Importance The effects of recruitment maneuvers and positive end-expiratory pressure (PEEP) titration on clinical outcomes in patients with acute respiratory distress syndrome (ARDS) remain uncertain. Objective To determine if lung recruitment associated with PEEP titration according to the best respiratory-system compliance decreases 28-day mortality of patients with moderate to severe ARDS compared with a conventional low-PEEP strategy. Design, Setting, and Participants Multicenter, randomized trial conducted at 120 intensive care units (ICUs) from 9 countries from November 17, 2011, through April 25, 2017, enrolling adults with moderate to severe ARDS. Interventions An experimental strategy with a lung recruitment maneuver and PEEP titration according to the best respiratory–system compliance (n = 501; experimental group) or a control strategy of low PEEP (n = 509). All patients received volume-assist control mode until weaning. Main Outcomes and Measures The primary outcome was all-cause mortality until 28 days. Secondary outcomes were length of ICU and hospital stay; ventilator-free days through day 28; pneumothorax requiring drainage within 7 days; barotrauma within 7 days; and ICU, in-hospital, and 6-month mortality. Results A total of 1010 patients (37.5% female; mean [SD] age, 50.9 [17.4] years) were enrolled and followed up. At 28 days, 277 of 501 patients (55.3%) in the experimental group and 251 of 509 patients (49.3%) in the control group had died (hazard ratio [HR], 1.20; 95% CI, 1.01 to 1.42; P = .041). Compared with the control group, the experimental group strategy increased 6-month mortality (65.3% vs 59.9%; HR, 1.18; 95% CI, 1.01 to 1.38; P = .04), decreased the number of mean ventilator-free days (5.3 vs 6.4; difference, −1.1; 95% CI, −2.1 to −0.1; P = .03), increased the risk of pneumothorax requiring drainage (3.2% vs 1.2%; difference, 2.0%; 95% CI, 0.0% to 4.0%; P = .03), and the risk of barotrauma (5.6% vs 1.6%; difference, 4.0%; 95% CI, 1.5% to 6.5%; P = .001). There were no significant differences in the length of ICU stay, length of hospital stay, ICU mortality, and in-hospital mortality. Conclusions and Relevance In patients with moderate to severe ARDS, a strategy with lung recruitment and titrated PEEP compared with low PEEP increased 28-day all-cause mortality. These findings do not support the routine use of lung recruitment maneuver and PEEP titration in these patients. Trial Registration clinicaltrials.gov Identifier:NCT01374022

- Abstract
IMPORTANCE: The effects of recruitment maneuvers and positive end-expiratory pressure (PEEP) titration on clinical outcomes in patients with acute respiratory distress syndrome (ARDS) remain uncertain. OBJECTIVE: To determine if lung recruitment associated with PEEP titration according to the best respiratory-system compliance decreases 28-day mortality of patients with moderate to severe ARDS compared with a conventional low-PEEP strategy. DESIGN, SETTING, AND PARTICIPANTS: Multicenter, randomized trial conducted at 120 intensive care units (ICUs) from 9 countries from November 17, 2011, through April 25, 2017, enrolling adults with moderate to severe ARDS. INTERVENTIONS: An experimental strategy with a lung recruitment maneuver and PEEP titration according to the best respiratory-system compliance (n = 501; experimental group) or a control strategy of low PEEP (n = 509). All patients received volume-assist control mode until weaning. MAIN OUTCOMES AND MEASURES: The primary outcome was all-cause mortality until 28 days. Secondary outcomes were length of ICU and hospital stay; ventilator-free days through day 28; pneumothorax requiring drainage within 7 days; barotrauma within 7 days; and ICU, in-hospital, and 6-month mortality. RESULTS: A total of 1010 patients (37.5% female; mean [SD] age, 50.9 [17.4] years) were enrolled and followed up. At 28 days, 277 of 501 patients (55.3%) in the experimental group and 251 of 509 patients (49.3%) in the control group had died (hazard ratio [HR], 1.20; 95% CI, 1.01 to 1.42; P = .041). Compared with the control group, the experimental group strategy increased 6-month mortality (65.3% vs 59.9%; HR, 1.18; 95% CI, 1.01 to 1.38; P = .04), decreased the number of mean ventilator-free days (5.3 vs 6.4; difference, −1.1; 95% CI, −2.1 to −0.1; P = .03), increased the risk of pneumothorax requiring drainage (3.2% vs 1.2%; difference, 2.0%; 95% CI, 0.0% to 4.0%; P = .03), and the risk of barotrauma (5.6% vs 1.6%; difference, 4.0%; 95% CI

