1. Impact of Helicobacter pylori infection status on outcomes among patients with advanced gastric cancer treated with immune checkpoint inhibitors

Abstract: Background Gut microbiota composition can influence cancer immunotherapy response. Recent evidence suggests Helicobacter pylori infection may reduce immune checkpoint inhibitor (ICI) efficacy in lung cancer and melanoma, but thorough characterization of this association in patients with gastric cancer is lacking. We aimed to determine the impact of H. pylori on survival in this population.Methods This single-center, retrospective study included all ICI-treated individuals with metastatic gastric cancer and documented H. pylori status at Memorial Sloan Kettering between July 2013 and October 2021. H. pylori-positive status was defined as history of infection obtained via breath test, stool antigen test, histopathology, and/or chart documentation. Negative status was defined as explicitly negative testing, histopathology, and/or chart documentation. Primary outcomes were progression-free survival (PFS) and overall survival (OS).Results Of 215 included patients, 49 had documented history of H. pylori infection. Compared with H. pylori-negative patients, positive individuals tended to be younger, non-white, and Hispanic with non-cardia and intestinal-type gastric cancer. Abstract: Summary: Emerging evidence from the western literature suggests an increasing focus on applying nature-based interventions for mental health improvements. However, in Indigenous communities, caring for country has always been central to the Indigenous way of life. Knowing that nature-based interventions effectively improve mental health outcomes, this review collated evidence on the application of caring for country in improving social and emotional well-being (SEWB) of Indigenous peoples in Australia and New Zealand. Three studies from Australia and one from New Zealand, explored the role of country or whenua (land) in the lives of Indigenous people. Participation in caring-for country activities was associated with lower levels of psychological distress and strengthened guardianship relationship with country, which positively affected SEWB. This systematic review offers preliminary evidence on the role of caring for country activities in improving the SEWB of Indigenous peoples and highlights the need for strengths-based approaches to improve the SEWB of Indigenous peoples. 3. Mist1 Expression Is Required for Paneth Cell MaturationSummary

Abstract: Background: Paneth cells are professional secretory cells found within the small intestinal crypt epithelium. Although their role as part of the innate immune complex providing antimicrobial secretory products is well-known, the mechanisms that control secretory capacity are not well-understood. MIST1 is a scaling factor that is thought to control secretory capacity of exocrine cells. Methods: Mist1+/+ and Mist1–/– mice were used to evaluate the function of MIST1 in small intestinal Paneth cells. We used histologic and immunofluorescence staining to evaluate small intestinal tissue for proliferation and lineage allocation. Total RNA was isolated to evaluate gene expression. Enteroid culture was used to evaluate the impact of the absence of MIST1 expression on intestinal stem cell function. Results: Absence of MIST1 resulted in increased numbers of Paneth cells exhibiting an intermediate cell phenotype but otherwise did not alter overall epithelial cell lineage allocation. Muc2 and lysozyme staining confirmed the presence of intermediate cells at the crypt base of Mist1–/– mice. These changes were not associated with changes in mRNA expression of transcription factors associated with lineage allocation, and they were not abrogated by inhibition of Notch signaling. However, the absence of MIST1 expression was associated with alterations in Paneth cell morphology including decreased granule size and distended rough endoplasmic reticulum. Absence of MIST1 was associated with increased budding of enteroid cultures; however, there was no evidence of increased intestinal stem cell numbers in vivo. Conclusions: MIST1 plays an important role in organization of the Paneth cell secretory apparatus and managing endoplasmic reticulum stress. 4. Non-Gestational Ovarian Choriocarcinoma: A Rare Ovarian Cancer Subtype

Abstract: Non-Gestational Ovarian Choriocarcinoma (NGOC) is an extremely rare ovarian tumor, with an incidence of less than 0.6% of malignant ovarian germ cell tumors. Its close pathologic resemblance to Gestational Ovarian Choriocarcinoma (GOC), however, requires special attention as the treatments differ greatly. NGOC typically affects patients in late adolescence or early reproductive years. As a result, NGOCs are often misdiagnosed as ectopic pregnancies due to their common presentation of bleeding, abdominal pain, adnexal mass, and positive serum beta-HCG. On pathologic examination, the tumor is indistinguishable from GOC, and only after review of tissue for paternal genetic components can the diagnosis of NGOC be made. Imaging studies often show highly vascular lesions with further investigation with computer topography (CT) sometimes showing metastatic lesions in the lungs, pelvis, vagina, and liver. These lesions are often hemorrhagic and can lead to catastrophic bleeding. Treatment is vastly different from GOC; NGOC requires treatment with both surgical resection and chemotherapy, with Bleomycin, Etoposide, and Cisplatin (BEP) being the most used regimen. With correct diagnosis and treatment, patients can often receive fertility sparing treatment with long term survival.

5. Depletion of enteric bacteria diminishes leukocyte infiltration following doxorubicin-induced small intestinal damage in mice.

Abstract: BACKGROUND & AIMS:While enteric bacteria have been shown to play a critical role in other forms of intestinal damage, their role in mediating the response to the chemotherapeutic drug Doxorubicin (Doxo) is unclear. In this study, we used a mouse model of intestinal bacterial depletion to evaluate the role enteric bacteria play in mediating Doxo-induced small intestinal damage and, more specifically, in mediating chemokine expression and leukocyte infiltration following Doxo treatment. An understanding of this pathway may allow for development of intervention strategies to reduce chemotherapy-induced small intestinal damage. METHODS:Mice were treated with (Abx) or without (NoAbx) oral antibiotics in drinking water for four weeks and then with Doxo. Jejunal tissues were collected at various time points following Doxo treatment and stained and analyzed for apoptosis, crypt damage and restitution, and macrophage and neutrophil number. In addition, RNA expression of inflammatory markers (TNFα, IL1-β, IL-10) and cytokines (CCL2, CC7, KC) was assessed by qRT-PCR. RESULTS:In NoAbx mice Doxo-induced damage was associated with rapid induction of apoptosis in jejunal crypt epithelium and an increase weight loss and crypt loss. In addition, we observed an increase in immune-modulating chemokines CCL2, CCL7 and KC and infiltration of macrophages and neutrophils. In contrast, while still positive for induction of apoptosis following Doxo treatment, Abx mice showed neither the overall weight loss nor crypt loss seen in NoAbx mice nor the increased chemokine expression and leukocyte infiltration. CONCLUSION:Enteric bacteria play a critical role in Doxo-induced small intestinal damage and are associated with an increase in immune-modulating chemokines and cells. Manipulation of enteric bacteria or the damage pathway may allow for prevention or treatment of chemotherapy-induced small intestinal damage.

