Abstract

Aim: To compare the incidence of adverse events (AEs) related to antiobesity medications (AOMs; glucagon‐like peptide‐1 receptor agonists [GLP‐1RAs] vs. non‐GLP‐1RAs) after bariatric surgery. Methods: This single‐centre retrospective cohort included patients (aged 16–65 years) who had undergone laparoscopic Roux‐en‐Y gastric bypass or sleeve gastrectomy (cohort entry date) and initiated AOMs. Participants were categorized as users of US Food and Drug Administration (FDA)‐approved, off‐label, or GLP‐1RA AOMs if documented as receiving the medication on or after cohort entry date. Non‐GLP‐1RA AOMs were phentermine, orlistat, topiramate, canagliflozin, dapagliflozin, empagliflozin, naltrexone, bupropion/naltrexone and phentermine/topiramate. GLP‐1RA AOMs included: semaglutide, dulaglutide, exenatide and liraglutide. The primary outcome was AE incidence. Logistic regression was used to determine the association of AOM exposure with AEs. Results: We identified 599 patients meeting our inclusion criteria, 83% of whom were female. Their median (interquartile range [IQR]) age was 47.8 (40.9–55.4) years. The median duration of surgery to AOM exposure was 30 months. GLP‐1RAs use was not associated with higher odds of AEs: adjusted odds ratio (aOR) 1.1 (95% confidence interval [CI] 0.5–2.6) and aOR 1.1 (95% CI 0.6–2.3) for GLP‐1RA versus FDA‐approved and off‐label AOM use, respectively. AOM initiation ≥12 months after surgery was associated with lower risk of AEs compared to <12 months (aOR 0.01 [95% CI 0.0–0.01]; p < 0.001). Conclusion: Our results showed that GLP‐1RA AOMs were not associated with an increased risk of AEs compared to non‐GLP‐1RA AOMs in patients who had previously undergone bariatric surgery. 15. Non-Operative Approach to Contained Perforated Marginal Ulcers: A Systematic Review and Case Series

Abstract

Background Perforated marginal ulcers (PMUs) are a rare but known complication of bariatric surgery. Management typically involves prompt surgical intervention, but limited data exists on non-operative approaches. This study reviews published data on non-operative management of PMUs and presents a case series of patients who were managed non-operatively. Our hypothesis is that certain patients with signs of perforation can be successfully managed non-operatively with close observation.Methods We completed a systematic review searching PubMed, Embase, Web of Science, Cochrane, and clinicaltrials.gov. Ultimately 3 studies described the presentation and non-operative management of 5 patients. Additionally, we prospectively collected data from our institution on all patients who presented between Dec. 2022 and Dec. 2023 with PMUs confirmed on imaging and managed non-operatively.Results In our literature review, three patients had Roux-en-Y gastric bypass (RYGB), while two had one anastomosis gastric bypass. One patient required surgery two days after admission. Another underwent elective conversion surgery weeks later for a non-healing ulcer. Two received endoscopic interventions. One patient recovered with nil-per-os (NPO) status, and intravenous proton pump inhibitor (PPI) treatment. The patients in our case series presented with normal vital signs, an average of 30 months after RYGB, and with CT scan signs of perforation. None of these patients required surgical or endoscopic intervention.Conclusion In conclusion, while perforated marginal ulcers have traditionally been considered a surgical emergency, some patients can be successfully treated with non-operative management. More research is needed to identify the clinical presentation features, comorbidities, and imaging findings of this group.

Abstract

Background: Combination anti‐obesity medications (AOMs) to treat postoperative bariatric surgery weight regain have limited data on their use in the clinical setting. Understanding the optimal treatment protocol in this cohort will maximize weight loss outcomes. Methods: A retrospective review of bariatric surgery patients (N = 44) presenting with weight regain at a single academic multidisciplinary obesity center who were prescribed AOM(s) plus intensive lifestyle modification for 12 months. Results: Age: 28–76 years old, 93% female, mean weight 110.2 ± 20.3 kg, BMI 39.7 ± 7.4 kg/m2, presenting 5.2 ± 1.6 years post‐bariatric surgery [27 (61.4%), 14 (31.8%), and 3 (6.8%) laparoscopic Roux‐en‐Y gastric bypass (RYGB), laparoscopic vertical sleeve gastrectomy (VSG), and open RYGB, respectively], with 15.1 ± 11.1 kg mean weight gain from nadir. Mean weight loss after medical intervention at 3‐, 6‐, and 12‐month time points was 4.4 ± 4.6 kg, 7.3 ± 7.0 kg, and 10.7 ± 9.2 kg, respectively. At 12 months, individuals prescribed 3 or more AOMs lost more weight than those prescribed one (−14.5 ± 9.0 kg vs. −4.9 ± 5.7 kg, p < 0.05) irrespective of age, gender, number of comorbidities, initial weight or BMI, type of surgery, or GLP1 use. RYGB patients lost less weight overall (7.4% vs. 14.8% VSG respectively; p < 0.05). 28. Benefits of Adjuvant Medical Weight Loss Intervention in Setting of Weight Regain and Inadequate Weight Loss After Weight Loss Surgery

