Knowledge of how the Brazilian public health system works in theory and practice should be disseminated to students during their graduation. In Dentistry, the Flexnerian model influenced the training of undergraduates for decades. However, the need for training focused on the Unified Health System and the dentist’s generalist profile was reiterated after revisions to the National Curriculum Guidelines for the course. Thus, this study aims to report experiences and internships in the reality of the Unified Health System and their contributions to Dentistry education, aligned with the National Curriculum Guidelines of the course. The report reveals the uncomfortable awakening in Dentistry education focused only on clinic or office work, and the interdisciplinary experiences provide the adoption of skills that will contribute to future dentists. Thus, there is significant cooperation for forming strategic subjects in the care and management of the Brazilian public health system, as per the expected profile for Dentistry graduates O conhecimento de como funciona, na teoria e na prática, o sistema público de saúde brasileiro deve ser disseminado aos estudantes durante a sua graduação. Na Odontologia, o modelo flexneriano influenciou a formação dos graduandos por décadas, contudo, após revisões das Diretrizes Curriculares Nacionais do curso, reiterou-se a necessidade da formação voltada para o Sistema Único de Saúde e do perfil generalista do cirurgião-dentista. Assim, o objetivo deste estudo é relatar experiências das vivências e estágios na realidade do SUS e suas contribuições para formação em Odontologia, em consonância com as Diretrizes Curriculares Nacionais do curso. Com o relato notase um despertar incômodo na formação em Odontologia voltada apenas para atuação em clínica ou consultório, e as vivências interdisciplinares propiciam a adoção de competências que contribuirão para os futuros cirurgiões-dentistas. Assim, existe grande cooperação para a formação de sujeitos estratégicos na assistência e gestão do sistema de saúde público brasileiro, conforme perfil esperado para o egresso do curso de Odontologia

Methods: An interrupted time series ecological study was carried out whose unit of analysis was Maranhão. The exposure variable was the COVID-19 pandemic, dichotomized into pre pandemic (Jan/2015 to Mar/2020) and pandemic (Apr/2020 to Jun/2021), considering the first case of the disease in the state. Data were taken from epidemiological bulletins available on portals of the State Health Department of Maranhão. The outcome variables were calculated by the ratio between the number of procedures related to disease prevention and health promotion (RPP-APS), urgency (RPU-APS) and Curative procedures (RPC-APS), and the census projection of the population of the state, multiplied by one thousand, monthly. The numerator data came from the Health Information System for Primary Care, and the denominator data from the Brazilian Institute of Geography and Statistics. Different autoregressive, integrated and seasonally moving average (SARIMA) models were tested (α=5%), the ones with the best fit were selected based on the criterion of parsimony of AIC (Akaike Information Criterion), BIC (Bayesian Information Criterion), MAPE (Mean Absolute Percentage Error), RMSE (Root Mean Squared Error) and the Ljung-Box and Box-Pierce tests. Results: The autoregressive, integrated and seasonally moving average (SARIMA) models with the best adjustments were: (0.1.0) (1.0.0) in the RPP-APS; (2.1.0) (1.0.0) in RPU-APS; and (0.1.0) (1.0.0) in RPC-APS. The pandemic caused a significant reduction in preventive procedures (Xreg=-6.52; p-value=0.0004) and in dressings (Xreg=-5.18; p value=0.0004). Although there was also a reduction in emergency procedures, there was no statistical difference in this indicator Xreg=-0,03; p-valor=0,1507). Conclusion: The COVID-19 pandemic had an impact on the reduction in the use of preventive and curative oral health services in PHC in Maranhão; and the actions taken in the state were not able to mitigate such effects until the first half of 2021. Constant discussion is recommended about the strategies that must be maintained and the necessary adjustments so that health units are adapted during the pandemic, in case of new waves, and, in the future, in the post-pandemic period. Objetivo: Analisar indicadores de uso de serviços de saúde bucal (USSB) da Atenção Primária à Saúde (APS) no Maranhão, antes e durante a pandemia de COVID-19. Métodos: Foi realizado um estudo ecológico de série temporal interrompida cuja unidade de análise foi o Maranhão. A variável exposição foi a pandemia de COVID-19, dicotomizada em pré-pandemia (jan/2015 a mar/2020) e pandemia (abr/2020 a jun/2021), considerando o primeiro caso da doença no estado. Os dados foram retirados dos boletins epidemiológicos disponibilizados em portais da Secretaria Estadual de Saúde do Maranhão. As variáveis de desfecho foram calculadas pela razão entre a quantidade de procedimentos relacionados à prevenção de doença e promoção da saúde (RPP-APS), de urgência (RPU-APS) e Procedimentos curativos (RPC-APS), e a projeção censitária da população do estado, multiplicado por mil, mensalmente. Os dados do numerador foram provenientes do Sistema de Informação em Saúde para a Atenção Básica, e do denominador, do Instituto Brasileiro de Geografia e Estatística. Diferentes modelos autorregressivos, integrados e de médias móveis com sazonalidade (SARIMA) foram testados (α=5%), os com melhores ajustes foram selecionados a partir do critério de parcimônia dos valores de AIC (Akaike Information Criterion), BIC (Bayesian Information Criterion), MAPE (Mean Absolute Percentual Error), RMSE (Root Mean Squared Error) e dos testes de Ljung-Box e Box-Pierce. Resultados: Os modelos autorregressivos, integrados e de médias móveis com sazonalidade (SARIMA) com melhores ajustes foram: (0,1,0) (1,0,0) na RPP-APS; (2,1,0) (1,0,0) na RPU-APS; e (0,1,0) (1,0,0) na RPC-APS. A pandemia causou redução significante nos procedimentos preventivos (Xreg=-6,52; p-valor=0,0004) e nos curativos (Xreg=-5,18; p-valor=0,0004). Embora também tenha ocorrido redução nos procedimentos de urgência, não houve diferença estatística neste indicador (Xreg=-0,03; p-valor=0,1507). Conclusão: A pandemia de COVID-19 impactou na redução do uso dos serviços preventivos e curativos de saúde bucal na APS do Maranhão; e as ações empreendidas no estado não foram capazes de mitigar tais efeitos até o primeiro semestre de 2021. Recomenda-se constante discussão acerca das estratégias que devem ser mantidas e de ajustes necessários para que as unidades de saúde sejam adaptadas durante a pandemia, em caso de novas ondas, e, futuramente, no pós-pandemia.

