Contains fulltext : 299952.pdf (Publisher’s version ) (Open Access), BACKGROUND: KBG syndrome is caused by haploinsufficiency of ANKRD11 and is characterised by macrodontia of upper central incisors, distinctive facial features, short stature, skeletal anomalies, developmental delay, brain malformations and seizures. The central nervous system (CNS) and skeletal features remain poorly defined. METHODS: CNS and/or skeletal imaging were collected from molecularly confirmed individuals with KBG syndrome through an international network. We evaluated the original imaging and compared our results with data in the literature. RESULTS: We identified 53 individuals, 44 with CNS and 40 with skeletal imaging. Common CNS findings included incomplete hippocampal inversion and posterior fossa malformations; these were significantly more common than previously reported (63.4% and 65.9% vs 1.1% and 24.7%, respectively). Additional features included patulous internal auditory canal, never described before in KBG syndrome, and the recurrence of ventriculomegaly, encephalic cysts, empty sella and low-lying conus medullaris. We found no correlation between these structural anomalies and epilepsy or intellectual disability. Prevalent skeletal findings comprised abnormalities of the spine including scoliosis, coccygeal anomalies and cervical ribs. Hand X-rays revealed frequent abnormalities of carpal bone morphology and maturation, including a greater delay in ossification compared with metacarpal/phalanx bones. CONCLUSION: This cohort enabled us to describe the prevalence of very heterogeneous neuroradiological and skeletal anomalies in KBG syndrome. Knowledge of the spectrum of such anomalies will aid diagnostic accuracy, improve patient care and provide a reference for future research on the effects of ANKRD11 variants in skeletal and brain development.

- Abstract
Background. High uptake of antiretroviral treatment (ART) is essential to reduce human immunodeficiency virus (HIV) transmission and related mortality; however, gaps in care exist. We aimed to construct the continuum of HIV care (CoC) in 2016 in 11 European Union (EU) countries, overall and by key population and sex. To estimate progress toward the Joint United Nations Programme on HIV/AIDS (UNAIDS) 90-90-90 target, we compared 2016 to 2013 estimates for the same countries, representing 73% of the population in the region. Methods. A CoC with the following 4 stages was constructed: number of people living with HIV (PLHIV); proportion of PLHIV diagnosed; proportion of those diagnosed who ever initiated ART; and proportion of those ever treated who achieved viral suppression at their last visit. Results. We estimated that 87% of PLHIV were diagnosed; 92% of those diagnosed had ever initiated ART; and 91% of those ever on ART, or 73% of all PLHIV, were virally suppressed. Corresponding figures for men having sex with men were: 86%, 93%, 93%, 74%; for people who inject drugs: 94%, 88%, 85%, 70%; and for heterosexuals: 86%, 92%, 91%, 72%. The proportion suppressed of all PLHIV ranged from 59% to 86% across countries. Conclusions. The EU is close to the 90-90-90 target and achieved the UNAIDS target of 73% of all PLHIV virally suppressed, significant progress since 2013 when 60% of all PLHIV were virally suppressed. Strengthening of testing programs and treatment support, along with prevention interventions, are needed to achieve HIV epidemic control.

Background High uptake of antiretroviral treatment (ART) is essential to reduce human immunodeficiency virus (HIV) transmission and related mortality; however, gaps in care exist. We aimed to construct the continuum of HIV care (CoC) in 2016 in 11 European Union (EU) countries, overall and by key population and sex. To estimate progress toward the Joint United Nations Programme on HIV/AIDS (UNAIDS) 90-90-90 target, we compared 2016 to 2013 estimates for the same countries, representing 73% of the population in the region. Methods A CoC with the following 4 stages was constructed: number of people living with HIV (PLHIV); proportion of PLHIV diagnosed; proportion of those diagnosed who ever initiated ART; and proportion of those ever treated who achieved viral suppression at their last visit. Results We estimated that 87% of PLHIV were diagnosed; 92% of those diagnosed had ever initiated ART; and 91% of those ever on ART, or 73% of all PLHIV, were virally suppressed. Corresponding figures for men having sex with men were: 86%, 93%, 93%, 74%; for people who inject drugs: 94%, 88%, 85%, 70%; and for heterosexuals: 86%, 92%, 91%, 72%. The proportion suppressed of all PLHIV ranged from 59% to 86% across countries. Conclusions The EU is close to the 90-90-90 target and achieved the UNAIDS target of 73% of all PLHIV virally suppressed, significant progress since 2013 when 60% of all PLHIV were virally suppressed. Strengthening of testing programs and treatment support, along with prevention interventions, are needed to achieve HIV epidemic control., Standardized definitions were used to estimate a 4-stage continuum of human immunodeficiency virus (HIV) care in 11 European Union (EU) countries in 2016. The EU is close to the 90-90-90 target, with the main challenge being the percentage of undiagnosed infections.

