Abstract

Background Immune checkpoint inhibitors have changed previous treatment paradigm of advanced urothelial carcinoma (UC). The ARON-2 study (NCT05290038) aimed to assess the real-world effectiveness of pembrolizumab in patients recurred or progressed after platinum-based chemotherapy. Patients and Methods Medical records of patients with documented metastatic UC treated by pembrolizumab as second-line therapy were retrospectively collected from 88 institutions in 23 countries. Patients were assessed for overall survival (OS), progression-free survival (PFS) and overall response rate (ORR). Cox proportional hazards models were adopted to explore the presence of prognostic factors. Results In total, 836 patients were included: 544 patients (65%) received pembrolizumab after progression to first-line platinum-based chemotherapy in the metastatic setting (cohort A) and 292 (35%) after recurring within < 12 months since the completion of adjuvant or neoadjuvant chemotherapy (cohort B). The median follow-up time was 15.3 months. The median OS and the ORR were 10.5 months and 31% in the overall study population, 9.1 months and 29% in cohort A and 14.6 months and 37% in cohort B. At multivariate analysis, ECOG-PS >= 2, bone metastases, liver metastases and pembrolizumab setting (cohort A vs B) proved to be significantly associated with worst OS and PFS. Stratified by the presence of 0, 1-2 or 3-4 prognostic factors, the median OS was 29.4, 12.5 and 4.1 months (p < 0.001), while the median PFS was 12.2, 6.4 and 2.8 months, respectively (p < 0.001). Conclusions Our study confirms that pembrolizumab is effective in the advanced UC real-world context, showing outcome differences between patients recurred or progressed after platinum-based chemotherapy.

23. De novo leaf transcriptome of a triploid linalool chemotype of Lippia alba(Mill.) N.E.Br. ex Britton & P. Wilson

Abstract

Lippia alba(Mill.) N.E.Br. ex Britton & P.Wilson is a well-known folk medicine plant belonging to family Verbenaceae. Despite its pharmacological aspects, biological interest, and intriguing polyploidization system, the molecular network is much less clear. This paper provides a transcriptome of Bushy Matgrass, aiming to shed light on its terpene metabolic pathway, bringing forward the protein structure and phylogeny of important terpene synthases found, such as those responsible for production of geraniol and nerolidol/linalool. A total of 47,498,310 paired-end reads were obtained and 38,096 unigenes generated by de novo assembly, of which 530 were associated with the terpene route. For validation, we submitted linalool-dominant triploid accessions to greenhouse and open field cultivation conditions, followed by RT-qPCR analysis using key terpene-related genes discovered in the NGS trial. Furthermore, we identified an abundance of transcription factors mRNA linked to terpene and stress, suggesting an epigenetic link between ploidy and chemotype. To the best of our knowledge, this is the first comprehensive transcriptomic analysis of LippiaL. genus and may serve as a baseline for further molecular studies.

Abstract

Motivation: The BioTIME database contains raw data on species identities and abundances in ecological assemblages through time. These data enable users to calculate temporal trends in biodiversity within and amongst assemblages using a broad range of metrics. BioTIME is being developed as a community‐led open‐source database of biodiversity time series. Our goal is to accelerate and facilitate quantitative analysis of temporal patterns of biodiversity in the Anthropocene. Main types of variables included: The database contains 8,777,413 species abundance records, from assemblages consistently sampled for a minimum of 2 years, which need not necessarily be consecutive. In addition, the database contains metadata relating to sampling methodology and contextual information about each record. Spatial location and grain: BioTIME is a global database of 547,161 unique sampling locations spanning the marine, freshwater and terrestrial realms. Grain size varies across datasets from 0.0000000158 km² (158 cm²) to 100 km² (1,000,000,000,000 cm²). Time period and grain: BioTIME records span from 1874 to 2016. The minimal temporal grain across all datasets in BioTIME is a year. Major taxa and level of measurement: BioTIME includes data from 44,440 species across the plant and animal kingdoms, ranging from plants, plankton and terrestrial invertebrates to small and large vertebrates. Abstract

O objetivo principal desse trabalho é o de apresentar os resultados do projeto intitulado ?Geotecnologias na integração, espacialização, análise e visualização de dados de pesquisa da cafeicultura mineira?. Esse projeto teve como objetivo principal integrar dados de pesquisa da cafeicultura mineira em um único banco de dados geográfico, visando a gestão da informação e a produção de novos conhecimentos por meio da análise espacial dessa base integrada e disponibiliza-los na internet. As informações foram disponibilizadas na internet por meio do site www.epamig.ufla.br/geosolos. O banco de dados está disponível para consultas textuais e espaciais por meio do web mapping i3Geo. O principal impacto desse projeto foi o de prover um ambiente integrado de consulta, que permite à cadeia produtiva do café, acessar dados, mapas e informações sobre a cafeicultura mineira e sua relação com o ambiente de uma forma inovadora.

