Objective Clinicians are recommended to use the clinical reasoning framework developed by the International Federation of Orthopaedic Manipulative Physical Therapists (IFOMPT) to provide guidance regarding assessment of the cervical spine and potential for cervical artery dysfunction prior to manual therapy and exercise. However, the interexaminer agreement and reliability of this framework is unknown. This study aimed to estimate the interexaminer agreement and reliability of the IFOMPT framework among physical therapists in primary care. Methods Ninety-six patients who consulted a physical therapist for neck pain or headache were included in the study. Each patient was tested independently by 2 physical therapists, from a group of 17 physical therapists (10 pairs) across The Netherlands. Patients and examiners were blinded to the test results. The overall interexaminer agreement, specific agreement per risk category (high-, intermediate-, and low-risk), and interexaminer reliability (weighted κ) were calculated. Results Overall agreement was 71% (specific agreement in high-risk category = 63%; specific agreement in intermediate-risk category = 38%; specific agreement in low-risk category = 84%). Overall reliability was moderate (weighted κ = 0.39; 95% CI = 0.21–0.57) and varied considerably between pairs of physical therapists (κ = 0.14–1.00). Conclusion The IFOMPT framework showed an insufficient interexaminer agreement and fair interexaminer reliability among physical therapists when screening the increased risks for vascular complications following manual therapy and exercise prior to treatment. Impact The IFOMPT framework contributes to the safety of manual therapy and exercise. It is widely adopted in clinical practice and educational programs, but the measurement properties are unknown. This project describes the agreement and reliability of the IFOMPT framework.

Objective To observe the influence of Nie-pinching the spine manipulation on the excretory rate of urine D-xylose in the infants with diarrhea due to spleen deficiency, and to assess the clinical effects.

To observe the clinical effect of Nie-pinching spine coupled with point application and injection for cerebral palsy (CP) in children with spleen deficiency. A total of 70 CP children with spleen deficiency were randomly allocated into a treatment group (n=35) and a control group (n=35). Children in the treatment group were treated with comprehensive rehabilitation training, Nie-pinching spine, point application and point injection, whereas children in the control group were treated with comprehensive rehabilitation training and oral administration of multi-enzyme tablets. One course of treatment made up of 20 d. There was a 20-day interval between two courses. The cases were treated for a total of 3 courses. Then the therapeutic efficacies were evaluated using scores of spleen-deficiency symptoms and gross motor function measure (GMFM). After 3 courses of treatment, the total effective rates for spleen-deficiency symptoms and rehabilitation were 85.7% and 77.1% respectively in the treatment group, versus 51.4% and 57.1% respectively in the control group, showing statistical differences (both P

BACKGROUND: While both manipulative treatment and physical exercises are used to treat cervical pain, it remains unclear which is most effective. OBJECTIVE: To compare the short-term effects of high-velocity, low-amplitude manipulation techniques (MT) with those of home-exercise (HE) with stretching and low-intensity (10% of max) isometric contractions on pain and function. METHODS: Single-blind randomized clinical trial was performed. A total of 27 asymptomatic subjects were randomly assigned to 2 groups: manipulation techniques (MT, n= 13) and home exercise (HE, n= 14). The visual analogue scale (VAS); neck disability index (NDI); pressure pain thresholds; cervical spine range of motion and electromyography during the cranio-cervical flexion test was measured before and one week after the intervention. RESULTS: After the intervention, both groups showed improved (P less than 0.05) NDI and VAS scores and flexion in both rotation ranges compared with the pre-intervention values. For the NDI, pain intensity, and neck flexion, the effects sizes were large; for the majority of the other measurements, the effect sizes were small to moderate. The MT group showed significantly better results than the HE group for 2 out of 17 tests. Background: Cervical high-velocity low-amplitude (HVLA) manipulation technique is among the oldest and most frequently used chiropractic manual therapy, but the physiologic and biomechanics effects were not completely clear. Objective: This review aims to describe the effects of cervical HVLA manipulation techniques on range of motion, strength, and cardiovascular performance. Methods/Design: A systematic search was conducted of the electronic databases from January 2000 to August 2016: PubMed (n = 131), ScienceDirect (n = 101), Scopus (n = 991), PEDro (n = 33), CINAHL (n = 884), and SciELO (n = 5). Two independent reviewers conducted the screening process to determine article eligibility. The intervention that included randomized controlled trials was thrust, or HVLA, manipulative therapy directed to the cervical spine. Methodological quality was assessed using the Cochrane risk-of-bias tool. The initial search rendered 2145 articles. After screening titles and abstracts, 11 articles remained for full-text review. Results: The review shows that cervical HVLA manipulation treatment results in a large effect size (d > 0.80) on increasing cervical range of motion and mouth opening. In patients with lateral epicondylalgia, cervical HVLA manipulation resulted in increased pain-free handgrip strength, with large effect sizes (1.44 and 0.78, respectively). Finally, in subjects with hypertension the blood pressure seemed to decrease after cervical HVLA manipulation. Higher quality studies are needed to develop a stronger evidence-based foundation for HVLA manipulation techniques as a treatment for cervical conditions. © Copyright 2017, Mary Ann Liebert, Inc.

