Abstract Background Splenectomy is sometimes necessary after abdominal trauma, but splenectomized patients are at risk of sepsis due to impaired immunological functions. To overcome this risk, autotransplantation of the spleen by using a new technique has been proposed, but so far, a demonstration of functionality of the transplanted tissue is lacking. Methods We therefore evaluated 5 patients who underwent a splenic autotransplant in comparison with 5 splenectomized patients without splenic autotransplant and 7 normal subjects. Results We confirmed that the patients not undergoing autotransplantation, when compared to normal subjects, had a higher platelet count, higher percentage of micronucleated reticulocytes (p = 0.002), increased levels of naive B lymphocytes (p = 0.01), a defect of class-switched memory (p = 0.001) and class-unswitched memory B cells (p = 0.002), and increased levels of PD1 on T lymphocytes CD8+ (p = 0.08). In contrast, no significant differences for any of the abovementioned parameters were recorded between patients who underwent spleen autotransplantation and normal subjects. Conclusion These findings suggest that splenic autotransplantation is able to restore an adequate hemocatheretic activity as well as recover the immunological deficit after splenectomy.

Background: Splenectomy is a surgical procedure indicated in a variety of medical conditions including trauma. Post‐operatively, there is a lifelong risk of developing overwhelming sepsis from encapsulated bacteria, most commonly due to Streptococcus pneumoniae. Splenic autotransplantation has been proposed as a method to recover splenic function in patients requiring splenectomy with otherwise normal spleens. This study aims to systematically review the literature to determine the efficacy of spleen autotransplantation. Methods: MEDLINE, PubMed and the Cochrane Library were searched for all studies assessing splenic autotransplantation (January 1947 to July 2018). Data were extracted on study characteristics, outcomes assessed, including spleen scintigraphy results, blood film counts and serum immunoglobulin (Ig) levels. Results: Data were obtained from 18 primary studies. All studies demonstrated return of regenerated spleen tissue in the majority of their patients (95.3%) on spleen scintigraphy. In 12 studies, 90.2% of patients had blood films return to normal following transplantation. Ig levels were shown to return to normal in all 12 studies where it was assessed. In 11 studies, 3.7% of patients had post‐operative complications. In five studies, 1.3% of patients had post‐operative infections in the follow‐up period. Conclusion: Splenic autotransplantation is a safe procedure with minimal complications that can return splenic filtration function and Ig levels to normal ranges. As complicações da esplenomegalia na doença de Gaucher, com repercussões de hiperesplenismo e compressões mecânicas, têm sido tratadas por meio de esplenectomia parcial. Contudo, verificou-se que o remanescente esplênico suprido pelos vasos hilares volta a crescer, com conseqüente recorrência da esplenomegalia e de todo o quadro clínico que a acompanha. Uma experiência superior a dezoito anos no tratamento da hipertensão porta, trauma esplênico, esplenomegalia mielóide, hipodesenvolvimento somático e sexual esplenomegálico, linfomas e cistoadenoma de cauda pancreática por meio de esplenectomia subtotal, preservando o pólo superior do baço irrigado apenas pelos vasos esplenogástricos, mostrou que o remanescente esplênico não aumenta as suas dimensões. Com base em evidências de que o pólo superior esplênico é suficiente para manter todas as funções do baço, realizamos em cinco pacientes com doença de Gaucher esplenectomia subtotal, mantendo o pólo superior do baço e sua vascularização esplenogástrica. O remanescente esplênico não modificou as suas dimensões durante o acompanhamento pós-operatório superior a doze anos e os parâmetros hematológicos normalizaram. Uma outra paciente, que possuía vasos esplenogástricos insuficientes para nutrir o pólo superior do baço, foi submetida a esplenectomia total, com implantes autógenos de tecido esplênico no omento maior. Os auto-implantes sobreviveram e foram funcionantes. Todos os seis pacientes tiveram evolução pós-operatória sem anormalidades relacionadas ao procedimento de conservação esplênica. Concluindo, em presença de esplenomegalia gigante, acompanhada de quadro clínico e hematológico grave, a cirurgia do baço conservadora, com destaque para a esplenectomia subtotal ou a esplenectomia total seguida de auto-implantes de tecido esplênico.Complications