Background: Tuberculous meningitis (TBM) is the most lethal form of tuberculosis with a mortality of ~50% in those co-infected with HIV-1. Current antibiotic regimens are based on those known to be effective in pulmonary TB and do not account for the differing ability of the drugs to penetrate the central nervous system (CNS). The host immune response drives pathology in TBM, yet effective host-directed therapies are scarce. There is sufficient data to suggest that higher doses of rifampicin (RIF), additional linezolid (LZD) and adjunctive aspirin (ASA) will be beneficial in TBM yet rigorous investigation of the safety of these interventions in the context of HIV associated TBM is required. We hypothesise that increased dose RIF, LZD and ASA used in combination and in addition to standard of care for the first 56 days of treatment with be safe and tolerated in HIV-1 infected people with TBM. Methods: In an open-label randomised parallel study, up to 100 participants will receive either; i) standard of care (n=40, control arm), ii) standard of care plus increased dose RIF (35mg/kg) and LZD (1200mg OD for 28 days, 600mg OD for 28 days) (n=30, experimental arm 1), or iii) as per experimental arm 1 plus additional ASA 1000mg OD (n=30, experimental arm 2). After 56 days participants will continue standard treatment as per national guidelines. The primary endpoint is death and the occurrence of solicited treatment-related adverse events at 56 days. In a planned pharmacokinetic (PK) sub-study we aim to assess PK/pharmacodynamic (PD) of oral vs IV rifampicin, describe LZD and RIF PK and cerebrospinal fluid concentrations, explore PK/PD relationships, and investigate drug-drug interactions between LZD and RIF. The Var prodelta progrades across a straight, narrow shelf (less than 2 km wide) with a very steep continental slope reaching locally more than 30°. Historically, the Var delta front is sadly famous for the 1979 catastrophic submarine landslide that resulted in several casualties and infrastructural damage. Geotechnical and geophysical investigations carried out in late 2007 to the east of the 1979 landslide scar provide evidence for the possible occurrence of a new important sedimentary collapse and submarine landslide. Geophysical data acquired in the area show the presence of several seafloor morphological steps rooted to shallow subsurface seismic reflections. Moreover, in situ piezocone measurements demonstrate the presence of several shear zones at the border of the shelf break at different depths below the seafloor. The aim of this technical note is to present and discuss acquired geotechnical and geophysical data in terms of failure mechanisms and submarine landslides. Both geophysical and geotechnical data suggest the start-up of a progressive failure mechanism and reveal the possible occurrence of a submarine landslide and the urgent need for mitigation procedures. Historiquement, le delta du Var est malheureusement celebre pour la catastrophe de 1979 provoquee par un glissement sous-marin ayant entraine des victimes et des dommages materiels. Les explorations geotechniques et geophysiques menees fin 2007 dans la partie orientale de la cicatrice du glissement de 1979 temoignent de la forte probabilite que des effondrements seedimentaires a venir provoquent a nouveau des glissements sous-marins. Les donnees geophysiques acquises sur la zone montrent l'existence de plusieurs terrasses morphologiques sur le fond de la mer connectees en profondeur a des surfaces sismiques superficielles. De plus, les mesures in-situ des piezocones demontrent la presence de plusieurs zones de cisaillement en rebord de plateau a plusieurs profondeurs sous le fond de la mer. L'objectif de cette note est de presenter et discuter les donnees geotechniques et geophysiques acquises en relation avec les mecanismes de rupture et de glissement sous-marin. 14. Unlocking the performance potential of functionally diverse teams: The paradoxical role of leader mood

