In cases of chronic instability of the lateral ligament complex following an ankle sprain, operative stabilization should be considered when conservative treatment fails. The purpose of this study is to determine the outcome of a percutaneous stabilization of the lateral ligament complex with a gracilis tendon auto-graft, after an adjuvant arthroscopy of the ankle joint. We retrospectively reviewed the medical files of patients who underwent this surgery performed by the senior author. Between 2012 and 2015, 18 ankles were stabilized. Clinical results were assessed at final follow-up at a mean post-operative period of 25 months (range, 10-42 months). The mean post-operative AOFAS ankle-hindfoot score was 90 points (range, 48-100 points). The mean Karlsson Ankle Functional Score was 85 points (range, 37-100 points). The mean VAS score was 1.2 (range, 0-7). Concomitant procedures were performed on 14 out of 18 ankles. In conclusion, we state that this arthroscopically assisted percutaneous technique is a viable treatment option for chronic lateral ankle instability. It offers an alternative for the modified Broström procedure when tissue quality is poor and carries all the advantages of a minimally invasive procedure.

A survey of 43 cemented arthroplasties of the trapeziometacarpal joint with a la Caffinière prosthesis in 40 patients is presented. A 70% patient satisfaction rate was obtained, with a mean DASH score of 24.2. Good range of motion was preserved (opposition 9/10 on Kapandji's scale) and gripping force as well as key pinch force increased from 18 kg to 24 kg and from 5.3 to 6.0 kg respectively. However loosening was seen in 44% of the implants. Loosening was related to younger age. We conclude that despite good clinical outcome, radiographic loosening is a threat for the future of this implant.

The authors report the case of a 39-year-old woman with pain in the right dominant hand after initiating a crafts course. Clinical examination revealed tenderness on the volar side of the thumb and wrist. Simultaneous flexion of the distal interphalangeal joint of the index finger with flexion of the interphalangeal joint of the thumb was observed. Operative procedure confirmed the presumed diagnosis of an anomalous connection between the flexor pollicis longus and the flexor digitorum profundus tendon to the index finger, first described in 1979 by Linburg and Comstock.

Ligament reconstruction/tendon interposition arthroplasty (LRTIA) is a common procedure for basal joint osteoarthritis of the thumb. We evaluated 31 operations in 30 patients, 28 females, 2 males with a mean age of 57 years. The mean follow-up was 24 months (range 8 to 46 months). Pain reduction was 75% on average, with good preservation of motion and of first web space. Twenty-six patients were satisfied or very satisfied; five were not. The grip strength and key-pinch strength increased post-operatively, albeit not significantly.

We present the case of a 78-year-old rheumatoid patient with a bifocal stress fracture of the pubic rami after ipsilateral total knee arthroplasty. Many risk factors that may cause insufficiency fractures are combined in rheumatoid arthritis. These patients are used to chronic pain, and therefore the diagnosis is often made late. When a patient suffering from rheumatoid arthritis presents with recent pain in the pelvic or hip region after hip or knee arthroplasty, a stress fracture should always be suspected. A standard pelvic radiograph and skeletal scintigraphy establish the diagnosis. CT-scan may be useful in the differential diagnosis with less benign pathology. Rest and analgesic drugs are sufficient to let the fracture heal. Nonunion has never been reported for these fractures.

The authors report the case of an 8-year-old child presenting with painless swelling of the middle phalanx of the ring finger. Radiology demonstrated simple hypertrophy of cortical bone with a clear zone. Bone biopsy showed the typical features of osteoid osteoma. Osteoid osteoma is rarely painless and generally corresponds to a superficial tumour (hand, foot, skull and thorax).

The split posterior tibial tendon transfer procedure was first reported by Green for correction of equinovarus hindfoot deformity in patients with cerebral palsy. A modification of the split posterior tibial tendon transfer combined with an Achilles tendon lengthening is described in 17 children (21 procedures) with a minimum follow-up of 3 years. This modified technique is indicated in young children with a continuously spastic posterior tibial tendon to correct a dynamic equinovarus. It restores active dorsiflexion when the anterior tibial and extensor muscles are weak. The anterior half of the split tibialis posterior is transferred through the interosseus membrane to the dorsum of the foot. Excellent or good results and two poor results were noted after a mean follow-up of 29 months. In the patients with an excellent or good result, marked improvement of their equinovarus foot deformity in stance and swing phase of gait was seen. In two patients, the procedure failed because of technical errors.