Abstract

Coffee is the second most important agricultural exportation product in Brazil, constituting one of the main income sources of the Brazilian economy. The state of Minas Gerais is the country?s biggest coffee producer. Recently, coffees produced in the state have won national specialty coffee contests, which has increased their commercial value and established them in the market. Due to the necessity of more information on areas with potential for producing quality arabic coffees (Coffea arabica L.), the objective of this work was to relate the coffees entered in the Quality Contest-Coffees from Minas Gerais, in 2007 and 2008, with the environmental characteristics of the state?s districts. The samples were distributed in four stages, the first composed of all the coffees entered in the contest, and the last composed only of the pre-finalists. The samples were categorized into natural and CDs (pulped natural, demucilaged and demucilaged/pulped natural). The spatialization of the samples from both years was done using Kernel maps to visualize the intensity of sample concentrations in each stage of the contest. The results show that in the first stage the samples were well distributed, with medium, high and very high intensity focus. In the fourth stage, a high concentration in the Sul de Minas region was observed in both years for both coffee categories.

Abstract

Objectives: Opportunistic outreach services have been commissioned to overcome potential barriers to uptake, by offering health checks in accessible community venues. This study aimed to evaluate the ability of an outreach health check service to reach key target groups: men, people of South Asian ethnicity and people from deprived areas. The comparator was the health check service provided by GP practices. One aim was to investigate whether the addition of an outreach service would result in a higher percentage of health checks being done for people from the target groups compared to a GP-based service alone. The second aim was to assess which types of venues used for outreach health checks were most effective in reaching these groups. Study design: Evaluation of Public Health Programme with retrospective control group comparison. Methods: The percentages of completed health checks in men, people of South Asian ethnicity, and participants registered at general practices with lowest quintile area-level deprivation status were compared between all opportunistic community checks conducted by the Outreach Service over a ten month period and checks conducted in general practice in a partially-overlapping time period of the same financial year. For the venue-based comparison of Outreach Service checks, the number of checks per visit and percentage of checks in each target group were calculated for each venue. Results: Of 3849 Outreach Service checks, 38% were in men (compared to 50% of checks conducted in Primary Care), and 11% were in people of South Asian ethnicity (compared to 3% in Primary Care). 3558 Outreach check participants were registered with a general practice in the County (92%), and of these, 32% of checks were in people registered with a general practice in the lowest deprivation quintile (compared to 13% of checks in Primary Care). There were 519 visits by the outreach service to 23 different types of venue. Certain venues recorded large numbers of checks e.g. supermarkets and libraries, but they were not always the most efficient places to recruit people for checks. Mosques and bus stations were the venues with the broadest reach to all target groups. Other venues, despite having lower turnover or recruitment rates, were still good settings to reach specific target groups. Conclusion: The NHS Health Check can successfully be targeted at people from deprived areas and people of South Asian ethnicity using a targeted opportunistic community outreach approach. Our findings show that the reach to different groups varies substantially by venue, and therefore best results may be achieved by combining venues and strategies specific to the target group. 48. Hemobilia: transcatheter occlusive therapy and long-term follow-up.

Abstract

Eight patients with life-threatening hemobilia were treated by percutaneous transcatheter occlusive therapy. The bleeding was caused by a traumatic pseudoaneurysm of the hepatic artery in 6 cases (auto accident in 4, surgery in 1, biliary drainage in 1) and a true aneurysm of the hepatic artery in 2 (unknown etiology in 1 and mycotic in 1). Arterial catheterization was used in all cases except for one in which a direct percutaneous puncture was performed. Gelfoam alone was used as embolic material in 3 patients. In 1 patient each, the material used was gelfoam plus coils, coils alone, blood clot, n-butyl-cyanoacrylate and an occluding balloon catheter. In all cases the bleeding stopped and did not recur during the follow-up period which ranged from 9 months to 14 years. This experience indicates that transcatheter occlusive therapy is an effective method for the treatment of severe hemobilia. [ABSTRACT FROM AUTHOR]
