement Program Database from 2015 to 2017.Patients with the diagnosis of colonic neoplasia undergoing an elective colectomy during the study time period.Anastomotic leak, failure to rescue.A total of 30,180 elective colectomies for neoplasia were identified. The leak rate was 2.9% (n = 880). Compared to nonfrail patients, frail patients were at increased odds of anastomotic leak (frailty score = 1: OR 1.34, 95% CI 1.10-1.63; frailty score = 2: OR 1.32, 95% CI 1.04-1.68; frailty score = 3: OR = 2.41, 95% CI 1.47-3.96). After an anastomotic leak, compared to nonfrail patient, a greater proportion of frail patients experienced mortality (3.4% vs 5.9%), septic shock (16.1% vs 21.0%), myocardial infarction (1.1% vs 2.9%), and pneumonia (6.8% vs 11.8%). Furthermore, the odds of mortality, septic shock, myocardial infarction, and pneumonia increased in frail patients with higher frailty scores.Potential misclassification bias from lack of a strict definition of anastomotic leak and retrospective design of the study.Frail patients undergoing colectomy for colonic neoplasia are at increased risk of an anastomotic leak. Introduction Older adults comprise an increasingly large proportion of patients with traumatic brain injury (TBI) receiving care in inpatient rehabilitation facilities (IRF). However, high rates of comorbidities and evidence of declining preinjury health among older adults who sustain TBI raise questions about their ability to benefit from IRF care. Objective 1) describe the proportion of older adults with TBI who exhibited minimal detectable change (MDC) and a minimally clinically important difference (MCID) in motor function from IRF admission to discharge; and 2) identify characteristics associated with clinically meaningful improvement in motor function and better discharge functional status. Design In this retrospective cohort study, we utilized Medicare administrative data probabilistically linked to the National Trauma Data Bank to estimate the proportion of patients whose motor function improved during inpatient rehabilitation and identify factors associated with meaningful improvement in motor function and motor function at discharge. Setting Inpatient rehabilitation facilities in the US. Patients Fee-for-service Medicare beneficiaries with TBI. Main outcome measures Minimal Detectable Change (MDC) and Minimally Clinically Important Difference (MCID) in the Functional Independence Measure motor (FIM-M) score from admission to discharge, and FIM-M score at IRF discharge. Results From IRF admission to discharge 84% of patients achieved the MDC threshold, and 68% of patients achieved the MCID threshold for FIM-M scores. Factors associated with a higher probability of achieving the MCID for FIM-M scores included better admission motor and cognitive function, lower comorbidity burden, and a length of stay longer than 10 days but only among individuals with lower admission motor function. Older age was associated with a lower FIM-M discharge score, but not the probability of achieving the MCID in FIM-M score. Conclusion Older adults with TBI have the potential to improve their motor function with IRF care. Baseline functional status and comorbidity burden, rather than acute injury severity, should be used to guide care planning. This article is protected by copyright. All rights reserved.

We hypothesized that implementation of new ultra-restrictive transfusion protocol in adult surgical intensive care units (SICU) was safe and feasible during pandemic-associated shortage crises.Retrospective analysis two months pre- and post-implementation of ultra-restrictive transfusion protocol in March 2020 with hemoglobin cutoff of 6 g/dL (6.5 g/dL if ≥ 65 years old) for patients without COVID, active bleeding, or myocardial ischemia.We identified 16/93 and 27/168 patients PRE and POST meeting standard transfusion threshold (7 g/dL); within POST, 12 patients met ultra-restrictive cutoffs. There was no significant difference between PRE and POST in the rate of mortality, ischemic complications, or the number of transfusions per patient, however, the overall incidence of transfusion was lower in the POST group (7.1 vs 17.2%, p = 0.02). Patients received a mean (SD) of 4(3.8) and 2.4(1.5) PRBC transfusions pre- and post-implementation. Odds ratio of mortality in POST group was 0.62 (95%CI: 0.08-5.12) adjusted for age, sex, and SOFA score.Implementation of an ultra-restrictive transfusion protocol was feasible and effective as a blood- preservation strategy.