12. Representação social sobre o aprendizado de estudantes de Odontologia da Universidade Federal do Maranhão inseridos em Estágio Curricular na Atenção Primária à Saúde

This study aimed to identify the social representation on the learning of students of the Dentistry course at the Federal University of Maranhão, at the end of a curricular internship in Primary Health Care, and compare the results with the current National Curricular Guidelines. A qualitative study was carried out using a questionnaire, an instrument used to assess the internship, and only the first question was of interest to this study. In this question, students were asked to evoke five words that demonstrated their learning at the end of the internship in increasing order of importance. The questionnaire was completed by graduates from the 5th period between 2016 and 2019, totaling 100 evaluations. A structural approach and Prototypical Analysis were adopted, considering the frequency and order of words, composing a central and peripheral core. Software OpenEVOC® identified 500 evoked terms and 10 that established a social representation. The central core consisted of the following terms: planning, knowledge, and management; the first periphery: organization; the contrast zone: territorialization and humanization; and the second periphery: commitment, reality, patience, and goals. Evocations suggested that the internship consisted of activities that equipped the professionals with the necessary skills and abilities for the new professional profile. The social representation of students in the Dentistry course included in an environmental internship in Primary Health Care is marked by terms typical of implementing knowledge in the Brazilian public health system and shows the course’s curricular programs’ contents. O objetivo deste estudo foi identificar a representação social sobre o aprendizado de estudantes do curso de Odontologia da Universidade Federal do Maranhão, ao final de um estágio curricular no contexto da Atenção Primária à Saúde, e comparar os resultados com as Diretrizes Curriculares Nacionais vigentes. Foi realizado um estudo de natureza qualitativa, por meio de um questionário, instrumento utilizado para avaliar o estágio, do qual somente a primeira questão era de interesse para este estudo. Nessa questão era solicitado ao aluno que evocasse, em ordem crescente de importância, cinco palavras que demonstrassem seu aprendizado ao fim do estágio. O questionário foi preenchido pelos egressos do 5º período entre 2016 e 2019, totalizando 100 avaliações. Foi considerada uma abordagem estrutural e Análise Prototípica, na qual se considera a frequência e a ordem das palavras, compondo um núcleo central e periferias. O software utilizado, o OpenEVOC®, identificou 500 termos evocados e 10 que formaram a representação social. O núcleo central foi composto pelos termos: planejamento, conhecimento e gestão; a primeira periferia: organização; na zona de contraste: territorialização e humanização; e segunda periferia: comprometimento, realidade, paciência e metas. As evocações sugerem que o estágio foi composto por atividades que dotassem os profissionais de competências e habilidades necessárias do novo perfil de profissional. A representação social dos estudantes do curso de Odontologia, inseridos em um estágio ambientado na Atenção Primária à Saúde, é marcada por termos característicos da implementação de conhecimentos do sistema público de saúde brasileiro e reflete conteúdos conexos com as diretrizes curriculares do curso.