33rd European Photovoltaic Solar Energy Conference and Exhibition; 2921-2925, The project Sustainable School proposed by evaluating the environmental performance of the State High School Padre Nunes, in the City of Gravataí, State of Rio Grande do Sul, the preparation of a report containing suggestions for action in the management, teaching and school extension to provide support taking decisions by the school administration for the purpose of reducing the environmental impact of their actions and the creation of a map bringing together all the proposed changes in the school environment, including sustainable structures as educators elements. Building a tank with a capacity of 16 thousand liters of rainwater was carried out, construction of a solar thermal panel as water heaters with a capacity of 146 liters of clean water for use in school kitchen, installation of a photovoltaic module supply the pressurize for non-potable water distribution network supplied by the rainwater tank, several interventions to revitalize the school yard as educational mosaics and paintings, conducting agro-ecological planting structures as vertical gardens, herb spiral, compost, among others. In addition, training workshops were carried out with the scholarship students and the courses of different levels.

Background: Our aim was to explore relevance of the proportion between neoplastic cell component and tumor-associated stroma in order to assess its association with confirmed aggressive phenotypes of right/left colon and rectum cancers in a large series of patients. Methods: The quantification of stroma component was performed in patients diagnosed with colorectal adenocarcinoma who underwent surgical resection. The analyzed variables were age, gender, anatomical/pathological features, and tumor-stroma proportion. Tumor-stroma proportion was estimated based on slides used in routine pathology for determination of T status and was described as low, with a stromal percentage ≤50% or high, with a stromal percentage >50%. The tumor-stroma proportion was estimated by two observers, and the inter-observer agreement was assessed. Results: The sample included 390 colorectal adenocarcinoma patients. Stroma-rich tumors were observed in 53.3% of cases. Well-differentiated tumors had the lowest stromal proportions (p = 0.028). Stroma-poor tumors showed less depth of invasion (p

El cuidado sensible contempla la razón sensible sensible, la creatividad, la estética, la solidaridad, la libertad de asociación en relación con el cuidado humano. Objetivos: Reflexionar sobre las demandas del cuidado de enfermería en el orden sensible y Analizar los enfoques profesionales relativos a la sensibilidad en el cuidado. Metodología: Se trata de un estudio reflexivo-analítico realizado a partir de una revisión bibliográfica sobre el cuidado sensible y la sensibilidad. Levantamiento realizado en la Biblioteca Virtual de Saúde - BVE, de publicaciones de los últimos diez años, se encontraron 23 publicaciones indirectas, en textos completos en la literatura nacional, identificados con los descriptores: Enfermería, Cuidados de Enfermería y Humanización de la Asistencia. Resultados: El estudio reveló que el cuidado sensible a pesar de no ser descrito en la literatura de enfermería, se identifica con la solidariedad, amor y afectividad de los contactos profesionales; en la disponibilidad, en la entrega, en la escucha atenta; en el diálogo objetivo e interesado, en la conversación dedicada, em la conversación terapéutica o todo aquello que implica el diálogo afectivo en la práctica humanizada del cuidado. Conclusiones: Los resultados destacan la necesidad de cambios de paradigma en el cuidado, de otra manera más orgánica, afectiva, más humana, que invierta en la subjetividad del ser, valorando lo vivido, la complejidad de los sentimientos que acompañan los desórdenes físicos y emocionales del usuario de los servicios de salud. Objectives: To reflect on the problems of nursing in the sensitiveble care approach and analyze some ways forto approach professionals to approachon the sensitivity care. Methodology: This is a reflective study conducted from a literature review ofin texts on about sensitiveble care and sensitivity. The survey usedwas done in the database in the Virtual of Health Library in the last decadeand found 23 full text publications in national literature., Descriptors related to: Nursing, Nursing Care, and Humanized Assistance. Results: The study shows that sensitiveble care, although not a descriptor in the nursing literature, is expressed in solidarity, love and professional contacts, in the availability, dispositionsal, with one attentive listening, and interested in dialogue, in the conversation-dedicated therapy, and in everything that involves the affective dialogue. Conclusions: The study revealedevoked the need for paradigm shifts in nursing care, towards a another order more organic, affective, more humane order, that invests in the subjectivity, that valuesvaluing the lived complexity of feelings that accompany the physical and emotional disorders of the user of the consumer's healthy service.

