Abstract Background Whilst previous work has identified clustering of the active trachoma sign “trachomatous inflammation—follicular” (TF), there is limited understanding of the spatial structure of trachomatous trichiasis (TT), the rarer, end-stage, blinding form of disease. Here we use community-level TF prevalence, information on access to water and sanitation, and large-scale environmental and socio-economic indicators to model the spatial variation in community-level TT prevalence in Benin, Cote d’Ivoire, DRC, Guinea, Ethiopia, Malawi, Mozambique, Nigeria, Sudan and Uganda. Methods We fit binomial mixed models, with community-level random effects, separately for each country. In countries where spatial correlation was detected through a semi-variogram diagnostic check we then fitted a geostatistical model to the TT prevalence data including TF prevalence as an explanatory variable. Results The estimated regression relationship between community-level TF and TT was significant in eight countries. We estimate that a 10% increase in community-level TF prevalence leads to an increase in the odds for TT ranging from 20 to 86% when accounting for additional covariates. Conclusion We find evidence of an association between TF and TT in some parts of Africa. However, our results also suggest the presence of additional, country-specific, spatial risk factors which modulate the variation in TT risk.

BackgroundTrichiasis is present when one or more eyelashes touches the eye. Uncorrected, it can cause blindness. Accurate estimates of numbers affected, and their geographical distribution, help guide resource allocation.MethodsWe obtained district-level trichiasis prevalence estimates in adults for 44 endemic and previously-endemic countries. We used (1) the most recent data for a district, if more than one estimate was available; (2) age- and sex-standardized corrections of historic estimates, where raw data were available; (3) historic estimates adjusted using a mean adjustment factor for districts where raw data were unavailable; and (4) expert assessment of available data for districts for which no prevalence estimates were available.FindingsInternally age- and sex-standardized data represented 1,355 districts and contributed 662 thousand cases (95% confidence interval [CI] 324 thousand-1.1 million) to the global total. Age- and sex-standardized district-level prevalence estimates differed from raw estimates by a mean factor of 0.45 (range 0.03-2.28). Previously non- stratified estimates for 398 districts, adjusted by ×0.45, contributed a further 411 thousand cases (95% CI 283-557 thousand). Eight countries retained previous estimates, contributing 848 thousand cases (95% CI 225 thousand-1.7 million). New expert assessments in 14 countries contributed 862 thousand cases (95% CI 228 thousand-1.7 million). The global trichiasis burden in 2016 was 2.8 million cases (95% CI 1.1-5.2 million).InterpretationThe 2016 estimate is lower than previous estimates, probably due to more and better data; scale-up of trichiasis management services; and reductions in incidence due to lower active trachoma prevalence.

BACKGROUND:Onchocerciasis control in Côte d'Ivoire started with aerial insecticide spraying in 1974 and continued with community directed treatment with ivermectin (CDTi) from 1992 to the present. Onchocerciasis and lymphatic filariasis (LF) are co-endemic in 46 of the 81 health districts in the country. Fourteen and 12 districts are endemic for only LF or onchocerciasis, respectively. This paper aims to review the impact of past interventions on onchocerciasis in Côte d'Ivoire between 1975 and 2013, and review plans for disease elimination. METHODS:We reviewed microfilaria (MF, skin snip) prevalence and community microfilarial load (CMFL) data from published reports from 53 health districts during two major epidemiological assessment periods. Data from 1975 through 1991 provided information on the impact of vector control, and data from 1992 through 2016 provided information on the impact of CDTi. RESULTS:Weekly aerial insecticide spraying in 8 endemic districts between 1975 and 1991 reduced the overall MF prevalence by 68.1% from 43.5% to 13.9%. The CMFL also decreased in 7 out of 8 surveyed communities by 95.2% from 9.24 MF/snip to 0.44 MF/snip. Ivermectin distribution started in 1992. The coverage targets for control (65% of the total population) was reached in most endemic districts, and some areas achieved 80% coverage. Two sets of surveys were conducted to assess the impact of CDTi. Results from the first repeat surveys showed a significant decrease in overall MF prevalence (by 75.7%, from 41.6% to 10.1%). The second follow-up evaluation showed further improvement in most endemic districts and also documented major reductions in CMFL compared to baseline. CONCLUSIONS:Extensive data collected over many years document the very significant impact of interventions conducted by the National Onchocerciasis and other Eyes Diseases Control Programme during challenging times with periods of civil unrest. The Health Ministry has now integrated efforts to control neglected tropical diseases and adopted the goal of onchocerciasis elimination.

Lasiodiplodia species causing mango Stem-end rot disease (SER) are mainly controlled using synthetic products, which can harm humans and environmental health. Therefore, developing an eco-friendly control method, such as using plant extract products, is imperative. In this study, we evaluated the inhibitory effect of 3 biopesticides based on essential oils (ASTOUN 50 EC, FERCA 50 EC, and NECO 50 EC) at 300, 500, 700, 1000 ppm and Moringa oleifera leaves extracts (Methanolic and aqueous) at 5, 10, 15 and 20 g/250 ml on mycelial growth of Lasiodiplodia theobromae in vitro. Subsequently, 135 fruits (cv. Kent) per treatment were inoculated (1 × 105 spores/ml) and treated with each biopesticide (700, 1000 and 2000 ppm) and M. oleifera leaves extracts (15 and 20 g/250 ml) in vivo to evaluate their efficacy on mango SER development. The results showed that the biopesticides ASTOUN (Cymbopogon citratus) and NECO (Occimmun gratissimun) completely inhibited the mycelial growth of L. theobromae at 700 and 1000 ppm. Similarly, M. oleifera methanolic extract has the highest inhibitory rate (65.45 %) compared to aqueous extract (42.44%). Moreover, 1000 and 2000 ppm of biofungicides and 15 and 20 g/250 ml of M. oleifera methanolic extract significantly reduced mango SER development compared to the control. This study provides evidence that these plant extracts are effective alternatives to mango SER management that are consistent with sustainable agriculture principles, promote ecological balance, and reduce the environmental impact of conventional agriculture.
