Abstract

The binding of tritiated [beta]-endorphin (3H-[beta]-EP) to brain homogenates is described. This had been difficult to achieve due to the lack of availability of 3H-[beta]-EP and to technical difficulties associated with high non-specific binding of [beta]-EP. We now report that 3H-[beta]-EP binding is saturable, stereospecific, has high affinity and is inhibited by sodium. Its dissociation rate is ten-fold longer than that of naloxone. Its regional distribution exhibits interesting differences from naloxone and enkephalin binding. ACTH1-24 appears to displace it more effectively than it displaces 3H-naloxone. The results are discussed in terms of multiple transmitter systems and the multiple opiate receptor hypothesis.

Мета. Мета полягала в тому, щоб сформувати настанови з клінічної практики діагностики і лікування акромегалії.Автори. До складу робочої групи ввійшли голова, обраний Підкомітетом з клінічних настанов (ПКН) Ендокринологічного товариства, п’ять експертів у галузі та методолог. Автори не отримували жодного корпоративного фінансування або винагороди. Розробка цих настанов частково фінансувалася Європейським товариством ендокринологів.Доказові дані. Ці настанови, засновані на принципах доказової медицини, розроблено з використанням системи розробки, оцінки й експертизи рекомендацій (Grading of Recommendations, Assessment, Development and Evaluation) (GRADE), яка характеризує як ступінь переконливості рекомендацій, так і якість доказових даних. Робоча група проаналізувала основні доказові дані й ініціювала підготовку двох додаткових систематичних оглядів.Процес досягнення консенсусу. Процес досягнення консенсусу включав одне засідання групи, кілька телеконференцій та листування електронною поштою. Проекти настанов переглянуто комітетами й членами Ендокринологічного товариства і Європейського товариства ендокринологів.Висновки. Використовуючи підхід, заснований на принципах доказової медицини, ці настанови розглядають важливі клінічні питання, що стосуються обстеження та ведення пацієнтів з акромегалією, в тому числі належних біохімічних аналізів, розробки алгоритмів лікування (застосування медикаментозної монотерапії або комбінованої терапії) і ведення пацієнтів під час вагітності. Abstract

The hypothalamus regulates numerous autonomic responses and behaviors. The neuroactive substances corticotropin-releasing factor (CRF), arginine-vasopressin (AVP), histidine decarboxylase (HDC), melanin-concentrating hormone (MCH), and orexin/hypocretins (ORX) produced in the hypothalamus mediate a subset of these processes. Although the expression patterns of these genes have been well studied in rodents, less is known about them in humans. We combined classical histological techniques with in situ hybridization histochemistry to produce both 2D and 3D images and to visually align and quantify expression of the genes for these substances in nuclei of the human hypothalamus. The hypothalamus was arbitrarily divided into rostral, intermediate, and caudal regions. The rostral region, containing the paraventricular nucleus (PVN), was defined by discrete localization of CRF- and AVP-expressing neurons, whereas distinct relationships between HDC, MCH, and ORX mRNA-expressing neurons delineated specific levels within the intermediate and caudal regions. Quantitative mRNA signal intensity measurements revealed no significant differences in overall CRF or AVP expression at any rostrocaudal level of the PVN. HDC mRNA expression was highest at the level of the premammillary area, which included the dorsomedial and tuberomammillary nuclei as well as the dorsolateral hypothalamic area. In addition, the overall intensity of hybridization signal exhibited by both MCH and ORX mRNA-expressing neurons peaked in distinct intermediate and caudal hypothalamic regions. These results suggest that human hypothalamic neurons involved in the regulation of the HPA axis display distinct neurochemical patterns that may encompass multiple local nuclei. J. Comp. Abstract

Drugs known to inhibit phenylethanolamine-N-methyltransferase (EC 2.1.1.28), the final enzyme in the epinephrine synthetic pathway were administered to rats and their effects on pituitary-arenal function studied. 2, 3-Dichloro-[alpha]-methylbenzylamine (DCMB) produced dose-related increases in plasma corticosterone in basal and stressed rats. Evidence for the central nature of this tonic inhibitory effect of epinephrine was its continued presence in adrenal demedullated rats. 2-chloro-3-trifluoromethyl-[alpha]-benzylamine (CTFMB) administration elevated plasma corticosterone and this effect was highly correlated to the decrease in hypothalamic epinephrine concentration in both sham operated and adrenal demedullated rats. These findings argue for tonic epinergic inhibition of pituitary-adrenal function.