6. SATELLITE FLUORESCENCE AS A MEASURE OF OCEAN SURFACE CHLOROPHYLL

Abstract: Both the MODIS optical sensors on the NASA Aqua and Terra spacecraft and the MERIS instrument on the ESA Envisat measure spectral radiance in bands designed to allow detection of the fluorescence signal from surface chlorophyll in sea water, stimulated by ambient sun and sky light. We present images of fluorescence as derived from the level 1 MERIS data and compare the signal levels of the fluorescence with measurements of extracted chlorophyll from research cruises during 2002 to 2004, with satellite estimates of chlorophyll from the blue to green ratio observed by MERIS, and with fluorescence measurements made by MODIS. We show that the fluorescence signal is consistent between MERIS and MODIS, and that relation between fluorescence and chlorophyll concentration in surface waters is consistent with a simple model accounting for absorption of stimulating and emitted radiation by chlorophyll pigments. 7. DETECTION OF INTENSE PLANKTON BLOOMS USING THE MERIS OPTICAL IMAGER ON ENVISAT

Abstract: The European satellite sensor MERIS (Medium Resolution Imaging Spectrometer) can be used to detect a peak in the optical spectrum of water-leaving radiance at about 705 nm which provides a signature of intense plankton blooms. We present examples of this signature in images and spectra derived from MERIS data, and demonstrate how this signature arises using model results. The MERIS band at 709 nm, which is an essential part of the detection scheme, is not present in either Seawifs or MODIS. In many cases the higher spatial resolution (300 m) provided in the FR mode of MERIS data is important for detecting small area events. The combination of wide area coverage, 300m spatial resolution, and appropriate spectral bands makes MERIS a unique and potentially important tool for global, intense plankton bloom monitoring. Here we show bloom examples and describe two types of "false alarm" signals, one due to benthic vegetation and one that appears to be due to cosmic ray impacts on the MERIS CCD detectors

Abstract: ObjectiveTo explore the association of a functional germline variant in the 3'-UTR of KRAS with endometrial cancer risk, as well as the association of microRNA (miRNA) signatures and the KRAS-variant with clinical characteristics and survival outcomes in two prospective RTOG endometrial cancer trials.Methods/materialsThe association of the KRAS-variant with endometrial cancer risk was evaluated by case-control analysis of 467 women with type 1 or 2 endometrial cancer and 582 age-matched controls. miRNA and DNA were isolated for expression profiling and genotyping from tumor specimens of 46 women with type 1 endometrial cancer enrolled in RTOG trials 9708 and 9905. miRNA expression levels and KRAS-variant genotype were correlated with patient and tumor characteristics, and survival outcomes were evaluated by variant allele type.ResultsThe KRAS-variant was not significantly associated with overall endometrial cancer risk (14% controls and 17% type 1 cancers), although was enriched in type 2 endometrial cancers (24%, p = 0.2). In the combined analysis of RTOG 9708/9905, miRNA expression differed by age, presence of lymphovascular invasion and KRAS-variant status. Overall survival rates at 3 years for patients with the variant and wild-type alleles were 100% and 77% (HR 0.3, p = 0.24), respectively, favoring the variant.ConclusionsThe KRAS-variant may be a genetic marker of risk for type 2 endometrial cancers. In addition, tumor miRNA expression appears to be associated with patient age, lymphovascular invasion and the KRAS-variant, supporting the hypothesis that altered tumor biology can be measured by miRNA expression, and that the KRAS-variant likely impacts endometrial tumor biology.

9. Graduate Student Perceptions of the Use of Online Course Tools to Support Engagement

Abstract: Graduate students in six online courses were asked to complete a questionnaire related to dimensions of engagement including participation and interaction, performance, studying, and relevance of material. Students were asked to indicate the importance of various online course features (e.g., online discussions) in enhancing their engagement in each dimension using a Likert scale. Twenty-six (29%) students completed the questionnaire. Students rated most course management system features as extremely important or very important. When the ratings for the four engagement areas were grouped by course site feature, the feature with the highest mean rating was “instructor feedback on assignments/assessments.” The feature with the lowest mean rating was “online chats with other students.” While the practices of the instructor in the courses studied may have influenced the students’ perceptions, it is clear that students especially value contact with the instructor.

Abstract: Background: Hypertrophic pyloric stenosis in otherwise healthy neonates frequently requires urgent surgical procedure but anaesthesia care may result in respiratory complications, such as hypoxaemia, pulmonary aspiration of gastric contents, and postoperative apnoea. The primary aim was to study whether or not the incidence of difficult airway management and of hypoxaemia in neonates undergoing pyloric stenosis repair was higher than that in neonates undergoing other surgeries. Methods: Data on neonates and infants undergoing anaesthesia and surgery for pyloric stenosis were extracted from the NEonate and Children audiT of Anesthesia pRactice In Europe (NECTARINE) database, for secondary analysis. Results: We identified 310 infants who had anaesthesia for surgery for pyloric stenosis. Difficult airway management (more than two attempts at laryngoscopy) was higher in children with pyloric stenosis when compared with the entire NECTARINE cohort (7.9% [95% confidence interval {CI}, 5.22–11.53] vs 4.4% [95% CI, 1.99–6.58]; relative risk [RR]=1.81 [95% CI, 1.21–2.69]; P=0.004), whereas transient hypoxaemia with oxygen saturation

Abstract: Zygomatic implant (ZI) surgery and restoration is a complex procedure, requiring experienced clinicians to execute with success. This study assessed the opinion and clinical experiences of a cohort of practitioners involved in provision of zygomatic implant treatment on aspects of the surgical and prosthodontic procedures. A survey was distributed among all 176 delegates attending ‘Zygomatic 2019’ International Conference in the UK, and all delegates completed the survey. The respondents were clinicians from both prosthodontic and surgical specialties, with a range of experience. Strong agreements were found regarding the use of specific implant designs dependent on the anatomical situation, and that fixed restorations should be used where possible. There was limited agreement between respondents regarding surgical approaches to ZI treatment. The limited agreement found between respondents highlights the varied and conflicting literature available on ZI surgery. High-quality research is required to inform clinicians and assist education.

