Abstract

Experimental results obtained along the banks of theSauris's Lake, during two different groups of observations (full and reducedbasin), are here resumed. The agreement between observed and precedent calculated data, isvery remarkably. A new theory, moreover, is here showed and for t h e firsttime applied to an artificial lake.As the values of the periods for w-nodal oscillations (n > 1) both inthe theory and in the observation have been higher t h a n these regardinga lake with even depth, we have applied the below explained theory to theSauris's Lake.I t is considered a basin whose longitudinal section forms a right-angledtriangle; the shorter cathetus figures the dam.The theoretical and the observed results among themselves perfectlyagreed.The above mentioned theory, because of the easy applications, willbe used for others similar lakes.

Abstract

Background The targeting accuracy of proton therapy (PT) for moving soft-tissue tumours is expected to greatly improve by real-time magnetic resonance imaging (MRI) guidance. The integration of MRI and PT at the treatment isocenter would offer the opportunity of combining the unparalleled soft-tissue contrast and real-time imaging capabilities of MRI with the most conformal dose distribution and best dose steering capability provided by modern PT. However, hybrid systems for MR-integrated PT (MRiPT) have not been realized so far due to a number of hitherto open technological challenges. In recent years, various research groups have started addressing these challenges and exploring the technical feasibility and clinical potential of MRiPT. The aim of this contribution is to review the different aspects of MRiPT, to report on the status quo and to identify important future research topics. Methods Four aspects currently under study and their future directions are discussed: modelling and experimental investigations of electromagnetic interactions between the MRI and PT systems, integration of MRiPT workflows in clinical facilities, proton dose calculation algorithms in magnetic fields, and MRI-only based proton treatment planning approaches. Conclusions Although MRiPT is still in its infancy, significant progress on all four aspects has been made, showing promising results that justify further efforts for research and development to be undertaken. First non-clinical research solutions have recently been realized and are being thoroughly characterized. The prospect that first prototype MRiPT systems for clinical use will likely exist within the next 5 to 10 years seems realistic, but requires significant work to be performed by collaborative efforts of research groups and industrial partners.

11. Multi atlas based segmentation approach for neurosurgery planning and guidance

Abstract

Neurosurgical plannining and guidance improve treatment effects while minimizing the procedure invasiveness. In this context, the accurate identification of brain’s anatomical structures is a fundamental step for succesful surgery outcome. However, manual tissue identification is a time consuming process, not compatible with the clinical routine. Moreover, it suffers from inter- and intra-subjects variability, thus making weak the entire process. In this work, we tested a new multi atlas based segmentation algorithm for neurosurgery applications. The algorithm is part of an open source software, Plastimatch, featuring registration and label fusion capabilities. The procedure was tested on 8 brain’s structures (both left and right putamen, thalamus, hippocampus and caudate) of 20 healthy subjets. Segmentation quality was evaluated in terms of Dice Similarity Coefficient (DSC) and Hausdorff Distance (HD) between surface meshes. The ground truth was represented by manually drawn contours. The median value of DSC was generally higher than 0.80 (with the exception of hippocampus) and HD was lower than 2 mm. The inter-quartile variability for DSC and HD ranged respectively from 0.73 to 0.95 and from 0.90 mm to 2.24 mm. In the case of hippocampus the median±quartiles for DSC and HD were 0.78±0.04 and 1.83±0.37 mm respectively. Finally, the computation time was on average 32 minutes, allowing a feasible clinical usage. In conclusion, the proposed methodology is proved to be transferable into the clinical routine, in order to segment anatomical structures for neurosurgery planning and guidance.

Abstract

Purpose: To improve the contouring of clinical target volume for the radiotherapy of neck Hodgkin/non-Hodgkin lymphoma by localizing the prechemotherapy gross target volume onto the simulation computed tomography using [18F]-fluorodeoxyglucose positron emission tomography/computed tomography.Material and Methods: The gross target volume delineated on prechemotherapy [18F]-fluorodeoxyglucose positron emission tomography/computed tomography images was warped onto simulation computed tomography using deformable image registration. Fifteen patients with neck Hodgkin/non-Hodgkin lymphoma were analyzed. Quality of image registration was measured by computing the Dice similarity coefficient on warped organs at risk. Five radiation oncologists visually scored the localization of automatic gross target volume, ranking it from 1 (wrong) to 5 (excellent). Deformable registration was compared to rigid registration by computing the overlap index between the automatic gross target volume and the planned clinical target volume and quantifying the V95coverage.Results: The Dice similarity coefficient was 0.80 ± 0.07 (median ± quartiles). The physicians’ survey had a median score equal to 4 (good). By comparing the rigid versus deformable registration, the overlap index increased from a factor of about 4 and the V95(percentage of volume receiving the 95% of the prescribed dose) went from 0.84 ± 0.38 to 0.99 ± 0.10 (median ± quartiles).Conclusion: This study demonstrates the impact of using deformable registration between prechemotherapy [18F]-fluorodeoxyglucose positron emission tomography/computed tomography and simulation computed tomography, in order to automatically localize the gross target volume for radiotherapy treatment of patients with Hodgkin/non-Hodgkin lymphoma.

Abstract

Abstract Background The purpose of this study is to investigate intra-fraction setup variability in hypo-fractionated cranial and body radiotherapy; this is achieved by means of integrated infrared optical localization and stereoscopic kV X-ray imaging. Method and Materials We analyzed data coming from 87 patients treated with hypo-fractionated radiotherapy at cranial and extra-cranial sites. Patient setup was realized through the ExacTrac X-ray 6D system (BrainLAB, Germany), consisting of 2 infrared TV cameras for external fiducial localization and X-ray imaging in double projection for image registration. Before irradiation, patients were pre-aligned relying on optical marker localization. Patient position was refined through the automatic matching of X-ray images to digitally reconstructed radiographs, providing 6 corrective parameters that were automatically applied using a robotic couch. Infrared patient localization and X-ray imaging were performed at the end of treatment, thus providing independent measures of intra-fraction motion. Results According to optical measurements, the size of intra-fraction motion was (median ± quartile) 0.3 ± 0.3 mm, 0.6 ± 0.6 mm, 0.7 ± 0.6 mm for cranial, abdominal and lung patients, respectively. X-ray image registration estimated larger intra-fraction motion, equal to 0.9 ± 0.8 mm, 1.3 ± 1.2 mm, 1.8 ± 2.2 mm, correspondingly. Conclusion Optical tracking highlighted negligible intra-fraction motion at both cranial and extra-cranial sites. The larger motion detected by X-ray image registration showed significant inter-patient variability, in contrast to infrared optical tracking measurement. Infrared localization is put forward as the optimal strategy to monitor intra-fraction motion, featuring robustness, flexibility and less invasivity with respect to X-ray based techniques.