In a multisource, lagged design field study of 66 consulting teams, we investigated the role of leader mood in unlocking the performance potential of functionally diverse teams. In line with our hypotheses, we found that, given high levels of leader positive mood, functional diversity was positively related to collective team identification. In contrast, given high levels of leader negative mood, functional diversity was positively associated with information elaboration in teams. Furthermore, results showed that functional diversity was most strongly related to team performance when both leader positive mood and leader negative mood were high. This study highlights the value of examining seemingly contradictory leadership aspects in the effort to gain a fuller understanding of how to foster performance in diverse teams. Practitioner points To effectively lead diverse teams, leaders need to navigate between the need to promote unique ideas (i.e., information elaboration) and the simultaneous need to pull together diverse members towards a common identity. Leader mood addresses both of these needs. When the team leader exhibited a positive mood, team functional diversity was positively related to members’ identification with the team. By contrast, when the team leader displayed a negative mood, team functional diversity was positively related to information elaboration. Over a 12‐day period, diverse teams performed best when the leader showed both positive and negative mood. Leaders of diverse teams are required to be sensitive to the affective tone of their team and aware of how their emotional displays influence team members’ moods and behaviours as well as team processes.

We describe an X-linked genetic syndrome associated with mutations in TAF1 and manifesting with global developmental delay, intellectual disability (ID), characteristic facial dysmorphology, generalized hypotonia, and variable neurologic features, all in male individuals. Simultaneous studies using diverse strategies led to the identification of nine families with overlapping clinical presentations and affected by de novo or maternally inherited single-nucleotide changes. Two additional families harboring large duplications involving TAF1 were also found to share phenotypic overlap with the probands harboring single-nucleotide changes, but they also demonstrated a severe neurodegeneration phenotype. Functional analysis with RNA-seq for one of the families suggested that the phenotype is associated with downregulation of a set of genes notably enriched with genes regulated by E-box proteins. In addition, knockdown and mutant studies of this gene in zebrafish have shown a quantifiable, albeit small, effect on a neuronal phenotype. Our results suggest that mutations in TAF1 play a critical role in the development of this X-linked ID syndrome. © 2015 The Authors.

22. Team diversity, team processes, and team performance: The moderating role of leader mood

Drawing on the Categorization-Elaboration (van Knippenberg, De Dreu, & Homan, 2004) and the Emotions-as-Social- Information model (Van Kleef, 2009), respectively, we hypothesized that leader mood will impact the respective relationships between team diversity and collective team identification and elaboration of task-relevant information. We further hypothesized that the association between elaboration and objective team performance will be moderated by team identification. Testing these hypotheses with data collected from 59 financial services teams at three different time points, we found that, given high levels of leader positive mood, team diversity was positively related to team identification and negatively associated with elaboration of task-relevant information. In contrast, given high levels of leader negative mood, diversity was negatively related to team identification and positively associated with elaboration of task-relevant information. Such incongruence between identification and elaboration poses an intriguing challenge with respect to predictions based on the CEM and previous research on team diversity. Also in contrast to previous theory and research, we found that elaboration of task-relevant information was conducive to objective team performance only when collective team identification was high. This study contributes to the literature by illustrating the central role of the leader in unlocking the potential of diversity, by shedding light on the link between team diversity and affect, and by providing new insights into the role of information elaboration in diverse teams.

