The aim of this study was to assess the short-term hemodynamic effects of increasing doses of spironolactone (25, 50, and 75 mg/day) on oliguric patients (5 men, mean age 47 [plus or minus] 12 years) undergoing hemodielysis for chronic renal impairment. Parameters of interest included heart rate (HR), cardiac output, systemic vascular resistance (SVR), arterial pressure, right atriel pressere, and pulmonary capillary wedge pressure (PCWP). The study also evaluated how spironolactone modified the effects on arterial and right atriel pressures and PCWP of infusion of increasing doses of noreplnephrine (20, 40, and 100 ng/ kg/min) and angietensin II (2, 4, and 10 ng/kg/ rain). Short- and long-term clinical effects of the anglotensin-converting enzyme (ACE) Inhibitor captopril in severe congestive heart failure (CHF) were evaluated during a 3-year open study of 124 inpatients with New York Heart Association (NYHA) functional class III or IV CHF refractory to treatment with cardiac glycosides and high doses of loop diuretlcs. Captopril was added to each patient's regimen, which comprised combinations of furosemide (124 patients), digitalis (117 patients), and spironolactone (90 patients). A large-scale, open, nonrandomized, multicenter, 90-day study of the safety and efficacy of a thiazide diuretic and aldosterone antagonist combination Aldactazinelt, 25 mg spironolactone and 15 mg altizide, 1/day) as monotherapy was performed in 946 patients with mild to moderate hypertension (diastolic blood pressure [BP] between 90 and 120 mm Hg). Adverse effects were assessed, and body weight, heart rate, serum potassium, creatinine and uric acid measurements were monitored. On day 45 of the study, BP was normalized diastolic BP (Am J Cardiol 1990;65:20K-23K), Spironolactone antagonizes the actions of the adrenal hormone aldosterone, which functions to regulate the metabolism of sodium, chloride, and potassium. It causes a mild increase in the excretion of sodium without causing losses in potassium and magnesium from the kidney. Thiazide diuretics directly affect the transport of sodium and chloride in the kidneys. Both agents have been used to treat hypertension. Spironolactone may be combined with thiazide diuretics to decrease the potassium and magnesium losses associated with thiazide use. The safety and effectiveness of combination therapy with spironolactone and the thiazide diuretic altizide were assessed in 946 patients with mild to moderate hypertension. Body weight, heart rate, and blood levels of potassium, creatinine, and uric acid were measured. Within 45 days of starting the combined treatment, 72 percent of the patients had achieved good blood pressure control with one tablet per day, containing 25 milligrams (mg) spironolactone and 15 mg altizide. The dose of the combined antihypertensive medication was increased to two tablets per day in patients whose blood pressure had not recovered to normal levels. Blood pressure was controlled in 83 percent of the patients within 90 days of treatment. Combination therapy did not affect body weight, heart rate, or laboratory measurements. However, six patients discontinued treatment; hypokalemia, or decreased blood potassium levels, developed in four patients, and elevated blood creatinine levels occurred in two patients. The blood levels of uric acid were increased in 5.5 percent of patients. Only five percent of the patients experienced adverse effects from drug treatment. These results demonstrate that the combination of spironolactone and altizide is a safe, effective, and inexpensive means of treating mild to moderate hypertension.
