Abstract

Tremendous developments in energy storage and conversion technologies urges researchers to develop inexpensive, greatly efficient, durable and metal-free electrocatalysts for tri-functional electrochemical reactions, namely oxygen reduction reactions (ORRs), oxygen evolution reactions (OERs) and hydrogen evolution reactions (HERs). In these regards, this present study focuses upon the synthesis of porous carbon (PC) or N-doped porous carbon (N-PC) acquired from golden shower pods biomass (GSB) via solvent-free synthesis. Raman spectroscopy, X-ray diffraction (XRD) and X-ray photoelectron spectroscopy (XPS) studies confirmed the doping of nitrogen in N-PC. In addition, morphological analysis via field emission scanning electron microscopy (FESEM) and transmission electron microscopy (TEM) provide evidence of the sheet-like porous structure of N-PC. ORR results from N-PC show the four-electron pathway (average n = 3.6) for ORRs with a Tafel slope of 86 mV dec−1 and a half-wave potential of 0.76 V. For OERs and HERs, N-PC@Ni shows better overpotential values of 314 and 179 mV at 10 mA cm−2, and its corresponding Tafel slopes are 132 and 98 mV dec−1, respectively. The chronopotentiometry curve of N-PC@Ni reveals better stability toward OER and HER at 50 mA cm−2 for 8 h. These consequences provide new pathways to fabricate efficient electrocatalysts of metal-free heteroatom-doped porous carbon from bio-waste/biomass for energy application in water splitting and metal air batteries.

Abstract

Background and objectives: Documentation is important for quality improvement, education, and research. There is currently a lack of recommendations regarding key aspects of documentation in regional anesthesia. The aim of this study was to establish recommendations for documentation in regional anesthesia. Methods: Following the formation of the executive committee and a directed literature review, a long list of potential documentation components was created. A modified Delphi process was then employed to achieve consensus amongst a group of international experts in regional anesthesia. This consisted of 2 rounds of anonymous electronic voting and a final virtual round table discussion with live polling on items not yet excluded or accepted from previous rounds. Progression or exclusion of potential components through the rounds was based on the achievement of strong consensus. Strong consensus was defined as ≥75% agreement and weak consensus as 50%-74% agreement. Results: Seventy-seven collaborators participated in both rounds 1 and 2, while 50 collaborators took part in round 3. In total, experts voted on 83 items and achieved a strong consensus on 51 items, weak consensus on 3 and rejected 29. Conclusion: By means of a modified Delphi process, we have established expert consensus on documentation in regional anesthesia.

We hypothesize that a TQL block, when compared to fascia iliaca block (FIB), will provide better analgesia and less motor block in the initial 24-h postoperative period.Patients and Methods: Fifty patients undergoing elective hip replacement surgery, ASA I–III, were included in the study. Patients were randomized into two groups. Patients in group A received spinal anesthesia followed by FIB. Patients in group B received spinal anesthesia followed by TQLB. Postoperative pain scores and motor block were assessed at 6 and 24 hours. The primary outcome measure was 24 h total morphine consumption. Secondary outcome measures included pain score (VNS) and motor block (modified Bromage scale) at 6 and 24 h postoperatively.Results: There was no statistical difference in morphine consumption between the two groups (p-value 0.699). There was no difference in pain scores at 6 h (p-value 0.540) or 24 h (p-value 0.383). There was no difference in motor block at 6 h (p-value 0.497) or at 24 h (p-value 0.773).Conclusion: Transmuscular quadratus lumborum block along with spinal anesthesia for patients undergoing elective hip replacement surgery does not reduce opioid consumption or motor weakness when compared to fascia iliaca block. The results and conclusion apply to a dose of 20 mL of 0.25% bupivacaine used in each group.Keywords: pain scores, motor block, opioid consumption