Abstract: Background: Neonates and infants are susceptible to hypoxaemia in the perioperative period. The aim of this study was to analyse interventions related to anaesthesia tracheal intubations in this European cohort and identify their clinical consequences. Methods: We performed a secondary analysis of tracheal intubations of the European multicentre observational trial (NEonate and Children audiT of Anaesthesia pRactice IN Europe [NECTARINE]) in neonates and small infants with difficult tracheal intubation. The primary endpoint was the incidence of difficult intubation and the related complications. The secondary endpoints were the risk factors for severe hypoxaemia attributed to difficult airway management, and 30 and 90 day outcomes. Results: Tracheal intubation was planned in 4683 procedures. Difficult tracheal intubation, defined as two failed attempts of direct laryngoscopy, occurred in 266 children (271 procedures) with an incidence (95% confidence interval [CI]) of 5.8% (95% CI, 5.1e6.5). Abstract: BACKGROUND Little is known about current clinical practice concerning peri-operative red blood cell transfusion in neonates and small infants. Guidelines suggest transfusions based on haemoglobin thresholds ranging from 8.5 to 12��g��dl-1, distinguishing between children from birth to day 7 (week 1), from day 8 to day 14 (week 2) or from day 15 (��week 3) onwards. OBJECTIVE To observe peri-operative red blood cell transfusion practice according to guidelines in relation to patient outcome. DESIGN A multicentre observational study. SETTING The NEonate-Children sTudy of Anaesthesia pRactice IN Europe (NECTARINE) trial recruited patients up to 60 weeks' postmenstrual age undergoing anaesthesia for surgical or diagnostic procedures from 165 centres in 31 European countries between March 2016 and January 2017. PATIENTS The data included 5609 patients undergoing 6542 procedures. Inclusion criteria was a peri-operative red blood cell transfusion. MAIN OUTCOME MEASURES The primary endpoint was the haemoglobin level triggering a transfusion for neonates in week 1, week 2 and week 3. Secondary endpoints were transfusion volumes, 'delta haemoglobin' (preprocedure - transfusion-triggering) and 30-day and 90-day morbidity and mortality. RESULTS Peri-operative red blood cell transfusions were recorded during 447 procedures (6.9%). The median haemoglobin levels triggering a transfusion were 9.6 [IQR 8.7 to 10.9] g��dl-1 for neonates in week 1, 9.6 [7.7 to 10.4] g��dl-1 in week 2 and 8.0 [7.3 to 9.0] g��dl-1 in week 3. The median transfusion volume was 17.1 [11.1 to 26.4] ml��kg-1 with a median delta haemoglobin of 1.8 [0.0 to 3.6] g��dl-1. Thirty-day morbidity was 47.8% with an overall mortality of 11.3%. CONCLUSIONS Results indicate lower transfusion-triggering haemoglobin thresholds in clinical practice than suggested by current guidelines. The high morbidity and mortality of this NECTARINE sub-cohort calls for investigative action and evidence-based guidelines addressing peri-operative red blood cell transfusions strategies. Abstract: Immune-related adverse events (irAEs) frequently complicate treatment with immune checkpoint blockade (ICB) targeting CTLA-4, PD-1, and PD-L1, which are commonly used to treat solid and hematologic malignancies. The skin and gastrointestinal (GI) tract are most frequently affected by irAEs. While extensive efforts to further characterize organ-specific adverse events have contributed to the understanding and management of individual toxicities, investigations into the relationship between multi-organ toxicities have been limited. Therefore, we aimed to conduct a characterization of irAEs occurring in both the skin and gut. A retrospective analysis of two cohorts of patients treated with ICB at Memorial Sloan Kettering Cancer Center was conducted, including a cohort of patients with cutaneous irAEs (ircAEs) confirmed by dermatologists (n = 152) and a cohort of patients with biopsy-proven immune-related colitis (n = 246). Among both cohorts, 15% (61/398) of patients developed both skin and GI irAEs, of which 72% (44/61) patients had ircAEs preceding GI irAEs (p = 0.00013). Our study suggests that in the subset of patients who develop both ircAEs and GI irAEs, ircAEs are likely to occur first. Further prospective studies with larger sample sizes are needed to validate our findings, to assess the overall incidence of co-incident irAEs, and to determine whether ircAEs are predictors of other irAEs. This analysis highlights the development of multi-system dermatologic and gastrointestinal irAEs and underscores the importance of oncologists, gastroenterologists, and dermatologists confronted with an ircAE to remain alert for additional irAEs.

Abstract: Connection with Country, community, and culture lies at the heart of Aboriginal and Torres Strait Islander peoples’ health and wellbeing. Although there is some evidence on the role of cultural identity on the mental health of Indigenous adults, this relationship is relatively unexplored in the context of Indigenous Australian children. Robust empirical evidence on the role of cultural identity for social and emotional wellbeing is necessary to design and develop effective interventions and approaches for improving the mental health outcomes for Indigenous Australian children. Drawing on data from the Longitudinal Study of Indigenous Children (LSIC), we explore social and emotional wellbeing in Indigenous Australian children and assesses whether cultural identity protects against social-emotional problems in Indigenous children. The results show that Indigenous children with strong cultural identity and knowledge are less likely to experience social and emotional problems than their counterparts. Our work provides further evidence to support the change from a deficit narrative to a strengths-based discourse for improved health and wellbeing of Indigenous Australian children.

