Abstract

Outpatient urodynamic monitoring (AUM) - registration of the patients urination parameters for a certain time (for example, during the day), which is carried out in natural conditions for him. Monitoring allows you to identify hidden or periodically occurring (circadian) changes, which are not always possible to record with a single study. The research was searched in the PubMed and Elibrary.ru databases using the keywords "home uroflowmetry", "outpatient urodynamic examination", "outpatient urodynamic monitoring", "outpatient uroflowmetric monitoring". The difficulties in performing AUM lie not only in the hardware (the presence of a portable device, minimally invasive or non-invasive research, software that guarantees the storage and transmission of data), but also the difficulties associated with teaching patients and overcoming conservatism on the part of medical personnel. Today, there are several urodynamic devices and uroflowmeters designed for research at home. Outpatient (home) urodynamic monitoring is one of the most promising studies for implementation in everyday clinical practice in the provision of medical care to patients with urination disorders on the background of benign prostatic hyperplasia. The integration of home uroflowmetric monitoring into the eHealth system, of course, remains an inevitable trend in the development of modern healthcare.

Abstract

Worldwide data for cancer survival are scarce. We aimed to initiate worldwide surveillance of cancer survival by central analysis of population-based registry data, as a metric of the eff ectiveness of health systems, and to inform global policy on cancer control. Methods Individual tumour records were submitted by 279 population-based cancer registries in 67 countries for 25·7 million adults (age 15–99 years) and 75 000 children (age 0–14 years) diagnosed with cancer during 1995–2009 and followed up to Dec 31, 2009, or later. We looked at cancers of the stomach, colon, rectum, liver, lung, breast (women), cervix, ovary, and prostate in adults, and adult and childhood leukaemia. Standardised quality control procedures were applied; errors were corrected by the registry concerned. We estimated 5-year net survival, adjusted for background mortality in every country or region by age (single year), sex, and calendar year, and by race or ethnic origin in some countries. Estimates were age-standardised with the International Cancer Survival Standard weights. Findings 5-year survival from colon, rectal, and breast cancers has increased steadily in most developed countries. For patients diagnosed during 2005–09, survival for colon and rectal cancer reached 60% or more in 22 countries around the world; for breast cancer, 5-year survival rose to 85% or higher in 17 countries worldwide. Liver and lung cancer remain lethal in all nations: for both cancers, 5-year survival is below 20% everywhere in Europe, in the range 15–19% in North America, and as low as 7–9% in Mongolia and Thailand. Striking rises in 5-year survival from prostate cancer have occurred in many countries: survival rose by 10–20% between 1995–99 and 2005–09 in 22 countries in South America, Asia, and Europe, but survival still varies widely around the world, from less than 60% in Bulgaria and Thailand to 95% or more in Brazil, Puerto Rico, and the USA. For cervical cancer, national estimates of 5-year survival range from less than 50% to more than 70%; regional variations are much wider, and improvements between 1995–99 and 2005–09 have generally been slight. For women diagnosed with ovarian cancer in 2005–09, 5-year survival was 40% or higher only in Ecuador, the USA, and 17 countries in Asia and Europe. 5-year survival for stomach cancer in 2005–09 was high (54–58%) in Japan and South Korea, compared with less than 40% in other countries. By contrast, 5-year survival from adult leukaemia in Japan and South Korea (18–23%) is lower than in most other countries. 5-year survival from childhood acute lymphoblastic leukaemia is less than 60% in several countries, but as high as 90% in Canada and four European countries, which suggests major defi ciencies in the management of a largely curable disease. Interpretation International comparison of survival trends reveals very wide diff erences that are likely to be attributable to diff erences in access to early diagnosis and optimum treatment. Abstract