Chest wall stabilization (CWS) improves outcomes for patients with chest wall injury (CWI). We hypothesized that patients treated at centers with higher annual CWS volumes experience superior outcomes.A retrospective study of adults with acute CWI undergoing surgical stabilization of rib or sternal fractures within the 2019 Trauma Quality Improvement Program database, excluding those with 24-hour mortality or any Abbreviated Injury Scale body region of six, was conducted. Hospitals were grouped in quartiles by annual CWS volume. Our primary outcome was a composite of in-hospital mortality, ventilator-associated pneumonia, acute respiratory distress syndrome, sepsis, and unplanned intubation or intensive care unit readmission. Regression was controlled for age, sex, Injury Severity Scale, flail chest, medical comorbidities, and Abbreviated Injury Scale chest. We performed cut-point analysis and compared patient outcomes from high- and low-volume centers.We included 3,207 patients undergoing CWS at 430 hospitals with annual volumes ranging from 1 to 66. There were no differences between groups in age, sex, or Injury Severity Scale. Patients in the highest volume quartile (Q4) experienced significantly lower rates of the primary outcome (Q4, 14%; Q3, 18.4%; Q2, 17.4%; Q1, 22.1%) and significantly shorter hospital and intensive care unit lengths of stay. Q4 versus Q1 had lower adjusted odds of the primary outcome (odds ratio, 0.58; 95% confidence interval, 0.43-0.80). An optimal cut point of 12.5 procedures annually was used to define high- and low-volume centers. Patients treated at high-volume centers experienced significantly lower rates of the primary composite outcome, in-hospital mortality, and deep venous thrombosis with shorter lengths of stay and higher rates of home discharge.Center-specific CWS volume is associated with superior in-hospital patient outcomes. These findings support efforts to establish CWI centers of excellence. Further investigation should explore the impact of center-specific volume on patient-reported outcomes including pain and postdischarge quality of life.Prognostic and Epidemiologic; Level III.

Early administration of tranexamic acid (TXA) has been widely implemented for the treatment of presumed hyperfibrinolysis in hemorrhagic shock. We aimed to characterize the liberal use of TXA and whether unjustified administration was associated with increased venous thrombotic events (VTEs).We identified injured patients who received TXA between January 2016 and January 2018 by querying our Level 1 trauma center's registry. We retrospectively reviewed medical records and radiologic images to classify whether patients had a hemorrhagic injury that would have benefited from TXA (justified) or not (unjustified).Ninety-five patients received TXA for traumatic injuries, 42.1% were given by emergency medical services. TXA was considered unjustified in 35.8% of the patients retrospectively and in 52% of the patients when given by emergency medical services. Compared with unjustified administration, patients in the justified group were younger (47.6 versus 58.4; P = 0.02), more hypotensive in the field (systolic blood pressure: 107 ± 31 versus 137 ± 32 mm Hg; P 0.001) and in the emergency department (systolic blood pressure: 97 ± 27 versus 128 ± 27; P 0.001), and more tachycardic in emergency department (heart rate: 99 ± 29 versus 88 ± 19; P = 0.04). The justified group also had higher injury severity score (median 24 versus 11; P 0.001), was transfused more often (81.7% versus 20.6%; P 0.001), and had higher in-hospital mortality (39.3% versus 2.9%; P 0.001), but there was no difference in the rate of VTE (8.2% versus 5.9%).Our results highlight a high rate of unjustified administration, especially in the prehospital setting. Hypotension and tachycardia were indications of correct use. Although we did not observe a difference in VTE rates between the groups, though, our study was underpowered to detect a difference. Cautious implementation of TXA in resuscitation protocols is encouraged in the meantime. Nonetheless, adverse events associated with unjustified TXA administration should be further evaluated.

OBJECTIVE To identify patient, injury, and functional status characteristics associated with successful discharge to the community following a skilled nursing facility (SNF) stay among older adults hospitalized following traumatic brain injury (TBI). SETTING Skilled nursing facilities. PARTICIPANTS Medicare fee-for-service beneficiaries admitted to an SNF after hospitalization for TBI. DESIGN Retrospective cohort study using Medicare administrative data merged with the National Trauma Data Bank using a multilayered Bayesian record linkage approach. MAIN OUTCOME MEASURE Successful community discharge: discharged alive within 100 days of SNF admission and remaining in the community for 30 days or more without dying or admission to a healthcare facility. RESULTS Medicaid enrollment, incontinence, decreased independence with activities of daily living, and cognitive impairment were associated with lower odds of successful discharge, whereas race "other" was associated with higher odds of successful discharge. Injury factors including worse injury severity (Glasgow Coma Scale and Abbreviated Injury Scale scores) and fall-related injury mechanism were not associated with successful discharge. CONCLUSION Among older adults with TBI who discharge to an SNF, sociodemographic and functional status characteristics are associated with successful discharge and may be useful to clinicians for discharge planning. Acute injury severity indices may have limited utility in predicting discharge disposition once a patient is admitted to an SNF for post-acute care.

