1. Thoracopelvic assisted movement training to improve gait and balance in elderly at risk of falling: a case series

This report describes the feasibility of using a new thoracopelvic assisted movement device to improve gait and balance in an elderly population with increased risk for falls. Methods: In this case series, 19 older adults were recruited from an assisted living facility. All had gait difficulties (gait speed MDC in their clinical measures after 6 treatment sessions, and more than half improved >MDC after 12 sessions. Six subjects (32%) improved their Timed Up and Go time by >4 seconds after 6 sessions, and 10 (53%) after 12 sessions. After the intervention, 4 subjects (21%) improved their 10-meter Walk Test velocity from limited community ambulation (0.4–0.8 m/s) to functional community ambulation (>0.8 m/s). Conclusion: Thoracopelvic assisted movement training that mimics normal walking pattern may have clinical implications, by improving skills that enhance balance and gait function. Additional randomized, controlled studies are required to examine the effects of this intervention on larger cohorts with a variety of subjects. Keywords: gait, balance, older adults, training

The purpose of this study was to test whether individuals with hemiparesis who have varied gait speeds, which place them in different functional categories, benefit to the same extent from the application of FES.Methods: Thirty-six subjects with chronic hemiparesis demonstrating foot-drop and deficits in knee and/or hip control were fitted with a dual-channel FES system activating the dorsiflexors and hamstring muscles. Gait was assessed during a 2-minute walk test with and without stimulation. A second assessment was conducted after 6 weeks of daily use. Analysis was performed with the subjects stratified into three functional ambulation classes according to their initial gait categories.Results: The dual-channel FES improved the gait velocity of all three subgroups. No minimal gait velocity was required in order to gain benefits from FES. For example, subjects with limited household ambulation capabilities improved their gait speed by 63.3% (from 0.30 ± 0.09 m/sec to 0.49 ± 0.20 m/sec; P < 0.01), while subjects with functional community ambulation capabilities improved their gait speed by 25.5% (from 0.90 ± 0.11 m/sec to 1.13 ± 0.22 m/sec; P < 0.01).Conclusion: Dual-channel FES positively affects gait velocity in patients with chronic hemiparesis, regardless of their initial gait velocity. Furthermore, gait velocity gains may be large enough to change an individual's ambulation status to a higher functional category.Keywords: hemiparesis, functional electrical stimulation, gait velocity, ambulation

Abstract

A method for the quantitative assessment of spatio-temporal structuring of brain activity is presented. This approach is employed in a longitudinal case study of a child with frontal lobe epilepsy (FLE) and tested against an age-matched control group. Several correlation measures that are sensitive to linear and/or non-linear relations in multichannel scalp EEG are combined with an hierarchical cluster algorithm. Beside a quantitative description of the overall degree of synchronization the spatial relations are investigated by means of the cluster characteristics. The chosen information measures not only demonstrate their suitability in the characterization of the ictal and interictal phases but they also follow the course of delayed recovery of the psychiatric symptomatology during successful medication. The results based on this single case study suggest testing this approach for quantitative control of therapy in an extended clinical trial.

13. Towards variance-matrix characterization of complementarity relations in a continuous variable system

We discuss complementarity relations in a bipartite continuous variable system. Building up from the work done on discrete d-dimensional systems, we prove that for symmetric two-mode states, quantum complementarity relations can be put in a simple relation with the elements of the variance matrix. When this condition is not satisfied, such a connection becomes non-trivial. Abstract

Mathematical models for ships’ consumption are in a central role in assessing the CO₂ emissions of marine traffic. Moreover, such models are needed when optimizing the ways the vessels are operated (e.g. routing). Nowadays, many ships are equipped with data collection systems, enabling data-based calibration of the models. Typically this calibration is done independently for each ship. In this paper, we demonstrate a hierarchical Bayesian approach, where we fit a single model over many vessels, with the assumption that the parameters of vessels of similar characteristics are likely close to each other. The benefits of such an approach are two-fold; (1) we can borrow information about parameters that are not well informed by the vessel-specific data using data from similar ships, and (2) we can use the hierarchical model to predict the behavior of a vessel from which we have no data, based only on its characteristics. In this paper, we discuss the basic concept and present a simple version of the model using cruise vessels. We apply the Stan modeling tool for the fitting and use real data from 64 ships collected via the commercial Eniram platform. The prediction accuracy of the model is compared to an existing data-free method. We demonstrate that the accuracy of such an approach can improve upon the classical resistance calculation-based methods.

Abstract

The 'MHC-I (major histocompatibility complex class I)-opathy' concept describes a family of inflammatory conditions with overlapping clinical manifestations and a strong genetic link to the MHC-I antigen presentation pathway. Classical MHC-I-opathies such as spondyloarthritis, Behcet's disease, psoriasis and birdshot uveitis are widely recognised for their strong association with certain MHC-I alleles and gene variants of the antigen processing aminopeptidases ERAP1 and ERAP2 that implicates altered MHC-I peptide presentation to CD8+T cells in the pathogenesis. Progress in understanding the cause and treatment of these disorders is hampered by patient phenotypic heterogeneity and lack of systematic investigation of the MHC-I pathway.Here, we discuss new insights into the biology of MHC-I-opathies that strongly advocate for disease-overarching and integrated molecular and clinical investigation to decipher underlying disease mechanisms. Because this requires transformative multidisciplinary collaboration, we introduce the EULAR study group on MHC-I-opathies to unite clinical expertise in rheumatology, dermatology and ophthalmology, with fundamental and translational researchers from multiple disciplines such as immunology, genomics and proteomics, alongside patient partners. We prioritise standardisation of disease phenotypes and scientific nomenclature and propose interdisciplinary genetic and translational studies to exploit emerging therapeutic strategies to understand MHC-I-mediated disease mechanisms. Abstract

