2. Acinetobacter infections prevalence and frequency of the antibiotics resistance: comparative study of intensive care units versus other hospital units

INTRODUCTION: this study aims to determine the Acinetobacter sp clinical isolates frequency and its antibiotic susceptibility pattern by comparing results obtained from the Intensive Care Units (ICUs) to that of other units at the Mohammed V Military Teaching Hospital in Rabat. METHODS: this is a retrospective study over a 2-years period where we collected all clinical isolates of Acinetobacter sp obtained from samples for infection diagnosis performed on hospitalized patients between 2012 to 2014. RESULTS: during the study period, 441 clinical and non-repetitive isolates of Acinetobacter sp were collected representing 6.94% of all bacterial clinical isolates (n=6352) and 9.6% of Gram negative rods (n=4569). More than a half of the isolates were from the ICUs and were obtained from 293 infected patients of which 65, 2% (191 cases) were males (sex ratio = 1.9) and the median age was 56 years (interquartile range: 42-68 years). Acinetobacter clinical isolates were obtained from respiratory samples (44.67%) followed by blood cultures (14.51%). The resistance to ciprofloxacin, ceftazidime, piperacillin / tazobactam, imipenem, amikacin, tobramycin, netilmicin,rifampicin and colistin was respectively 87%, 86%, 79%, 76%; 52%, 43%, 33% 32% and 1.7%. The difference in resistance between the ICUs and the other units was statistically significant (p =0.05) except for colistin, tetracycline and rifampicin. CONCLUSION: this paper shows that solving the problem of prevalence and high rate of multidrug resistant Acinetobacter infection which represents a therapeutic impasse, requires the control of the hospital environment and optimizing hands hygiene and antibiotics use in the hospital.

Light‐chain multiple myeloma is a malignant hemopathy characterized by medullary invasion by tumor plasma cells, with the secretion of free light chains of immunoglobulins in the blood giving signs of bone disorders. However, it can be asymptomatic or can start by less clear clinical manifestations such as recurrent infections, anemic syndromes, renal failure, neurological symptoms, pathological fractures, or even amylosis 1. Revelation through pulmonary embolism is exceptional. Through this observation, we are emphasizing on the particularities of light‐chain myeloma.

Multiple Myeloma is a blood cancer type B characterized by clonal proliferation of malignant plasma cells, the median age at diagnosis is 70 years, cases among younger patients are rare, and less than 0, 3 % patients are younger than 30 years in most series. We report a case of a 25-year- old women; she was suffering of a sacred pain radiating towards the left lower limb and fatigue for three months, a magnetic resonance imaging for dorso-lumbar spine showed an expansive process and multiple osteolytic lesions in 2nd, 3rd and 4th sacred vertebras. She underwent a laparotomy that revealed a heavy mass invading the sacrum and the rectum. The histological examination with immunocytochemistry analysis revealed a plasmocytoma and the diagnosis of symptomatic multiple myeloma stage III was established then she received 2 courses of chemotherapy. Despite the rarity of multiple myeloma among young patients, this diagnosis should be evoked when clinical, biological and radiological signs are in favor. It appears that there is no difference between younger and elderly patients on the presentation of the disease, although a longer survival has been reported among young patients.

Background Post cardiac surgery mediastinitis is the major infectious complication, despite the development of surgical techniques and the application of strict preventive measures. The Haemophilus influenzae mediastinitis is very rare. The mediastinitis caused by the association between Haemophilus influenzae and Aggregatibacter aphrophilus has never been described to our knowledge. Case presentation We report the case of an exceptional combination of Haemophilus influenzae and Aggregatibacter aphrophilus in a patient operated for single bypass which is complicated by mediastinitis the 10th day after the surgical act. Conclusion The conclusion to be drawn from this work is to think in unusual seeds in case of mediastinitis post cardiac surgery for the elaboration of recommendations for antibiotic prophylaxis.

Scolymus hispanicus or the Golden Thistle, locally known as ‘Guernina’ or ‘Taghediwt’, is one of the most appreciated wild vegetables in Morocco. This study aims to characterise the functional chemical and pharmacological variability of Scolymus hispanicus parts (roots, stems, leaves and flowers). The chemical analysis revealed higher content of α-tocopherol in the flowers (2.79 ± 0.07 mg/100 g) and lead to the identification of 3 flavonoids and 13 phenolic acids, with high content of gallic acid in leaves (187.01 ± 10.19 mg/kg); chlorogenic (936.18 ± 92.66 mg/kg) and caffeic (4400.14 ± 191.43 mg/kg) acids in flowers, roots were much more higher in sinapic acid (0.25 ± 0.03 mg/kg) compared to the other parts. Moreover, Scolymus hispanicus ethanolic extracts exhibited interesting antioxidant and antimicrobial properties, promising anti-amylase and anti-glucosidase activities and relevant diuretic effect that confirms its traditional uses.