20. Biofuel production from novel Prunus domesticakernel oil: process optimization technique

Abstract

Biodiesel obtained from low-cost non-edible oils is the most promising alternative fuel for conventional diesel fuel. In this current work, Prunus domesticakernel oil was used as a feedstock for synthesizing methyl ester. The FFA value of the kernel oil was examined by employing the isopropyl alcohol technique and found to be 11.63 mg KOH/g. Hence, oil was processed with a two-step transesterification process using acid (H2SO4) and base (NaOH) catalyst reaction to convert into biodiesel. To obtain maximum yield and high-quality biodiesel, an optimization technique was employed. In this technique, the process parameters such as experimental duration, reaction temperature, NaOH concentration, and methanol to oil ratio were optimized based on Taguchi technique. The investigations revealed that 150 min, 60°C, 8:1 ratio, and 1 wt% NaOH were optimal process parameters obtained with a reaction efficiency of 97.86%. The experimental analysis considered that the methanol to oil ratio was happened to be the most substantial entity using ANOVA. The biodiesel Prunus domesticamethyl ester produced matched and fulfilled the standards EN14214.

Abstract

BACKGROUND: Magnesium has been implicated in the pathogenesis of acute myocardial infarction and its complication like arrhythmia. Magnesium improves myocardial metabolism, inhibits calcium accumulation and myocardial cell death. It improves vascular tone, peripheral vascular resistance, after load and cardiac output, reduces cardiac arrhythmias and improves lipid metabolism. Magnesium also reduces vulnerability to oxygen derived free radicals, improves endothelial function and inhibits platelet function including platelet aggregation and adhesion. OBJECTIVE: To know the relationship between the serum magnesium levels and arrhythmias in patients with acute myocardial infarction. METHOD: By using simple random method, 50 cases of acute myocardial infarction, admitted in Coimbatore Medical College and Hospital. RESULTS: There is a significant difference in the magnesium levels in patients with arrhythmias and without arrhythmias. CONCLUSION: In acute myocardial infarction, patients with low magnesium levels are more prone to get arrhythmias. So magnesium treatment can be considered in patients of acute myocardial infarction with low magnesium levels.

Abstract

This study is aimed at analyzing the clinical symptomatology and hematological evaluation with an emphasis on platelet indices in relation to predicting the outcome of the febrile thrombocytopenic patients admitted in Coimbatore medical college hospital..This is a prospective study involving 100 adult patients who presented to our hospital with fever and thrombocytopenia (platelet1,50,000). This study excluded patients with known causes of thrombocytopenia like ITP and patients on chemotherapy etc.Out of 100 patients 34 were dengue positive, 66 were dengue negative. Dengue specific symptoms like myalgia and retro-orbital pain were present in 58.88% of dengue positive and 10.60% of dengue negative patients. Laboratory evaluation revealed sharp rise in hematocrit with fall in platelet count in both the groups more significant in dengue positive group. Bleeding manifestation and rashes were 29.4% and 26.4% in dengue positive, 12.12% and 7.57% in dengue negative group respectively. MPV was significantly lower in patients with bleeding manifestations irrespective of platelet count in both the groups. Mortality in our study was 2%.MPV is an independent predictor of bleeding manifestation and poor outcome. Dengue virus may suppress the bone marrow as evidenced by alteration in MPV in addition to other mechanisms of thrombocytopenia.

Abstract

Abstract Background Production or consumption of protons in growth medium during microbial metabolism plays an important role in determining the pH of the environment. Such pH changes resulting from microbial metabolism may influence the geochemical speciation of many elements in subsurface environments. Protons produced or consumed during microbial growth were measured by determining the amount of acid or base added in a 5 L batch bioreactor equipped with pH control for different species including Escherichia coli, Geobacter sulfurreducens, and Geobacter metallireducens. Results An in silico model was used to predict the proton secretion or consumption rates and the results were compared with the data. The data was found to confirm predictions of proton consumption during aerobic growth of E. coli with acetate as the carbon source. However, in contrast to proton consumption observed during aerobic growth of E. coli with acetate, proton secretion was observed during growth of Geobacter species with acetate as the donor and Fe(III) as the extracellular electron acceptor. Conclusions In this study, we have also shown that the final pH of the medium can be either acidic or basic depending on the choice of the electron acceptor for the same electron donor. In all cases, the in silico model could predict qualitatively the proton production/consumption rates obtained from the experimental data. Abstract