Despite notable scientific and medical advances, broader political, socioeconomic and behavioural factors continue to undercut the response to the COVID-19 pandemic1,2. Here we convened, as part of this Delphi study, a diverse, multidisciplinary panel of 386 academic, health, non-governmental organization, government and other experts in COVID-19 response from 112 countries and territories to recommend specific actions to end this persistent global threat to public health. The panel developed a set of 41 consensus statements and 57 recommendations to governments, health systems, industry and other key stakeholders across six domains: communication; health systems; vaccination; prevention; treatment and care; and inequities. In the wake of nearly three years of fragmented global and national responses, it is instructive to note that three of the highest-ranked recommendations call for the adoption of whole-of-society and whole-of-government approaches1, while maintaining proven prevention measures using a vaccines-plus approach2 that employs a range of public health and financial support measures to complement vaccination. Other recommendations with at least 99% combined agreement advise governments and other stakeholders to improve communication, rebuild public trust and engage communities3 in the management of pandemic responses. The findings of the study, which have been further endorsed by 184 organizations globally, include points of unanimous agreement, as well as six recommendations with >5% disagreement, that provide health and social policy actions to address inadequacies in the pandemic response and help to bring this public health threat to an end.

The field of veterinary oncology has advanced greatly over the last decade, and veterinarians have an important role in advising owners on whether and when certain treatments, such as radiotherapy, are indicated for dogs and cats. Using an online questionnaire (N=636), we investigated what treatment Austrian, Danish and UK veterinarians would recommend for a cat and a dog in a disease scenario where adjunctive radiotherapy would be optimal (a cat with a feline injection-site sarcoma and a dog with a soft tissue sarcoma). In both scenarios, around 40% of the veterinarians suggested radiotherapy, around 10% advised against it and around 27% did not make any recommendation. No significant differences were found in the advice given based on species. However, we identified that Danish veterinarians were significantly less likely to suggest radiotherapy compared to their UK and Austrian colleagues. Further, we found that veterinarians with additional qualifications or a greater interest in advancing veterinary medicine were more likely to recommend radiotherapy. Even though veterinarians would recommend radiotherapy for both species equally, the fact that approximately one quarter of veterinarians would not make any recommendation raises a potential ethical challenge since it may lead to different access of treatment for animal patients suffering from cancer.

Abstract

In recent years, due to the development of standardized diagnostic protocols associated with an improvement in the associated technology, the diagnosis of pelvic endometriosis using imaging is becoming a reality. In particular, transvaginal ultrasound and magnetic resonance are today the two imaging techniques that can accurately identify the majority of the phenotypes of endometriosis. This review focuses not only on these most common imaging modalities but also on some additional radiological techniques that were proposed for rectosigmoid colon endometriosis, such as double-contrast barium enema, rectal endoscopic ultrasonography, multidetector computed tomography enema, computed tomography colonography and positron emission tomography-computed tomography with 16 alpha-[18F]fluoro-17 beta-estradiol.

Abstract

This article provides a retrospective analysis of patients with peripartum cardiomyopathy (PPCMP), a condition characterized by acute cardiac decompensation during the peripartum period. The study, conducted at the Medical University of Vienna, identified 31 patients with PPCMP. Eight patients were diagnosed with PPCMP before delivery and underwent Caesarean section, while the remaining patients developed PPCMP after delivery. The median time to diagnosis was one day, and all patients survived and showed full recovery of left ventricular ejection fraction within four months. The study concludes that with prompt diagnosis and optimal therapy, good maternal outcomes can be expected for patients with PPCMP. [Extracted from the article]

Abstract

Estimating parameters of chaotic geophysical models is challenging due to their inherent unpredictability. These models cannot be calibrated with standard least squares or filtering methods if observations are temporally sparse. Obvious remedies, such as averaging over temporal and spatial data to characterize the mean behavior, do not capture the subtleties of the underlying dynamics. We perform Bayesian inference of parameters in high-dimensional and computationally demanding chaotic dynamical systems by combining two approaches: (i) measuring model–data mismatch by comparing chaotic attractors and (ii) mitigating the computational cost of inference by using surrogate models. Specifically, we construct a likelihood function suited to chaotic models by evaluating a distribution over distances between points in the phase space; this distribution defines a summary statistic that depends on the geometry of the attractor, rather than on pointwise matching of trajectories. This statistic is computationally expensive to simulate, compounding the usual challenges of Bayesian computation with physical models. Thus, we develop an inexpensive surrogate for the log likelihood with the local approximation Markov chain Monte Carlo method, which in our simulations reduces the time required for accurate inference by orders of magnitude. We investigate the behavior of the resulting algorithm with two smaller-scale problems and then use a quasi-geostrophic model to demonstrate its large-scale application.