Abstract: Background: Neonates and infants requiring anaesthesia are at risk of physiological instability and complications, but triggers for peri-anaesthetic interventions and associations with subsequent outcome are unknown. Methods: This prospective, observational study recruited patients up to 60 weeks' postmenstrual age undergoing anaesthesia for surgical or diagnostic procedures from 165 centres in 31 European countries between March 2016 and January 2017. The primary aim was to identify thresholds of pre-determined physiological variables that triggered a medical intervention. The secondary aims were to evaluate morbidities, mortality at 30 and 90 days, or both, and associations with critical events. Results: Infants (n=5609) born at mean (standard deviation [SD]) 36.2 (4.4) weeks postmenstrual age (35.7% preterm) underwent 6542 procedures within 63 (48) days of birth. Abstract: To systematically review the effects of exercise interventions that may enhance quality of life (QOL) in individuals with traumatic brain injury (TBI).A systematic search was conducted using five databases up to April 2018. Studies were included if QOL was quantified following an exercise programme for people with a TBI. Methodological quality was assessed using a validated scoring checklist. Two independent reviewers assessed study inclusion and methodological quality.Thirteen studies met the inclusion criteria (seven RCTs, six non-RCTs). The median total scores for the quality assessment tool were 26.1 (RCTs), and 21.3 (non-RCTs), out of 33. Eight out of the 13 studies reported improved QOL following an exercise programme. The duration of the interventions varied from 8-12 weeks. The most common programmes involved moderate to vigorous exercise; with a frequency and duration of 3-5 times/week for 30-60 minutes.Due to the diversity of the exercise training interventions, heterogeneity of patient characteristics, multitude of QOL instruments and outcome domains assessed, it was not possible to draw any definitive conclusion about the effectiveness of exercise interventions. However, this review identified positive trends to enhance various aspects of QOL measured using a range of assessment tools.

Abstract: BACKGROUND: Recurrent Clostridiodies difficile infection (CDI) contributes to morbidity and mortality in cancer patients. Fecal microbiota transplantation (FMT) has been proven to be effective in treatment of recurrent CDI, but immunocompromised patients have been excluded from prospective studies due to safety concerns. The aim of this study was to investigate the safety of fecal microbiota transplantation (FMT) for recurrent CDI in immunocompromised patients with solid tumor malignancy undergoing chemotherapy. METHODS: This was a single center, prospective observational study of patients at a tertiary care cancer center of 10 patients with recurrent CDI who were at least 18 years of age, with a solid tumor malignancy who had received chemotherapy within the previous 6 months. Patients received FMT either by upper endoscopy or colonoscopy and were followed for 6 months. Safety was a primary outcome measured by infections occurring within 2 weeks of FMT. Efficacy of FMT was also evaluated. RESULTS: Nineteen patients were evaluated. On applying exclusion criteria, 10 were included in the study. One patient requested to be off study within 2 weeks and was considered a treatment failure. Seven received FMT via upper endoscopy, 3 via colonoscopy. There were no infectious complications from FMT. Eight patients (80%) were cured after the first FMT. All eight patients went on to restart oncologic treatment with an average of 32.5 days after FMT. CONCLUSIONS: FMT is safe and effective for recurrent CDI in solid tumor patients undergoing chemotherapy. Patients can resume oncologic treatment after FMT.

Methods: A repeated cross-sectional study was conducted between January 2014 and December 2017. Medical, nursing, dentistry, and allied health students who completed a rural/remote placement were invited to complete a survey questionnaire on placement experience. Information on students’ sociodemographic factors, discipline, placement experience, placement satisfaction, and rural/remote practice intentions was collected. Modified Poisson regression was performed to determine the relationships between placement satisfaction and future rural practice intentions. Qualitative data were analyzed by thematic analysis. Results: A total of 873 students responded. The majority of the survey respondents were females (70.06%), with a median age of 22 years (IQR 21–25 years), and 44.46% of respondents were medical students. Students satisfied with their placement were 2.10 times more likely to have rural/remote practice intention than their counterparts. Of all components of rural/remote placement experience, satisfaction with the placement supervision had the highest impact on changing students’ rural/remote practice intentions from negative to positive. The major themes from qualitative analysis were as follows: “wide variety of experience and hands-on learning opportunities,” “multidisciplinary exposure at home and workplace,” “support from the local University Department of Rural Health (UDRH),” “learning of indigenous culture,” and “experiencing challenges of rural health care services.” Conclusion: There is a strong association between positive placement experience and future rural/remote practice intentions. Therefore, facilitation of positive placement experiences in remote and rural locations could be a key strategy in addressing rural health workforce maldistribution. Abstract: © 2018 Elsevier Ltd. Descending stairs requires elevated joint moment-generating capability in the lower limbs, making it a challenging daily activity, particularly for older individuals. The aim of the study was to investigate the influence of three different strategies for descending standard and increased height stairs: step-over-step (SoS), step-by-step (SbS) and side-step (SS) on lower limb kinetics in older people. Eleven participants (mean ± SD age: 74.8 ± 3.1 years, height: 1.63 ± 0.07 m, mass: 67.7 ± 9.5 kg) descended a four-step custom built instrumented staircase at a self-selected speed, adopting each of the three strategies, at two configurations: a step-rise height of 170 mm (standard; STD) and a step-rise height of 255 mm (increased; INC). 3D motion capture, synchronised with embedded force plates enabled the calculation of joint kinetics of lead and trail limbs. The aim of this qualitative study was to provide an understanding of the lived experiences of students undertaking placements in various nonmetropolitan locations across Australia. In addition to providing their suggestions to improve rural placements, the study provides insight into factors contributing to positive and negative experiences that influence students’ future rural practice intentions. Methods: Responses to open-ended survey questions from 3,204 students from multiple health professions and universities were analyzed using two independent methods applied concurrently: manual thematic analysis and computerized content analysis using Leximancer software. Results: The core concept identified from the thematic analysis was “ruralization of students’ horizons,” a construct representing the importance of preparing health professional students for practice in nonmetropolitan locations. Ruralization embodies three interrelated themes, “preparation and support,” “rural or remote health experience,” and “rural lifestyle and socialization,” each of which includes multiple subthemes. From the content analysis, factors that promoted students’ rural practice intentions were having a “positive” practice experience, interactions with “supportive staff,” and interactions with the “community” in general. It was apparent that “difficulties,” eg, with “accommodation,” “Internet” access, “transport,” and “financial” support, negatively impacted students’ placement experience and rural practice intentions. Conclusions: The study findings have policy and practice implications for continuing to support students undertaking regional, rural, and remote placements and preparing them for future practice in nonmetropolitan locations. This study may, therefore, further inform ongoing strategies for improving rural placement experiences and enhancing rural health workforce recruitment, retention, and capacity building. Abstract: Purpose This study explored the link between sleep and emotional and behavioural problems and assessed whether cultural attachment reduces the risk of emotional and behavioural problems in Aboriginal and Torres Strait Islander (Indigenous) children. Methods The data from wave 5 to wave 10 of the Footprints in Time cohort were used. Multi-trajectory modelling was used to identify sleep trajectories using weekday sleep duration, weekday bedtimes, wake times, and sleep problems (waves 5, 7 & 10). Trajectories of emotional and behavioural problems were derived from the Strengths and Difficulties Questionnaire (SDQ) data (waves 6, 8 & 10). Cultural attachment assessment included the knowledge of Indigenous language, clan, people, family stories/history and other cultural practice. Multivariable logistic regression models were used to assess the link between sleep and emotional and behavioural problems. Results Analysis of sleep data from 1270 Indigenous children (50.6% females, mean age 6.3 years (±1.5)) identified four distinct trajectories: early sleepers/early risers (19.3%); early/long sleepers (22.1%), normative sleepers (47.8%), and late sleepers (10.8%). Three emotional and behavioural problem trajectories emerged: low stable (49.1%), high decreasing (40.5%), and high stable (10.4%). Early sleepers//early risers (OR: 0.48, 95% CI: 0.28–0.82) and children with strong cultural attachment (OR: 0.47, 95% CI: 0.27–0.82) had lower odds of being in the high emotional and behavioural problem trajectory group. Conclusions Early bedtime in children may reduce the risk of future emotional and behavioural problems. The protective effect of cultural attachment further highlights the need for strengths-based approaches to reduce mental health issues in Indigenous children.