Injured patients benefit from direct transport to a trauma center; however, it is unknown whether patients with traumatic out-of-hospital cardiac arrest (OHCA) benefit from initial resuscitation at the nearest emergency department (ED) if a trauma center is farther away. We hypothesized that patients with traumatic OHCA transported directly to a trauma center have less in-hospital mortality after initial resuscitation compared to those transferred from non-trauma centers.We examined patients presenting with traumatic OHCA within our institutional trauma registry and the National Trauma Data Bank (NTDB) and excluded patients with ED mortality. Our primary outcome was all-cause mortality during index hospitalization; multiple logistic regression controlled for age, sex, injury severity score, mechanism of injury, signs of life, emergency surgery, and level I trauma center designation.We identified 271 and 1,138 adult patients with traumatic OHCA in our registry and the NTDB; 28% and 16% were transferred from another facility, respectively. Following initial resuscitation, patients transferred to a trauma center had higher in-hospital mortality than those transported directly in both our local and national cohorts (aOR: 2.27, 95%CI: 1.03-4.98, and aOR: 2.66, 95%CI: 1.35 - 5.26, respectively).Patients with traumatic OHCA transported directly to a trauma center may have increased survival to discharge compared to those transferred from another facility, even accounting for initial resuscitation. Further investigation should examine the impact of both physiologic and logistic factors including distance to trauma center, traffic, and weather patterns that may impact prehospital decision-making and destination selection.

Patients' trauma burdens are a combination of anatomic damage, physiologic derangement, and the resultant depletion of reserve. Typically, Injury Severity Score (ISS)15 defines major anatomic injury and Revised Trauma Score (RTS)7.84 defines major physiologic derangement, but there is no standard definition for reserve. The Need For Trauma Intervention (NFTI) identifies severely depleted reserves (NFTI+) with emergent interventions and/or early mortality. We hypothesized NFTI would have stronger associations with outcomes and better model fit than ISS and RTS.Thirty-eight adult and pediatric U.S. trauma centers submitted data for 88,488 encounters. Mixed models tested ISS greater than 15, RTS less than 7.84, and NFTI's associations with complications, survivors' discharge to continuing care, and survivors' length of stay (LOS).The NFTI had stronger associations with complications and LOS than ISS and RTS (odds ratios [99.5% confidence interval]: NFTI = 9.44 [8.46-10.53]; ISS = 5.94 [5.36-6.60], RTS = 4.79 [4.29-5.34]; LOS incidence rate ratios (99.5% confidence interval): NFTI = 3.15 [3.08-3.22], ISS = 2.87 [2.80-2.94], RTS = 2.37 [2.30-2.45]). NFTI was more strongly associated with continuing care discharge but not significantly more than ISS (relative risk [99.5% confidence interval]: NFTI = 2.59 [2.52-2.66], ISS = 2.51 [2.44-2.59], RTS = 2.37 [2.28-2.46]). Cross-validation revealed that in all cases NFTI's model provided a much better fit than ISS greater than 15 or RTS less than 7.84.In this multicenter study, NFTI had better model fit and stronger associations with the outcomes than ISS and RTS. By determining depletion of reserve via resource consumption, NFTI+ may be a better definition of major trauma than the standard definitions of ISS greater than 15 and RTS less than 7.84. Using NFTI may improve retrospective triage monitoring and statistical risk adjustments.Prognostic, level IV.

BackgroundThe American College of Surgeons Resources for Optimal Care of the Injured Patient recommends using hypotension, defined as systolic blood pressure ≤90 mm Hg, as an indicator of a full team trauma activation. We hypothesized that an elevated shock index (SI) predicts significant traumatic injuries better than hypotension alone.MethodsThis is a retrospective cohort study analyzing full team trauma activations between February 2018 and January 2020, excluding transfers and those who had missing values for prehospital blood pressure or heart rate. We reviewed patients’ demographics, prehospital and emergency department vitals, injury pattern, need for operation, and clinical outcomes. The primary outcome was rate of significant injury defined as identified injured liver, spleen, or kidney, pelvis fracture, long bone fracture, significant extremity soft tissue damage, hemothorax, or pneumothorax.ResultsAmong 544 patients, 82 (15.1%) had prehospital hypotension and 492 had normal blood pressure. Of the patients with prehospital hypotension, 34 (41.5%) had a significant injury. There was no difference in age, gender, medical history, or injury pattern between the two groups. There was no difference between the two groups in rate of serious injury (41.5% vs. 46.1%, NS), need for emergent operation (31.7% vs. 28.1%, NS) or death (20.7% vs. 18.8%, NS). On the other hand, SI ≥1 was associated with increased rate of serious injury (54.6% vs. 43.4%, p=0.04). On a logistic regression analysis, prehospital hypotension was not associated with significant injury or need for emergent operation (OR 0.83, 95% CI 0.51 to 1.33 and OR 1.32, 95% CI 0.79 to 2.25, respectively). SI ≥1 was associated with both increased odds of significant injury and need for emergent operation (OR 1.57, 95% CI 1.01 to 2.44 and OR 1.64, 95% CI 1.01 to 2.66).DiscussionSI was a better indicator and could replace hypotension to better categorize and triage patients in need of higher level of care.Level of evidencePrognostic and epidemiologic, level III.

