Abstract

We prove the saturation of a generalized partially hyperbolic attractor of a $C^2$ map. As a consequence, we show that any generalized partially hyperbolic horseshoe-like attractor of a $C^1$-generic diffeomorphism has zero volume. In contrast, by modification of Poincar\'e cross section of the geometric model, we build a $C^1$-diffeomorphism with a partially hyperbolic horseshoe-like attractor of positive volume.

Abstract

This research aims to develop data envelopment analysis (DEA) for predicting the supply chain sustainability of the concrete industry using stochastic variables. In the first phase, the mines and companies that organized the four supply chains of the Guilan concrete industry based on competitive elements were identified. After that, based on economic, social, and environmental indexes, the sustainability of the supply chains was chosen using the Fuzzy Delphi model and input and output of mines and concrete companies, which include controllable, desirable, and undesirable inputs and outputs. Finally, data envelopment analysis was used to measure the supply chain sustainability of the concrete industry employing crisp data and a linear model. The errors were entered into the model with a stochastic element based on the probability of the errors. In other words, according to stochastic data envelopment analysis, all input and output data were considered randomly. The output data shows that none of the four supply chains of the Guilan concrete industry is sustainable. [ABSTRACT FROM AUTHOR]

4. Particularités cliniques et thérapeutiques des carcinomes épidermoïdes de la thyroïde : étude de cinq cas

Abstract

Résumé: La tumeur de Herrenschmidt ou carcinome épidermoïde primitif de la thyroïde (CET) est une tumeur rare. Elle représente moins de 1 % de l’ensemble des cancers thyroïdiens. Elle est caractérisée par son évolution rapide et agressive qui ressemble à celle du carcinome anaplasique. Notre travail repose sur une étude rétrospective de cinq cas de carcinome épidermoïde pur de la thyroïde confirmé histologiquement. Tous nos patients étaient de sexe féminin, avec une moyenne d’âge de 57 ans, le motif de consultation était un goitre, caractérisé par sa consistance dure, pierreuse, son caractère adhérant. Les adénopathies cervicales ont été palpées chez une seule patiente. La thyroïdectomie totale curative n’a été possible que chez deux patientes. Le pronostic était globalement médiocre avec une survie de l’ordre de quelques mois et surtout de mauvaise qualité. La chirurgie radicale — quand c’est possible — nous paraît la seule arme potentiellement curative, susceptible d’améliorer les résultats. Cependant, le diagnostic tardif ainsi que l’infiltration rapide des tissus voisins n’autorisent le plus souvent qu’une thyroïdectomie palliative, permettant de remédier transitoirement aux phénomènes compressifs.

5. Le phéochromocytome surrénalien malin : difficultés diagnostiques et modalités thérapeutiques

Abstract

Résumé: Introduction: Les phéochromocytomes sont des tumeurs rares, développées aux dépens de la médullosurrénale et des paraganglions. Elles synthétisent et stockent les catécholamines dont la sécrétion excessive est responsable de la symptomatologie clinique. Le diagnostic de malignité reste un dogme entre chirurgien, endocrinologue, anatomopathologiste et oncologue. Il n’est posé que par la présence demétastases des organes dépourvus de tissu chromaffine ou par l’apparition de récidives néoplasiques. Quatre cas sont étudiés et analysés dans cet article avec une revue de la littérature. Matériels et méthodes: Durant 20 ans, 50 patients porteurs de phéochromocytomes étaient pris en charge dans notre service dont quatre (soit 8 %) étaient malins. Tous nos patients étaient de sexe masculin, leur âge variait entre 17 et 48 ans. Les circonstances de découvertes étaient: des métastases pulmonaires et une carcinose péritonéale diffuse chez deux patients. Une récidive controlatérale avec des métastases hépatiques a été observée, 13 ans après la première surrénalectomie, chez un de nos patients. Un phéochromocytome malin était découvert dans un contexte familial, avec une hypertension artérielle (HTA) et une rétinopathie bilatérale chez un jeune de 17 ans. Le couple scanner-dérivés méthoxylés était suffisant pour affirmer le diagnostic de phéochromocytome dans 75 % des cas. Résultats: Un cas était opéré en urgence pour hémopéritoine sur carcinose diffuse. Une surrénalectomie bilatérale avec hépatectomie droite était réalisée dans un cas, et une surrénalectomie droite dans un autre. Le patient présentant des métastases pulmonaires était jugé inopérable. L’évolution était marquée par un décès postopératoire immédiat (hémopéritoine exploré en urgence) et par une bonne évolution à long terme, avec une normalisation des chiffres tensionnels et les dérivés méthoxylés chez deux patients. Conclusion: La malignité du phéochromocytome ne se définit que par l’existence de métastases ou de récidives néoplasiques. La chirurgie du phéochromocytome malin, même au stade métastatique, augmente la survie. La chimioradiothérapie et la métaiodobenzylguanidine (MIBG) font partie de l’arsenal thérapeutique. Abstract

