The current economic scale is bigger and bigger, the social material living standard also along is also getting higher and higher with the rapid economic growth. However, the problems caused by economic development are also increasing, on the one hand, there is the contradiction between supply and demand caused by resource consumption and shortage of resources; on the other hand, there also is contradiction between the great pollution and destruction in the ecological environment and the public's increasingly demanding ecological environment. Especially, the contradiction between the ecological environment and the social environment has become the focus of attention of the Chinese public. Therefore, the ecological environment protection becomes the current consensus either from the national level or the social level, how to manage and protect the ecological environment is also a question of the current social thinking. The common practice of ecological environment protection is to control and protect, at the same time, governance is to restore the damaged environment. There are many means for protection, such as energy conservation, emission reduction, monitoring and so on. Energy saving and emission reduction not only mean that the consumption of resources is reduced, but also mean that the discharge of pollutants and destroy the ecological environment are reduced. And monitoring refers to the existing ecological environment monitoring; the change of ecological environment is detected by real-time observation, so that counter measures are made according to the changes.

Its use in preterm neonates is off-label. Considering the dearth of data, we carried out a retrospective study evaluating the furosemide use and its effects on the electrolyte abnormalities in critically ill neonates.Methods: Critically ill neonates receiving at least one dose of furosemide during their stay in the NICU were recruited. The following details were obtained: demographic characteristics and furosemide details (dose, frequency, route, and duration). Urine output, body weight, serum creatinine, electrolytes (sodium, potassium, calcium, bicarbonate, chloride, and magnesium) during furosemide therapy were extracted.Results: Ninety neonates were recruited. Elevated serum creatinine was observed in 21.1% of the patients, and electrolyte disturbances were observed in 52.2%. Those with electrolyte disturbances had significantly greater cumulative doses compared to those without [5.5 (1– 34) vs 3.9 (0.9– 30.2) mg/kg; p = 0.01]. Cumulative doses adjusted to body-weight were significantly lower in very preterm and extremely preterm neonates compared to late preterm category. A significant area-under-the-curve was observed for the cumulative dose (0.66; 95% CI: 0.54– 0.78; p = 0.01) in predicting the risk of electrolyte abnormalities with a cut-off value of 4 mg/kg. Eight neonates received more than 10 mg/kg cumulative dose of furosemide of which one died. No significant differences were observed between the proportion of neonates with electrolyte disturbances with cumulative furosemide dose above 10 mg/kg (p = 0.3) and with mortality (p = 0.3).Conclusion: We observed that our critically ill neonates received relatively higher cumulative doses of furosemide and electrolyte disturbances were observed in nearly half of the population. A cumulative dose of 4 mg/kg increases the risk of electrolyte abnormalities, particularly in preterm neonates. More diligence in the dose titration coupled with deprescribing and intense monitoring of all the potential adverse effects in this vulnerable population is needed.Keywords: furosemide, preterm neonates, off-label drug use