Abstract text BACKGROUND & AIMTransmuscular Quadratus lumborum (TQL) block has been described recently as an effective option for post-operative analgesia in patients undergoing total hip replacements with single injection providing analgesia for up to 24 hours. We hypothesise that a single injection TQL block when compared to single injection fascia iliaca block (FIB) will provide better analgesia and less motor block in the initial 24 hour period. METHODSEthics committee approval was taken and this prospective, semi-blind, randomised controlled trial was conducted in patients undergoing elective total hip replacement surgeries. Patients were randomised to either to group A wherein they received Spinal Anaesthesia with 3.2 ml 0.5% bupivacaine followed by usg guided FIB with 20 ml 0.25% bupivacaine or to group B wherein they received Spinal Anaesthesia with 3.2 ml 0.5% bupivacaine followed by usg guided QLB with 20 ml 0.25% bupivacaine. All patients received morphine PCA in post-operative period and were reviewed at 6 hours and 24 hours to assess the pain scores and motor block.The Primary Outcome Measure was 24 hour morphine consumption between the two groups and secondary outcome measure were Pain Score (VNS) and Motor block (Modified Bromage Scale) at 6 and 24 hours. RESULTSThere was no statistical difference in Morphine consumption between the 2 groups (P 0.699). There was also no difference in Pain scores at 6 hours (P 0.540) and 24 hours (P 0.383). There was no difference noted in Motor block at 6 hours (P 0.497) and at 24 hours (P 0.773)CONCLUSIONThe result of this study demonstrates that TQL doesnu2019t provide superior analgesia or less motor block then FIB in patients undergoing elective hip replacement surgery.ABSTRACT TOPICPERIPHERAL NERVE BLOCKS

Abstract

The objective of this thesis was to enhance operator performance to improve efficiency, safety and efficacy of neuraxial blocks. Methodology Efficiency In study 1 and 1a, a metric based assessment tool for labour epidural catheter placement was developed and validated. The effect of proficiency based progression training (PBP) in provision of labour epidural analgesia was then studied. Safety Study 2 was undertaken to improve the accuracy of palpated landmarks for administering spinal anaesthesia in patients undergoing caesarean section. Efficacy In study 3, clinically relevant association and correlation(s) between i. ultrasound images and ii. Magnetic Resonance Imaging (MRI) of lumbar spine was studied. In study 4a and 4b, landmark-guided midline approach was compared to pre-procedure ultrasound guided paramedian techniques in spinal anaesthesia. Results PBP (study 1, 1a, n=17) group had a significantly (p= 0.04) lower failure rate (13.3%) compared to simulation only group (28.7%). Abstract

Background Training procedural skills using proficiencybased progression (PBP) methodology has consistently resulted in error reduction. We hypothesised that implementation of metric-based PBP training and a valid assessment tool would decrease the failure rate of epidural analgesia during labour when compared to standard simulation-based training. Methods Detailed, procedure-specific metrics for labour epidural catheter placement were developed based on carefully elicited expert input. Proficiency was defined using criteria derived from clinical performance of experienced practitioners. A PBP curriculum was developed to train medical personnel on these specific metrics and to eliminate errors in a simulation environment. Seventeen novice anaesthetic trainees were randomly allocated to undergo PBP training (Group P) or simulation only training (Group S). Following training, data from the first 10 labour epidurals performed by each participant were recorded. The primary outcome measure was epidural failure rate. Results A total of 74 metrics were developed and validated. The inter-rater reliability (IRR) of the derived assessment tool was 0.88. Of 17 trainees recruited, eight were randomly allocated to group S and six to group P (three trainees did not complete the study). Data from 140 clinical procedures were collected. The incidence of epidural failure was reduced by 54% with PBP training (28.7% in Group S vs 13.3% in Group P, absolute risk reduction 15.4% with 95% CI 2% to 28.8%, p=0.04). Conclusion Procedure-specific metrics developed for labour epidural catheter placement discriminated the performance of experts and novices with an IRR of 0.88. Proficiency-based progression training resulted in a lower incidence of epidural failure compared to simulation only training. Trial registration number NCT02179879. NCT02185079; Post-results.
