BACKGROUND: In a randomized, controlled trial that compared liraglutide, a glucagon-like peptide 1 analogue, with placebo in patients with type 2 diabetes and high cardiovascular risk who were receiving usual care, we found that liraglutide resulted in lower risks of the primary end point (nonfatal myocardial infarction, nonfatal stroke, or death from cardiovascular causes) and death. However, the long-term effects of liraglutide on renal outcomes in patients with type 2 diabetes are unknown. METHODS: We report the prespecified secondary renal outcomes of that randomized, controlled trial in which patients were assigned to receive liraglutide or placebo. The secondary renal outcome was a composite of new-onset persistent macroalbuminuria, persistent doubling of the serum creatinine level, end-stage renal disease, or death due to renal disease. The risk of renal outcomes was determined with the use of time-to-event analyses with an intention-to-treat approach. Changes in the estimated glomerular filtration rate and albuminuria were also analyzed. RESULTS: A total of 9340 patients underwent randomization, and the median follow-up of the patients was 3.84 years. The renal outcome occurred in fewer participants in the liraglutide group than in the placebo group (268 of 4668 patients vs. 337 of 4672; hazard ratio, 0.78; 95% confidence interval [CI], 0.67 to 0.92; P=0.003). This result was driven primarily by the new onset of persistent macroalbuminuria, which occurred in fewer participants in the liraglutide group than in the placebo group (161 vs. 215 patients; hazard ratio, 0.74; 95% CI, 0.60 to 0.91; P=0.004). The rates of renal adverse events were similar in the liraglutide group and the placebo group (15.1 events and 16.5 events per 1000 patient-years), including the rate of acute kidney injury (7.1 and 6.2 events per 1000 patient-years, respectively). CONCLUSIONS: This prespecified secondary analysis shows that, when added to usual care, liraglutide resulted in lower rates of the development and progression of diabetic kidney disease than placebo. Transdisciplinary researchers collaborate with diverse partners outside of academia to tackle sustainability problems. The patterns and practices of social interaction and the contextual nature of transdisciplinary research result in different performance expectations than traditional, curiosity-driven research. Documenting patterns of interaction can inform project success and affirm progress toward interim outcomes on the way to achieve sustainability impacts. Yet providing credible and robust indicators of research activity remains challenging. We provide quantitative and qualitative indicators for assessing transdisciplinary practices and patterns through social network analysis (SNA). Our assessment developed four criteria to reveal how SNA metrics provide insight into (1) diversity of participants; (2) whether and how integration and collaboration are occurring, (3) the relative degrees of network stability and fragility, and (4) how the network is structured to achieve its goals. - Abstract
This thesis investigates the Cultural Studies paradigm 'hybridity' through an analysis of Hong Kong culture. In Cultural Studies, 'hybridity' is usually applied to cultures that have been influenced by another culture, resulting in a loss of identity, leading to a cultural mixture. As a former Colony comprising predominantly of a southern Chinese population, Hong Kong has been influenced by British culture. However, the question is whether Hong Kong culture has been 'hybridises', or, the Hong Kong people, in mastering two cultures, have become bicultural, and use their biculturalism bilaterally, as particular situations require. The study also researches the condition of Hong Kong culture when exported overseas through migration, and remigration, especially among Chinese. Other areas researched are Hong Kong culture's relationship to identity and ethnicity through film and language. The primary research data is sourced from surveys and interviews with Hong Kong Chinese people in Australia and Hong Kong. Secondary sources include written media, computer generated media, film, and television. The research uses a multiple design format of field: historical; content analysis; textual analysis; forensic, and anecdotal material. The findings will show that Hong Kong culture is dominated by Chinese cultures rather than Western culture and as such may not be regarded as a 'hybrid' culture, but as a conglomerate of independently used cultures. My research findings challenge the validity of the Cultural Studies use of hybridity in association with colonialism, and opens the way for other cultures designated as 'hybrid' to be re-examined within a similar research framework to this study.