Early identification of traumatic brain injury (TBI) with head CT HCT should expedite operative decision-making and improve outcome. We aimed to determine whether an early HCT protocol in TBI patients would improve outcome.A multidisciplinary protocol to obtain an HCT within 30 minutes from arrival for patients with GCS ≤ 13 was instituted on 1/1/2015. Our trauma registry was queried for patients evaluated between 3/2012 and 12/2015. Outcomes included compliance with protocol and in-hospital mortality.346 patients presented with GCS ≤ 13. Patients PRE- (n=264) and POST-protocol (n=82) were similar in demographic and physiologic characteristics. Time to HCT was lower (35 vs. 77 min; p0.001). POST-protocol had lower odds of mortality (OR 0.65, 95% CI 0.43-0.99) adjusting for age, gender, ISS and GCS.Implementing a protocol of early HCT for TBI optimized performance of the trauma team. Time to HCT could serve as a quality metric in TBI.

Use of anticoagulant and antiplatelet medications (AAMs) is increasing significantly with our growing population of older adults. AAMs worsen outcomes in trauma patients. Our goal was to improve collaboration between trauma and outpatient providers and to improve safety in making decisions on anticoagulant and antiplatelet medications(AAMs) after injuries.A risk management initiative.Patients that suffered traumatic injury while on anticoagulation or antiplatelets medications at a level I university trauma center.IRB approval was obtained to review records for medications, demographics, mechanism and type of injury, and indication for preinjury AAM use. Inpatient trauma team providers contacted the primary prescriber. A collaborative decision was made regarding AAM plans.One hundred and five patients, mean age 79 years, were followed. The three most common AAMs were warfarin (69 patients), clopidogrel (24), and Factor Xa inhibitors (16). Atrial fibrillation was the most common indication for AAMs (70 patients), venous thrombosis (14) and TIA/CVA (11). Falls were the most frequent injury mechanism, 79.4%. Soft tissue hematomas (27.4%), TBI (16%), and pelvic fractures (12.3%) were the most common injuries. In 56.6% AAMs were held until follow-up, 31.1% had AAMs resumed at discharge, and AAMs were held indefinitely in 12.3%. Patients discharged to home versus facility (37 vs 18% p0.05),75 years of age (47 vs 27% p0.05) were more likely to have AAMs resumed at discharge. Patients who suffered falls versus MVC mechanism were less likely to have AAMs resumed at discharge (28 vs 82% p0.05). CHA2DS2-VASc scores were similar between decision groups.This is the first description of mandatory communication between trauma and outpatient providers to guide decision making on AAMs after injury. Efforts should be made to determine if this mitigates risk by following patients longterm. This communication should become standard for a population that is often elderly, frail, and at risk of repeat injuries.

Diverticular disease is common in the Western population and can cause considerable morbidity. The prevalence of colonic diverticulosis reaches 60% by the age of 60 years. Small bowel diverticulosis is much rarer and, when present, most commonly occurs in the duodenum. We herein report an elderly woman with jejunal diverticulosis complicated by diverticulitis and small bowel obstruction, who subsequently underwent small bowel resection and primary anastomosis. As demonstrated by this case, jejunal diverticulitis can cause serious complications and given the possibility of recurrence and serious complications, surgical options should be discussed early in the course of medical care.

Objectives The Cribari Matrix Method (CMM) is the current standard to identify over/undertriage but requires manual trauma triage reviews to address its inadequacies. The Standardized Triage Assessment Tool (STAT) partially emulates triage review by combining CMM with the Need For Trauma Intervention, an indicator of major trauma. This study aimed to validate STAT in a multicenter sample. Methods Thirty-eight adult and pediatric US trauma centers submitted data for 97,282 encounters. Mixed models estimated the effects of overtriage and undertriage versus appropriate triage on the odds of complication, odds of discharge to a continuing care facility, and differences in length of stay for both CMM and STAT. Significance was assessed at p Results Overtriage (53.49% vs. 30.79%) and undertriage (17.19% vs. 3.55%) rates were notably lower with STAT than with CMM. CMM and STAT had significant associations with all outcomes, with overtriages demonstrating lower injury burdens and undertriages showing higher injury burdens than appropriately triaged patients. STAT indicated significantly stronger associations with outcomes than CMM, except in odds of discharge to continuing care facility among patients who received a full trauma team activation where STAT and CMM were similar. Conclusions This multicenter study strongly indicates STAT safely and accurately flags fewer cases for triage reviews, thereby reducing the subjectivity introduced by manual triage determinations. This may enable better refinement of activation criteria and reduced workload.