Abstract: Purpose Despite a significant burden of poor sleep, Aboriginal and Torres Strait Islander peoples’ (Indigenous Australians) conceptualisation of sleep health is poorly understood. This research explored Indigenous Australians’ understanding and interpretation of sleep health and how that affects their health. Methods Indigenous people from remote Queensland were invited to participate in focus group discussions exploring their understanding of sleep health, the link between dreaming and sleep, and perceived implications of poor sleep. Participants were also asked to complete an adapted pictorial Epworth Sleepiness Scale (ESS). Descriptive statistics were used to summarise ESS data and participants’ demographic data. Thematic analysis was used to analyse focus group data. Results A total of 29 Indigenous Australians (82% females), median age 39 years (Interquartile range 26–51 years) from various geographical areas within North West Queensland participated in focus group discussions (n=6). The following themes emerged from the data: interconnection among sleep, emotional and physical health; challenges and successes in obtaining healthy sleep; the impact of dreams on waking life; and lack of support from health services in managing sleep issues. Scores from the modified pictorial scale indicate 24% of the participants had excessive daytime sleepiness (ESS score>10 points). Conclusion Indigenous Australians’ conceptualisation of sleep health is different from the western interpretation of sleep health. In particular, the connection between dreams and sleep is not adequately captured in current tools and resources to promote sleep health. This will limit effective prevention and management of sleep issues in Indigenous communities.

Abstract: We extend our previous observations of the spring bloom in the Strait of Georgia, Canada using Moderate Resolution Imaging Spectroradiometer (MODIS) satellite images, to include both a longer time period (2000–2016) and a wider variety of ‘seeding’ bloom events. We have reported on blooms in inlets to the north of the Strait, which appear to seed the main spring bloom in a characteristic ‘Malaspina Dragon’ pattern, leading to significantly earlier spring bloom dates in some years. Here, we double the time period of those observations. We find no clear repetitions of the earlier dragon pattern, but several instances of seeding from other inlets, with varying impacts on spring bloom timing. In situ monitoring has increased in coverage in recent years in the main body of the Strait, and we have deployed a fluorometer to record blooms at one inlet location, but observations of blooms in inlets rely mostly on satellites. Abstract: Objective: To illustrate and discuss sources of gait deviations (experimental, genuine and intentional) during a gait analysis and how these deviations inform clinical decision making. Methods A case study of a 24-year old male diagnosed with Alkaptonuria undergoing a routine gait analysis. A 3D motion capture with the Helen-Hayes marker set was used to quantify lower-limb joint kinematics during barefoot walking along a 10 m walkway at a self-selected pace. Additional 2D video data were recorded in the sagittal and frontal plane. The patient reported no aches or pains in any joint and described his lifestyle as active. \ud Results: Temporal-spatial parameters were within normal ranges for his age and sex. Three sources of gait deviations were identified; the posteriorly rotated pelvis was due to an experimental error and marker misplacement, the increased rotation of the pelvis in the horizontal plane was genuine and observed in both 3D gait curves and in 2D video analysis, finally the inconsistency in knee flexion/extension combined with a seemingly innocuous interest in the consequences of abnormal gait suggested an intentional gait deviation. \ud Conclusions: Gait analysis is an important analytical tool in the management of a variety of conditions that negatively impact on movement. Experienced gait analysts have the ability to recognise genuine gait adaptations that forms part of the decision-making process for that patient. However, their role also necessitates the ability to identify and correct for experimental errors and critically evaluate when a deviation may not be genuine.