Extra pulmonary small cell carcinomas are extremely rare, accounting for 0.1-0.4% of all malignancies and 2.5-4% of small cell carcinomas. In the head and neck region, the most common primary sites are the larynx, salivary gland, nasal cavity, paranasal sinus, tonsil, and oral cavity. Small cell carcinomas occurring in nasopharynx are extremely rare, to our best knowledge, only four cases of primary nasopharyngeal small cell carcinoma has been described in English literature. We report another case of nasopharyngeal small cell carcinoma arising in a 46-year-old woman. She accused, for five months, nasal blockage, right hypoacousia, exophthalmos and reduced visual acuity. Cerebral and facial magnetic resonance imaging showed an extensive mass of the nasopharynx with the invasion of sphenoidal body, the diagnosis of disseminated small cell carcinoma of nasopahrynx was established. The patient received three courses of systemic chemotherapy; unfortunately, she died after the third course. The prognosis of such entity is poor and is similar to that for patients with extensive small cell lung cancer.

Treatment of Acinetobacter baumannii meningitis is an important challenge due to the accumulation of resistance of this bacteria and low meningeal diffusion of several antimicrobial requiring use of an antimicrobial effective combination to eradicate these species. We report a case of Acinetobacter baumannii multidrug-resistant nosocomial meningitis which was successfully treated with intravenous and intrathecal colistin associated with rifampicin.

11. Étude de la flore fongique et physicochimique de l’eau à l’hôpital militaire d’instruction Mohamed V de Rabat

Introduction Il est bien etabli que le reseau d’eau hospitalier peut etre un reservoir de pathogenes susceptibles de provoquer des infections nosocomiales chez des patients a risque. L’objectif de ce travail etait de determiner le profil ecologique de la flore fongique hydrique de notre environnement hospitalier et de proposer des recommandations et des precautions permettant de limiter l’exposition de ces patients aux spores fongiques. Materiels et methodes Il s’agit d’une enquete prospective et transversale menee, d’une part, au sein des services hospitaliers (reanimation medicale, reanimation chirurgicale, hematologie clinique et bloc operatoire central) et au laboratoire de parasitologie et de mycologie de l’HMIMV de Rabat, et d’une part, au centre de recherche du service de toxicologie de la faculte de medecine et de pharmacie de Rabat. Pour chacun des 4 services concernes, 2 points de prelevement ont ete definis avec la realisation de 3 prelevements d’eau (eau 1 er jet, eau froide, eau chaude) et au moins 2 prelevements de surfaces (robinetterie, siphon, pommeau de douche). Les prelevements d’eau ont ete recueillis dans des bouteilles steriles d’au moins 1 L et ont ete places a +4 °C jusqu’a leur filtration (filtration sous pression), puis chaque membrane a ete deposee face non filtrante sur une gelose MALT. Concernant les surfaces, elles ont ete prelevees par ecouvillonnage : au minimum 2 ecouvillons par point de prelevement puis ensemences par epuisement sur gelose MALT dans des boites de petri. La duree d’incubation etait d’au moins 28 jours entre 27–30 °C. Resultats L’etude a porte sur 8 points de prelevements soit 22 prelevements d’eau et 22 prelevements de surfaces exploitables. Sur les 22 prelevements d’eau exploitables, 18 etaient positifs (soit 81 %) et pour les surfaces 20 etaient positifs (90 %). La repartition des champignons isoles a partir des echantillons d’eau et de surface revele un total de 12 especes differentes. Aspergillus (26 %), Acremonium et mucorales (12 %), Fusarium (10 %) etaient les plus frequemment isoles. La reanimation chirurgicale etait le service le plus colonise par les champignons (42 %). D’autre part, les resultats physicochimiques de l’eau ont objective que l’eau de l’hopital est conforme aux recommandations de l’OMS. Conclusion Notre etude a montre que l’eau de l’HMIMV est un reservoir potentiel des germes fongiques avec une positivite de 81 %. Ces pathogenes opportunistes, presents naturellement dans l’eau ou introduits de facon retrograde a partir de l’environnement hospitalier persistent et proliferent dans les reseaux d’eau au sein des biofilms. Des lors qu’ils sont relargues, les patients sont exposes aces pathogenes au cours des differents usages de l’eau : par ingestion d’eau contaminee, inhalation d’aerosols (douches) ou contact direct ou indirect via des dispositifs medicaux rinces avec une eau contaminee. Ces infections peuvent etre severes, parfois fatales. D’ou l’interet de mettre en place de mesures de surveillance et de protection.