INTRODUCTION Arterial cannulation is frequently performed on intensive care unit (ICU) and operating room patients; a 1% complication rate has been reported. Investigators applied simulation to study clinical providers' arterial catheter (AC) insertion performance and to assess for interdisciplinary and intradisciplinary variation that may contribute to complications. METHODS Anesthesia, medical critical care, and surgical critical care providers with AC insertion experience were enrolled at 2 academic hospitals. Each subject completed a simulated AC insertion on an in situ task trainer. Using a Delphi-derived checklist that incorporated published recommendations, expert opinion, and institutional requirements, 2 investigators completed offline video reviews to compare subjects' technical performance. RESULTS Ten anesthesia, 11 medical ICU (MICU, 1 excluded), and 10 surgical ICU (SICU) subjects with significant between-group differences in training level and AC insertion experience were enrolled for 2 years. Differences in procedural planning, equipment preparation, and patient preparation steps did not attain significance across groups except for anesthesia participants using only ad hoc AC kits, and MICU and SICU subjects preferentially using commercial kits (P < 0.001). Time-outs were completed by 1 anesthesia subject, 5 MICU subjects, and 4 SICU subjects (P = 0.29, NS). For proceduralist preparation steps, fewer anesthesiology subjects donned gowns (P < 0.001). Only MICU subjects used ultrasound guidance (P = 0.0053), and only MICU (100%) and SICU (100%) subjects sutured ACs in place. Overall observance of sterile technique was similar across groups at 70% to 100% (P = 0.32). CONCLUSIONS Simulated AC insertions revealed procedural performance variability that may derive from individual provider differences, discipline-based practice parameters, and setting-specific cultural factors.

Research Objectives Medicare administrative data is a valuable resource for understanding post-acute care outcomes of older adults. However, these records lack information about injury severity that is useful for understanding post-acute care among those with TBI. The Injury Severity Score (ISS) reflects anatomic injury severity and is calculated by trauma registries, but can also be calculated from ICD-9 codes available in administrative data. Our objective is to examine the agreement between ICD-9 derived ISS scores and those sourced from trauma registries. Design Design: We linked Medicare administrative data to the National Trauma Data Bank (NTDB) using a Bayesian record linkage algorithm. We then examined the agreement between trauma registry- ISS scores and those calculated from acute and post-acute ICD-9 codes using Injury Categorization Software (ICDPIC) software. Setting U.S. Inpatient Rehabilitation Facilities (IRF) and Skilled Nursing Facilities (SNF). Participants Medicare beneficiaries who discharged to a SNF or an IRF following hospitalization for TBI between 2011 and 2015. Interventions N/A. Main Outcome Measures Injury Severity Score. Results For scores derived from acute claims, the absolute agreement with trauma registry scores was 11.2% (8.3, 14.2), with 41.8% (37.4, 46.2) of scores being within +/- 5 points. Trauma registry sourced ISS scores were more often higher than the ISS from the trauma registry. In the majority of cases ISS scores could not be calculated from post-acute claims, due to a lack of injury specific ICD-9 codes. Conclusions There is limited agreement between trauma registry ISS scores and those calculated from the ICD-9 codes of acute claims. The scores derived from acute claims may have utility for adjustment, but may underestimate anatomic injury severity of older adults with TBI. Research Objectives Older adults comprise a growing proportion of patients with TBI in inpatient rehabilitation facilities (IRF). However, concerns about their tolerance of rehabilitation-intensive IRF care remain. Centers for Medicare & Medicaid defined ‘short stays’ (Length of stay (LOS) Design In this retrospective cohort study we used Medicare administrative data linked to the National Trauma Data Bank via a Bayesian record linkage algorithm to examine the relationship between age, chronic conditions, functional status, and an ‘unsuccessful IRF admission.’ We used a two-level hierarchical logistic regression model, adjusting for demographics, injury characteristics, and facility effects. Setting U.S. Inpatient Rehabilitation Facilities. Participants Medicare fee-for-service beneficiaries hospitalized with TBI between 2011 and 2015. Interventions N/A. Main Outcome Measures Unsuccessful IRF admission (‘short stays’ or ‘short-stay transfers’). Results Cognitive function in the lowest quartile (OR:2.5 (1.0, 6.3), or motor function in the lowest two quartiles (OR:3.1(1.4, 7.1), OR:6.2 (2.4, 15.6)) were associated with higher odds of an ‘unsuccessful IRF admission’ compared to the highest quartile of each domain. The model differentiated ‘unsuccessful IRF admissions’ (AUC:0.74 (0.69, 0.80), however, 46% of those in cognitive quartile 1, and 52% of those in motor quartiles 1 and 2 were able to discharge home. Conclusions Among older adults with TBI, significant impairments in cognitive and motor function are associated with increased risk of an ‘unsuccessful IRF admission’. Factors unaccounted for in our models, potentially acute management characteristics, may help to differentiate those who are unlikely to benefit from IRF care. Many studies have described the detrimental effect of lack of health insurance on trauma-related outcomes. It is unclear, though, whether these effects are related to pre-injury health status, access to trauma centers, or differences in quality of care after presentation. The aim of this study was to determine if patient and insurance type affect outcomes after trauma surgery.We conducted a retrospective chart review of prospectively collected data at the American College of Surgeons level 1 trauma registry in Rhode Island. All blunt trauma patients aged 18-45 observed from 2004 to 2014 were included. Patients were divided into one of four groups on the basis of their type of insurance: Private/commercial, Medicare, Medicaid, and uninsured. Co-morbidities and infections were recorded. Analysis of variance or the Mann-Whitney U test, as appropriate, was used to analyze the data.A total of 8,018 patients were included. Uninsured patients were more likely to be male and younger, whereas the Medicare patient group had significantly fewer male patients. Rates of co-morbidities were highest in the Medicare group (28.1%) versus the private insurance (16.7%), Medicaid (19.9%), and uninsured (12.9%) groups (p 0.05). However, among patients with any co-morbidity, there was no difference in the average number of co-morbidities between insurance groups. The rate of infection was highest in Medicaid patients (7.7%) versus private (5.6%), Medicare (6.3%), and uninsured (4.3%) patients (p 0.05). Only Medicaid was associated with a significantly greater risk of developing a post-injury infection (odds ratio 1.6; 95% confidence interval 1.1-2.3).The presence of insurance, namely Medicaid, does not equate to diagnosis and management of conditions that affect trauma outcomes. Medicaid is associated with worse pre-trauma health maintenance and a greater risk of infection.