The Nice Slope, situated in the French part of the Ligurian Margin, representsa landslide-prone area, where different factors favour slope instability: (i)Seismicity up to ~M6, (ii) rapid sediment deposition in the Var fluvial system,(iii) groundwater discharge along the coastal aquifer, (iv) creeping ofsensitive clay layers, and (v) human activity (land reclamation, construction).In 1979, a catastrophic failure occurred in the Var pro-delta, that destroyedthe Nice new harbour extension, mobilised ~8.7x106 m3 material, and initiated atsunami wave of 2-3m. This event led extensive research efforts first directedat understanding the triggers of the failure. In later years, some furtherstudies allowed to detect the occurrence of mechanically weak sediments outsidethe area that failed in 1979. This, along with the detection of freshened porewater and excess pore pressure development raised concerns as to the presentday and future slope stability. Hence, at present, progressive weakening of the shallow sediment is monitoredusing a long-term array in collaboration between Ifremer Brest and MARUMBremen. It comprises two multi-level pore pressure probes (Ifremer piezometers,one installed in the 1979 scar, one in the unaffected slope further E) as wellas a tilt-meter instrument (Ifremer) that will observe the deformation rate ofthe upper 8 m of sediment in real-time. In addition, an osmotically drivenfluid sampling-lance (MARUM) collects pore fluids for geochemical studies atthe levels of the pore pressure ports of the piezometers to identifyground-water discharge. With temporary OBS installations and onshoreseismometers in place, the array will allow us to distinguish between thevarious factors that are presently affecting the stability of the NiceSlope.

Pore pressure and shear strength are major controlling parameters for slope stability, which can be measured in situ using CPT (cone penetration testing) instruments. This paper presents results from initial tests with two free-fall CPT probes deployed in the neotectonically active submarine slope of northern Crete, Greece. Research expedition P336 investigated landslide-prone areas in the Cretan Sea using multibeam swathmapping, seismic reflection profiling, in situ CPT measurements, and gravity coring. Several large landslide complexes at the NE Cretan Margin as well as a small, but steep landslide scarp structure further east were identified on the seismic profiles. CPT devices were deployed in undisturbed slope sediments, across the slide scar, and in the main body of the slide, and remained stuck in the sediment for ~10 minutes to monitor pore pressure dissipation upon insertion. Excess pore pressure after insertion is in a range around 60 kPa in background sediment, and exceeds 80 kPa in the slide deposits. Cone resistance of the slope sediment ranges between 300 and 500 kPa, corresponding to undrained shear strengths of up to 40 kPa. The slid sediments (specifically the headwall material with <10 kPa strength) show velocity-weakening behaviour during ring shear experiments, indicating that those sediments are unlikely to show stable creep and instead may fail catastrophically. The in situ measurement of seafloor physical properties such as pore pressure, shear strength or compressibility poses a challenge to engineers, in particular in the marine realm. We present the design and first use of a marine, deep-water free-fall instrument for cone penetration testing (CPT). The probe can be operated in up to 4000 m water depth to measure cone resistance, sleeve friction, deceleration, temperature and tilt as well as pore pressure in u1 and u3 position. In this paper we discuss the advantages and disadvantages of the current prototype design, and dwell on the differences between quasi-static versus dynamic cone penetration testing. Subaquatic mass movements are common in marine and lacustrine environments, but due to their barely pre- dictable nature, direct observations of these processes are limited so that knowledge is only indirectly obtained by investigating the resulting mass-transport deposits (MTDs). Most research focuses on the most common fron- tally emergent slides, fast-moving events able to generate turbidity currents and tsunamis. Geohazards of fron- tally confined slides and mechanisms behind their typical fold-and-thrust deformation structures are however still poorly understood. We investigate frontally confined MTDs in Lake Lucerne (Switzerland) by integrating bathymetric and high-res- olution seismic data with geotechnical information derived from in situ Cone Penetrometer Tests and short core analysis. Investigated MTDs consist of three units: i) a mass-slide deposit, located at the base of the slope consisting of a coherent slope sequence, ii) a fold-and-thrust system developed in basin sediments, and iii) an overrunning mass flow deposit, consisting of remolded slope sediments. The deformed and thrusted basin sed- iments show higher undrained shear strength compared to the undisturbed basin sequence. We propose that this strengthening is caused by lateral compression leading to fluid expulsion in the high-plasticity basin sedi- ments by the bulldozing sliding mass. Relative kinematic indicators document that the fold-and-thrust deforma- tion structures occur rapidly. Thus, they should be considered in tsunami hazard analysis. Furthermore, our data highlight that the slope angle of the gliding surface and basin topography are key controlling factors for slope sta- bility and propagation of basin-plain deformations, respectively. Our integrated study supports and refines prop- agation models proposed in marine environments, revealing the potential of investigating smaller-scale easier- to-access MTDs in lakes.