13. Persistem iniquidades sociais na distribuição da cárie dentária em adolescentes maranhenses? Contribuições de um estudo de base populacional

Resumo O objetivo foi analisar a associação de fatores socioeconômicos com a prevalência de cárie dentária em adolescentes de São Luís, Maranhão, para responder se as iniquidades sociais persistem na distribuição desta doença. Este é um estudo transversal aninhado a uma coorte prospectiva. Incluímos 2.413 adolescentes de 18-19 anos, avaliados em 2016 (2º seguimento). O desfecho foi a ocorrência de dentes com cárie dentária não tratada (sim ou não), avaliada pelo índice CPO-D. Características socioeconômicas e demográficas foram as variáveis independentes. Foram realizadas análises estatísticas descritivas e de regressão de Poisson, calculando-se razões de prevalência (RPs) brutas e ajustadas (alpha=5%). Pertencer às classes econômicas C (RP=1,23; IC95%:1,11-1,37) ou D-E (RP=1,48; IC95%: 1,32-1,65), estar casado/morar com companheiro (RP=1,22; IC95%:1,07-1,39), ter pais separados (RP=1,11; IC95%1,03-1,19) e maior número de pessoas na residência (RP=1,05; IC95%:1,03-1,07) foram associadas a maior prevalência de cárie dentária. Apesar da implementação da Política Nacional de Saúde Bucal, as iniquidades sociais em saúde bucal de adolescentes persistem. É fundamental que o modelo de atenção à saúde vigente busque a reorientação das estratégias de educação em saúde, direcionando-as a populações vulneráveis. - Abstract
This article aims to evaluate the effect of the COVID-19 pandemic on malnutrition among children under two years of age enrolled in the Bolsa Família Program (BFP). Ecological study of interrupted time series (ITS), with low weight for age, stunting, and overweight as time-dependent variables of malnutrition, extracted monthly (Jan/2008 to June/2021) from the Food and Nutrition Surveillance System. The COVID-19 pandemic was the exposure, dichotomized into pre-pandemic and pandemic. In RStudio, the trend was obtained by Prais-Winsten regression, and the effect of the pandemic on the time-dependent variables was determined by SARIMA modeling, estimating the regression coefficients (RC) adjusted for trend and seasonality (α = 5%). The pandemic was associated with an increase in: i) low weight for age in the South (RC = 0.94; p < 0.001) and Southeast (RC = 1.97; p < 0.001); ii) height deficit in the Midwest (RC = 2.4; p = 0.01), South (RC = 2.15; p < 0.001) and Southeast (RC = 2.96; p < 0.001); and iii) and overweight in the North (RC = 1.51; p = 0.04), Midwest (RC = 2.29; p = 0.01), South (RC = 2.83; p < 0.001), and Southeast (RC = 0.72; p = 0.04). The pandemic increased underweight in the South and Southeast, and the double burden of malnutrition in the Midwest, South, and Southeast. In the Northeast and North, higher rates of malnutrition still persist.

Objective: To analyze characteristics of the home visit (HV) in Brazil, 2012 and 2017., Methods: Ecological study, with panel data whose units of analysis were the Primary Health Care teams in Brazil, participants of the 1st and 3rd cycles of the Program to Improve Access and Quality of Primary Care of the Unified Health System. Descriptive, inferential and spatial analyzes (alpha=5%) were performed., Results: There was an increase in the proportion of teams that carried out home visits at a frequency defined based on risk and vulnerability analysis and actively searched for people with respiratory symptoms and women with delayed and altered cytopathological examination. In the heat maps, the Northeast, Southeast and South regions had a higher concentration of teams that carried out the HV and carried out an active search., Conclusion: The maintenance and qualification of HVs must be a priority in Brazil, since there are few countries in the world with such capillarity of health services, reaching the homes of millions of people.
