2. Ectopic pregnancy on the uterosacral ligament post-double frozen embryo transfer in an endometriosis patient: case report and review of management and treatment options.

Abstract

BACKGROUND: Ectopic pregnancy (EP) is a serious obstetric condition that can be life-threatening, with various risk factors contributing to its development. In particular, in vitro fertilization (IVF) techniques may lead to an increased rate EP. Additionally, also endometriosis seems to be related to this particular obstetrical condition. CASE REPORT: We report a rare case of ectopic pregnancy on the uterosacral ligament (USL) along with a suspected coexisting tubal ectopic pregnancy following IVF. The patient is a 48-year-old woman in menopause, with a history of pelvic endometriosis, who experienced sudden abdominal pain and vomiting at eight weeks of amenorrhea after undergoing a double heterologous frozen embryo transfer. Thorough examination and pelvic ultrasound, we diagnosed a hemoperitoneum due to a suspected heterotopic EP on the left USL and contralateral tube. Due to the sudden worsening of the patient's condition, we opted for a surgical procedure. An urgent laparotomy revealed a severe hemoperitoneum caused by an EP implanted on the left USL and a malacic, bleeding contralateral tube, both of which were removed, and hemostasis was then guaranteed. The histopathologic exam confirmed the EP on the left USL and an edematous tube without product of conception (POC). CONCLUSIONS: Comparing our case with others reported in the current literature, it appears that the etiopathogenetic mechanisms leading to this urgent obstetrical condition are various and not fully understood. Despite those circumstances, the present case highlights the importance of considering non-tubal ectopic pregnancies in the context of risk factors, including IVF techniques, endometriosis, and advanced age, in cases of abdominal pain and hemoperitoneum after a single or double embryo transfer. The treatment, which involves different professional figures, should be executed as soon as possible, with the aim of preserving the patient's life and any future desire for pregnancies. Abstract

BACKGROUND: Literature shows that occurrence of comorbidities in people with severe acquired brain injury (sABI) is a common problem in rehabilitation stay. Consequently, patients could require an increase of interventions for diagnosis and treatment of clinical conditions, with a reduction of the rehabilitative take in charge for both clinical and organizational aspects. AIM: The first aim was to evaluate the rate of clinical conditions of sABI patients at admission in rehabilitation and the types of rehabilitative interventions performed in the first week; second objective was to explore the impact of clinical conditions on real rehabilitative take in charge. DESIGN: Cross sectional study. SETTING: Inpatient rehabilitation centers. POPULATION: The study included data from 586 sABI patients. METHODS: Collected data regarded anamnestic information, functional status assessed by means of Glasgow Outcome Scale, Levels of cognitive functioning, Early Rehabilitation Barthel Index, comorbidities at admission and type of rehabilitative interventions carried out in first week of rehabilitation stay. Spearman correlation coefficients were applied to detect possible correlations between the number of treatments in first week and clinical variables; through a multiple regression analysis the effect of patient’s characteristics on rehabilitative take in charge was explored. RESULTS: Data from the sABI patients: mean age 55.1±17.1 years; etiology of sABI was vascular in 315 patients (53.8%), anoxic in 83 (14.2%), neoplastic in 17 (2.9%), infectious in 15 (2.6%), traumatic in 150 (25.6%); 6 subjects (1%) presented a mixed etiology. Need of cardiorespiratory monitoring, pressure sores, infections or presence of multi drug resistant bacteria were the most frequent comorbidities. Abstract

Mountain building in the Al-Hajar Mountains (NE Oman) occurred during two major shortening stages, related to the convergence between Africa–Arabia and Eurasia, separated by nearly 30 Ma of tectonic quiescence. Most of the shortening was accommodated during the Late Cretaceous, when northward subduction of the Neo-Tethys Ocean was followed by the ophiolites obduction on top of the former Mesozoic margin. This shortening event lasted until the latest Santonian – early Campanian. Maastrichtian to Eocene carbonates unconformably overlie the eroded nappes and seal the Cretaceous foredeep. These neo-autochthonous post-nappe sedimentary rocks were deformed, along with the underlying Cretaceous tectonic pile, during the second shortening event, itself including two main exhumation stages. In this study we combine remotely sensed structural data, seismic interpretation, field-based structural investigations and apatite (U–Th)/He (AHe) cooling ages to obtain new insights into the Cenozoic deformation stage. Seismic interpretation indicates the occurrence of a late Eocene flexural basin, later deformed by an Oligocene thrusting event, during which the post-nappe succession and the underlying Cretaceous nappes of the internal foredeep were uplifted. This stage was followed by folding of the post-nappe succession during the Miocene. AHe data from detrital siliciclastic deposits in the frontal area of the mountain chain provide cooling ages spanning from 17.3 to 42 Ma, consistent with available data for the structural culminations of Oman. Our work points out how renewal of flexural subsidence in the foredeep and uplift of the mountain belt were coeval processes, followed by layer-parallel shortening preceding final fold amplification. Abstract