INTRODUCTION: Blood shortage have emerged as complicating and strategic hurdle during COVID-19 pandemic We hypothesized that adopting an ultra-restrictive transfusion protocol to preserve blood products use in surgical intensive care units (SICU) is safe and effective compared to current practices METHODS: Early in the COVID-19 pandemic (on 3/20/2020), our SICU adopted a protocol to change transfusion cutoff to hemoglobin (Hb) of 6 g/dl (or 6 5 g/dl for patients 65 years and older) instead of 7 g/dl We excluded patients who were COVID+, actively bleeding, or had signs of cardiac ischemia on admission We compared patients admitted to SICU during 2 months before (PRE) and 2 months after (POST) implementing the protocol who met the transfusion cutoff Our primary outcome was in-hospital mortality The Sequential Organ Failure Assessment (SOFA) score on the day of transfusion was calculated Secondary outcomes were new ischemic changes and total number of packed red blood cells (PRBC) transfused RESULTS: Our cohort had 261 patients (PRE n=93, POST n=168) Of those, 41 patients had Hb drop below 7 (16 in PRE, 17 2% and 27 in POST, 16 1%) In Post group, 12 patients had Hb drop below 6 and required transfusion (7 1%, p=0 02) There was no difference in gender or age between patients who met transfusion cutoff in either group There was also no difference in mortality (PRE: 5, 31 3% and POST: 3, 25%, NS) or ischemic complications (PRE: 2, 12 5% and POST: 4, 33 3%, NS) Odds ratio of mortality in POST group was 0 62 (95%CI:0 08-5 12) adjusted for age, gender, and SOFA score Mortality rate in POST patients who had Hb>6 g/dl (n=15) was 13 3% PRBC transfusion per patient was on average 4±3 8 units in PRE group and 2 4±1 5 units in POST group (NS) This represents a potential preservation of 60-100 PRBC units during the 2 months period of implementing the protocol and a potential reduction of 55-73% in blood product utilization CONCLUSIONS: Ultra-restrictive transfusion protocol appears to be safe and effective in preserving blood products utilization during a shortage crisis These results are limited by the small sample size and a large multi-center study is warranted However, these results could present a promising option during a subsequent pandemic crises or other similar disasters