Abstract

Background Currently, there is no nationally accepted protocol for addressing weight regain or inadequate weight loss after MBS.Objectives To devise, implement, and evaluate a protocol targeting weight regain or inadequate weight loss in MBS patients at our institution.Setting Vanderbilt University Medical Center, Nashville, TN, United StatesMethods Patients at least 6 months following primary sleeve gastrectomy (SG) or Roux-en-Y gastric bypass (RYGB) who achieved or were trending toward <50% excess body weight loss or who regained ≥10% of their lowest postoperative weight, were identified and referred for medical weight loss (MWL) intervention. Exclusion criteria were body mass index (BMI) ≤ 27 kg/m2, treatment with adjustable gastric banding, and conversion from SG to RYGB.Results 2274 patients who were >6 months out from surgery were evaluated over 12 months. 93 patients (86% female) met criteria for inclusion. 69 (74%) patients agreed to intervention and were followed for an average of 165 days (SD 106.89 days), demonstrating a mean weight change of −5.11 kg (SD 6.86 kg), and BMI change of −1.81 kg/m2(SD 2.37 kg/m2). Patients who spent <90 days in a MWL program demonstrated less average weight loss (1.75 kg vs 6.48 kg) (P= .0042), and less change in BMI (−.63 kg/m2vs −2.29 kg/m2) (P= .0037) when compared to patients who spent >90 days in the MWL intervention.Conclusions This study identifies criteria for intervention in patients suffering weight regain or inadequate weight loss after MBS and demonstrates that standardized identification and referral for treatment results in modest weight loss.

29. Monitoring of CO2 Fluxes from Differing Vegetation Communities using the Eddy Covariance Approach

Abstract

Coastal vegetated ecosystems, including wetlands and saltmarsh, have been widely recognised as providing a range of environmental, social, and cultural benefits. They have the ability to store more carbon than nearly any other vegetated system, making them the target of increasing study due to the potential play a role in the mitigation of some effects of climate change. Wetland vegetation have been recognised for having a high carbon sequestration rate. But the carbon cycling dynamics between vegetation types in Australian saltmarshes is lesser known. Framed within a small wetland on the New South Wales South Coast, this research project aimed to monitor greenhouse gas fluxes and how environmental variables influence these fluxes. Monitoring was carried out through the erection of two eddy covariance flux towers which measure fluxes of CO2 from the wetland surface. Soil and water sampling was conducted to analyse salinity, bulk density, carbon content and pH, while measuring biomass across vegetation communities. Using these measurements, expected differences in fluxes between two vegetation communities with differing vegetation structures and compositions were identified. These two communities were differentiated by their dominant species, one by Sarcocornia quinqueflora, and the other by Juncus kraussii. The comparison of the two communities allowed for potential drivers of CO2 fluxes to be identified, based on the sampled environmental variables, and their location within the wider wetland itself. Fluxes of CO2 differed between the two monitored vegetation communities. This difference occurred when comparing both the 24-hour average of half-hourly flux data and the daytime-only half-hourly average flux data. The Juncus-dominated community had a stronger negative CO2 flux than the Sarcocornia-dominated community, indicating a stronger ability to drawn down more carbon out of the atmosphere (-0.078 ± 0.013 and -0.029 ± 0.0063 mg CO2 m-2 s-1 respectively). Abstract

Preference for patterns of outcomes that improve over time is termed negative time preference, in economics. In lay terms, this concept equates to "saving the best for last." Generally, adults tend to prefer to postpone their more preferred outcomes when options are presented as a sequence of events. Event sequencing seems particularly relevant for children who exhibit food refusal or selectivity. Preference for the sequencing of bites when an array involved preferred and relatively non‐preferred foods was evaluated. Participants experienced pre‐programed bite sequences that improved, worsened, or remained fixed across trials, and we assessed participants' preference for each of the bite sequences. Three of the four participants preferred bite sequences that began with a highly preferred food and either worsened or remained fixed over time, whereas one participant preferred the improving sequence of bite presentation, or in other words, one participant preferred to "save the best for last." Abstract

Molecular signaling events associated with the necroinflammatory changes in nonalcoholic steatohepatitis (NASH) are not well understood.To understand the molecular basis of NASH, we evaluated reversible phosphorylation events in hepatic tissue derived from Class III obese subjects by phosphoproteomic means with the aim of highlighting key regulatory pathways that distinguish NASH from non-alcoholic fatty liver disease (also known as simple steatosis; SS).Class III obese subjects undergoing bariatric surgery underwent liver biopsy (eight normal patients, eight with simple steatosis, and eight NASH patients). Our strategy was unbiased, comparing global differences in liver protein reversible phosphorylation events across the 24 subjects.Of the 3078 phosphorylation sites assigned (2465 phosphoserine, 445 phosphothreonine, 165 phosphotyrosine), 53 were altered by a factor of 2 among cohorts, and of those, 12 were significantly increased or decreased by ANOVA (P0.05).Statistical analyses of canonical signaling pathways identified carbohydrate metabolism and RNA post-transcriptional modification among the most over-represented networks.Collectively, these results raise the possibility of abnormalities in carbohydrate metabolism as an important trigger for the development of NASH, in parallel with already established abnormalities in lipid metabolism.