Background Literature shows that occurrence of comorbidities in people with severe acquired brain injury (sABI) is a common problem in rehabilitation stay. Consequently, patients could require an increase of interventions for diagnosis and treatment of clinical conditions, with a reduction of the rehabilitative take in charge for both clinical and organizational aspects. Aim The first aim was to evaluate the rate of clinical conditions of sABI patients at admission in rehabilitation and the types of rehabilitative interventions performed in the first week; second objective was to explore the impact of clinical conditions on real rehabilitative take in charge. Design Cross sectional study. Setting Inpatient rehabilitation centers. Population The study included data from 586 sABI patients. Methods Collected data regarded anamnestic information, functional status assessed by means of Glasgow Outcome Scale, Levels of cognitive functioning, Early Rehabilitation Barthel Index, comorbidities at admission and type of rehabilitative interventions carried out in first week of rehabilitation stay. Spearman correlation coefficients were applied to detect possible correlations between the number of treatments in first week and clinical variables; through a multiple regression analysis the effect of patient's characteristics on rehabilitative take in charge was explored. Results Data from the sABI patients: mean age 55.1±17.1 years; etiology of sABI was vascular in 315 patients (53.8%), anoxic in 83 (14.2%), neoplastic in 17 (2.9%), infectious in 15 (2.6%), traumatic in 150 (25.6%); 6 subjects (1%) presented a mixed etiology. Need of cardiorespiratory monitoring, pressure sores, infections or presence of multi drug resistant bacteria were the most frequent comorbidities. Passive mobilization, sitting positioning, arousal/awareness stimulation, evaluation and management of dysphagia were the interventions most frequently carried out in the first week. The regression analysis showed that severe neurological and clinical conditions, acute organ failure, cardio-respiratory instability and paroxysmal sympathetic hyperactivity significantly limit access to rehabilitative sessions. Conclusions In sABI patients clinical comorbidities requiring elevated care assistance are frequent at admission in rehabilitation from acute wards and may interfere with rehabilitative take in charge. Clinical rehabilitation impact The knowledge of clinical complexity of sABI patients may improve their care pathways, promoting early and appropriate transition from acute care to rehabilitation settings.

Abstract

Field data and seismic reflection profiles of various resolutions, calibrated by deep well logs, have been used to unravel the tectonic evolution of the Crati Basin (southern Italy). The study area is located in the northern portion of the Calabrian Arc, a well-developed arc-shaped feature of the circum-Mediterranean belts, consisting of a series of ophiolite-bearing tectonic units and overlying basement nappes. NW–SE oriented left-lateral strike-slip faults exerted a major control on the tectonic evolution of northern-central Calabria, from Middle Miocene to Lower Pleistocene times. Such faults, arranged in an en-echelon geometry and dissecting the pre-existing Late Oligocene–Early Miocene orogenic belt, led to a structural setting including major N-S striking synforms – as the offshore Paola Basin and the Crati Basin are interpreted based on our results – separated by a broad antiformal ridge. Since the Middle Pleistocene, both E- and W-dipping normal faults developed in the southernmost sector of the Crati Basin, probably as a consequence of both uplift of the orogenic edifice and Tyrrhenian back-arc extension. The pre-existing regional strike-slip faults became inactive in this sector of the belt. However, working as persistent barriers, it is envisaged here that they inhibited the southern propagation of the newly formed normal faults, which therefore propagated towards the north. A minimum value of cumulative displacement of ca. 600 m has been unraveled for the central sector of the Crati Basin since Middle Pleistocene times. This yields a vertical strain rate of ca. 0.9 mm/y during the last 700 ka.

Abstract

Richter syndrome (RS) represents the transformation of chronic lymphocytic leukaemia (CLL) to aggressive lymphoma. Risk factors of CLL transformation to RS are only partly known. We explored the role of the host genetic background as a risk factor for RS occurrence. Forty-five single nucleotide polimorphisms (SNPs) known to be relevant for CLL prognosis were genotyped in a consecutive cohort of 331 CLL, of which 21 had transformed to RS. After correcting for multiple testing and adjusting for previously reported RS risk factors, the LRP4 rs2306029 TT variant genotype was the sole SNP independently associated with a higher risk of RS transformation (Hazard Ratio: 4·17; P = 0·001; q = 0·047). The enrichment of LRP4 TT genotype in RS was confirmed in an independent series (n = 44) used for validation purposes. The LRP4 protein was expressed in CLL (n =66). Bioinformatic analysis scored LRP4 rs2306029 as a variant with possible deleterious and damaging variant of LRP4. LRP4 genotyping may help the recognition of patients with increased risk of RS at the time of CLL diagnosis.

Abstract

Objective The importance of prenatal pediatric surgical consultation has been recently emphasized. The impact of prenatal surgical consultations on the activity of a neonatal surgery unit and the capacity of tertiary prenatal ultrasound centers to detect false positives diagnosed by primary centers were investigated. Methods Review of a seven year experience at a prenatal diagnosis center separate from our hospital. All women with suspected fetal anomalies were scanned at least three times during pregnancy. Those anomalies assumed to be surgically correctable (except cardiac, urorenal and neurological ones) prompted monthly consultations at the center. All consultations were <> ones, done onsite, together with a sonogram. The relationship between prenatal consultations and surgical activity was analyzed. Results During the period 1992-1998, 4,123 scans were performed. Suspected anomalies were confirmed in 3120 fetuses (sensibility 75.6%, specificity 99.5%, false positive rate 24.3%). Of these, 523 fetuses were referred for surgical consultation (1554 surgical consultations). Anomalies included: thorax 24.17% (sequestration, CDH, CCAM, esophageal atresia, pleural effusion), abdomen 49.31% (cysts, intestinal atresia, echogenic bowel, calcifications); abdominal wall defects 9.16% (omphalocele, gastroschisis), tumors 8.79% (lymphangioma, teratoma, neuroblastoma); other defects 8.57%. Amniocentesis was performed in 89% cases. Fetal intervention was required in 12% (ovarian cysts [is greater than] 4.5 cm and pleural effusions). Pregnancy was terminated in 7.1% cases (omphalocele and lymphangioma).Timing and mode of delivery were planned in all cases. A steady increase in major anomalies antenatally diagnosed and treated in our unit was observed. We recorded a +371,6% increase in prenatal consultations and +415% increase in operation rate notwithstanding the natality decrease. Conclusion <> consultations increases the activity of neonatal surgery referral centers.