The analysis of the main clinical indices of purulent sinusitis has been studied in 102 children who are ailing with acute respiratory diseases. It has been established, that the majority of clinical indices had high sensitiveness and low specificity. The acute purulent sinusitis was diagnosed in every fifth preschool child and more than 40% school-age children. The X-ray examination of paranasal sinuses is expediently for decreasing the part of mistakes in diagnostics of purulent sinusitis in children who are in poor health., Проведено исследование основных клинических показателей гнойного синусита у 102 детей, которые подвержены острым респираторным заболеваниям. Установлено, что большинство клинических диагностических показателей имели высокую чувствительность и низкую специфичность. Острый гнойный синусит диагностировали у каждого пятого дошкольника и более чем у 40 % школьников группы наблюдения. Для уменьшения доли ошибок при диагностировании гнойного синусита у часто и длительно болеющих детей целесообразным является проведение рентгенологического исследования околоносовых пазух., Проведене дослідження основних клінічних показників гнійного синуїту у 102 дітей, що часто хворіють гострими респіраторними захворюваннями. Встановлено, що більшість клінічних діагностичних показників мали високу чутливість і низьку специфічність. Гострий гнійний синуїт діагностували в кожного п’ятого дошкільняти та більше, ніж у 40% школярів групи спостереження. Для зменшення помилок при діагностуванні гнійного синуїту у дітей, що часто тривало хворіють, доцільним є проведення рентгенологічного дослідження біляносових пазух.

Abstract

Observations of local variations in the intensity and spectral composition of the cathodoluminescence of lunar regolith are described. These variations are caused by structural features, by the kind and quantity of the material, and by the admixtures. In addition, cathodoluminescent emission is also associated with a change in the temperature of the lunar surface. Periodic diurnal illumination and darkening of the moon is responsible for a temperature gradient normal to the lines of longitude. These observations made of regolith, using a scanning electron microscope with an attachment for temperature variation, simulate the indicated thermal changes of the cathodoluminescent properties of lunar material.

Abstract

Scanning electron microscopy was used in studying the morphology and cathodoluminescence of lunar regolith particles. Surface and structure of two groups of particles are differentiated: (1) Crystalline with well defined facets and spalling surfaces, which are grains of minerals and rock fragments: and (2) amorphous, fused, and partially or entirely glazed particles. Local melting of particles and the round openings on their surfaces are attributed to secondary influence on the regolith of factors of lunar weathering and above all micrometeoric impacts.

40. Hydrogen Migration and HCl Elimination Promoted by Metal-Centered Nucleophilic Attack on Osmium Hydrido−Carbynes

Abstract

The carbynes [OsHCl2(CCH2R)L2] (R = Ph, CH3; L = PiPr3) react with CO to give, after 1,2-hydride migration from osmium to Cα of the carbyne ligand, the osmium carbene complexes [OsCl2(CO)(&dbd;CHCH2R)L2]. For R = Ph, reactions with excess CO lead to HCl elimination to form [OsCl(CO)2((E)-CH&dbd;CHPh)L2], which reacts with liberated HCl at Cα to yield [OsCl2(CO)2L2] and CH2&dbd;CHPh. Abstraction of chloride from either the carbynes or carbenes with NaBAr‘4 (Ar‘ = 3,5-C6H3(CF3)2) yields the corresponding coordinatively unsaturated carbyne and carbene complexes [OsHCl(CCH2R)L2][BAr‘4] and [OsCl(CO)(&dbd;CHCH2R)L2][BAr‘4], respectively. The carbynes [OsHCl(CCH2Ph)L2]+ react with reagent L‘ (L‘ = CO, HCCR‘) to initially give the carbenes [OsCl(L‘)(&dbd;CHCH2Ph)L2]+ after 1,2-hydride migration to Cα of the carbyne ligand. These carbenes react further, eliminating HCl and undergoing selective protonation to yield either [OsCl(CO)3L2]+ and CH2&dbd;CHPh (with L‘ = CO) or the σ-vinyl carbyne complexes [OsCl(CCH2R‘)((E)-CH&dbd;CHPh)L2]+ (with L‘ = HCCR‘).