Monitoring and mapping eroded lands by gully erosion is an essential step to control gully networks. Advances in remote sensing and aerial photography have enabled users to capture data with variant temporal and spatial resolution that is needed in different fields. In addition, introducing different types of unmanned aerial vehicles (UAV) enabled to carry imaging payload. The orthophoto and digital elevation model (DTM) produced from aerial images taken by Aeria-X camera mounted on Sensefly eBee-X drone was employed to identify and map eroded areas by gully in Kajoo-Gargaroo watershed in Chabahar, south-eastern part of Iran. Digitizing gully boarders manually is a tiring and time-consuming process for the operators. Maximum likelihood algorithm as one of the machine learning algorithm was also used to classify orthophoto in order to extract gully borders in the study area. In this study a new algorithm based on analysing geometric features and clustering of the DTM was used to map gullies automatically. The results of the proposed method and machine learning algorithm were compared with the manually digitized gully map. Quantitative evaluation demonstrates that our proposed method reaches better overall accuracy compared to machine learning algorithm with the increase of 7.2 percent in overall accuracy. Abstract

1.Illegal hunting of ungulates can reduce the prey base of carnivores, which can increase human‐carnivore conflict (HCC) through livestock depredation. However, the relationship between ungulate poaching, wild prey abundance and livestock depredation has rarely been empirically studied. 2.We surveyed 18 sites across the Hyrcanian forest in northern Iran; a global biodiversity hotspot under pressure from illegal hunting of ungulates, prey depletion, livestock grazing and HCC. We conducted three field surveys across 1204 km in 93 4×4 km cells to count signs of ungulate poaching as well as encounters with livestock and prey species of the Persian leopard Panthera pardus saxicolor and the grey wolf Canis lupus. We documented sheep/goat and cattle depredation from interviews with 201 herders and analysed the effects of illegal hunting of ungulates, forest cover, IUCN categories of reserves, elevation, distance to villages, and wild prey and livestock encounter rates on carnivore depredation rates using generalized linear models. 3.Illegal hunting of ungulates was the most influential depredation predictor. An increase in the illegal hunting of ungulates by one sign/km significantly increased depredation by up to four times. We also found significantly lower levels of ungulate poaching in national parks (IUCN category II) compared to protected areas (V), wildlife refuges (IV) and no‐hunting areas, though poaching signs were frequently found in most cells (58%). Encounters with livestock was inversely linked to wild prey species, but positively coupled with signs of ungulate poaching. 4.Synthesis and applications. Our study reveals that: (i) an increase in the intensity of illegal hunting of ungulates can intensify livestock depredation by carnivores; (ii) future efforts in reducing human‐carnivore conflict (HCC) to acceptable levels require a combination of law enforcement, prey recovery approaches and mitigation measures; (iii) it is essential to understand the root caus