Purpose: MYRF-related cardiac-urogenital syndrome (MYRF-CUGS) is a rare condition associated with heterozygous MYRF variants. The description of MYRF-CUGS phenotype is mostly based on postnatal cases and 36 affected individuals have been published so far. We aim now to delineate the prenatal phenotype of MYRF-CUGS by reporting clinical data from fetuses and neonates with a pathogenic MYRF variant., Methods: Detailed radiographic, pathological, clinical, and molecular data from 12 prenatal cases were collected through an international collaborative study. Adding the five fetuses previously published, we were able to study a cohort of 17 cases., Results: Main ultrasound-accessible manifestations of MYRF-CUGS include congenital heart defects (13/17, 76%), congenital diaphragmatic hernia (10/17, 59%) and disorders of sexual differentiation in 46, XY fetuses (7/14; 50%). Postnatal examination and/or autopsy data highlighted additional birth defects and neurological findings with a large spectrum of severity. Molecular results revealed ten previously unpublished variants, one missense and nine predicted truncating variants (three frameshift, three nonsense and three splice site variants)., Conclusion: We report the first prenatal cohort of MYRF-CUGS, allowing us to further characterize the variable expressivity of this rare disorder in fetuses. Severe congenital anomalies with a poor prognosis are more frequent than previously described in postnatal cases. - Abstract
Background: KBG syndrome is caused by haploinsufficiency of ANKRD11 and is characterised by macrodontia of upper central incisors, distinctive facial features, short stature, skeletal anomalies, developmental delay, brain malformations and seizures. The central nervous system (CNS) and skeletal features remain poorly defined., Methods: CNS and/or skeletal imaging were collected from molecularly confirmed individuals with KBG syndrome through an international network. We evaluated the original imaging and compared our results with data in the literature., Results: We identified 53 individuals, 44 with CNS and 40 with skeletal imaging. Common CNS findings included incomplete hippocampal inversion and posterior fossa malformations; these were significantly more common than previously reported (63.4% and 65.9% vs 1.1% and 24.7%, respectively). Additional features included patulous internal auditory canal, never described before in KBG syndrome, and the recurrence of ventriculomegaly, encephalic cysts, empty sella and low-lying conus medullaris. We found no correlation between these structural anomalies and epilepsy or intellectual disability. Prevalent skeletal findings comprised abnormalities of the spine including scoliosis, coccygeal anomalies and cervical ribs. Hand X-rays revealed frequent abnormalities of carpal bone morphology and maturation, including a greater delay in ossification compared with metacarpal/phalanx bones., Conclusion: This cohort enabled us to describe the prevalence of very heterogeneous neuroradiological and skeletal anomalies in KBG syndrome. Purpose: De novo variants in CUL3 (Cullin-3 ubiquitin ligase) have been strongly associated with neurodevelopmental disorders (NDDs), but no large case series have been reported so far. Here we aimed to collect sporadic cases carrying rare variants in CUL3, describe the genotype-phenotype correlation, and investigate the underlying pathogenic mechanism., Methods: Genetic data and detailed clinical records were collected via multi-center collaboration. Dysmorphic facial features were analyzed using GestaltMatcher. Variant effects on CUL3 protein stability were assessed using patient-derived T-cells., Results: We assembled a cohort of 35 individuals with heterozygous CUL3 variants presenting a syndromic NDD characterized by intellectual disability with or without autistic features. Of these, 33 have loss-of-function (LoF) and two have missense variants. CUL3 LoF variants in patients may affect protein stability leading to perturbations in protein homeostasis, as evidenced by decreased ubiquitin-protein conjugates in vitro . Specifically, we show that cyclin E1 (CCNE1) and 4E-BP1 (EIF4EBP1), two prominent substrates of CUL3, fail to be targeted for proteasomal degradation in patient-derived cells., Conclusion: Our study further refines the clinical and mutational spectrum of CUL3 -associated NDDs, expands the spectrum of cullin RING E3 ligase-associated neuropsychiatric disorders, and suggests haploinsufficiency via LoF variants is the predominant pathogenic mechanism.