Background The population of older adults is rapidly growing and more older patients are presenting with abdominal trauma. Outcomes have not been well defined for patients that require a damage control approach(DCL). Methods This was a retrospective study at a level one trauma center of patients age 65 years and older with abdominal trauma that required DCL. Outcomes reviewed included mortality, length of stay, discharge disposition. Presenting vital signs and laboratories were reviewed to identify predictors of mortality. Results 31 older patients(mean age 75.2 years) underwent DCL. Twenty-four of 31(77.4%) older patients died. Seven of 7 older DCL survivors were discharged to a rehabilitation center or nursing home. In comparisons of older DCL nonsurvivors and survivors there were not differences in presenting HR(90 versus 96; p = 0.56) or SBP in the emergency room(107 versus 116; p = 0.51). No differences in initial lactate or change in lactate concentration were found between nonsurvivors and survivors. Fifteen of 24 nonsurvivors died from multisystem organ failure. Conclusions/Implications The mortality rate of older patients that require damage control approach for is extremely high. Presenting vital signs and laboratory markers may not be useful in older patients to predict mortality.

OBJECTIVE To describe the natural history of patients with traumatic brain injury (TBI) admitted to skilled nursing facilities (SNFs) following hospitalizations. SETTING Between 2005 and 2014. PARTICIPANTS Adults who had incident admissions to skilled nursing facilities (SNFs) with a diagnosis of TBI. DESIGN Retrospective review of the Minimum Data Set. MAIN MEASURES Main variables were cognitive and physical function, length of stay, presence of feeding tube, terminal condition, and dementia. RESULTS Incident admissions to SNFs increased annually from 17 247 patients to 20 787 from 2005 to 2014. The percentage of patients with activities of daily living score 23 or more decreased from 25% to 14% (P < .05). The overall percentage of patients with severe cognitive impairment decreased from 18% to 10% (P < .05). More patients had a diagnosis of dementia in 2014 compared with previous years (P < .05), and the presence of a terminal condition increased from 1% to 1.5% over the 10-year period (P < .05). The percentage of patients who stayed fewer than 30 days was noted to increase steadily over the 10 years, starting with 48% in 2005 and ending with 53% in 2013 (P < .05). CONCLUSION Understanding past trends in TBI admissions to SNFs is necessary to guide appropriate discharge and predict future demand, as well as inform SNF policy and practice necessary to care for this subgroup of patients.

Background Traumatic brain injury is a leading cause of death and disability in the United States. In survivors, traumatic brain injury remains a leading contributor to long-term disability and results in many patients being admitted to skilled nursing facilities for postacute care. Despite this very large population of traumatic brain injury patients, very little is known about the long-term outcomes of traumatic brain injury survivors, including rates of discharge to home or risk of death in long-term nursing facilities. We hypothesized that patient demographics and functional status influence outcomes of patients with traumatic brain injury admitted to skilled nursing facilities. Methods We conducted a retrospective cohort study of Medicare fee-for-service beneficiaries aged 65 and older discharged alive and directly from hospital to a skilled nursing facility between 2011 and 2014 using the prospectively maintained Federal Minimum Data Set combined with Medicare claims data and the Centers for Medicare and Medicaid Services Vital Status files. Records were reviewed for demographic and clinical characteristics at admission to the skilled nursing facility, including age, sex, cognitive function, ability to communicate, and motor function. Activities of daily living were reassessed at discharge to calculate functional improvement. We used robust Poisson regression with skilled nursing facility fixed effects to calculate relative risks and 99% confidence intervals for mortality and functional improvement associated with the demographic and clinical characteristics present at admission. Linear regression was used to calculate adjusted mean duration of stay. Results Overall, 87,292 Medicare fee-for-service beneficiaries with traumatic brain injury were admitted to skilled nursing facilities. The mean age was 84 years, with 74% of patients older than age 80. Generally, older age, male sex, and poor cognitive or functional status at admission to a skilled nursing facility were associated with increased risk for poorer outcomes. Older patients (age ≥80 years) with traumatic brain injury had a 1.5 times greater risk of death within 30 days of admission compared with adults younger than 80 years (relative risk = 1.49, 99% confidence interval = 1.36, 1.64). Women were 37% less likely to die than men were (relative risk = 0.63, 99% confidence interval = 0.59, 0.68). The risk of death was greater for patients with poor cognitive function (relative risk = 2.55, 99% confidence interval = 2.32, 2.77), substantial motor impairment (relative risk = 2.44, 99% confidence interval = 2.16, 2.77), and patients with impairment in communication (relative risk = 2.58, 99% confidence interval = 2.32, 2.86) compared with those without the respective deficits. One year after admission, these risk factors continued to confer excess risk for mortality. Duration of stay was somewhat greater for older patients (30.1 compared with 27.5 average days) and patients with cognitive impairment (31.7 vs 27.5 average days). At discharge, patients with cognitive impairment (relative risk = 0.86, 99% confidence interval = 0.83, 0.88) and impairment in the ability to communicate (relative risk = 0.67, 99% confidence interval = 0.54, 0.82) were less likely to improve in physical function. Conclusion Our results suggest that among patients with traumatic brain injury admitted to skilled nursing facilities, the likelihood of adverse outcomes varies significantly by key demographic and clinical characteristics. These findings may facilitate setting expectations among patients and families as well as providers when these patients are admitted to skilled nursing facilities for rehabilitation after their acute episode.