Abstract

The Grand Bazaar was constructed, there have been significant changes in shopping practices. Particularly the spread of Global trade boosted the opening of new shopping centres, high-street department stores and shopping malls. Despite all these changes in shopping habits, the Grand Bazaar still continues to be an actively used shopping area; it has never lost its customer interest and its socially vivid atmosphere. This research explores the reasons why the Grand Bazaar has not been lost its social dynamism over the course of time? This question has various answers, but this research argues that one of them is particularly related to its peculiar spatial formation that encourages social interactions. In this account; this research methodologically focuses on two main components, these are: how the Grand Bazaar was spatially constructed and how it was experienced by users. These purposes will be accomplished by the analyses of the writings and drawings of the important Western travellers. It is revealed that articulated in topographical account, the Grand Bazaar eliminates the boundaries between the spaces and thus between the people. This continually enhances the mutual relationships between inside and outside. This study focuses on the main interest of the topographic approach which is not much the representative qualities; re-expression of the exterior appearance of the building in shape and profile or any stylistic display, but it is more to create an open venue or a stage which enhances the spatial and social connectivity between interior and exterior worlds, ergo between individual and collective lives. As a result, the study demonstrates how the Grand Bazaar's topographical attitude lends a 'formative' aspect to an architectural context. The Grand Bazaar tries to abolish borders by emphasizing topographical aspects such as permeability, spatiality, and delay in order to create an in-between environment, and also a common social ground.

Abstract

The aim of this study was to investigate whether mutations in the glucocorticoid receptor could account for the increasing unresponsiveness of patients with chronic lymphatic leukemia (CLL) to combination chemotherapy. The receptor was tested immunocytochemically, in steroid binding assays, and by a mutation screening (denaturing gradient gel electrophoresis) of the receptor-cDNA. The receptor concentration, as measured by staining and steroid binding test, varied considerably but showed no clear correlation to clinical response. Using a highly sensitive mutation screening assay of the DNA- and the steroid-binding region, none of the treated patients revealed any mutation, suggesting that the glucocorticoid receptor in the CLL patients tested is not altered in these domains. In one individual who had not been treated before analysis a silent mutation was found in one receptor allele. The results suggest that mechanisms other than altered ligand or DNA binding of the receptor may be responsible for the lack of response to chemotherapy. This conclusion is discussed in relation to the mechanism of corticoid resistance in mouse and human lymphoma cells in culture.

Abstract

Introduction : L’os est le troisième site préférentiel pour les métastases après le poumon et le foie. Il devient douloureux dans 70% et peut altérer de manière significative la qualité de vie du patient atteint de cancer. La physiopathologie de la douleur due aux métastases osseuses est actuellement mieux comprise. La douleur osseuse comprend deux composantes essentielles : inflammatoire et neuropathique. Les options thérapeutiques sont nombreuses et variées. Discussion : Il est important de connaître les différentes options thérapeutiques et leurs champs d’action afin de choisir le traitement approprié. Toutefois, l’approche multimodale est souvent nécessaire et comprend des moyens médicamenteux (les analgésiques, les bisphosphonates et le dénosumab) et des moyens non médicamenteux notamment la radiothérapie et la cimentoplastie. Les analgésiques comprennent 3 paliers de l’OMS et seront indiqués en fonction de l’intensité de la douleur. Les bisphosphonates et le dénosumab diminuent la résorption osseuse en agissant sur les ostéoclastes et ont démontré leur effet antalgique en particulier dans le cancer de la prostate avec métastases osseuses. La radiothérapie permet d’obtenir un effet antalgique résultant de l’effet radiotoxique direct sur les cellules tumorales et de la réaction inflammatoire de l’environnement osseux qui en découle. Alors que la radiothérapie externe est le traitement de choix pour pallier les douleurs de métastases osseuses localisées, la radiothérapie métabolique par son action systémique est mieux adaptée en cas de métastases diffuses. La cimentoplastie consiste à injecter du ciment dans le corps vertébral lésé mais son rôle antalgique reste à prouver. Le traitement anti-tumoral, lorsque la tumeur est sensible peut agir sur la douleur en réduisant la tumeur. Conclusion : L’approche thérapeutique doit être individualisée pour chaque patient et nécessite une bonne compréhension des mécanismes de la douleur cancéreuse. A l’heure actuelle, il n’existe pas d’algorithme de choix entre les différents moyens thérapeutiques, d’où la nécessité d’une approche multidisciplinaire réunissant des spécialistes dans le traitement du cancer et de la douleur, un médecin nucléaire, un réanimateur, un chirurgien et un médecin de soins palliatifs afin de discuter le meilleur traitement pour chaque patient., Maroc Médical, Vol. 35, No 4 (2013)
