- Abstract
Going through the different phases of the activity of the clinical laboratory, the need for harmonization and standardization are examined and the activities directed to improve the situation are proposed. Regarding the analytical phase the progress in standardization requires an amelioration of the introduction in the daily practice the concept of metrological traceability and measurement uncertainty. Also, a clear definition of the analytical performance specifications and their use within the various aspects of the activity (internal quality control, method verification and development, external quality assessment) are a requirement to define common targets and harmonized level of analytical quality. The preanalytical phase harmonization requires intervention in standardizing the blood drawing and especially the indications for the storage and transportation of the different types of samples. In the post-analytical phase harmonization initiatives involve the standardized description of the measurands, the form of the report, the unity of measurement and the reference intervals. For the units of measurement and the reference intervals a series of minimal requirements are presented: elimination of /mL and mEq/L, use of g/L or mg/L for proteins; need for sex and age-related intervals, clear specifications for decision limits.