Abstract

Ductal carcinoma in situ; Tumor microenvironment; Whole genome sequencing Carcinoma ductal in situ; Microambiente tumoral; Secuenciación del genoma completo Carcinoma ductal in situ; Microambient tumoral; Seqüenciació del genoma complet Ductal carcinoma in situ (DCIS) is the most common precursor of invasive breast cancer (IBC), with variable propensity for progression. We perform multiscale, integrated molecular profiling of DCIS with clinical outcomes by analyzing 774 DCIS samples from 542 patients with 7.3 years median follow-up from the Translational Breast Cancer Research Consortium 038 study and the Resource of Archival Breast Tissue cohorts. We identify 812 genes associated with ipsilateral recurrence within 5 years from treatment and develop a classifier that predicts DCIS or IBC recurrence in both cohorts. Pathways associated with recurrence include proliferation, immune response, and metabolism. Distinct stromal expression patterns and immune cell compositions are identified. Our multiscale approach employed in situ methods to generate a spatially resolved atlas of breast precancers, where complementary modalities can be directly compared and correlated with conventional pathology findings, disease states, and clinical outcome. Abstract

In this study, we aimed to modify the immune response in the long term after allogeneic bone marrow transplantation (allo-BMT) by using the proteasome inhibitor ixazomib (IXZ) at the late stages of the post-transplant period. This approach facilitated the immune reconstitution after transplantation. IXZ significantly prolonged survival and decreased the risk of chronic graft-versus-host disease (cGvHD) in two different murine models without hampering the graft-versus-leukemia (GvL) effect, as confirmed by bioluminescence assays. Remarkably, the use of IXZ was related to an increase of regulatory T cells both in peripheral blood and in the GvHD target organs and a decrease of effector donor T cells. Regarding B cells, IXZ treated mice had faster recovery of B cells in PB and of pre-pro-B cells in the bone marrow. Mice receiving ixazomib had a lower number of neutrophils in the GvHD target organs as compared to the vehicle group. Abstract

Breast cancer; Cancer Càncer de mama; Càncer Cáncer de mama; Cáncer The METRIC study (NCT#0199733) explored a novel antibody–drug conjugate, glembatumumab vedotin (GV), targeting gpNMB that is overexpressed in ~40% of patients with triple-negative breast cancer (TNBC) and associated with poor prognosis. The study was a randomized, open-label, phase 2b study that evaluated progression-free survival (PFS) of GV compared with capecitabine in gpNMB-overexpressing TNBC. Patients who had previously received anthracycline and taxane-based therapy were randomized 2:1 to receive, GV (1.88 mg/kg IV q21 days) or capecitabine (2500 mg/m2 PO daily d1–14 q21 days). The primary endpoint was RECIST 1.1 PFS per independent, blinded central review. In all, 327 patients were randomized to GV (213 treated) or capecitabine (92 treated). Median PFS was 2.9 months for GV vs. 2.8 months for capecitabine. The most common grade ≥3 toxicities for GV were neutropenia, rash, and leukopenia, and for capecitabine were fatigue, diarrhea, and palmar-plantar erythrodysesthesia. The study did not meet the primary endpoint of improved PFS over capecitabine or demonstrate a relative risk/benefit improvement over capecitabine. Funding provided by Celldex Therapeutics, Inc.

Abstract

Circulating tumour DNA (ctDNA); Liquid biopsy; Precision medicine DNA tumoral circulant (ctDNA); Biòpsia líquida; Medicina de precisió ADN tumoral circulante (ctDNA); Biopsia líquida; Medicina de precisión Circulating tumour DNA (ctDNA) assays conducted on plasma are rapidly developing a strong evidence base for use in patients with cancer. The European Society for Medical Oncology convened an expert working group to review the analytical and clinical validity and utility of ctDNA assays. For patients with advanced cancer, validated and adequately sensitive ctDNA assays have utility in identifying actionable mutations to direct targeted therapy, and may be used in routine clinical practice, provided the limitations of the assays are taken into account. Tissue-based testing remains the preferred test for many cancer patients, due to limitations of ctDNA assays detecting fusion events and copy number changes, although ctDNA assays may be routinely used when faster results will be clinically important, or when tissue biopsies are not possible or inappropriate. Reflex tumour testing should be considered following a non-informative ctDNA result, due to false-negative results with ctDNA testing. In patients treated for early-stage cancers, detection of molecular residual disease or molecular relapse, has high evidence of clinical validity in anticipating future relapse in many cancers. Molecular residual disease/molecular relapse detection cannot be recommended in routine clinical practice, as currently there is no evidence for clinical utility in directing treatment. Additional potential applications of ctDNA assays, under research development and not recommended for routine practice, include identifying patients not responding to therapy with early dynamic changes in ctDNA levels, monitoring therapy for the development of resistance mutations before clinical progression, and in screening asymptomatic people for cancer. Recommendations for reporting of results, future development of ctDNA assays and future clinical research are made.