Background High-velocity, low-amplitude (HVLA) manipulation techniques are habitually used on the cervical spine but the effects are not completely clear. The aim of this prospective comparative trial was to evaluate effects of an indiscriminate manipulation on the C5 (AMC5) a manipulation treatment based on a previous evaluation (MT) and a sham intervention (ST) on cervical spine range of motion (ROM); cervical flexion isometric peak force; EMG activation of sternocleidomastoid muscle (SCM) during the cranio-cervical flexion test (CCFT); and EMG signals of right and left biceps at rest were analyzed. Methods/Design Randomised controlled pilot study and intention-to-treat analysis was performed. Setting The study was conducted at an osteopathic clinic. Methods The outcomes were measured pre and immediately post intervention. Participants A total of 36 asymptomatic subjects (18 male, mean age 30 years) were randomly enrolled into 3 groups: AMC5 (n = 12), MT (n = 12), and ST (n = 12). Results Significant changes (p less than 0.1) were found in the cervical flexion isometric peak force (?13.15%), however, the effect size was considered moderate (d = 0.52). The extension (10.44%) and left rotation ROM (12.25%) showed significant improvement in MT group. During CCFT significant changes were not reported. Conclusions The current pilot study suggested that a tendency toward a decrease in the isometric strength peak in the cervical flexion of the MT group may appear. In cervical ROM the MT group achieved significant effects in extension and left rotation movement. © 2016 Elsevier Ltd

Cauda equina syndrome seems to be a very rare complication of spinal manipulations. Only few cases, in fact, were referred in literature in the past decades. Most of them are very old and poorly documented evoking doubts about the pathogenetic relationship between the spinal maneuvers and the onset of the syndrome. We observed and treated a 42-year-old patient who complained a rapid onset of saddle hypoparesthesia and urine retention only a few hours after the spinal manipulation performed for L5-S1 herniated disc. The comparison of the two following MRIs performed before and after the manipulations seems to prove a close pathogenetic relationship. The patient was operated soon after the admission to our emergency department and 1 year later he referred an incomplete recovery of the syndrome. The case offered the opportunity to update the literature. The review revealed only three cases from the beginning of the current century that confirm the rarity of the syndrome. Based on the data emerging from the official literature, safety of the manipulations and its pathogenetic aspects in causing lumbar radiculopathies are discussed.

High-velocity spinal manipulation is commonly adopted for treating chronic low-back pain (CLBP) and has been associated with changes in muscle activity, but the evidence is controversial. The aim of this study was to analyse the immediate effects of high-velocity spine manipulation on paraspinal activity during flexion-extension trunk movements. Forty nonspecific CLBP patients were randomised into two groups, manipulation (n = 20) and control (n = 20). While the manipulation group received high-velocity spine manipulation at the L4-L5 level, the control group remained lying in the same position. EMG-related variables, perceived pain intensity (100 mm VAS) and finger-floor distance were collected before and after spinal manipulation at the L4-L5 level. EMG surface signals from the right and left paraspinal muscles (L5-S1 level) were acquired during trunk flexion-extension cycles. EMG activity during the static relaxation phase was significantly reduced following intervention for the manipulation group but not for the control group. The extension-phase EMG activity was also reduced after manipulation, but the flexion-phase EMG levels remained unchanged. Accordingly, the percent changes in FRR and ERR were significantly larger for the manipulation group compared to the control. The results suggest that a high-velocity spinal manipulation is able to acutely reduce abnormal EMG activity during the full-flexion static phase and activation during the extension phase.