Attempts are being made to find a cure or at least a better treatment that can slow down the progression of Parkinson?s disease, but recently, no intervention has been discovered. A better understanding of the basic mechanisms that underlie this disease is of great importance for the development of new drugs. Lifestyle factors, such diet, can influence different biological mechanisms involved in PD, such as the purinergic system, the dopaminergic system, neuroinflammation and microbiota. In this thesis, these factors will be explored in different animal models. In Chapter 2, the interaction of adenosine receptors with dopaminergic receptors and the potential role of adenosine receptor ligands in the treatment of PD are reviewed. In this chapter, the potential relation of adenosine with lifestyle and diabetes is discussed as well. In Chapter 3, we investigated the interaction of purinergic and dopaminergic receptors in a zebrafish model of PD. After intra-encephalic injection of 6-OHDA, zebrafish presented behavioral changes and slight effect on the dopaminergic system. In Chapter 4, we investigated the feasibility of in vivo PET imaging in living healthy adult zebrafish and in a zebrafish model of inflammation. This study demonstrated that in vivo PET imaging with 18F-FDG and 18F-NaF in living zebrafish is feasible and differences in uptake can be seen in a model of inflammation. In Chapter 5, we analyzed the impact of a cafeteria diet on the reward system in rats. Our study suggests that D2 receptors play a role in obesity and consumption of a cafeteria diet, but no alterations after challenge was observed. To elucidate the influence of a high fat diet (HFD) on PD progression, we investigated the effect of a HFD on the availability of D2 receptors and behavioral parameters in rats, in Chapter 6. The HFD aggravated the damage in the PD model, suggesting a detrimental role of the HFD on the onset or progression of PD. In Chapter 7, we suggest that the documented impact of HFD on neuroinflammation may be mediated by the gut microbiome in the 6-OHDA rat model of PD and it is independent of peripheral inflammation. The knowledge of the basic mechanisms underlying PD and their relationship with changes in lifestyle can help scientists to better understand the factors that trigger the onset of the disease and aid the development of new treatments and diagnostic tools. These studies can help reveal of the interaction between behavior, microbiota, dopaminergic and purinergic response, and inflammation in the gut and the brain. Uma melhor compreens?o dos mecanismos b?sicos que est?o por tr?s dessa doen?a ? de grande import?ncia para o desenvolvimento de novos medicamentos. Fatores do estilo de vida, como a dieta, podem influenciar diversos mecanismos biol?gicos envolvidos na DP, como o sistema purin?rgico, o sistema dopamin?rgico, a neuroinflama??o e a microbiota. Nesta tese, esses fatores ser?o explorados em diferentes modelos animais. 13. Gatekeeping Through Remote Family Physician Consultations in Tertiary University Hospital During the COVID-19 Pandemic: A Cross-Sectional Study.

- Abstract
Objective : To explore the potential of gatekeeping for specialized consultations and patient care via remote interactions with family physicians. Methods: This cross-sectional study was conducted at a tertiary hospital between November 2020 and December 2021, when specialized consultations were canceled due to the COVID-19 pandemic. Patients who were evaluated for remote consultation with family physicians were included. Remote consultations were done by a family physician team facilitated through the TelessaúdeRS-UFRGS service. The primary outcome was potential patients eligible for discharge from specialty ambulatory to primary care. Data Sources and Analytic Sample : Data were collected from hospital records. Candidates for remote consult included stable health conditions, indicating the absence of acute or decompensated symptoms as reported in the consult request via the online platform, absence of necessity for any medical procedures or scheduled surgeries, and absence of time-sensitive situations. The prevalence of the outcome was estimated at a corresponding 95% confidence interval. The chi-square test compared the outcome according to COVID-19 mortality waves and specialty groups. Results : At the outset, 2,429 consultations were assessed against the study's eligibility criteria. Among these, 2,160 consultations were included, of whom 776 were candidates for family physician team consultation. Subsequently, the remote family physicians team conducted 557 (23% of the original sample) consultations. Overall, 10% (95% CI: 9-11) had the potential to be discharged from specialty care. Patients' age was linked to discharged likelihood. Prevalence rates varied across specialty groups (highest in surgical patients) and COVID-19 waves (highest in the second wave). Conclusions : This study, conducted within a tertiary hospital's specialty outpatient clinic, highlights the potential of remote consultations with a family physician team in identifying cases suitable for management in primary care settings. Our findings demonstrate that 10% of cases assessed through remote consultations exhibited potential for primary care management.