Abstract: The body wall muscle of a Drosophila larva is generated by fusion between founder cells and fusion-competent myoblasts (FCMs). Initially, a founder cell recognizes and fuses with one or two FCMs to form a muscle precursor, then the developing syncitia fuses with additional FCMs to form a muscle fiber. These interactions require members of the immunoglobulin superfamily (IgSF), with Kin-of-IrreC (Kirre) and Roughest (Rst) functioning redundantly in the founder cell and Sticks-and-stones (Sns) serving as their ligand in the FCMs. Previous studies have not resolved the role of Hibris (Hbs), a paralog of Sns, suggesting that it functions as a positive regulator of myoblast fusion and as a negative regulator that antagonizes the activity of Sns. The results herein resolve this issue, demonstrating that sns and hbs function redundantly in the formation of several muscle precursors, and that loss of one copy of sns enhances the myoblast fusion phenotype of hbs mutants. We further show that excess Hbs rescues some fusion in sns mutant embryos beyond precursor formation, consistent with its ability to drive myoblast fusion, but show using chimeric molecules that Hbs functions less efficiently than Sns. In conjunction with a physical association between Hbs and SNS in cis, these data account for the previously observed UAS-hbs overexpression phenotypes. Lastly, we demonstrate that either an Hbs or Sns cytodomain is essential for muscle precursor formation, and signaling from IgSF members found exclusively in the founder cells is not sufficient to direct precursor formation.

36. Resistance training as a treatment for older persons with peripheral artery disease: a systematic review and meta-analysis

Abstract: Resistance training (RT) improves walking ability in persons with peripheral artery disease. We conducted a meta-analysis of randomised controlled trials (RCTs) investigating the effect of RT on peripheral artery disease (as measured by walking ability).We included RCTs that investigated the effect of RT on treadmill and/or 6 min walk (6-MWT) distances. RT intensity was assessed according to the American College of Sports Medicine guidelines by 1 repetition maximum or rating of perceived exertion. Standardised mean (SMD) and mean differences (MD) were calculated using a random-effects inverse variance model. Heterogeneity and bias were assessed using RevMan V.5.3. Meta-regression and meta-analysis of variance were performed as moderator analyses.Databases (Medline, Embase, Web of Science, Cinahl and Google Scholar) were searched until July 2018.Fifteen trials isolated RT; 7 trials compared RT with aerobic exercise. We analysed 826 patients (n=363 completing RT), with a mean age of 67.1±3.8 years. Training ranged from low-high intensity, 2-7 times per week for 17±7 weeks, with a mix of upper, lower or whole body training. Overall RT significantly improved constant load treadmill claudication onset (COD) (SMD 0.66 [0.40, 0.93], p0.00001) and total walking distance (WD) (SMD 0.51 [0.23, 0.79], p=0.0003), progressive treadmill COD (SMD 0.56 [0.00, 1.13], p=0.05) and total WD (SMD 0.45 [0.08, 0.83], p=0.02), and 6-MWT COD (MD 82.23 m [40.91, 123.54], p0.0001). Intensity played a role in improvement, with high-intensity training yielding the greatest improvement (p=0.02).RT clinically improved treadmill and flat ground walking ability in persons with peripheral artery disease. Higher intensity training was associated with better outcomes. Abstract: OBJECTIVES: The extent to which gastrocnemius muscle and Achilles tendon properties contribute to the impaired walking endurance of claudicants is not known. METHODS: Ultrasound images quantified muscle architecture of the lateral and medial gastrocnemius (GL and GM) and were combined with dynamometry during plantarflexor contractions to calculate tendon stress, strain, stiffness, the Young modulus, and hysteresis. Key parameters were entered into multiple regression models to explain walking endurance. RESULTS: Worse disease severity was significantly associated with longer fascicle: tendon length ratios (GL R = -0.789 and GM R = -0.828) and increased tendon hysteresis (R = -0.740). Walking endurance could be explained by GL and GM pennation angle, maximum tendon force, tendon hysteresis, and disease severity (R(2) = ∼0.6). CONCLUSIONS: Peripheral arterial disease was associated with functionally important changes in muscle and tendon properties, including the utilization of stored elastic energy. Abstract: This chapter discusses strategies that older and younger people employ to negotiate stairs based on experiments performed on an instrumented staircase in lab environment aiming at identifying ways to reduce stair fall risk for the elderly. Stair negotiation was found to be more demanding for the knee and ankle joint muscles in older than younger adults, with the demand increasing further when the step-rise was higher. During descent of stairs with higher step-rises, older adults shifted the centre of mass (COM) posteriorly, behind the centre of pressure (COP) to prevent forward falling. A decreased step-going resulted in a slower descent of the centre of mass in the older adults and standing on a single leg for longer than younger adults. A greater reliance on the handrails and rotation of the body in the direction of the handrail was also observed when the step-going was decreased during descent, which allowed this task to be performed with better dynamic stability, by maintaining the COM closer to the COP. These findings have important implications for stair design and exercise programs aiming at improving safety on stairs for the elderly.

Abstract: The purpose of this study was to analyze and record the characteristics of grandparents who are raising school-aged children as they relate to how the United States is responding to their needs and the needs of their grandchildren. The researcher sought to determine the characteristics of grandparent-headed households with no parent present in regards to housing, nutritional assistance, transportation, employment, age of children in the household, language spoken in the home, and family type, as measured by the 2010 United States Census Bureau. This study was conducted using a quantitative, descriptive research design. The data used for this study were indirect estimates produced by statistical model-based methods using sample survey, decennial census, and administrative data sources compiled by the United States Census Bureau. The Census Bureau collects data about American families for the nation, states and communities, and data related to socioeconomic status, family composition, and language spoken were used to more fully understand characteristics of households where grandparents are raising grandchildren outside of the traditional 1-2 parent American households. Participants included the 92 grandparent-headed households from among the 15,482 participants in the American Community Survey, which included 556 households where grandparents were living with grandchildren. To address the research questions, descriptive statistics were used, including frequencies and percentages. The statistical package SPSS was used to analyze the data. The results of this study revealed that of the grandparents who lived with their grandchildren where there was no parent present, 100% lived in housing units, 75% participated in the yearly food stamp/supplemental nutrition assistance program, 36% had no transportation, 26% were female householders who were unemployed and did not have a husband living in the household, 74% had related children who were 6-17 years old