A proportion of trauma patients present for evaluation in a delayed fashion after injury, likely due to a variety of medical and nonmedical reasons. There has been little investigation into the characteristics and outcomes of trauma patients who present delayed. We hypothesize that trauma patients who present in a delayed fashion are a unique population at risk of increased trauma-related complications.This was a retrospective review from 2010-2015 at a Level I trauma center. Patients were termed delayed if they presented24 hours after injury. Patients admitted within 24 hours of their injury were the comparison group. Charts were reviewed for demographics, mechanism, comorbidities, complications and outcomes. A subgroup analysis was done on patients who suffered falls.During the 5-y period, 11,705 patients were admitted. A total of 588 patients (5%) presented24 h after their injury. Patients in the delayed group were older (65 versus 55 y, P 0.001) and more likely to have psychiatric comorbidities (33% vs. 24%, P = 0.0001) than the control group. They were also more likely to suffer substance withdrawal (8.9% vs. 4.1%, P 0.001) but had toxicology testing for drugs and alcohol done at significantly lower rates. Patients that presented delayed after falls were similar in age and injury severity score (ISS) but more likely to suffer substance withdrawal when compared to those with falls that presented within 24 hours. Patients with falls that presented delayed had toxicology testing at significantly lower rates than the comparison group.Trauma patients that present to the hospital in a delayed fashion have unique characteristics and are more likely to suffer negative outcomes including substance withdrawal. Future goals will include exploring strategies for early intervention, such as automatic withdrawal monitoring and social work referral for all patients who present in a delayed fashion.

Nutrition assessment is a means of identifying critical care patients at greatest risk for complications. It should include manifestations both of disease-related malnutrition and starvation. Critically ill patients are at risk for accelerated loss of lean body mass with even greater loss of weight if nutrient intake is poor or absent. Recognizing those patients who are unable to eat and barriers that interfere with adequate nourishment are important aspects of clinical decision-making. This chapter discusses at risk patient populations, dietary intake analysis, delays and obstacles to feeding, nutrition-focused history and physical examination, laboratory evaluation, and body composition assessment. The multidisciplinary critical care team should discuss relevant components of the nutrition assessment daily. Ongoing evaluation of nutrition adequacy is essential for optimal patient care.

Recent studies have shown that the occurrence rate of bloodstream infections associated with arterial catheters is 0.9-3.4/1,000 catheter-days, which is comparable to that of central venous catheters. In 2011, the Centers for Disease Control and Prevention published new guidelines recommending the use of limited barrier precautions during arterial catheter insertion, consisting of sterile gloves, a surgical cap, a surgical mask, and a small sterile drape. The goal of this study was to assess the attitudes and current infection prevention practices used by clinicians during insertion of arterial catheters in ICUs in the United States.An anonymous, 22-question web-based survey of infection prevention practices during arterial catheter insertion.Clinician members of the Society of Critical Care Medicine.Eleven thousand three hundred sixty-one physicians, nurse practitioners, physician assistants, respiratory therapists, and registered nurses who elect to receive e-mails from the Society of Critical Care Medicine.None.There were 1,265 responses (11% response rate), with 1,029 eligible participants after exclusions were applied. Only 44% of participants reported using the Centers for Disease Control and Prevention-recommended barrier precautions during arterial catheter insertion, and only 15% reported using full barrier precautions. The mean and median estimates of the incidence density of bloodstream infections associated with arterial catheters were 0.3/1,000 catheter-days and 0.1/1,000 catheter-days, respectively. Thirty-nine percent of participants reported that they would support mandatory use of full barrier precautions during arterial catheter insertion.Barrier precautions are used inconsistently by critical care clinicians during arterial catheter insertion in the ICU setting. Less than half of clinicians surveyed were in compliance with current Centers for Disease Control and Prevention guidelines. Clinicians significantly underestimated the infectious risk posed by arterial catheters, and support for mandatory use of full barrier precautions was low. Further studies are warranted to determine the optimal preventive strategies for reducing bloodstream infections associated with arterial catheters.