- Abstract
Introduction: Death is a natural phenomenon inherent to the human condition and an inevitable consequence of life. As future professionals, nursing students should face the process of death and recognize their limitations, because emotional aspects can influence the quality of health care, especially in Spanish-speaking countries. This study aimed to quantify the level of fear of death among nursing students in a higher education institution in a region of Colombia. Methods: A quantitative correlational study including 258 university students selected by stratified sampling were administered the Collet-Lester Fear of Death Scale; the results were analyzed for the age, religion, marital status, number of children, and academic level of the respondents. Results: On average, the students were 22.9 years old; 83.3% of them were female, 75.6% were Catholic, 14.1% had at least one child, and 66.7% had experienced bereavement in the family. An association was found between the female gender and the fear of one's own death ( p = .025). - Abstract
This cross-sectional study aims to describe doubts regarding the diagnosis and treatment of syphilis in pregnancy among primary care professionals in a telehealth service. All teleconsultations (TCs) offered through TelessaúdeRS-UFRGS to primary health care (PHC) services in the state of Rio Grande do Sul between 2018 and 2021 involving syphilis in pregnancy were included. A total of 356 (TCs) were analyzed. The main doubts about syphilis during pregnancy raised by primary care professionals were related to the need for retreatment (35%), diagnostic definition (23%) and initial treatment (16%). In addition, 95% of TCs were suitable for diagnosing and treating syphilis based on the 2020 Brazilian Ministry of Health guideline. - Abstract
Several standardized systems for nongynecological cytopathology have been published following the successful implementation of The Bethesda System for Reporting Cervical Cytology. Each of these systems comprises a set of reporting categories accompanied by a risk of malignancy. However, in most cases, these risk of malignancy estimates have not been based on high-quality evidence and often may not be consider proper "risks" (because they have been estimated based on cross-sectional studies). This commentary discusses the problems related to the data used to generate these risks. To make nongynecological cytopathology reporting more evidence-based, large-scale prospective cohort studies and randomized trials, in addition to high-quality systematic reviews and meta-analyses, should be performed., (© 2023 American Cancer Society.)

18. Comprehensive Genomic Studies on the Cell Blocks of Pancreatic Cancer.

- Abstract
Pancreatic cancer is one of the deadliest malignancies, characterized by late-stage diagnosis and limited treatment options. Comprehensive genomic profiling plays an important role in understanding the molecular mechanisms underlying the disease and identifying potential therapeutic targets. Cell blocks (CBs), derived from EUS-FNA, have become valuable resources for diagnosis and genomic analysis. We examine the molecular profile of pancreatic ductal adenocarcinoma (PDAC) using specimens obtained from CB EUS-FNA, across a large gene panel, within the framework of next-generation sequencing (NGS). Our findings revealed that over half (55%) of PDAC CB cases provided adequate nucleic acid for next-generation sequencing, with tumor cell percentages averaging above 30%. Despite challenges such as low DNA quantification and degraded DNA, sequencing reads showed satisfactory quality control statistics, demonstrating the detection of genomic alterations. Most cases (84.6%) harbored at least one gene variant, including clinically significant gene mutation variants such as KRAS, TP53, and CDKN2A. Even at minimal concentrations, as long as the extracted DNA is of high quality, performing comprehensive molecular profiling on PDAC samples from cell blocks has remained feasible. This strategy has yielded valuable information about the diagnosis, genetic landscape, and potential therapeutic targets, aligning closely with a precision cytopathology approach.

Introduction: The characterization of tumor microenvironment (TME) related factors and their impact on tumor progression have attracted much interest. We investigated cancer cells and cancer-associated fibroblasts (CAFs) to evaluate biomarkers that are associated with neoplastic progression, observing them in different interface zones of colorectal cancer., Methods: On 357 CRC tissue microarrays, using immunohistochemistry, we examined the associations of podoplanin and α-SMA expressed in cancer cells and CAFs and evaluated them in different areas: tumor core, invasive front, tumor budding, tumor-stroma ratio (TSR) scoring, and desmoplastic stroma., Results: CAFs expressing α-SMA were found in more than 90% of the cases. Podoplanin+ was detected in cancer cells and CAFs, with positivities of 38.6% and 70%, respectively. Higher α-SMA+ CAFs and podoplanin+ cancer cells were observed predominantly at the TSR score area: 94.3% and 64.3% of cases, respectively. The status of podoplanin in CAFs+ was higher in the desmoplastic area (71.6%). Stroma-high tumors showed increased expression of α-SMA and podoplanin in comparison with stroma-low tumors. The status of podoplanin in cancer cells was observed in association with lymphatic invasion and distant metastasis., Conclusion: The substance of the CRC was composed predominantly of the surrounding stroma-α-SMA+ CAFs. Podoplanin expressed in the prognosticator zones was associated with unfavorable pathological features. 20. Tissue management in precision medicine: What the pathologist needs to know in the molecular era.