12. Procalcitonine et candidoses invasives en réanimation

Introduction Le diagnostic des candidoses systemiques est difficile a etablir vu la non-specificite des signes cliniques et les difficultes du diagnostic biologique, or tout retard diagnostique complique la prise en charge therapeutique de ces infections graves. C’est dans ce cadre que s’inscrit le dosage de la procalcitonine (PCT) dont le role en tant que marqueur dans les infections bacteriennes, a ete reconnu alors que dans les infections fongiques invasives, en particulier, les candidoses invasives, ce role reste controverse. Les auteurs proposent dans cette etude, d’evaluer l’interet du dosage de la procalcitonine serique chez des patients en milieu de reanimation comme un possible indicateur predictif des candidoses invasives. Patients et methode Il s’agit d’une etude prospective monocentrique sur une periode de 7 mois. Elle porte sur tous les patients hospitalises en reanimation et presentant une fievre resistant a une antibiotherapie a large spectre. Nous avons exclu tous les patients avec des affections connues pour augmenter la procalcitonine. La PCT a ete dosee par technique VIDAS ® a l’admission, j0, j2, j4, j6 et j8. Nous avons retenu la valeur la plus elevee qui correspond a la PCT max. En parallele, nous avons procede a la recherche bi-hebdomadaire de l’Ag mannane et des anticorps anti- Candida , a la determination de l’index de colonisation et a la realisation des hemocultures. Resultats Durant la periode d’etude, 49 patients ont ete retenus. Quatre patients sont classes dans le groupe infection fongique prouvee, 17 dans le groupe probable et 28 patients dans le groupe sans infection. L’âge moyen est de 62 ans. Le sex-ratio H/F est de 1,44. La PCT moyenne est de 3,6 ng/mL avec des extremes de 0 a 56,4 ng/mL. La PCT moyenne dans les 2 groupes avec infection fongique est de 4,96 ng/mL versus 0,65 ng/mL dans le groupe sans infection ( p Conclusion Notre etude a montre un interet non negligeable dans les infections fongiques invasives. Cependant, de grandes etudes prospectives sont necessaires pour determiner la valeur diagnostique de la PCT dans les candidoses invasives.

Introduction Les infections mycosiques du pied constituent un motif frequent de consultation chez les diabetiques, le diabete constituant a la fois un facteur favorisant et aggravant les lesions cutaneo-muqueuses. L’objectif de ce travail etait d’etudier la frequence des mycoses des pieds chez le diabetique, d’identifier la flore mycologique locale responsable des lesions du pied chez le diabetique, de determiner les facteurs favorisant la survenue de mycoses et les eventuelles complications secondaires. Materiels et methodes Il s’agit d’une etude prospective descriptive realisee sur une periode de 3 mois allant de novembre 2016 a janvier 2017 au sein du laboratoire de parasitologie et de mycologie de l’HMIMV de rabat, et en collaboration avec le service de dermatologie de l’HMIMV de rabat. Dans notre etude nous avons retenu tous les patients diabetiques vus en consultation de diabetologie, presentant des lesions cutanees des pieds evocatrices d’atteinte mycosique. L’examen mycologique comportait un examen direct et une culture. L’analyse statistique a ete faite en utilisant le logiciel SPSS 10.0. Les tests utilises etaient le test Chi 2 pour les variables qualitatives et le test de Student pour les variables quantitatives. Resultats Durant la periode etudiee, 29 patients diabetiques ont ete vu en consultation de mycologie. L’âge des patients, la neuropathie peripherique, l’hypercholesterolemie, la nephropathie ont constitue les facteurs favorisant l’apparition de mycose des pieds chez nos patients. L’onychomycodystrophie totale a constitue le principal siege des mycoses suivie par l’onychomycose disto-laterale et les leuconychies. Trichophyton rubrum etait le champignon le plus frequent dans toutes les localisations. Aucune complication n’a ete observee chez nos patients. Conclusion La prise en charge des mycoses du pied chez le diabetique doit inclure le diagnostic mycologique ainsi que le changement des facteurs favorisant leur survenue. Aussi, les praticiens doivent conseiller les patients en matiere d’hygiene des pieds et d’environnement.

Introduction L’aspergillose pulmonaire invasive (API), due au champignon filamenteux Aspergillus spp, est une infection opportuniste associee a une mortalite elevee. L’API demeure une infection tres difficile a diagnostiquer. Par consequent, la necessite d’un nouveau biomarqueur proteique precoce est interessante. Materiel et methodes Il s’agit d’une etude experimentale qui s’est deroule entre septembre 2015 et juin 2016, au laboratoire de parasitologie-mycologie de l’HMIMV, le laboratoire de biotechnologie medicale et l’animalerie centrale de la faculte de medecine et de pharmacie de Rabat. Des rats albinos mâles immunodeprimes (neutroniques = 6, temoins = 3) ont ete exposes a des spores d’ Aspergillus niger pendant 30 minutes. Le liquide de lavage broncho-alveolaire (LBA) et le sang total ont ete preleves chez les rats neutropeniques et les rats temoins au 2 e , 5 e et 7 e jours post-infection. Une antigenemie galactomannane et un MALDI-TOF ont ete utilises sur le LBA et le serum. Resultats L’antigene galactomannane a ete retrouve chez 4 des 6 rats neutroniques (index ≥ 0,5). Pour la spectrometrie de masse, il y avait trois pics principaux a 3,5, 9,1 et 10,2 kDa chez tous les rats malades. Il y avait une difference significative dans l’intensite des pics entre les rats neutropeniques et les rats temoins. Conclusion La recherche de nouveaux biomarqueurs dans le serum et/ou le LBA est interessante pour ameliorer le diagnostic precoce de l’API chez les patients neutropeniques.
