This contribution tries to find anwers to the question how to accurately approach the process of clarification of important geographical topics and problems. In the Introduction it explains the matter of the differences among information, know-ledge, opinions, statements as value-neutral enities and attitudes, the foundation of which are values, functioning as standards, according to which we evaluate our behaviour and actions of others. In the next part of our contribution we clarify basic principles of the informative approach and "the approach of attitudes to-wards teaching Geography." We explain basic differences in the application of these two approaches on the topic example of the migration of the population and we state the reasons which led us into preferring teaching methods and acti-vities developing all parts of students´attitudes.

Abstract Close vital signs monitoring is crucial for the clinical management of patients with dengue. We investigated performance of a non-invasive wearable utilising photoplethysmography (PPG), to provide real-time risk prediction in hospitalised individuals. We performed a prospective observational clinical study in Vietnam between January 2020 and October 2022: 153 patients were included in analyses, providing 1353 h of PPG data. Using a multi-modal transformer approach, 10-min PPG waveform segments and basic clinical data (age, sex, clinical features on admission) were used as features to continuously forecast clinical state 2 h ahead. Summary: Background: Dengue epidemics impose considerable strain on healthcare resources. Real-time continuous and non-invasive monitoring of patients admitted to the hospital could lead to improved care and outcomes. We evaluated the performance of a commercially available wearable (SmartCare) utilising photoplethysmography (PPG) to stratify clinical risk for a cohort of hospitalised patients with dengue in Vietnam. Methods: We performed a prospective observational study for adult and paediatric patients with a clinical diagnosis of dengue at the Hospital for Tropical Disease, Ho Chi Minh City, Vietnam. Patients underwent PPG monitoring early during admission alongside standard clinical care. PPG waveforms were analysed using machine learning models. Adult patients were classified between 3 severity classes: i) uncomplicated (ward-based), ii) moderate-severe (emergency department-based), and iii) severe (ICU-based). Data from paediatric patients were split into 2 classes: i) severe (during ICU stay) and ii) follow-up (14–21 days after the illness onset). Model performances were evaluated using standard classification metrics and 5-fold stratified cross-validation. Findings: We included PPG and clinical data from 132 adults and 15 paediatric patients with a median age of 28 (IQR, 21–35) and 12 (IQR, 9–13) years respectively. 1781 h of PPG data were available for analysis. The best performing convolutional neural network models (CNN) achieved a precision of 0.785 and recall of 0.771 in classifying adult patients according to severity class and a precision of 0.891 and recall of 0.891 in classifying between disease and post-disease state in paediatric patients. Interpretation: We demonstrate that the use of a low-cost wearable provided clinically actionable data to differentiate between patients with dengue of varying severity. Continuous monitoring and connectivity to early warning systems could significantly benefit clinical care in dengue, particularly within an endemic setting. Work is currently underway to implement these models for dynamic risk predictions and assist in individualised patient care. Electrocardiogram (ECG) and photoplethysmogram (PPG) are commonly used to determine the vital signs of heart rate, respiratory rate, and oxygen saturation in patient monitoring. In addition to simple observation of those summarized indexes, waveform signals can be analyzed to provide deeper insights into disease pathophysiology and support clinical decisions. Such data, generated from continuous patient monitoring from both conventional bedside and low-cost wearable monitors, are increasingly accessible. However, the recorded waveforms suffer from considerable noise and artifacts and, hence, are not necessarily used prior to certain quality control (QC) measures, especially by those with limited programming experience. Various signal quality indices (SQIs) have been proposed to indicate signal quality. To facilitate and harmonize a wider usage of SQIs in practice, we present a Python package, named vital_sqi, which provides a unified interface to the state-of-the-art SQIs for ECG and PPG signals. The vital_sqi package provides with seven different peak detectors and access to more than 70 SQIs by using different settings. The vital_sqi package is designed with pipelines and graphical user interfaces to enable users of various programming fluency to use the package. Multiple SQI extraction pipelines can take the PPG and ECG waveforms and generate a bespoke SQI table. As these SQI scores represent the signal features, they can be input in any quality classifier. The package provides functions to build simple rule-based decision systems for signal segment quality classification using user-defined SQI thresholds. An experiment with a carefully annotated PPG dataset suggests thresholds for relevant PPG SQIs.

BackgroundCritically ill patients often require complex clinical care by highly trained staff within a specialized intensive care unit (ICU) with advanced equipment. There are currently limited data on the costs of critical care in low-and middle-income countries (LMICs). This study aims to investigate the direct-medical costs of key infectious disease (tetanus, sepsis, and dengue) patients admitted to ICU in a hospital in Ho Chi Minh City (HCMC), Vietnam, and explores how the costs and cost drivers can vary between the different diseases.MethodsWe calculated the direct medical costs for patients requiring critical care for tetanus, dengue and sepsis. Costing data (stratified into different cost categories) were extracted from the bills of patients hospitalized to the adult ICU with a dengue, sepsis and tetanus diagnosis that were enrolled in three studies conducted at the Hospital for Tropical Diseases in HCMC from January 2017 to December 2019. The costs were considered from the health sector perspective. The total sample size in this study was 342 patients.ResultsICU care was associated with significant direct medical costs. For patients that did not require mechanical ventilation, the median total ICU cost per patient varied between US$64.40 and US$675 for the different diseases. The costs were higher for patients that required mechanical ventilation, with the median total ICU cost per patient for the different diseases varying between US$2,590 and US$4,250. The main cost drivers varied according to disease and associated severity.ConclusionThis study demonstrates the notable cost of ICU care in Vietnam and in similar LMIC settings. Future studies are needed to further evaluate the costs and economic burden incurred by ICU patients. The data also highlight the importance of evaluating novel critical care interventions that could reduce the costs of ICU care.

This paper focuses on the application of Claroline courses and learning management system in lower Secondary Education. First part is briefly describing Claroline, its history and features. Next part elaborates related workflows for teachers using Claroline.Workflows from techer’s view guides form User account creation, thru creating new course (using main page, course description, storing and sharing documents and links, evaluating excerises and assignements). These workflows were implemented in as the pilot project at the Elementary School Narnia in Banska Bystrica on the courses Geography 6, Geography 5, and Information Technology (5th and 6th grade) in the second school term of 2011/2012. Final third part of the paper evulating experiences and opinions of pupils regarding usability of the Claroline LMS. Data was collected in survey (questionary in electronic form). The elearning environment was gradually accepted by pupils (and later also by their parents).

Despite the fact that digital technologies are more and more used in the learning and education process, there is still lack of professional evaluation tools capable of assessing the quality of used digital teaching aids in a comprehensive and objective manner. Construction of the Comprehensive Evaluation of Electronic Learning Tools and Educational Software (CEELTES) tool was preceded by several surveys and knowledge obtained in the course of creation of digital learning and teaching aids and implementation thereof in the teaching process. The evaluation tool as such consists of sets (catalogues) of criteria divided into four separately assessed areas - the area of technical, technological and user attributes; the area of criteria evaluating the content, operation, information structuring and processing; the area of criteria evaluating the information processing in terms of learning, recognition, and education needs; and, finally, the area of criteria evaluating the psychological and pedagogical aspects of a digital product. The specified areas are assessed independently, separately, by a specialist in the given science discipline. The final evaluation of the assessed digital product objectifies (quantifies) the overall rate of appropriateness of inclusion of a particular digital teaching aid in the teaching process.