Abstract: The purpose of this study was to explore the community college students perceptions on the use and effects of social media and social networking sites as well as any differences in perceptions based on students demographic characteristics. A community college in the state of Mississippi was chosen for this study. This community college is a comprehensive educational institution, accredited by the Commission on Colleges of the Southern Association of Colleges and Schools. The population for this study was currently enrolled freshman and sophomore students at one of the multiple locations. The respondents in this study were gathered by using a convenience sampling of students enrolled in academic core courses and electives or career and technical programs of study during the spring 2018 semester. The instrument that used for data collection in this study was a modified version of the Social Media Updates Survey (Pew Research Center, 2016). This study was quantitative in design, and a descriptive research methodology was used to conduct the study. The results revealed that students 25 years and older had significantly different social media habits than the students in the other age groups. Female students used social media more often than male students. More females than males used social media to interact with family. The data revealed that students mostly used mobile devices/tables to access social media and social networking sites. More students from academic programs accessed social media using desktops and laptops. More students from career technical education accessed social media using mobile devices. The most preferred social media websites were Facebook, YouTube, Instagram and SnapChat. Abstract: The purpose of the study was to determine administrators self-assessed perceived knowledge, importance, and perceptions of the International Society for Technology in Education Standards for Administrators (ISTE Standards-A) and the Virginia Technology Standards for Instructional Personnel (VTSIP). A survey instrument based on the ISTE Standards-A and the VTSIP was used in the study. 43 administrators in Virginia participated in the study. The participants were members of the Virginia Association of Elementary or Secondary School Principals. The study found that administrators perceived their knowledge of the ISTE Standards-A as good and their knowledge of the VTSIP as excellent. Administrators strongly agreed that both the ISTE Standards-A and the VTSIP were important in their practice. Additionally, their perceptions of both the ISTE Standards-A and the VTSIP were excellent. There were no statistically significant differences found between the perception of the standards and most of the demographic variables. However, there was a statistically significant difference found between male and female perceptions of the VTSIP. Also, there was a statistically significant difference found between administrators perceptions of the ISTE Standards-A and perceptions of the VTSIP. The study provided quantitative data on administrators perceived knowledge, importance and perceptions of practicing the ISTE Standards-A and the VTSIP. The findings of this study provide valuable information for administrators as they investigate their technology leadership and note areas of improvement, and for the Virginia Department of Education, local education agencies and universities. Abstract: Objective To determine the lower limb joint kinetic strategies during stair descent in claudicants with peripheral arterial disease (PAD-IC). Design Cross-sectional observation study. Setting University laboratory. Participants A total of 22 participants; 10 healthy controls and 12 patients diagnosed with PAD-IC. Main outcome measures Between-group comparisons of ground reaction force (GRF) and, hip, knee and ankle kinetics during steady-state stair descent. Results The claudicating-limb group demonstrated reduced vertical and posterior GRF compared to healthy controls (ES = −1.46 [−2.32,−0.69] and ES = −1.08 [−0.42,−0.26]) as well as demonstrating an greater contribution to support moment from the ankle and trends towards a smaller hip contribution (42 ± 14% vs 28 ± 7%, P = .005 and Hip 16 ± 8% vs 21 ± 11%, P = .056, respectively). A unique sub-group was identified within the PAD-IC cohort demonstrating different hip moment strategies during weight acceptance: a novel hip extensor group (PAD-IC HExt) and stereotypical hip flexor group (PAD-IC HFlex). Compared to both healthy controls and the PAD-IC HFlex groups, the PAD-IC HExt group demonstrated increased hip extensor moment (ES = 3.05 [1.67,4.42] and ES = 3.62 [1.89,5.35]) and reduced knee extensor moment (ES = −2.00 [−3.15,−0.85] and ES = −1.36 [−2.60,−0.11] respectively) during weight acceptance. Conclusions A novel hip extensor strategy was identified in a sub-group of claudicants which acts to reduce the demand on the knee extensors, but not the plantarflexors. Weakness in the knee extensors may prevent redistribution of the task demand, typically seen in older adults in stair descent, away from the functionally limited plantarflexor muscle group. Further investigation into multi-level joint strength and the relationship to functional tasks is warranted to inform targeted intervention programmes.