However, despite an increasing recognition of its importance, large studies evaluating the incidence patterns over time, risk factors and burden of CAP in COPD are currently lacking.Methods: A retrospective observational study using a large UK-based database of linked primary and secondary care records was conducted. Patients with a diagnosis of COPD aged ≥40 years were followed up for 5 years from January 1, 2010. CAP and exacerbation episodes were identified from hospital discharge data and primary care coding records, and rates were calculated per month, adjusting for mortality, and displayed over time. In addition, baseline factors predicting future risk of CAP and hospital admission with CAP were identified.Results: A total of 14,513 COPD patients were identified: 13.4% (n=1,938) had ≥1 CAP episode, of whom 18.8% suffered from recurrent (≥2) CAP. Background: The COVID-19 pandemic has led to more than 760,000 deaths worldwide (correct as of 16th August 2020). Studies suggest a hyperinflammatory response is a major cause of disease severity and death. Identitfying COVID-19 patients with hyperinflammation may identify subgroups who could benefit from targeted immunomodulatory treatments. Analysis of cytokine levels at the point of diagnosis of SARS-CoV-2 infection can identify patients at risk of deterioration.Methods: We used a multiplex cytokine assay to measure serum IL-6, IL-8, TNF, IL-1β, GM-CSF, IL-10, IL-33 and IFN-γ in 100 hospitalised patients with confirmed COVID-19 at admission to University Hospital Southampton (UK). Demographic, clinical and outcome data were collected for analysis.Results: Age > 70 years was the strongest predictor of death (OR 28, 95% CI 5.94, 139.45). IL-6, IL-8, TNF, IL-1β and IL-33 were significantly associated with adverse outcome. Clinical parameters were predictive of poor outcome (AUROC 0.71), addition of a combined cytokine panel significantly improved the predictability (AUROC 0.85). In those ≤70 years, IL-33 and TNF were predictive of poor outcome (AUROC 0.83 and 0.84), addition of a combined cytokine panel demonstrated greater predictability of poor outcome than clinical parameters alone (AUROC 0.92 vs 0.77).Conclusions: A combined cytokine panel improves the accuracy of the predictive value for adverse outcome beyond standard clinical data alone. - Abstract
This report evaluates the AGARD FMP Specialists' Meeting on Piloted Aircraft Environment Simulation Techniques held in Brussels on 24-27 April 1978. Following an introduction outlining the role of piloted simulation, the report considers the papers presented during the meeting. This is followed by a report of the round-table discussion, including contributions from the floor. Finally, an overall evaluation of the meeting is made. The general quality of the papers was high and a broad coverage of the topic of the meeting was achieved. Detailed descriptions of techniques, as well as of actual hardware for satisfying various needs, were given. A few papers gave a critical apprasial of the deficiencies of simulators for particular purposes. Nevertheless, criteria for judging or specifying simulators were still lacking and greater emphasis on the role of specific features of simulators in satisfying users' requirements is clearly needed. The scientific and mathematical description of atmospheric characteristics is now well advanced; the techniques of presentation to the pilot still need further refinement. A particular difficulty is the inability of current visual displays to adequately represent atmospheric phenomena via the visual channel. The role of visual displays and motion platforms in providing cues to the pilot was a subject of considerable debate and was certainly not resolved at the meeting. It is an area requiring urgent study. Also the differing requirements of simulators in the training and research and development roles need to be delineated and defined; not least of the difficulties may be the identification of the training needs. The three types of furnace used in the aluminium industry all have several general drawbacks. Resistance-heated furnaces have high energy, investment, maintenance and operating costs, induction furnaces are energy-inefficient and have high maintenance and labour costs, while gas- or oil-fired furnaces generate noise and harmful combustion products and have low recovery rates. A new design, the Thermoblower furnace, has been proposed. The furnace uses as its heating source a unit in which hot air or inert gases may be quickly and efficiently heated to 1 000 degrees C in an extremely controlled manner. Release of the hot air or gas through nozzles can rapidly heat the furnace by forced convection. Introduction and Objectives Chronic obstructive pulmonary disease (COPD) is a complex inflammatory disease of the lungs Initiated by inhalation of toxic particles or gases. Periodic exacerbations triggered by respiratory pathogens are a major cause of morbidity/mortality in these patients. Microbial pathogens are recognised by pattern recognition receptors such as the toll-like receptors (TLRs), initiating innate immune defences. We hypothesised that abnormal TLR expression, and not resident inflammatory cell load in the lung parenchyma, contributes to exacerbation in COPD. Methods Human lung tissue, distant from tumour margins, was taken fromex-smoker patients undergoing lobectomy for lung cancer. Patients were classified according to GOLD