Objectives: To assess the differentiated effects of population-based disease management programs (DMPs) for type 2 diabetes on intermediary clinical outcomes in The Netherlands.Methods: Data covering a period from 20 to 24 months between January 2008 and December 2010 were collected from 18 Dutch care groups (primary care provider networks that have bundled payment contracts for delivery of diabetes DMPs). Meta-analysis and meta-regression. methods were used to conduct differentiated analyses of these programs' effects over time on 4 clinical indicators: glycated hemoglobin, low-density lipoprotein, systolic blood pressure, and body mass index. Heterogeneous average results were stratified according to various patient and process characteristics to investigate whether differences in these features could explain variation in outcomes.Results: Between 56% and 71% of patients (N = 105,056) had valid first- and second-year measurements of the study outcomes. Although average changes in these measures over time were small, stratified analyses demonstrated that clinically relevant improvements were achieved in patients with poor first-year health values. Interactions with age, disease duration, comorbidity, and smoking status were not consistent across outcomes; nonetheless, heterogeneity in results decreased considerably when simultaneously correcting for known patient characteristics. Positive effects tended to diminish with longer length of follow-up, while greater measurement frequency was associated with improved results, especially in patients with poor health.Conclusions: Our data suggest that tailored disease management, in which not only evidence-based guidelines but also patient characteristics directly determine care processes, including self-management support, has great potential to improve the cost-effectiveness of current chronic care delivery. This book explores the extent to which ten characteristics of the concept of disease management are advisable in the long-term for certain types of patient care in the Netherlands. The care in mind for this concept covers certain patient populations as well as a number of health problems. For this purpose, the authors have taken as their starting point the following definition of disease management: the programmatical and systematic approach of specific diseases and health problems by using the management instruments that aim at the advancement of quality and efficiency. This definition is based on the literature study that was carried out by C. Spreeuwenberg for his research into the material covered in the first chapter. The ten characteristics of disease management that were derived from the general literature by the aforementioned author are as follows: 1. relates to one health problem and is aimed at clearly-defined patient populations and sub-populations 2. methodological education and the advancement of self-management 3. orientation on the integration of the various parts of the care processes including preventive interventions 4. disease management protocols that are based on evidence-based diagnostics and treatment 5. classification of patients in clinical pathways based on sub-characteristics 6. substitution of care from physicians to nurses 7. use of information and communication technology 8. focus on the use of a range of management instruments such as benchmarking and feedback 9. large-scale and a robust organizational structure 10. direction and funding coming from a central point The characteristics are thus classified according to patient care (characteristics 1 through 5) organization of the patient care (characteristics 6 and 7) and the management of patient care (characteristics 8 through 10). The patient populations that were studied were those of cystic fibrosis (chapter 2), diabetes (chapter 3), depression (chapter 4), heart failure (chapter 5), and palliative patients (chapter 6). The choice for these patient populations was made by representatives of the Dutch Council for the Chronically Ill and Disabled (CG Raad). The idea was to cover a wide range of disorders affecting the young (cystic fibrosis) to the old (heart failure, palliative care) to psychiatric (depression) and somatic illnesses (other patient populations) and on the common disorders (diabetes, heart failure) and less common disorders (cystic fibrosis, palliative care). Authors were sought with a wide range of expertise on the development of integrated care for the patient population concerned. They used knowledge from five sources: 1) their own personal experience and expertise (see author information in appendix II), 2) recent published articles covering the period 1999-2004, specifically searched through Pubmed and Google; 3) the travel reports of Marianne Acampo who visited one location per patient group in the United States, 4) expert meetings in the spring of 2004 whereby leading care professionals for each patient population, project leaders and policy-makers in the area of integrated care in the Netherlands were present, and, 5) individual analytical and judgemental skills. On the basis of this last-mentioned factor, the authors reached their conclusions on whether or not disease management should eventually be introduced in the Netherlands for their respective patient populations. In dit boek wordt voor een aantal patiëntengroepen en gezondheidsproblemen nagegaan, in hoeverre het wenselijk is dat tien kenmerken van het concept disease management op langere termijn gaan gelden voor de patiëntenzorg aan deze groepen. Hierbij zijn de auteurs uitgegaan van de volgende begripsomschrijving van disease management: de programmatische en systematische aanpak van specifieke ziekten en gezondheidsproblemen door middel van het gebruik van managementinstrumenten met als doel de kwaliteit en de doelmatigheid te bevorderen. Deze begripsomschrijving is gebaseerd op de literatuurstudie die Spreeuwenberg ten behoeve van het eerste hoofdstuk uitvoerde. De tien kenmerken van disease management die Spreeuwenberg uit de algemene literatuur afleidde, waren de volgende: 1. betrekking op één gezondheidsprobleem en gericht op welomschreven (sub)populaties van patiënten 2. methodische educatie en bevordering van zelfmanagement 3. gerichtheid op samenhang tussen de diverse onderdelen van het zorgproces, inclusief preventieve interventies 4. geprotocolleerde en op ‘evidence’ gebaseerde diagnostiek en behandeling 5. indeling van patiënten in behandelstromen op grond van subkenmerken 6. taakherschikking van artsen naar verpleegkundigen 7. gebruik van informatie- en communicatietechnologie 8. focus op gebruik van een scala van managementinstrumenten zoals op informatie gebaseerde benchmarking en feedback 9. grootschaligheid en een robuuste organisatiestructuur 10. centrale sturing en bekostiging. De kenmerken zijn hierbij gerangschikt naar patiëntenzorg (kenmerk 1 tot en met 5), organisatie van de patiëntenzorg (kenmerk 6 en 7) en de aansturing van de patiëntenzorg (kenmerk 8 tot en met 10). De bestudeerde patiëntengroepen betreffen patiënten met cystic fibrosis (hoofdstuk 2), diabetes (hoofdstuk 3), depressie (hoofdstuk 4), hartfalen (hoofdstuk 5) en palliatieve zorg aan patiënten (hoofdstuk 6). Vertegenwoordigers van de Chronisch Zieken en Gehandicapten Raad (CG Raad) hebben de keuze voor deze vijf groepen gemaakt. Men wilde een brede spreiding over jong (cystic fibrosis) en oud (hartfalen, palliatieve zorg), over psychiatrische (depressie) en somatische ziektebeelden (overige patiëntengroepen) en over vaak (diabetes, hartfalen) en minder vaak voorkomende ziektebeelden (cystic fibrosis, palliatieve zorg). Bij deze patiëntengroepen zijn auteurs aangezocht met brede kennis van de ontwikkeling van samenhangende zorg aan deze groepen. Zij maakten gebruik van kennis uit vijf bronnen: 1) hun eigen ervaring en expertise, waarvan hun CV's in bijlage II getuigen; 2) recente artikelen over de periode 1999-2004, speciaal opgezocht via Pubmed en Google; 3) de reisverslagen van Marianne Acampo, die per patiënt een site in de Verenigde Staten bezocht. Amerikaanse patiëntenverenigingen hadden haar deze sites aanbevolen als zijne vooraanstaande, met disease management nauw verwante, innovatieve centra; 4) expert meetings in het voorjaar van 2004, met per patiëntengroep vooraanstaande zorgverleneres, projectleiders en beleidsmakers op het terrein van ketenzorg in Nederland en 5) eigen analytisch en oordeelkundig vermogen. Op basis van dat laatste kwamen de auteurs tot oordelen over het al dan niet op termijn voor hun patiëntengroep wenselijk zijn van disease management in Nederland.