BACKGROUND AND OBJECTIVES:Osteopathic manipulation is indicated for pain, myofascial tensions and/or decreased movement amplitude. This study aimed at checking whether osteopathic manipulation with cervical rhythmic articulatory technique generates abnormal blood flow velocity oscillations or risks to internal carotid, vertebral and basilar arteries circulation.METHODS:The sample was made up of 58 individuals with chronic mechanical cervical pain (40 females and 18 males), with mean age of 36 years, submitted to internal carotid, vertebral and basilar arteries ultrasound before and after a single osteopathic manipulation with cervical rhythmic articulatory technique. Individuals were evaluated by ultrasound in three moments: control evaluation, rest control evaluation and study evaluation. Separation was sequential and methods were randomly and blindly applied.RESULTS:Ultrasound has shown no significant differences in the comparison of flow velocity variables means among evaluations. However, a slight increase in vertebral, intracranial and basilar arteries blood flow was observed after osteopathic manipulation with cervical rhythmic articulatory technique in the study evaluation, without statistical significance.CONCLUSION:In this studied population, osteopathic manipulation with cervical rhythmic articulatory technique has not generated significant blood flow velocity oscillation of internal carotid, vertebral and basilar arteries and has not posed risk to brain circulation. JUSTIFICATIVA E OBJETIVOS:Na presença de dor, tensões miofasciais e/ou redução da amplitude de movimento, é indicada a manipulação osteopática. O objetivo deste estudo foi verificar se a manipulação osteopática com técnica articulatória rítmica cervical gera oscilações anormais de velocidade de fluxo sanguíneo ou riscos à circulação das artérias carótidas internas, vertebrais e basilar.MÉTODOS:A casuística foi constituída por 58 indivíduos com cervicalgia mecânica crônica (40 mulheres, 18 homens) com idade média de 36 anos, submetidos a ultrassonografia das artérias carótidas internas, vertebrais e basilar antes e após única manipulação osteopática com técnica articulatória rítmica cervical. Os indivíduos foram analisados pela ultrassonografia em três momentos: exame controle, exame controle de repouso e exame estudo. A separação se deu de forma sequencial e os métodos de forma randomizada e encoberta.RESULTADOS:A ultrassonografia demonstrou que não existe diferença significativa em nenhum dos casos analisados, no comparativo das médias das variáveis de velocidade de fluxo entre os exames. Porém, foi observado um discreto aumento na velocidade de fluxo sanguíneo das artérias vertebral, intracraniana e basilar, após a manipulação osteopática com técnica articulatória rítmica cervical no exame estudo sem significância estatística.CONCLUSÃO:Nessa população estudada, a manipulação osteopática com técnica articulatória rítmica cervical não gerou significativa oscilação da velocidade de fluxo sanguíneo das artérias carótidas internas, vertebrais e basilar e não ofereceu risco à circulação cerebral.

Cervical spine manipulation (CSM) is a commonly utilized intervention, but its use remains controversial.To retrospectively analyze all available documented case reports in the literature describing patients who had experienced severe adverse events (AEs) after receiving CSM to determine if the CSM was used appropriately, and if these types of AEs could have been prevented using sound clinical reasoning on the part of the clinician.PubMed and the Cumulative Index to Nursing and Allied Health were systematically searched for case reports between 1950 and 2010 of AEs following CSM.Case reports were included if they were peer-reviewed; published between 1950 and 2010; case reports or case series; and had CSM as an intervention. Articles were excluded if the AE occurred without CSM (e.g. spontaneous); they were systematic or literature reviews. Data extracted from each case report included: gender; age; who performed the CSM and why; presence of contraindications; the number of manipulation interventions performed; initial symptoms experienced after the CSM; and type of resultant AE.Based on the information gathered, CSMs were categorized as appropriate or inappropriate, and AEs were categorized as preventable, unpreventable, or unknown. Chi-square analysis with an alpha level of 0.05 was used to determine if there was a difference in proportion between six categories: appropriate/preventable, appropriate/unpreventable, appropriate/unknown, inappropriate/preventable, inappropriate/unpreventable, and inappropriate/unknown.One hundred thirty four cases, reported in 93 case reports, were reviewed. There was no significant difference in proportions between appropriateness and preventability, P = .46. Of the 134 cases, 60 (44.8%) were categorized as preventable, 14 (10.4%) were unpreventable and 60 (44.8%) were categorized as 'unknown'. CSM was performed appropriately in 80.6% of cases. Death resulted in 5.2% (n = 7) of the cases, mostly caused by arterial dissection.There may have been discrepancies between what was reported in the cases and what actually occurred, since physicians dealing with the effects of the AE, rather than the clinician performing the CSM, published many of the cases.This review showed that, if all contraindications and red flags were ruled out, there was potential for a clinician to prevent 44.8% of AEs associated with CSM. Additionally, 10.4% of the events were unpreventable, suggesting some inherent risk associated with CSM even after a thorough exam and proper clinical reasoning.

Low Back Pain (LBP) is one of the main causes of disability in the whole world. In most cases, its presentation is considered benign and usually managed quite easily. However, in a low percentage of cases LBP is secondary to a serious pathology. Patients with Multiple Sclerosis (MS), for instance, are often affected also by LBP, and LBP could also be an initial symptom of MS. Moreover, patients with MS can present with a w ide range of clinical symptoms, as the Restless Legs Syndrome – RLS. In these specific cases, the ability to recognize a serious pathology is a key component of the physical therapist practice. Both conditions, nonspecific LBP and RLS, have been found in a patient that was referred with diagnosis of bilateral sciatica.

Vertebral artery injuries are identified more frequently now than in the past for both penetrating and blunt trauma. This is as a result of increased suspicion and awareness, liberal use of color flow Doppler, CT angiography, and traditional four-vessel angiography. The vast majority of patients who are not exsanguinating at the time of presentation can be managed nonoperatively or by angiographic embolization. Operative management should be reserved only for patients with severe active bleeding or when interventional radiology fails. The surgical exposure is complex and demands excellent knowledge of the local anatomy.