- Abstract
As social exclusion can be linked to worse health and overall reduced quality of life, we describe social exclusion in people with diabetes and assess whether diabetes can be considered as a risk factor for social exclusion. We analyzed two waves (2014, 2017, N = 6604) from a survey of community-dwelling people aged > 40 using linear regression, group comparison and generalized estimating equations to explore the association between diabetes, social exclusion, socioeconomic, physical and psychosocial variables. In the entire cohort, diabetes was cross-sectionally associated with social exclusion after adjusting for covariates (p = 0.001). In people with diabetes, social exclusion was further associated with self-esteem (p < 0.001), loneliness (p = < 0.001), income (p = 0.017), depression (p = 0.001), physical diseases (p = 0.04), and network size (p = 0.043). Longitudinal data revealed that higher levels of social exclusion were already present before the diagnosis of diabetes, and future social exclusion was predicted by self-esteem, loneliness, depression, and income, but not by diabetes (p = .221). We conclude that diabetes is not a driver of social exclusion. Instead, both seem to co-occur as a consequence of health-related and psychosocial variables., (© 2023. The Author(s).)

- Abstract
Background: Drug regimens that include intensified antibiotics alongside effective anti-inflammatory therapies may improve outcomes in tuberculous meningitis (TBM). Safety data on their use in combination and in the context of human immunodeficiency virus (HIV) are needed to inform clinical trial design., Methods: We conducted a phase 2, open-label, parallel-design, randomized, controlled trial to assess the safety of high-dose rifampicin, linezolid, and high-dose aspirin in HIV-associated TBM. Participants were randomized (1.4:1:1) to 3 treatment arms (1, standard of care [SOC]; 2, SOC + additional rifampicin [up to 35 mg/kg/d] + linezolid 1200 mg/d reducing after 28 days to 600 mg/d; 3, as per arm 2 + aspirin 1000 mg/d) for 56 days, when the primary outcome of adverse events of special interest (AESI) or death was assessed., Results: A total of 52 participants with HIV-associated TBM were randomized; 59% had mild disease (British Medical Research Council (MRC) grade 1) vs 39% (grade 2) vs 2% (grade 3). AESI or death occurred in 10 of 16 (63%; arm 3) vs 4 of 14 (29%; arm 2) vs 6 of 20 (30%; arm 1; P = .083). The cumulative proportion of AESI or death (Kaplan-Meier) demonstrated worse outcomes in arm 3 vs arm 1 (P = .04); however, only 1 event in arm 3 was attributable to aspirin and was mild. There was no difference in efficacy (modified Rankin scale) between arms., Conclusions: High-dose rifampicin and adjunctive linezolid can safely be added to the standard of care in HIV-associated TBM. - Abstract
Background: Since the first description of a BRWD3-associated nonsydromic intellectual disability (ID) disorder in 2007, 21 additional families have been reported in the literature., Methods: Using exome sequencing (ES) and international data sharing, we identified 14 additional unrelated individuals with pathogenic BRWD3 variants (12 males and 2 females, including one with skewed X-inactivation). We reviewed the 31 previously published cases in the literature with clinical data available, and describe the collective phenotypes of 43 males and 2 females, with 33 different BRWD3 variants., Results: The most common features in males (excluding one patient with a mosaic variant) included ID (39/39 males), speech delay (24/25 males), postnatal macrocephaly (28/35 males) with prominent forehead (18/25 males) and large ears (14/26 males), and obesity (12/27 males). Both females presented with macrocephaly, speech delay, and epilepsy, while epilepsy was only observed in 4/41 males. Among the 28 variants with available segregation reported, 19 were inherited from unaffected mothers and 9 were de novo., Conclusion: This study demonstrates that the BRWD3-related phenotypes are largely non-specific, leading to difficulty in clinical recognition of this disorder. A genotype-first approach, however, allows for the more efficient diagnosis of the BRWD3-related nonsyndromic ID. 44. Geotechnical characterization and stability analysis of subaqueous slopes in Lake Lucerne (Switzerland).