of splenomegaly in Gaucher's disease, such as hypersplenism and mechanical compressions, have been treated by partial splenectomy. However, the splenic remnant, vascularized by the hilar vessels, may enlarge leading to recurrent splenomegaly and all its adverse effects. More than eighteen years of experience treating portal hypertension, splenic trauma, myeloid splenomegaly, splenomegalic somatic and sexual hypodevelopment, chronic lymphocytic lymphoma and cystadenoma of pancreatic tail by subtotal splenectomy, maintaining the upper pole supplied only by the splenogastric vessels showed that the splenic remnant does not increase its size. Based on evidence that the hilar vessels may contribute to the increase of the splenic remnant, we performed in five cases with Gaucher's disease subtotal splenectomy, maintaining the upper pole supplied only by the splenogastric vessels. The splenic remnant did not modify in size during a postoperative follow-up of more than twelve years, and the hematological parameters remained normal. Another patient whose splenogastric vessels were insufficient to maintain the upper splenic pole, was submitted to total splenectomy and autologous splenic tissue transplantation on the greater omentum. The autotransplant survived and presented with function. All six patients had uneventful postoperative follow-up in relation to the splenic conservative procedure. In conclusion, in presence of giant splenomegaly with severe hematologic and clinical symptoms the spleen must be resected in a conservative fashion such as subtotal splenectomy or total splenectomy followed by autotransplants of splenic tissue.

Here, BALB/c mice subjected to splenectomy and autogenous grafting of spleen were evaluated after 3 and 7 days of intravenous L. monocytogenes infection. The group of splenectomized animals (SP) presented a lower number of bacteria in the liver in comparison with both the sham-operated control group (CT) and the group that received splenic autotransplantation (AT) in the retroperitoneal site. The AT group presented bacterial counts in the liver similar to the CT group. SP animals showed larger production of interferon-gamma (IFN-γ) and nitric oxide (NO) in the liver in comparison with CT and AT, this being associated with greater accumulation of mononuclear cells. IFN-γ production by spleen cells after stimulation with heat-killed Listeria was similar between the AT and CT groups, suggesting that the implanted fragments behaved like the original organ. The autogenous grafting of spleen fragments reverses the resistance to L. monocytogenes infection found in splenectomized mice, associated with a reduced IFN-γ and NO production in the liver. The present study shows that splenic autotransplantation restores the function of the spleen in splenectomized mice, even though in this case it does favor the susceptibility to L. monocytogenes infection. Minor splenic injuries from blunt trauma can be treated conservatively, whereas high-grade injuries commonly associated with multiple trauma require surgical treatment and usually removal of the organ. Although splenectomy is nowadays routinely performed laparoscopically for the treatment of hematological pathologies, in an emergency the operational procedure is performed through conventional laparotomy worldwide, advocating the need for hemostasis. Progress in surgical skill and new developments in equipment allow us to treat also patients affected by severe splenic blunt trauma minimally invasively. In this study we analyzed 12 patients who consecutively came under our observation during a 2-year period and, being affected by severe spleen injury from blunt trauma requiring surgery, underwent emergency laparoscopy. All of them had Injury Severity Score (ISS) ≥ 20 with Glasgow Coma Score (GCS) ≥ 10. Laparoscopic splenectomy was performed in ten of the cases utilizing a quick hemostatic technique. In one case bleeding was controlled without removal of the organ and in another case laparoscopy revealed that the supposed hemoperitoneum and splenic rupture were in fact the rupture of a giant splenic cyst. The median operative time to reach hemostasis was 17 min (13–125 min) and the median overall operative time was 120 min (55–210 min). All operations were performed fully laparoscopically. Neither mortality nor morbidity related to abdominal problems was observed. Median postoperative stay was 4 days (3–11 days). Laparoscopic approach to splenic blunt trauma requiring surgery is a safe and effective procedure. The described technique allows laparoscopic splenectomy to be performed in an