Natural polysaccharides, e.g., starch, cellulose and sodium alginate have been highlighted as unconventional chromophores owing to their chain structures containing clustered electron-rich groups and the rigidification imposed by inter/intramolecular interactions. On account of the abundant hydroxyl groups and dense packing of low-substituted (< 5 %) mannan chains, we have investigated the laser-induced fluorescence of mannan-rich vegetable ivory seeds (Phytelephas macrocarpa), both in the native state and after thermal aging. The untreated material emitted fluorescence at 580 nm (yellow-orange) when excited at 532 nm (green). This luminescence is intrinsic to the polysaccharide matrix abundant in crystalline homomannan, as demonstrated by lignocellulosic analyses, fluorescence microscopy, NMR, Raman, FTIR and XRD. Thermal aging at 140 °C and above intensified the yellow-orange fluorescence and caused the material to fluoresce when excited by a near-infrared laser (785 nm). In view of the clustering-triggered emission mechanism, the fluorescence of the untreated material can be attributed to hydroxyl clusters and the conformational rigidification in mannan I crystals. On the other hand, thermal aging caused dehydration and oxidative degradation of mannan chains, inducing the substitution of hydroxyl groups by carbonyls. Context: In the absence of any specific treatment available for COVID-19, people started practicing traditional nonpharmacological preventive home remedies such as salt water gargling and steam inhalation. The available research evidence on some of these measures opines that steam inhalation, saline gargling, and povidone-iodine gargling does have virucidal properties and do provide symptomatic relief., Aims: The aim is to test this hypothesis, and the present trial was undertaken with an objective to assess the effect of steam inhalation, saline gargling, and povidone-iodine gargling among the COVID-19-positive patients with respect to early test negativity and clinical recovery., Methodology: Open-labeled, parallel, randomized controlled trial was conducted among asymptomatic or mild COVID-19-positive patients in Bangalore from September 2020 to February 2021. In each group of steam inhalation, saline gargling, povidone-iodine gargling, and control, twenty participants were allocated. Daily follow-up was done for 21 days to assess early test negativity and clinical recovery. Trial Registry Number: Clinical Trial Registry India/2020/09/027687., Results: Among 80 participants recruited, 65 (81.3%) were symptomatic. Early test negativity was seen in povidone-iodine gargling group of 6 days (KaplanMeier survival curve, BreslowGeneralized Wilcoxon test P = 0.7 as per the intention-to-treat and as per-protocol P = 0.8). Schistosomiasis is a neglected disease that affects millions of people around the world, being common in the state of Maranhão. A total of 225 rodents of the Holochilus sciureus species from the Western Lowland Maranhão were studied, of which 144 animals (64%) exhibited Schistosoma eggs in their feces samples. Macroscopic lesions characterized as well-defined whitish areas on the liver and spleen surfaces were observed. Histopathological examination revealed multifocal granulomas in the esophagus, liver, spleen, pancreas and duodenum, with structures compatible with Schistosoma mansoni eggs, as well as severe hepatic micro-vacuolar degeneration, multifocal and coalescent, with proliferation of random bile ducts and associated epithelial hyperplasia to areas of fibrosis. Adult forms of the parasite were observed in the blood vessels of the portal space. The lungs exhibited moderate and diffuse interstitial pneumonia with intralesional S. mansoni eggs. In the kidneys, hyaline cylinders were observed in the pelvis and diffuse hemorrhage. In conclusion, H. sciureus displays a pathological picture similar to human being. This rodent plays a role as sentinel in Baixada Maranhense.

We investigated the effects of acetylsalicylic acid (ASA) on the total myenteric neuronal population in the descending colon in Trypanosoma cruzi-infected mice. Thirty-five male Swiss mice, 60 days old, were divided into a control group (C group), control group treated with ASA (CA group), infected group (I group), and infected group treated with ASA (IA group). A total of 1300 trypomastigotes of the Y strain of T. cruzi were intraperitoneally inoculated in the IA and I groups. The CA and IA groups were treated with ASA intraperitoneally. At 75 days post-infection (dpi), all of the animals were sacrificed. Neurons in the colon were stained with Giemsa, quantified, and measured. No difference in the course of infection was observed between the IA and I groups, reflected by the parasitemia curve. Acetylsalicylic acid treatment in the CA and IA groups did not alter the total number of myenteric neurons compared with the C and I groups. The CA and IA groups exhibited an increase in the nuclear area, cytoplasmic area, and neuronal body area compared with the C and I groups. Future studies should elucidate the mechanism of action of ASA against Chagas' disease in the chronic phase.

