Abstract

This book presents a state-of-the-art, robust, and adaptable process, the Theory of Being, that offers strategies for working across Difference, and for embarking on constructive dialogue around the issues that drive us apart, both individually and collectively. Whether around racial, gender, and/or social class inequity, core beliefs, uses of power or other points of cultural conflict, this book offers a research-validated approach, developed and refined over twenty years, to engage in difficult dialogues. "The Theory of Being" includes personal, relational, and community practices that support individuals and communities to better work through the difficult dialogues necessary to transform systems of structural inequity. It describes and offers applications of Being to help the reader understand and apply principles and practices that invite openness to controversy through facilitating deep reflection and shifting the focus of conflict from individuals to centering the issue of contention as a Third Thing about which participants can more safely express experiences and emotions. Via cases and narratives, the editors and contributors demonstrate how, through productively situating feelings of vulnerability and anger, individuals, organizations, and communities can work together to continuously evolve responsive, inclusive, and equitable practices that value social and cultural differences. This book focuses on strategies for the "how" we interact, demonstrating an orientation to process rather than prioritizing outcomes. A process-orientation can increase the quality of interaction between individuals, and the likelihood of traversing problems associated with controversial social difference in ways that result in sustainable strategies to disrupt systems of oppression. A range of applications exemplify this approach throughout the text. The primary audience is higher education leaders and leaders-in-training including student affairs professional staff, campus administrators, higher education and student affairs faculty, and undergraduate and graduate students. Abstract

Background: Hyperinsulinism (HI) due to dysregulation of pancreatic beta-cell insulin secretion is the most common and most severe cause of persistent hypoglycemia in infants and children. In the 65 years since HI in children was first described, there has been a dramatic advancement in the diagnostic tools available, including new genetic techniques and novel radiologic imaging for focal HI; however, there have been almost no new therapeutic modalities since the development of diazoxide. Summary: Recent advances in neonatal research and genetics have improved our understanding of the pathophysiology of both transient and persistent forms of neonatal hyperinsulinism. Rapid turnaround of genetic test results combined with advanced radiologic imaging can permit identification and localization of surgically-curable focal lesions in a large proportion of children with congenital forms of HI, but are only available in certain centers in "developed" countries. Diazoxide, the only drug currently approved for treating HI, was recently designated as an "essential medicine" by the World Health Organization but has been approved in only 16% of Latin American countries and remains unavailable in many under-developed areas of the world. Novel treatments for HI are emerging, but they await completion of safety and efficacy trials before being considered for clinical use. Key Messages: This international consensus statement on diagnosis and management of HI was developed in order to assist specialists, general pediatricians, and neonatologists in early recognition and treatment of HI with the ultimate aim of reducing the prevalence of brain injury caused by hypoglycemia. A previous statement on diagnosis and management of HI in Japan was published in 2017. The current document provides an updated guideline for management of infants and children with HI and includes potential accommodations for less-developed regions of the world where resources may be limited. Abstract

The importance of glucokinase (GK) in the regulation of insulin secretion has been highlighted by the phenotypes of individuals with activating and inactivating mutations in the glucokinase gene (GCK). Here we report 10 individuals with congenital hyperinsulinism (HI) caused by eight unique activating mutations of GCK. Six are novel and located near previously identified activating mutations sites. The first recognized episode of hypoglycemia in these patients occurred between birth and 24 years, and the severity of the phenotype was also variable. Mutant enzymes were expressed and purified for enzyme kinetics in vitro. Mutant enzymes had low glucose half-saturation concentration values and an increased enzyme activity index compared with wild-type GK. We performed functional evaluation of islets from the pancreata of three children with GCK-HI who required pancreatectomy. Basal insulin secretion in perifused GCK-HI islets was normal, and the response to glyburide was preserved. However, the threshold for glucose-stimulated insulin secretion in perifused glucokinase hyperinsulinism (GCK-HI) islets was decreased, and glucagon secretion was greatly suppressed. Our evaluation of novel GCK disease-associated mutations revealed that the detrimental effects of these mutations on glucose homeostasis can be attributed not only to a lowering of the glucose threshold of insulin secretion but also to a decreased counterregulatory glucagon secretory response. 24. Characterisation of chicken RXR-γ

Abstract

The clone pR2 was isolated from a chick cDNA library and its insert was sequenced. The predicted amino acid sequence, encoded by the insert, was analysed and the protein was found to be a new member of the steroid/thyroid receptor superfamily. Further analysis revealed that it was a member of a newly discovered class of retinoid receptors, the retinoid X receptors (RXRs). By comparing the amino acid identities with other RXRs isolated from the mouse it was confirmed that our clone encoded chicken RXR-γ (cRXR-γ). At this time the ligand for the RXRs was unknown. All-trans retinoic acid had been shown to activate members of the RXR family but this was considered to represent a spurious cross-reaction. In order to identify the nature of the ligand for RXRs, co-transfections were carried out in Baby Hamster Kidney cells with a construct encoding cRXR-γ and a reporter construct consisting of the chloramphenicol acetyl transferase (CAT) gene under the control of a promoter containing a response element which will bind to the RXRs. Using this system a number of retinoids were tested for their ability to activate the reporter. Of all the retinoids tested, none were found to bring about reporter activation at physiological levels. It was later reported that an isomer of retinoic acid, 9-cis retinoic acid, was the ligand for the RXRs. Structural similarities between 9-cis retinoic acid and the eicosanoids raised the possibility that eicosanoids may also activate RXRs in vivo. Using a similar transfection system it was demonstrated that lipoxygenase metabolites of arachidonic acid (the precursor of eicosanoids) are capable of activating cRXR-γ at submicromolar concentrations. Consistent with this, molecular modelling predicts conformational similarities between some lipoxygenase products and 9-cis retinoic acid. This may explain why certain eicosanoids (the hydroxyeicosatetraenoic acids) mimic some of the actions of retinoids in cell-based assays. As a preliminary step in the analysis of the function of cRXR-γ, a system was set up to block the effect of the receptor by co-expression of a dominant negative mutant. QT6 cells were co-transfected with an expression construct for cRXR-γ and for the ligand binding domain of cRXR-γ. It was shown that by co-expressing this dominant negative mutant, cRXR-γ mediated transactivation was inhibited. A genomic clone was isolated for cRXR-γ and partially sequenced. This allowed the identification of an exon/intron boundary within the cDNA. From this genomic clone a region of 5'-sequence was subcloned into a CAT construct and transfected into QT6 cells. This showed that the sequence contained promoter activity. In summary, these results have lead to the identification of a novel clone as encoding a member of the RXRs. The activation of the receptor by a number of retinoid and non-retinoid compounds has been determined and a genomic clone isolated leading to the discovery of a region containing promoter activity.