41. Trinuclear and Tetranuclear Platinum−Iridium Cluster Complexes:  A Remarkable Metal for Metal Substitution Reaction

Abstract

The reaction of [Pt3(μ3-CO)(μ-dppm)3]2+, 1, dppm = Ph2PCH2PPh2, with [Ir(CO)4]- at low temperature gives [Pt3(μ3-CO)(μ-dppm)3{Ir(CO)4}]+, 2, and subsequent loss of CO leads to the butterfly complex [Pt3{Ir(CO)(μ-CO)2}(μ-CO)(μ-dppm)3]+, 3. Complex 3 reacts with L = P(OPh)3 to give [Pt3{IrL(μ-CO)2}(μ-CO)(μ-dppm)3]+, 4, which has been characterized by X-ray structure determination and has a butterfly structure with iridium at a wingtip position. Cluster 3 is thermally unstable and rearranges in solution to form the isomeric butterfly cluster cation [Pt2Ir{Pt(CO)(μ-CO)2}(CO)(μ-dppm)3]+, 5, in which the iridium atom now occupies a hinge position and whose formation involves phosphorus migration from platinum to iridium. Under CO, 5 eliminates one platinum atom to form the trimetallic complex [Pt2Ir(μ3-CO)(CO)(μ-dppm)3]+, 6. The overall reaction then involves displacement of a platinum atom from the 42-electron cluster cation 1 by {IrCO}- to give the 44-electron cation 6.

Abstract

A brief historical survey is given of work which was performed in the Soviet Union in the field of the physics of the gas discharge, with respect to the elementary processes originating in the passage of an electrical current through gases, and related problems. Early work in the investigation of the discharge is enumerated, with respect to the establishment of gas conductivity. Investigations which laid the foundation of the development of theoretical concepts of plasma are described, and also work performed in recent years. Works are considered which are devoted to the behavior of plasma in a magnetic field, plasma diagnostics, and various types of discharges. Abstract

The course of electrophilic addition to the ruthenium(II) chloro vinylidenes [Cp*Cl(PPh3)Ru(CCHR)] is influenced by the steric properties of the electrophile and ruthenium complex. Thus, H+ selectively adds to Cβ of the vinylidene ligand to yield the ruthenium(IV) carbynes [Cp*Cl(PPh3)Ru(CCH2R)][A] (A = BF4-, BArf4-), while the comparably larger Me+ (from MeOTf) abstracts Cl- to yield, after anion coordination, [Cp*(OTf)(PPh3)Ru(CCHR)] and MeCl.

Abstract

Variable-temperature kinetics studies of the reversible β-methyl elimination reaction of Cp2Hf(CH2CMe3)(μ-Me)B(C6F5)3 (I) to give Cp2Hf(Me)(μ-Me)B(C6F5)3 and CH2&dbd;CMe2 are reported. Activation enthalpies of 21.3(0.5) and 17.0(0.3) kcal/mol in toluene and chlorobenzene, respectively, are shown to represent the energy barrier for β-methyl migration accompanied by concomitant, solvent-assisted dissociation of [BMe(C6F5)3]- anion from the metal.

When examining samples of low conductivity with a scanning electron microscope (REM) charging of the body and surface takes place. This is brought about by the electrons of the probe. In the majority of cases this leads to a deterioration in the resolution and to a considerable distortion of the image. Abstract

The purpose of a stroboscopic or gating electron microscope is to observe the successive quasi-instantaneous stages of dynamic processes; if the frequency of the investigated process is synchronized with the gating there will be obtained stationary images of the surface structure regardless of the frequency characteristics of the screen. In the case of an emission system with a three-electrode objective a stroboscopic regime can be realized in different ways: supply of the microscope with high-voltage pulses, modulation of the potential on the focusing electrode, or deflection of the beam by means of appropriate deflecting plates. In the instrument employed in the present work pulse modulation was employed. A block diagram of the microscope and associated electronic equipment is shown in the figure. Some of the parameters of the equipment and particularly of the ion source used for inducing the secondary emission are described in the paper.