emergency, with much the same hemostatic efficacy as the open technique, but with much better outcome for the patient. Purpose: Atherosclerosis observations after splenectomy for trauma and hypersplenism suggests a possible role for the spleen in lipid metabolism. The authors examined the effects of splenectomy on serum lipids in rats and also cholesterol-fed rats with experimental atherosclerosis. Methods: This study was designed on rats. The rats were divided into five groups: splenectomy, normal diet (SP-N, n: 8), splenectomy, cholesterol-fed groups (SP-C, n: 8), splenic autotransplantation after splenectomy, normal diet (SA-N, n: 8), splenic autotransplantation after splenectomy, cholesterol-fed groups (SA-C, n: 8) and sham groups (n: 8). Total triglyceride, total cholesterol, HDL (high-density lipoprotein), LDL (low-density lipoprotein), and VLDL (very low-density lipoprotein) levels were determined in 40 rats. The rats were classified into five groups based on the surgical procedures. The spleens were removed and then the rats were fed a normal diet in Group SP-N (n = 8). The spleens were removed and then the rats were fed a diet containing 1% cholesterol in Group SP-C (n = 8). Splenectomy and splenic autotransplantations were performed and then the rats were fed a normal diet in Group SA-N (n = 8). Splenectomy and splenic autotransplantations were performed and then the rats were fed a diet containing 1% cholesterol in Group SA-C (n = 8). The rats were sham-operated in the control group (Group S, n = 8). An active splenic function was shown in rats that underwent splenic autotransplantation in both groups by using Technicium 99 m sulphurcolloide sintiscan on day 30. Blood lipid levels were repeated 6 months later. Results: There was no difference between pre- and postoperative lipid levels in the sham group and SA-N group (p >.05). All lipid levels including HDL were increased significantly in SP-C group (p <.05). Also VLDL and total tryglyceride levels were increased significantly in SP-N and SA-C groups (p <.05). Conclusions: This study showed that the spleen might have an important effect on lipid metabolism and splenic autotransplantation may be protective in conditions with increased lipid levels. Background. The loss of spleen may increase the incidence of overwhelming sepsis. To prevent this, splenic autotransplantation has been performed in humans and experimental animals. However, there is still controversy about the effectiveness of regenerated splenic tissue in preventing infection. This study explored the effectiveness of splenic tissue autotransplantation in restoring host defense. Materials and methods. Rabbits were divided into three groups: splenic autotransplantation, sham operation, and total splenectomy. Histomorphology, T-lymphocyte count, serum lysozyme levels, hemolysin titers, and pneumococcal clearance were observed as read-out parameters over 24 weeks. Results. Histological study showed that the white pulp was poorly developed and central arterioles were missing in the regenerated splenic tissue of the autotransplanted rabbits. The weight of regenerated spleens recovered 6 months later in the splenic autotransplantation group was 11% of that in the sham operation group and was significantly less than the weight at implantation. There was no significant difference in the number of T lymphocytes or level of serum lysozyme between the three groups. A poor antibody response by the rabbits in the splenic autotransplantation and total splenectomy groups was noted after the primary intravenous administration of sheep red blood cells compared to those of sham operation group. After the challenge with type 3 pneumococci intravenously, pneumococcal clearance from the bloodstream in the splenic autotransplantation group did not differ significantly from that in the total splenectomy group, but was markedly delayed compared with that in the sham operation group. Conclusions. The low quantity and poor quality of the regenerated splenic tissue contribute to the inferior immunoprotective ability of animals autotransplanted with one-third of the original spleen. Wistar rats submitted to isolated total splenectomy or total splenectomy combined with splenic autotransplantation were inoculated with 99mtechnetium-labeled Escherichia coli. Measurement of isotope uptake in the organs of the mononuclear phagocytic system showed a greater bacterial bloodstream clearance in rats with splenic autotransplantation. Although uptake of bacteria in the spleen was higher in the control group, the number of bacteria remaining in the bloodstream did not