Distribution of black spot defects and small clusters in 1 MeV krypton irradiated 3C-SiC has been investigated using advanced scanning transmission electron microscopy (STEM) and TEM. We find that two thirds of clusters smaller than 1 nm identified in STEM are invisible in TEM images. For clusters that are larger than 1 nm, STEM and TEM results match very well. A cluster dynamics model has been developed for SiC to reveal processes that contribute to evolution of defect clusters and validated against the (S)TEM results. Simulations showed that a model based on established properties of point defects (PDs) generation, reaction, clustering, and cluster dissociation, is unable to predict black spot defects distribution consistent with STEM observations. This failure suggests that additional phenomena not included in a simple point-defect picture may contribute to radiation-induced evolution of defect clusters in SiC and using our model we have determined the effects of a number of these additional phenomena on cluster evolution. - Abstract
Background & aims: An estimated 38% of adults worldwide have non-alcoholic fatty liver disease (NAFLD). From individual impacts to widespread public health and economic consequences, the implications of this disease are profound. This study aimed to develop an aligned, prioritised fatty liver disease research agenda for the global health community. Methods: Nine co-chairs drafted initial research priorities, subsequently reviewed by 40 core authors and debated during a three-day in-person meeting. Following a Delphi methodology, over two rounds, a large panel (R1 n = 344, R2 n = 288) reviewed the priorities, via Qualtrics XM, indicating agreement using a four-point Likert-scale and providing written feedback. The core group revised the draft priorities between rounds. In R2, panellists also ranked the priorities within six domains: epidemiology, models of care, treatment and care, education and awareness, patient and community perspectives, and leadership and public health policy. Results: The consensus-built fatty liver disease research agenda encompasses 28 priorities. The mean percentage of ‘agree’ responses increased from 78.3 in R1 to 81.1 in R2. Five priorities received unanimous combined agreement (‘agree’ + ‘somewhat agree’); the remaining 23 priorities had >90% combined agreement. While all but one of the priorities exhibited at least a super-majority of agreement (>66.7% ‘agree’), 13 priorities had [removed]90% combined agreement. Conclusions: Adopting this multidisciplinary consensus-built research priorities agenda can deliver a step-change in addressing fatty liver disease, mitigating against its individual and societal harms and proactively altering its natural history through prevention, identification, treatment, and care. This agenda should catalyse the global health community's efforts to advance and accelerate responses to this widespread and fast-growing public health threat. Chlamydia trachomatis is one among the sexually transmitted diseases causing genital tract infection frequently associated with complications of infertility. The aim of our study is to detect the presence of C. trachomatis infection (CTI) in female infertility by nested Polymerase Chain Reaction (PCR) in a tertiary care center. A cross-sectional study was done with 230 infertile women attending the OPD of Reproductive Medicine and Surgery. CTI was detected among the study participants by screening for momp and cryptic plasmid gene using nested PCR. Based on the history and clinical presentation, the enrolled patients categorized as primary and secondary infertility. The results of the nested PCR for the primary and secondary infertile women were tabulated and compared for the statistical significance using Epi info version 7 and Chi-square test. A p-value of < 0.05 considered significant. In the study, participants 1 (20%) was primary and 4 (80%) belonged to secondary infertility. Of the 230 infertile women screened 2.2% of them had PCR positive for either momp or cryptic plasmid gene. CTI was seen more (80%) in secondary infertile than in primary infertile women. CTI was seen more in the age group 26-30 years (60%) followed by 21-25 years and 31-35 years (20%). The results of our study showed CTI is associated with infertility and recurrent spontaneous abortion. It's imperative to screen for CTI by molecular method in young females which necessitates early therapy and prevention of long term complications like infertility. [ABSTRACT FROM AUTHOR]

In today’s world, mining and learning applications play an essential role in healthcare sectors and intend to transform all the data into an understandable form. However, the healthcare sectors require an automated disease prediction system for better medical analysis and emphasize better prediction accuracy for evaluation purposes. In this paper, a new automated prediction model based on Linearly Support Vector Regression and Stacked Linear Swarm Optimization (LSVR-SLSO) has been proposed to predict heart disease accurately. Primarily, the features are analyzed in a linear and non-linear manner using LSVR feature learning approaches. The extracted features are then fed into the SLSO model in order to extract the global optimal solutions. These global solutions will reduce the data dimensionality and computational complexity during the evaluation phase. Moreover, the optimal solution facilitates the proposed model to predict heart disease appropriately. The simulation can be carried out through the MATLAB environment by utilizing a publicly available benchmark heart disease dataset. The performance results evident that the proposed LSVR-SLSO model can efficiently predict heart disease with superior accuracy of 98%, precision of 98.76%, and recall of 99.7% when compared with conventional approaches. The better performance of the proposed model will pave the way to act as an effective clinical decision support tool for physicians during an emergency.