- Abstract
Precision medicine is "an emerging approach for disease treatment and prevention that takes into account individual variability in genes, environment, and lifestyle for each person." Among many medical specialists involved in precision medicine, the pathologists play an important and key role in the implementation and development of molecular tests that are in the center of decision of many therapeutic choices. Besides many laboratory procedures directly involved in the molecular tests, is fundamental to guarantee that tissues and cells collected for analysis be managed correctly before the DNA/RNA extraction. In this paper we explore the pivotal and interconnected points that can influence molecular studies, such as pre-analytical issues (fixation and decalcification); diagnosis and material selection, including the calculation of nuclei neoplastic fraction. The standardization of sample processing and morphological control ensures the accuracy of the diagnosis. Tissue or cytological samples constitutes the main foundation for the determination of biomarkers and development of druggable targets. Pathology and precision oncology still have a long way to go in terms of research and clinical practice: improving the accuracy and dissemination of molecular tests, learning in molecular tumor boards for advanced disease, and knowledge about early disease. Objective: to understand the perception of the multiprofessional team about the quality of health care provided to patients in palliative care in the Intensive Care Unit., Method: qualitative study, anchored to Donabedian's theoretical framework, through semi-structured interviews with 35 professionals working in the Intensive Care Unit. For data analysis, the Content Analysis technique was used., Results: three categories were pre-established: structure, process, and outcome, from which five subcategories emerged: Deficit in terms of numbers of workers and professional qualification; Ambience and palliative care; (In)existence of assistance based on the principles of palliative care; Failures in communication and in the interdisciplinary approach and Repercussions of (lack of) assistance., Conclusion: the study allowed understanding the institutional weaknesses for the operationalization of care provided to patients eligible for palliative care in the Intensive Care Unit setting. Thus, for this philosophy of care to be propagated, the co-participation of managers, professionals, patients, and family members is required, since these gaps cannot be filled without collective involvement.

- Abstract
We assessed the associations of social distancing and mask use with symptomatic, laboratory-confirmed severe acute respiratory syndrome coronavirus 2 infection in Porto Alegre, Brazil. We conducted a population-based case-control study during April-June 2020. Municipal authorities furnished case-patients, and controls were taken from representative household surveys. In adjusted logistic regression analyses of 271 case-patients and 1,396 controls, those reporting moderate to greatest adherence to social distancing had 59% (odds ratio [OR] 0.41, 95% CI 0.24-0.70) to 75% (OR 0.25, 95% CI 0.15-0.42) lower odds of infection. Lesser out-of-household exposure (vs. going out every day all day) reduced odds from 52% (OR 0.48, 95% CI 0.29-0.77) to 75% (OR 0.25, 95% CI 0.18-0.36). Mask use reduced odds of infection by 87% (OR 0.13, 95% CI 0.04-0.36). In conclusion, social distancing and mask use while outside the house provided major protection against symptomatic infection.

- Abstract
Purpose: To determine whether teleophthalmology can help physicians in assessing and managing eye conditions and to ascertain which clinical conditions can be addressed by teleophthalmology in primary care setting., Methods: We evaluated the resolution capacity of TeleOftalmo, strategy implemented in the public health system of southern Brazil. Resolution capacity was defined as the ability to fully address patients' eye complaints in primary care with remote assistance from ophthalmologists. Data from tele-eye reports were collected over 14 months. Resolution capacity was compared across different age groups and different ocular conditions., Results: Overall, 8,142 patients had a tele-eye report issued in the study period. Resolution capacity was achieved in 5,748 (70.6%) patients. When stratified into age groups, the lowest capacity was 43.1% among subjects aged ≥65 years, while the highest was 89.7% among subjects aged 13-17 years (p<0.001). Refractive error (70.3%) and presbyopia (56.3%) were the most prevalent conditions followed by cataract (12.4%) and suspected glaucoma (7.6%). Resolution capacity was higher in cases of refractive error, presbyopia, spasm of accommodation and lid disorders than in patients diagnosed with other condition (p<0.001)., Conclusions: With telemedicine support, primary care physicians solved over two-thirds of patients' eye or vision complaints. Refractive errors had high case resolution rates, thus having a great impact on reducing the number of referrals to specialty care.