differ between groups. Splenic autotransplantation seems to be the only alternative for preservation of splenic tissue after total splenectomy. This work was carried out to analyze the morphologic regeneration of autotransplanted splenic tissue in Wistar rats and to determine the bacterial phagocytic function of their macrophages. We utilized an experimental model including young and adult rats, of both sexes, submitted to total splenectomy combined with autotransplantation in the greater omentum of slices of the whole mass of spleen. Sixteen weeks later animals were intravenously inoculated with a suspension of Escherichia coli AB1157. There was regeneration of autotransplanted splenic tissue in all animals. A similar morphological aspect among all animals was observed, with splenic tissue showing red and white pulps with a moderate architectural disarrangement. Macrophages containing bacterial aggregates were observed, as well as macrophages with hemosiderin pigments inside the cytoplasm. Blood vessels showed preserved walls, with no signs of vasculitis or thrombosis. The present results suggest that splenic autotransplants in the greater omentum of the rat acquire the macro- and microscopic architecture of a normal spleen, with reduced dimensions, and preserve bacterial phagocyte function. In Japan since the 1960s, patients with gastric cancer have routinely had splenectomy combined with gastrectomy to ensure that lymph node dissection is complete. However, the influence of splenectomy on long-term immunity is unclear. Methods. Forty-nine gastric cancer patients who underwent total gastrectomy for cure with ( n = 25) and without ( n = 24) splenectomy were assessed for immunologic function, including the proportion of lymphocyte subsets, purified protein derivative from tuberculin (PPD) response, natural killer (NK) activity, and phytohemagglutinin (PHA) response. Results. Peripheral T-cell mediated functions, e.g., PPD and PHA response, were significantly suppressed in patients who underwent gastrectomy with splenectomy compared with those who had gastrectomy alone. Decreased T-cell subsets (CD 3+, 4+, 8+) and increased NK cell subsets (CD 16+, 57+) were observed in patients who underwent splenectomy. Patients who did not undergo splenectomy had immunologic responses within the normal range. Conclusions. Splenectomy decreased T-cell mediated responses over the long term. Recently preservation of traumatized spleen has been advocated. Splenic autotransplantation has been proposed as a method for preserving splenic function. This paper reports our experience with autotransplantation of splenic tissue in adults. During a 5-year period from 1981-1985, 102 patients with injured spleen underwent conservative surgery at our institute, 54 of whom received autotransplantation of splenic slices into omental pouches. One patient died of multiple organ failure 2 days after operation,. and another one developed adhesive intestinal obstruction and, therefore, required reoperation 5 months after the first surgical intervention. Histologic studies of this patient's transplants revealed extensive fibrosis. 42 of the 54 patients were followed up, only 10 maintained a regular follow-up schedule. It was found that the neutrophil leukocyte to lymphocyte ratio was reversed in 7 cases, and α-naphthyl-acetate esterase assay showed the excessive peripheral lymphocytes to be mainly B lymphocytes. The high incidence of overwhelming postsplenectomy infection caused by Streptococcus pneumoniae can be reduced by splenic autotranspiantation. In this study the effect of splenectomy and splenic autotransplantation on the immune response to S. pneumoniae infection was investigated. Balb/c mice were divided into three groups: splenectomized (SP), splenectomized and autotransplanted (AT), and sham operated control (CT). Five days post-infection the serum antibody levels were measured and the number of S. pneumoniae CR1, neutrophil accumulation and IL-17 production in the liver and lungs were investigated. SP mice showed greater number of bacteria in both organs and lower serum levels of S. pneumoniae-specific 1gM, lgG1 and lgc2a antibodies. IL-17 production and neutrophil recruitment to the liver and lungs were lower in SP mice, in comparison with both the Cr and the AT groups. Levels of S. pneumoniae-specific 1gM, CFU counts, neutrophil accumulation and lL-17 production did not differ significantly between the CF and AT groups. These results suggest that splenic autotransplantation restores the capacity of splenectomized mice to fight S. pneumoniae infection. [ABSTRACT FROM AUTHOR]