guidelines as healthy control subjects (HC) or those with COPD. Resected tissue was digested and cells analysed by flowcytometry for phenotypicmarkers of epithelial cells and inflammatory cell subtypes (macrophages, CD8 + and CD8- T lymphocytes) and the TLR2 and four expressions on these subtypes. Quantitative data of cell numbers and TLR staining intensity were compared using Mann-Whitney U tests. Results Seven COPD patients and nine age-matched HC were analysed. No significant differences in the numbers of inflammatory or epithelial cells in the parenchymal tissue of these groups were observed, although a trend was observed to a reduction in macrophage numbers in the COPD group (median HC=4.2, median COPD=3.2 p=0.17). Similarly, no significant difference was found in the level ofTLR2 or TLR4 expression on any of the cell types examined. However, a trend was observed towards a decrease in TLR2 expression on epithelial cells in the COPD patients (median sMFI 3399 (HC) vs 2462 (COPD), p=0.094). Conclusions This preliminary analysis has demonstrated that, as hypothesised, there was no significant difference in inflammatory cell load in parenchymal tissue between the two groups. The trend towards a reduced expression of TLR2 in the epithelial cells may reflect an abnormal down regulation of this receptor due to constant exposure to bacterial pathogens. The lack of surveillance of microbial pathogens by TLRs is a potential mechanism by which patients with COPD are more susceptible to infection by new bacterial strains and thus could contribute to exacerbation frequency. This paper contributes to the field by addressing the critical issue of enhancing the spatial and temporal resolution of health data. Although Bayesian methods are frequently employed to address this challenge in various disciplines, the application of Bayesian spatio-temporal models to burden of disease (BOD) studies remains limited. Our novelty lies in the exploration of two existing Bayesian models that we show to be applicable to a wide range of BOD data, including mortality and prevalence, thereby providing evidence to support the adoption of Bayesian modeling in full BOD studies in the future. We illustrate the benefits of Bayesian modeling with an Australian case study involving asthma and coronary heart disease. Mosquito-borne disease is a significant public health issue and within Australia Ross River virus (RRV) is the most reported. This study combines a mechanistic model of mosquito development for two mosquito vectors; Aedes vigilax and Aedes camptorhynchus, with climate projections from three climate models for two Representative Concentration Pathways (RCPs), to examine the possible effects of climate change and sea-level rise on a temperate tidal saltmarsh habitat in Perth, Western Australia. The projections were run under no accretion and accretion scenarios using a known mosquito habitat as a case study. This improves our understanding of the possible implications of sea-level rise, accretion and climate change for mosquito control programmes for similar habitats across temperate tidal areas found in Southwest Western Australia. The output of the model indicate that the proportion of the year mosquitoes are active increases. Population abundances of the two Aedes species increase markedly. The main drivers of changes in mosquito population abundances are increases in the frequency of inundation of the tidal wetland and size of the area inundated, increased minimum water temperature, and decreased daily temperature fluctuations as water depth increases due to sea level changes, particularly under the model with no accretion. The effects on mosquito populations are more marked for RCP 8.5 when compared to RCP 4.5 but were consistent among the three climate change models. The results indicate that Ae. vigilax is likely to be the most abundant species in 2030 and 2050, but that by 2070 Aedes camptorhynchus may become the more abundant species. This increase would put considerable pressure on existing mosquito control programmes and increase the risk of mosquito-borne disease and nuisance biting to the local community, and planning to mitigate these potential impacts should commence now.

Severe acute respiratory syndrome coronavirus 2 (SARS-Cov-2) has had a tremendous impact on humanity. Prevention of transmission by disinfection of surfaces and aerosols through a chemical-free method is highly desirable. Ultraviolet C (UVC) light is uniquely positioned to achieve inactivation of pathogens. We report the inactivation of SARS-CoV-2 virus by UVC radiation and explore its mechanisms. A dose of 50 mJ/cm
2 using a UVC laser at 266 nm achieved an inactivation efficiency of 99.89%, while infectious virions were undetectable at 75 mJ/cm2 indicating >99.99% inactivation. Infection by SARS-CoV-2 involves viral entry mediated by the spike glycoprotein (S), and viral reproduction, reliant on translation of its genome. We demonstrate that UVC radiation damages ribonucleic acid (RNA) and provide in-depth characterization of UVC-induced damage of the S protein. We find that UVC severely impacts SARS-CoV- 2 spike protein's ability to bind human angiotensin-converting enzyme 2 (hACE2) and this correlates with loss of native protein conformation and aromatic amino acid integrity. 48. A Multi-Species Simulation of Mosquito Disease Vector Development in Temperate Australian Tidal Wetlands Using Publicly Available Data.