Objective: To describe occurrences of septic glenohumeral arthritis among patients with arthropathy of the rotator cuff, and to highlight the importance of correct diagnosis and surgical procedures., Methodology: Eight surgical drains were installed in seven patients with glenohumeral pyoarthritis. All the patients presented arthropathy of the rotator cuff (four males and three females). Six patients presented pyoarthritis in the dominant shoulder. The age range was from 53 to 93 years (mean: 74 years). The mean duration of the symptoms before the surgical lavage was six weeks. Six patients underwent treatment consisting of a combination of arthroscopic irrigation and debridement, and one patient was treated by means of open arthrotomy. All the patients received systemic antibiotic therapy in accordance with their bacterial sensitivity., Results: All seven patients achieved satisfactory results, taking into consideration especially the improvement of pain and the patients' satisfaction. The functional assessment was performed using the University of California Los Angeles (UCLA) scale. Only one patient needed to go through another arthroscopic procedure. Staphylococcus aureus was isolated from four cultures and Escherichia coli from one culture. There were two situations in which the patients used empirical antibiotic therapy and the cultures showed negative results. Among the associated procedures, tenotomy of the biceps was performed in four cases, resection of the lateral third of the clavicle due to osteomyelitis in one case and arthrotomy of the knee in one case., Conclusion: Surgical treatment was effective in the cases of arthritis associated with arthropathy of the rotator cuff. In patients with arthropathy of the rotator cuff and subclinically altered laboratory signs, the possibility of pyoarthritis should always be suspected.

Objective: To describe the frequency and types of tracheal intubation complications and their main causes., Methods: Cross sectional study of patients who were submitted to tracheal intubation for more than 24 hours at the Pediatric ICU of Santa Casa de Misericórdia de São Paulo, between May 1998 and December 1999. Exclusion criteria were previous intubations, surgeries or traumas in the cervical region or oropharynx., Results: A study of 147 patients with ages varying from 1 month to 15 years and 3 months was carried out. An inadequate tracheal tube had been used in 31.3% of patients submitted to tracheal intubation and 14.3% needed 5 or more attempts to achieve intubation. Resident physicians had more difficulty with intubation. Most tracheal intubation attempts were related to increased traumas, hypoxia, bradycardia and worsening of the Downes score after extubation. Accidental extubation was observed in 21.8%, related to worsening in the score of Downes and need for reintubation. The resident physicians also caused a higher number of traumas and bradycardia., Conclusion: Most complications may be attributed to lack of experience and training of the physician performing the tracheal intubation. We should implement training programs and increase supervision during tracheal intubation to minimize these outcomes.

Unlabelled: The radial artery catheterization with the purpose of children's monitoring has been more and more used in Pediatric Intensive Care Units, and many times, is use is indispensable, like in cardiac surgery post-operative cases. However, there are only a few articles on the subject among us., Purpose: To analyse the complications of radial artery catheterization in the post-operative of cardiac surgery in children., Methods: We studied the complications of this procedure in 120 children in cardiac surgery post-operative range from 1 month to 2 years. The children were catheterized by percutaneous technique (n = 67) and by cutdown technique (n = 53). The following complications were analysed in relation to the placement time of the catheter and the used technique: local infection, hemorrhagic complication, ischemia, catheter occlusion and accidental catheter displacement., Results: Infection, ischemia and hemorrhage were more frequent and statistically significant in patients submitted to cutdown technique, while the catheter occlusion and accidental displacement were similar in both groups. The placement time of the catheter was an important variable in our study. All of the other infection cases occurred after 72 hours of the catheterization and the frequency of hemorrhage and ischemia was higher in the first 72 hours., Conclusion: The main complications of this proceeding are related to the technic utilized, time of utilization of the catheter and to the characteristics of the patients.