Abstract: The internal knee abduction moment (KAM) in osteoarthritis is reduced by increased lateral trunk lean (TL). Mechanistically, this occurs as the Centre of Mass (COM) moves further over the stance leg. Since the size of the base of support constrains the COM, an associated increase in step width (SW) would be expected to maintain stability. This study tested the effects of TL on SW and KAM in healthy participants (n = 21) who performed normal and 6° TL walks. The latter was controlled via audio-visual biofeedback. We found two distinct gait strategies in TL walk: widening the step width substantially (>50%) to permit an increase in the COM displacement (WSW, n = 13), or maintaining a baseline SW and minimally displacing the COM by moving the hip/pelvic complex in the opposite direction (NSW, n = 8). WSW doubled SW (11.3 ± 2.4 v. 24.7 ± 5.5 cm, p 0.05). These two distinct gait strategies resulted in unique patterns of KAM reduction across the stance phase. NSW reduced KAM impulse significantly in the initial half (0.08 ± 0.02 v. 0.06 ± 0.02, p = .04) but not in the later stance phase (0.07 ± 0.02 v. 0.07 ± 0.04, p > 0.05). Abstract: Background Little is known about the incidence of severe critical events in children undergoing general anaesthesia in Europe. We aimed to identify the incidence, nature, and outcome of severe critical events in children undergoing anaesthesia, and the associated potential risk factors. Methods The APRICOT study was a prospective observational multicentre cohort study of children from birth to 15 years of age undergoing elective or urgent anaesthesia for diagnostic or surgical procedures. Children were eligible for inclusion during a 2-week period determined prospectively by each centre. There were 261 participating centres across 33 European countries. The primary endpoint was the occurence of perioperative severe critical events requiring immediate intervention. A severe critical event was defined as the occurrence of respiratory, cardiac, allergic, or neurological complications requiring immediate intervention and that led (or could have led) to major disability or death. This study is registered with ClinicalTrials.gov, number NCT01878760. Findings Between April 1, 2014, and Jan 31, 2015, 31â127 anaesthetic procedures in 30â874 children with a mean age of 6Â·35 years (SD 4Â·50) were included. The incidence of perioperative severe critical events was 5Â·2% (95% CI 5Â·0â5Â·5) with an incidence of respiratory critical events of 3Â·1% (2Â·9â3Â·3). Cardiovascular instability occurred in 1Â·9% (1Â·7â2Â·1), with an immediate poor outcome in 5Â·4% (3Â·7â7Â·5) of these cases. The all-cause 30-day in-hospital mortality rate was 10 in 10â000. Abstract: The maximum chlorophyll index (MCI) from the Medium Resolution Imaging Spectrometer (MERIS) satellite imager gives a robust indicator of the presence of a variety of floating slicks, near-surface vegetation, and intense surface plankton blooms. The index responds to the presence of surface slicks of Trichodesmium due to the ‘red edge’ increase in radiance with increasing wavelength near 700 nm. Global composite images of this index can be used to map the distribution of surface slicks of Trichodesmium, showing seasonal and long-term variations. The MCI also responds to the ‘red edge’ in the spectral signature of Sargassum. The two species share some areas of common occurrence and it is important to distinguish between them. We have developed spectral techniques of distinguishing between these two, and for discriminating a variety of other confusing targets that occur in different areas. Abstract: Objective Intermittent claudication (IC) is frequently associated with deterioration in walking capacity and physical function, and it can often result in an impairment in balance. Whereas supervised exercise is recommended by the National Institute for Health and Care Excellence as the first-line treatment, the mechanism behind walking improvement is poorly understood. The existing literature suggests that there may be some physiologic change to the skeletal muscle contributing to the functional impairment, but these data are conflicting. We therefore sought to undertake a systematic review to clarify the muscle properties of patients with IC. Methods A systematic review of randomized and nonrandomized trials that investigated the role of muscle function in patients diagnosed with IC was undertaken using MEDLINE, Cochrane Central Register of Controlled Trials, and Embase databases. The searches were limited from 1947 to June 2016 in the English language. Results The search yielded a total of 506 articles, of which 206 were duplicate articles. Of the remaining 300, a total of 201 were excluded from full-text analysis; 99 full-text articles were assessed for eligibility, with 30 articles deemed appropriate for inclusion in the review. There were four main categories of functional outcome measures: muscle strength, muscle size, muscle fiber type, and muscle metabolism. A total of 2837 patients were included in the study. Nine studies reported on muscle strength, incorporating isometric, concentric, eccentric, and endurance measures. Eight studies reported on muscle size, incorporating circumference, computed tomography scans, and ultrasound imaging techniques. Eleven studies reported on muscle fibers, incorporating fiber type proportions, fiber size, and capillarity measures. Seven papers reported on muscle metabolism, incorporating adenosine diphosphate recovery and phosphocreatine recovery measures. Conclusions Previous literature has found clear evidence that strength (of the calf and thigh musculature) and calf characteristics are related to mortality and functional declines. However, this review has demonstrated the vast array of muscle groups assessed and multiple methods employed to determine strength; therefore, it is unclear exactly what measure of "strength" is impaired. Furthermore, the underlying morphologic causes of potential changes in strength are unclear. This information is essential for designing optimal exercise interventions. The data acquired during this systematic review are heterogeneous, with a substantial lack of high-quality intervention-based studies. Future research should endeavor to establish standardized testing procedures and to implement randomized controlled trials for targeted therapeutic interventions.

Abstract: In the summer of 2011, a major ‘Sargassum event’ brought large amounts of seaweed onto the beaches of the islands of the eastern Caribbean with significant effects on local tourism. We present satellite observations showing that the event had its origin north of the mouth of the Amazon in an area not previously associated with Sargassum growth. A significant concentration of Sargassum was detected in April, when it was centred at about 7° N latitude and 45° W longitude. By July it had spread to the coast of Africa in the east and to the Lesser Antilles and the Caribbean in the west. We have previously used images from MERIS (Medium Resolution Imaging Spectrometer) and MODIS (Moderate Resolution Imaging Spectroradiometer) to show the value of satellite observations in tracking patterns of Sargassum. Abstract: Medium Resolution Imaging Spectrometer (MERIS) satellite imagery of areas of mixed open water and ice near Antarctica shows an increase of a particular type of phytoplankton bloom by about a factor of 4 during the period 2003–2012. The increase has been mostly due to the detection of blooms over a larger area, increasing from about 24,000 km2 in 2003 to 100,000 km2 in 2012, but with some indication also of increasing bloom intensity. Blooms of this type have been previously identified as consistent with ‘superblooms’ near melting ice shelves, but can also be associated with newly forming ice. They occur in February to April of each year, with peak intensity in March, when new ice is starting to form. Although the total count shows a fairly steady increase, the blooms tend to occur at different longitudes in different years. Cause of the rapid increase is unclear. 50. Responsibility in transitioning teachers: What drives it when self-efficacy is lacking?

Abstract: Previous research has linked self-efficacy to classroom responsibility noting that teachers take on more classroom responsibility when they have confidence in themselves and their abilities. However, the gap to be addressed is with transitioning teachers who transitioned from other careers and those who entered alternate route programs. Previous research has found that these teachers lack self-efficacy because of lack of preparation and classroom experience, yet they still must take on the responsibilities of the classroom. The purpose of this qualitative study was to determine the internal factors that drive classroom responsibility for transitioning teachers who have low self-efficacy. In addition to that, it was also important to try to understand how self-efficacy can be developed in these teachers. The participants for this study were 4 teachers who either completed an alternate route program or transitioned to teaching from another career choice. They teach state-tested subject areas at a 5th and 6th grade elementary school in the rural South. Interviews were conducted with four transitioning teachers to be analyzed along with observations and artifacts. The study yielded the internal factors of motherhood, personal interests, and love as helping these teachers take responsibility for student learning. It was also found that verbal persuasion and vicarious experiences would help in developing self-efficacy in transitioning teachers. The current study challenges the idea of self-efficacy as the primary indicator of teacher responsibility. We are left to suggest that there are other realms to classroom responsibility, especially in transitioning teachers.