- Abstract
Tsunamis occur not only in marine settings but also in lacustrine environments. Most of the lacustrine tsunamis are caused by seismically- or aseismically-triggered mass movements. Therefore, an assessment of the stability of subaqueous slopes is crucial for tsunami hazard assessment in a lake. We selected Lake Lucerne (Switzerland) as a natural laboratory to perform an in-depth geotechnical characterization of its subaqueous slopes. This lake experienced documented tsunamis in 1601 and 1687. Some of its slopes still bear sediment volumes with a potential for tsunamigenic failure. To identify such slopes, we interpreted available reflection seismic data and analyzed the bathymetric map. Then, we performed 152 dynamic Cone Penetration Tests with pore pressure measurement (CPTu) and retrieved 49 sediment cores at different locations in the lake. These data were used to characterize the failure-prone sediments and to evaluate the present-day static stability of subaqueous slopes. Obtained results allowed the definition of three classes of slopes in terms of static stability: unstable slopes, stable slopes close to the unstable state, and stable areas. Non-deltaic slopes with thicker unconsolidated fine-grained sediment drape and moderate-to-high slope gradients (> 5-10°) have the lowest Factor of Safety. In agreement with previous studies, the failure plane for the non-deltaic slopes is embedded within the fine-grained glaciolacustrine sediments. Deltaic slopes with prevailing coarse-grained sediments mostly appear statically stable. Finally, we generalized the measured undrained shear strength profiles s u ( z ) into the depth-dependent power-law models. Background: Tuberculous meningitis (TBM) is the most lethal form of tuberculosis with a mortality of ~50% in those co-infected with HIV-1. Current antibiotic regimens are based on those known to be effective in pulmonary TB and do not account for the differing ability of the drugs to penetrate the central nervous system (CNS). The host immune response drives pathology in TBM, yet effective host-directed therapies are scarce. There is sufficient data to suggest that higher doses of rifampicin (RIF), additional linezolid (LZD) and adjunctive aspirin (ASA) will be beneficial in TBM yet rigorous investigation of the safety of these interventions in the context of HIV associated TBM is required. We hypothesise that increased dose RIF, LZD and ASA used in combination and in addition to standard of care for the first 56 days of treatment with be safe and tolerated in HIV-1 infected people with TBM. Methods: In an open-label randomised parallel study, up to 100 participants will receive either; i) standard of care (n=40, control arm), ii) standard of care plus increased dose RIF (35mg/kg) and LZD (1200mg OD for 28 days, 600mg OD for 28 days) (n=30, experimental arm 1), or iii) as per experimental arm 1 plus additional ASA 1000mg OD (n=30, experimental arm 2). After 56 days participants will continue standard treatment as per national guidelines. The primary endpoint is death and the occurrence of solicited treatment-related adverse events at 56 days. In a planned pharmacokinetic (PK) sub-study we aim to assess PK/pharmacodynamic (PD) of oral vs IV rifampicin, describe LZD and RIF PK and cerebrospinal fluid concentrations, explore PK/PD relationships, and investigate drug-drug interactions between LZD and RIF. - Abstract
Background/aim: Death receptor 6 (DR6) is a member of the tumor necrosis factor receptor superfamily. The expression of DR6 is elevated in different kinds of tumors including ovarian, breast cancer and adult sarcoma. In these tumors, the receptor may be handled as a new diagnostic and prognostic marker. Thus, we investigated the expression of DR6 in gliomas., Materials and Methods: Tumor and control tissues were extracted during neurosurgery and grouped according to the WHO classification. DR6 expression was investigated in low- and high-grade gliomas PCR (n=70), immunofluorescence staining (n=33) and western blot (n=58). Additional analysis of TCGA-data was performed to assess the general alteration of DR6 in cancer and influence of IDH-mutation on DR6 expression in gliomas., Results: The expression of DR6 was significantly enhanced in gliomas (p<0.05). It showed a trend towards rising expression with increasing malignancy of the tumor. Whilst the adoption of such practices is mainly considered a top-down approach to work design, we argue that successful utilization of time-spatial flexibility requires proactivity on the part of the employee in the form of time-spatial job crafting . Previous research has demonstrated that time-spatial flexibility can have both positive and negative effects on well-being, performance, and work-life balance; yet remains mute about the underlying reasons for this and how employees can handle the given flexibility. Drawing on research from work design, we posit that in order for employees to stay well and productive in this context, they need to engage in time-spatial job crafting (i.e., a context-specific form of job crafting that entails reflection on time and place), which can be considered a future work skill. We propose a theoretical model of time-spatial job crafting in which we discuss its components, shed light on its antecedents, and explain how time-spatial job crafting is related to positive work outcomes through a time/spatial-demands fit.