Worldwide, mosquito monitoring and control programs consume large amounts of resources in the effort to minimise mosquito-borne disease incidence. On-site larval monitoring is highly effective but time consuming. A number of mechanistic models of mosquito development have been developed to reduce the reliance on larval monitoring, but none for Ross River virus, the most commonly occurring mosquito-borne disease in Australia. This research modifies existing mechanistic models for malaria vectors and applies it to a wetland field site in Southwest, Western Australia. Environmental monitoring data were applied to an enzyme kinetic model of larval mosquito development to simulate timing of adult emergence and relative population abundance of three mosquito vectors of the Ross River virus for the period of 2018-2020. The model results were compared with field measured adult mosquitoes trapped using carbon dioxide light traps. The model showed different patterns of emergence for the three mosquito species, capturing inter-seasonal and inter-year variation, and correlated well with field adult trapping data. The model provides a useful tool to investigate the effects of different weather and environmental variables on larval and adult mosquito development and can be used to investigate the possible effects of changes to short-term and long-term sea level and climate changes.

While there is wide consensus regarding the importance of early intervention, health is rarely considered within priorities. Twenty-five children on the autism spectrum (M
age = 4.67, SD = 0.82) participated in a 12-week physical activity intervention. Primary objective was to examine impact of a physical activity intervention on physical activity, fitness and motor competence. Secondary objective was to examine associations between motor behavior and ASD symptoms. Ball skills (p < .001) and isometric push-up performance (p = .02) improved. Autism symptoms were associated with motor skills (r > - .49, p < .05). Study outcomes provide new knowledge regarding design, delivery, and measures for early interventions targeting health disparities in young children on the autism spectrum., (© 2021. Background: Allogeneic hematopoietic cell transplantation (allo-HCT) remains a curative option for patients with high-risk myeloid malignancies., Procedure: We present our 10-year experience (October 2012 to October 2021) of consecutive allo-HCT in patients with myeloid malignancies treated on the pediatric HCT service and conditioned with myeloablative targeted dose-busulfan (BU), fludarabine (FLU), and melphalan (MEL). Twenty-three children, adolescents, and young adult patients (CAYA) (median age 15.4 years) with acute myeloid leukemia (AML, n = 17), myelodysplastic syndrome (MDS, n = 4), or chronic myeloid leukemia (CML, n = 2) underwent allo-HCT post-BU-FLU-MEL. Four patients had treatment-related AML/MDS. Donor/stem cell source was matched sibling donor (MSD) PBSC (n = 7), matched unrelated donor (MUD) PBSC (n = 2), umbilical cord blood (UCB) (n = 3), or haploidentical-BMT (n = 11). Risk stratification was low (n = 2), intermediate (n = 15), high (n = 3), and very high risk (n = 1). The two patients with CML had failed tyrosine kinase inhibitor therapies., Results: With a median follow-up of 41.6 months, the relapse rate is only 4.5% with an overall survival (OS) 100%, progression-free survival (PFS) 95.5%, and graft-versus-host-free-relapse-free survival (GRFS) 67.8%. The donor source and the acute graft-versus-host disease (GvHD) prophylaxis regimen significantly impacted grade II-IV aGvHD 66.7% versus 19.2% (p = .039) and chronic graft-versus-host-disease (cGvHD) 66.7% versus 0% (p = .002) in the patients receiving MSD or MUD PBSC compared to haplo-BMT, respectively, resulting in improved GRFS in haplo-BMT, 83.3% compared to 40% matched donor peripheral blood stem cell transplant (PBSCT) (p = .025)., Conclusions: Our results demonstrate that BU-FLU-MEL is efficacious conditioning for disease control in young patients with myeloid malignancies undergoing MSD or alternative donor allo-HCT, but in the setting of PBSC grafts with cyclosporine A-methotrexate (CSA-MTX) GvHD prophylaxis, it results in an unacceptably high incidence of GvHD., (© 2022 Wiley Periodicals LLC.)