- Abstract
Purpose: Truncating mutations in the maternally imprinted, paternally expressed gene MAGEL2, which is located in the Prader-Willi critical region 15q11-13, have recently been reported to cause Schaaf-Yang syndrome, a Prader-Willi-like disease that manifests as developmental delay/intellectual disability, hypotonia, feeding difficulties, and autism spectrum disorder. The causality of the reported variants in the context of the patients' phenotypes was questioned, as MAGEL2 whole-gene deletions seem to cause little or no clinical phenotype., Methods: Here we report a total of 18 newly identified individuals with Schaaf-Yang syndrome from 14 families, including 1 family with 3 individuals found to be affected with a truncating variant of MAGEL2, 11 individuals who are clinically affected but were not tested molecularly, and a presymptomatic fetal sibling carrying the pathogenic MAGEL2 variant., Results: All cases harbor truncating mutations of MAGEL2, and nucleotides c.1990-1996 arise as a mutational hotspot, with 10 individuals and 1 fetus harboring a c.1996dupC (p.Q666fs) mutation and 2 fetuses harboring a c.1996delC (p.Q666fs) mutation. The phenotypic spectrum of Schaaf-Yang syndrome ranges from fetal akinesia to neurobehavioral disease and contractures of the small finger joints., Conclusion: This study provides strong evidence for the pathogenicity of truncating mutations of the paternal allele of MAGEL2, refines the associated clinical phenotypes, and highlights implications for genetic counseling for affected families.Genet Med 19 1, 45-52.

- Abstract
We describe an X-linked genetic syndrome associated with mutations in TAF1 and manifesting with global developmental delay, intellectual disability (ID), characteristic facial dysmorphology, generalized hypotonia, and variable neurologic features, all in male individuals. Simultaneous studies using diverse strategies led to the identification of nine families with overlapping clinical presentations and affected by de novo or maternally inherited single-nucleotide changes. Two additional families harboring large duplications involving TAF1 were also found to share phenotypic overlap with the probands harboring single-nucleotide changes, but they also demonstrated a severe neurodegeneration phenotype. Functional analysis with RNA-seq for one of the families suggested that the phenotype is associated with downregulation of a set of genes notably enriched with genes regulated by E-box proteins. In addition, knockdown and mutant studies of this gene in zebrafish have shown a quantifiable, albeit small, effect on a neuronal phenotype. - Abstract
Previous studies have suggested the efficacy of foscarnet combined with thymidine analogues as salvage therapy in late-stage HIV-1 infection. Here, we report on the first case of foscarnet therapy in a patient infected with HIV-2 exhibiting virologic failure. The patient was known to be HIV-2-infected since 1992 and had received 11 sequential lines of combined antiretroviral therapy (cART) with almost all the available antiretroviral agents including raltegravir. A marked decrease in HIV-2 plasma viral load of 1.48 log(10)copies/ml was observed at day 14 of foscarnet induction therapy associated with zidovudine and failing cART. An optimized cART was then introduced with lamivudine, zidovudine, lopinavir/r, etravirine and maraviroc. Four months after the end of foscarnet therapy, HIV-2 plasma viral load remained undetectable. This case report suggests that foscarnet may represent a therapeutic option for HIV-2-infected patients exhibiting multidrug resistance., (Copyright (c) 2009 Elsevier B.V. All rights reserved.)
